Usefulness of the double-guidewire technique for endoscopic procedures in the field of biliary and pancreatic diseases

  • The double-guidewire method has been increasingly used in endoscopic procedures for biliary and pancreatic diseases in recent years, including endoscopic retrograde cholangiopancreatography and endoscopic ultrasonography-related procedures. In addition, double-lumen catheters with uneven distal and proximal lumen openings have been introduced, making it possible to easily create a double-guidewire situation, and the usefulness of the double-guidewire technique using uneven double-lumen cannulas has been widely reported. Although the advantages of using two guidewires depend on the particular situation and the appropriate use of the two guidewires, deepening the knowledge of the double-guidewire method will contribute greatly to troubleshooting in daily practice. In this review, the usefulness of the double-guidewire technique is discussed with respect to two main areas: selective insertion of guidewires and devices and biliary cannulation.

  • 본 논문에서는 관상동맥 질환의 객관적 분석을 위해, 혈관단면영상에서의 ROI(Lumen, Media, Plaque)에 대한 정확한 분할과 분할한 영역에서 질병을 유발시키는 요소들에 대한 정량적 분석을 위한 소프트웨어를 개발하였다. 본 시스템은 Visual C++ 6.0을 이용하여 개발하였으며, 현미경으로부터 획득한 관상동맥 단면영상에 적용하여 Lumen, Media와 Plaque를 분할하고, 각 영역의 형태학적 특징을 추출하여 분석 결과를 파일로 저장할 수 있도록 구현하였다. 분석된 결과는 심장질환의 객관적 진단을 위한 보조판단근거로써 사용될 것으로 기대한다.

  • 만성동맥폐색증 환자의 혈관내 시술 시 발생되는 기술적 실패의 가장 많은 이유 중의 하나는 폐색부위를 내막하 진행 후 진강내 재진입의 실패에 기인한다. 진강내 재진입 카테터는 유도철사의 진강내 재진입을 용이하게 하여, 대부분 수술로 전환될 만성폐색질환에서 성공적인 혈관내 시술을 가능하게 한다. 본원에서는 표재성 대퇴동맥의 만성동맥폐색증의 혈관내 시술 시 기존의 장비로 진강내 재진입이 실패한 3예에서 $Outback^{(R)}$ $LTD^{TM}$ catheter를 이용하여 성공적으로 치료하였기에 문헌 고찰과 함께 보고하는 바이다.

속리산 참나무류 도관내강면적연대기와 기후인자와의 관계분석 (Relationships Between Vessel-lumen-area Time Series of Quercus spp. at Mt. Songni and Corresponding Climatic Factors)

  • 본 연구는 국내 속리산 참나무류 도관내강면적연대기를 작성하고, 작성된 연대기가 기후프록시로써의 활용 가능성이 있는지 확인하기 위해 수행되었다. 도관내강면적연대기 작성을 위해 3가지 측정 방법이 이용되었다. 방법 1과 2는 도관내강면적이 각각 $6,000{\mu}m^2$ (MVA-60)와 $7,500{\mu}m^2$ (MVA-75) 이상인 것만 이용하여 평균값으로 연대기를 작성하는 것이고, 방법 3은 각 연륜에서 가장 큰 도관내강면적(MAX)만을 이용하여 연대기를 작성하는 것이다. MVA-60과 MVA-75는 전년도 11월부터 당년 1월까지인 겨울철 강수량과 유의성에 근접한 정의 상관을 나타냈으나, MAX는 전년도 11월에 대해서만 유의성 있는 정의 상관을 나타냈다. 본 연구결과를 근거로 국내 참나무류의 도관내강면적은 연륜기후학 분야에서 기후프록시로써 활용이 가능함이 확인되었다.

