흉막에 발생한 소세포암 1예 (A Case of a Small Cell Carcinoma of the Pleura)

  • A extrapulmonary small cell carcinoma is a very rare disease, and a primary pleural manifestation is extremely rare. A diagnosis of a small cell carcinoma should be based on the cell morphology, histological pattern, and an immunohistochemical study. We recently experienced a case of small cell carcinoma (SCC) of the pleura in a 59-year-old man who had suffered from right pleuritic chest pain. A histopathological confirmation of SCC was made by a video-associated thoracoscopic lung biopsy. Systemic chemotherapy with etoposide and cisplatin was initiated.

  • Lung cancer remains the most common cause of cancer death in the United States and worldwide. About 80~90% of cases are smoking-related and smoking cessation programs are of great importance in reducing lung cancer risk. However, the lifetime risk for lung cancer remains elevated even in ex-smokers. Chemoprevention holds the promise to further reduce this risk and thus to decrease lung cancer incidence and mortality. Over the last decades, most chemoprevention trials for lung cancer have yielded negative outcomes. Population-based studies suggest that high intake of certain foods such as soy, red wine or green vegetables may be associated with decreased cancer risk. Because of these observations and their general safety, a plethora of natural compounds is currently being studied for the chemoprevention of cancer. In this review we discuss promising in vitro and in vivo data of novel natural compounds, their interference with molecular mechanisms responsible for lung cancer development and potential implications for their further preclinical and clinical investigation.

Prostatic Cancer Presenting as an Isolated Large Lung Mass

  • A hidden primary tumor presenting as an isolated lung mass is a diagnostic challenge to physicians because the diagnosis of lung cancer is likely to be made if the histologic findings are not inconsistent with lung cancer. A large lung mass was found incidentally in a 59-year-old man. Although adenocarcinoma was diagnosed by percutaneous needle biopsy, thyroid transcription factor-1 (TTF-1) immunostaining was negative, raising suspicion that there was another primary site. There was no abnormal finding except for the lung mass on a $^{18}FDG$-PET/CT scan and the patient did not complain of any discomfort. Finally, prostatic cancer was confirmed through the study of tumor markers and prostate-specific antigen (PSA) immunostaining. Because of the rare presentation of a single lung mass in malignancies that have another primary site, physicians should carefully review all data before making a final diagnosis of lung cancer.

  • Lymphoepithelioma-like carcinoma (LELC) of the lung is a very rare tumor. Originally described in the nasopharynx as lymphoepithelioma, this carcinoma has also been found in the stomach, esophagus, thymus, cervix, urinary bladder, skin, and salivary glands. Histologically, it is an undifferentiated carcinoma that has a syncytial appearance with tumor cells and is infiltrated by numerous lymphocytes, macrophages, and plasma cells. LELC of the lung occurs more commonly in Asians, particularly Chinese. Many studies have reported the association between Epstein-Barr virus (EBV) and LELC of the lung in Asian patients. A 45-year-old man had a solitary pulmonary nodule on a routine chest X-ray examination. As a malignant tumor was suspected, surgical resection was performed to establish the correct diagnosis. The pathology of the excised tumor demonstrated LELC of the lung. This is the first report of LELC of the lung in Korea.

Moxifloxacin의 Secretory $PLA_2$억제가 올레인 산으로 유도된 호중구성 급성 폐손상에 미치는 영향 (Moxifloxacin Ameliorates Oleic Acid-induced Acute Lung Injury by Modulation of Neutrophilic Oxidative Stress in Rats)

  • Background: Based on the known immunoregulatory functions of moxifloxacin on phagocytes, the therapeutic effect of moxifloxacin on oleic acid (OA)-induced acute lung injury (ALI) was investigated. Methods: Moxifloxacin (10 mg/kg) was given to male Sprague-Dawley rats that had been given oleic acid (OA, $30{\mu}L$) intravenously. Five hours after OA injection, parameters demonstrating ALI were assessed to measure the effects of moxifloxacin on acute lung injury. Results: The pathological findings of OA-induced ALI's was diminished by moxifloxacin. Through ultrastructural and $CeCl_3$ EM histochemistry, moxifloxacin was confirmed to be effective in decreasing oxidative stress in the lung as well. Indices of ALI, such as lung weight/body weight ratio, protein content in bronchoalveolar lavage fluid, and lung myeloperoxidase were decreased by moxifloxacin. In diaminobenzidine immunohistochemistry, fluorescent immunohistochemistry, and Western blotting of the lung, moxifloxacin had decreased the enhanced expression of secretory phospholipase $A_2$ ($sPLA_2$) by OA. Conclusion: We concluded that moxifloxacin was effective in lessening acute inflammatory pulmonary edema caused by OA, by inhibiting the neutrophilic respiratory burst, which was initiated by the activation of $sPLA_2$.

자가 호기말 양압(auto-PEEP)을 보인 환자에서 원인질환에 따른 PEEP적용 효과의 차이 (Different PEEP Effects on Lung Volume According to Underlying Lung Disease in Patients with Auto-PEEP)

  • 배 경 : 인공 호흡기 치료 중 auto-PEEP을 보이는 환자에게 PEEP을 적용할 경우에 폐 용적에 미치는 영향은 일정하지 않다. COPD와 같이 동적 호기류 제한이 있는 경우에 auto-PEEP 수치의 85%에 해당하는 PEEP까지는 안전하게 적용할 수 있으나, 동적 호기류 제한이 없으면 낮은 PEEP에서도 폐 용적의 증가가 나타난다. 이에 저자들은 auto-PEEP이 있는 환자에서 임상진단에 의한 질환별로 PEEP적용의 효과에 차이가 있는지 보고자 하였다. 대상 및 방법 : 인공 호흡 중 auto-PEEP을 보인 환자 16명을 대상으로 병력, 임상 증상 및 흉부방사선 소견을 기준으로 천식, COPD, 결핵성 변화군 등 3가지 환자 군으로 분 류한 뒤 각각 auto-PEEP 수치의 25, 50, 75, 100%에 해당하는 PEEP을 적용하고 포획 가스용적을 측정하여 폐 용적의 증가를 비교하였다. 결 과 : 천식 환자 군에서는 75% PEEP부터 폐 용적의 증가가 의미 있게 나타났다. COPD 군에서는 100% PEEP에서 폐 용적이 증가 하였다. 결핵성 변화 군에서는 낮은 PEEP에서도 폐 용적이 증가하는 경향을 보였으나 통계적으로 의미 있는 증가는 100% PEEP에서만 나타났다. 결 론 : Auto-PEEP을 보이는 환자에서 폐 용적에 대한 PEEP의 효과는 폐의 병리 상태에 따라 달랐다. 천식 환자 군에서는 auto-PEEP 수치의 50%이상에 해당하는 PEEP 적용은 포획 가스 용적의 증가를 가져올 수 있다.

  • A Literature study on the external treatment of nose diseases, the results are as follows; 1. Nose was called "Cheun-bin(天牝)", was outer orifice of the lung, was the gate of vital energe(氣), The main function of nose is divided three pacts. One is the respiration, another is the sense of smell, the other is the sounding consonance. 2. Nose is concerned with The Lung(肺), The Spleen(脾), The Heart(心), The Stomach(胃), The Large intestine(大腸), The Bladder(膀胱) in the organs, is concerned with Lung Meridean, Large intestine Meridean, Stomach Meridean, Heart Meridean, Spleen Meridean, Governor Vessel Meridean etc. 3. The method on the external treatment of nose diseases is classified with obstructive method, pouring method, blowing method, attaching method, plaster method, rubbing method, cleansing method, poulticing method. In frequency of practical use, Obstructive method is used 276 cases, Blowing method is used 123 cases, Attaching method is used 103 cases, Plaster method is used 76 cases. 4. In frequency of the medicinal works on the external treatment of nose diseases, Herba Asari(細辛) is used 134 cases, Pedicellus Melo(瓜 ) is used 85 cases, Radix Aconiti Praeparata(附子) is used 52 cases, Herba Agastachis(麝香) is used 51 cases, Semen Armeniacae Amarurn(杏仁) is used 44 cases etc. In frequency of a prescription on the external treatment of nose diseases, Tongcho-san(通草散) is used 9 cases, $Sesin-go{\cdot}Sinyi-go{\cdot}Changpo-san$ are used 8 cases, $Kwache-san{\cdot}Wungwhang-san$ are used 7 cases etc.