조명시뮬레이션 소프트웨어의 적합한 사용을 위한 기초적 연구 (A Fundamental Study for Proper Application of Lighting Simulation Software)

  • 조명시뮬레이션 소프트웨어는 크게 조명기구의 광학설계를 위한 소프트웨어와 조명계획안의 예측 및 평가를 위한 소프트웨어로 구분할 수 있으며, 그 사용이 점차 증가하고 있다. 조명기구의 광학설계를 위한 시뮬레이션 소프트웨어로는 Photopia 2.0이 대표적이며, 조명계획안의 적합성을 판단하기 위한 시뮬레이션 소프트웨어는 Lumen-micro 2000, Relux 2006, Lightscape 3.2와 AGI 32 등이 있다. 이러한 시뮬레이션 소프트웨어는 측정값과 시뮬레이션 결과값, 그리고 소프트웨어별 시뮬레이션 결과값이 상이하게 나오는 경우가 발생한다. 따라서 본 연구는 임의의 조명기구 측정 배광데이터와 Photopia 2.0의 시뮬레이션 배광데이터를 비교하고, 공간의 규모, 배광데이터, 마감재등의 같은 조건하에 Lumen-micro 2000, Relux 2006, Lightscape 3.2와 AGI 32의 시뮬레이션 결과를 비교하여 차이점의 원인에 분석하고자 한다.

등줄쥐(Apodemus agrarius coreae)의 정자유입에 따른 정소상체 상피세포의 형태적 변화 (Morphological Changes of Epithelial Cells of the Epididymides by Sperm Entrance in the Korean Striped Field Mouse, Apodemus agrarius coreae)

  • To investigate the morphological changes of epithelial cells of ductus epididymides by sperm entrance, the Korean striped field mouse, Apodemus agrarius coreae was studied with light and transmission electron microscopy. 1. The diameters of the caput epididymis (Cp) and its lumen were $135\sim145{\mu}(140{\pm}0.5{\mu})$ and $115\sim120{\mu}m(117{\pm}0.1{\mu}m)$. The length and width of the epithelium were $27.0\sim28.5{\mu}m(28.0{\pm}0.1{\mu}m)$ and $4.8\sim5.4{\mu}m(5.1{\pm}0.2{\mu}m)$. 2. The diameters of the corpus epididymis (Cr) and its lumen were $160\sim170{\mu}m(166{\pm}0.2{\mu}m)$ and $140\sim150{\mu}m(145{\pm}0.3{\mu}m)$. The length and width of the epithelium were $17.4\sim18.0{\mu}m(17.6{\pm}0.5{\mu}m)$ and $8.8\sim10.4{\mu}m(9.5{\pm}0.2{\mu}m)$. 3. The diameters of the cauda epididymis (Cu) and its lumen were $270\sim280{\mu}m(275{\pm}0.2{\mu}m)$ and $265\sim275{\mu}m(268{\pm}0.3{\mu}m)$. The length and width of the epithelium were $11.2\sim13.4{\mu}m(12.3{\pm}0.3{\mu}m)$ and $9.2\sim11.2{\mu}m(10.0{\pm}0.2{\mu}m)$. Therefore, the size of diameter and lumen of the ductus epididymides and the width of the epithelium increased from Cp and Cr to Cu, but the length of epithelial cells decreased from Ca and Cr to Cu. These data suggest that the morphological changes of epithelial cells of the ductus epididymides may be the results of the sperm entrance.

유치도요관 환자의 방광 세척에 따른 요로 감염 발생 빈도에 관한 연구 (A Study on Incidence of Bacteriuria according to Bladder Irrigation in Patients with Indwelling Catheter)

  • The Purpose of this study is for clinical nurses to be aware of the significance of prevention against bacteriuria caused form foley catheterization, through probing variables related to the occurrence of bacteriuria which appears as the most frequent occurring infection, and try to lower bacteriuria by applying to nursing care at the clinical. For this study 46 patients with catheterization and the closed drainage system were sampled and investigated from among patients at Intensive Care Unit in Two hospitals affiliated to K University in Seoul. Those patients sampled had not shown bacteriuria before foley catheterization. The research design is to explore the effectiveness of prevention against bacteriuria in accordance with bladder irrigation and no bladder irrigation. Especially, the frequency of occurrence of bacteriuria examined so as to compare the effectiveness of bladder irrigation depending on the type of foley catheter between 2-lumen foley catheter and 3-lumen foley catheter. The results Were as follows. 1. The occurrence of bacteriuria in patient with bladder irrigation was 21.7%, while in patient without bladder irrigation 26.1%. 2. The occurrence of bacteriuria in patient without bladder irrigation according to duration of Indwelling catheter, was 4.3% after 48 h, 8.7% after 72 h and 21.7% after 96 h. 3. In case of 2-lumen foley catheter the occurrence of bacteriuria in patient with bladder irrigation was 0% after 72 h, and 4.35% after 96 h. In case of 3-lumen foley catheter, the occurrence of bacteriuria in patient with bladder irrigation was 13% after 48 h, 8.7% after 72 h and 4.3% after 96 h. 4. The occurrence of bacteriuria according to duration of catheterization was 8.7% after 48 h, 8.7% after 72 h and 15.2% after 96 h. In sex, female was 35% and male 15.4%. 5. The occurrence of bacteriuria according to mental state was 15% in clear state, while 29.6% in mental disorder. 6. In regard to a kind of microorganism induced bacteriuria, Gram negative bacteria was 63.7%, Gram positve bacteria 36.3%.

Initial experience with Scepter Mini dual lumen balloon for embolization of cerebrovascular diseases

  • Objective: Endovascular treatment of cerebrovascular diseases is often challenging due to small caliber, tortuous distal vessels. Several devices and techniques have evolved to overcome these challenges. Recently, a low profile dual lumen microballoon catheter, specifically designed for distal navigation is employed for neurovascular procedures. Due to its recent advent, scarce data is available on clinical utility and safety of Scepter Mini. The aim of this case series is to report our initial experience with Scepter Mini in the management of various cerebrovascular diseases. Methods: All interventional neurovascular cases performed using Scepter Mini between January 2020 till April 2021 were included. Data regarding patient demographics, procedural details and complications was retrospectively collected from patient's electronic medical record and procedure reports. Results: Total twelve embolization procedures were performed in eleven patients, including six brain arteriovenous malformation, two dural arteriovenous fistula, one vein of Galen malformation and three hyper-vascular glomus tumor embolizations. All procedures were successfully performed with adequate penetration of the embolic agent. Complete embolization was performed in six procedures, while intended partial embolization was performed in the rest of procedures. Scepter Mini was solely used in ten procedures, however in the other two embolization procedures it was used as an additional conjunct tool to complete the intended embolization. No balloon related complication was observed in any procedure. Conclusions: Scepter Mini dual lumen microballoon catheter is safe and feasible for delivery of liquid embolic agents for cerebrovascular embolization procedures.

한우 및 재래산양 서비기관의 형태학적 연구 (Morphological studies on the vomeronasal organ of Korean native cattle and Korean native goats)

  • Morphological features of the vomeronasal organ of both Korean native cattle and Korean native goat were studied by gross, microscopic and histochemical examinations. Anatomical characteristics of the vomeronasl organ were similar in both Korean native cattle and Korean native goats. The vomeronasal organ is a tubular structure situated bilaterally at the base of the nasal septum, and enclosed by hyaline cartilage. Its lumen is semilunar to crescent in transverse sections. It join with the incisive duct through narrow duct. The lumen of the vomeronasal organ is lined with sensory and respiratory epithelia. The distribution pattern of vomeronasal mucosal epithelia varied by the position. In the anterior portion joining with nasal cavity, the lumen is lined with only respiratory epitheliu. In the middle portion, sensory epithelium appeared on the medial side, and respiratory epithelium on the lateral side. In the posterior, it is lined with sensory epithelium on the ventral side and lined with respiratory epithelium on the dorsal side. The vomeronasal gland composed of mucous and serous acini are distributed in the lamina propria under the respiratory epithelium, where venous sinuses are also well developed.

기관 삽관후 발생한 좌측 주기관지 파열 - 1례 보고 - (