Adverse Effects of Air Pollution on Pulmonary Diseases

  • Environmental exposure to air pollution is known to have adverse effects on various organs. Air pollution has greater effects on the pulmonary system as the lungs are directly exposed to contaminants in the air. Here, we review the associations of air pollution with the development, morbidity, and mortality of pulmonary diseases. Short-and long-term exposure to air pollution have been shown to increase mortality risk even at concentrations below the current national guidelines. Ambient air pollution has been shown to be associated with lung cancer. Particularly long-term exposure to particulate matter with a diameter <2.5 ㎛ (PM2.5) has been reported to be associated with lung cancer even at low concentrations. In addition, exposure to air pollution has been shown to increase the incidence risk of chronic obstructive pulmonary disease (COPD) and has been correlated with exacerbation and mortality of COPD. Air pollution has also been linked to exacerbation, mortality, and development of asthma. Exposure to nitrogen dioxide (NO2) has been demonstrated to be related to increased mortality in patients with idiopathic pulmonary fibrosis. Additionally, air pollution increases the incidence of infectious diseases, such as pneumonia, bronchitis, and tuberculosis. Furthermore, emerging evidence supports a link between air pollution and coronavirus disease 2019 transmission, susceptibility, severity and mortality. In conclusion, the stringency of air quality guidelines should be increased and further therapeutic trials are required in patients at high risk of adverse health effects of air pollution.

Comparison of Clinical and Radiologic Characteristics between Anthracofibrosis and Endobronchial Lung Cancer

  • Background: Endobronchial lung cancer (EBLC) and bronchial anthracofibrosis (BAF) share similar symptoms and radiological findings. The aim of this study was to describe clinical and radiological differences between BAF and EBLC, both of which were confirmed by bronchoscopy. Methods: This was a retrospective study of patients with BAF or EBLC from 2008 to 2014. Data were derived from a bronchoscopy registry made since January 1, 2008. Clinical and radiological characteristics of both diseases were analyzed. Results: Among 3,214 patients who underwent bronchoscopy, 167 and 117 patients were enrolled in BAF and EBLC groups, respectively. BAF occurred more predominantly in older non-smoking female patients with a higher chance of tuberculosis (38.3%) than EBLC (6.0%). Cough, sputum, and dyspnea were common symptoms reported for both groups. Bronchoscopic findings revealed that BAF lesions were more common in multiple lobar bronchi (85.0%) or bilateral bronchi (73.7%). Radiologic findings revealed that bronchial stenosis was the most commonly found lesion in both groups (49.1% and 78.6%, respectively). Rates of peribronchial calcification and bronchial wall thickening were higher in the BAF group. The number of patients with lymph node calcification was also higher in the BAF group. Conclusion: Results of this study demonstrated characteristics of clinical and radiologic findings of BAF and EBLC. Increasing the awareness of both diseases may help clinicians differentiate these two diseases from each other, thus avoiding unnecessary invasive diagnostic procedures.

단순 디지털 촬영과 저선량 CT의 폐기종 소견으로부터 폐쇄성 폐기능 장애 위험 비교 (Evaluation of Obstructive Pulmonary Function Impairment Risks in Pulmonary Emphysema Detected by Low-Dose CT: Compared with Simple Digital Radiography)

  • Background: Pulmonary emphysema (PE) is major cause of obstructive pulmonary function impairment (OPFI), which is diagnosed by spirometry. PE by high resolution CT is known to be correlated with OPFI. Recently, low dose CT (LDCT) has been increasingly used for screening interstitial lung diseases including PE. The aim of this study was to evaluate OPFI risks of subjects with PE detected by LDCT compared with those detected by simple digital radiography (SDR). Methods: LDCT and spirometry were administered to 266 inorganic dust exposed retired workers, from May 30, 2007 to August 31, 2008. This study was approved by our institutional review board and informed consent was obtained. OPFI risk was defined as less than 0.7 of forced expiratory volume in one second (FEV1)/forced vital capacity (FVC), and relative risk (RR) of OPFI of PE was calculated by multiple logistic regression analysis. Results: Of the 266 subjects, PE was found in 28 subjects (10.5%) by LDCT and in 11 subjects (4.1%) by SDR; agreement was relatively low (kappa value=0.32, p<0.001). FEV1 and FEV1/FVC were significantly different between PE and no PE groups determined by either SDR or LDCT. The differences between groups were larger when the groups were divided by the findings of SDR. When PE was present in either LDCT or SDR assays, the RRs of OPFI were 2.34 and 8.65, respectively. Conclusion: LDCT showed significantly higher sensitivity than SDR for detecting PE, especially low grade PE, in which pulmonary function is not affected. As a result, the OPFI risks in the PE group by LDCT was lower than that in the PE group by SDR.
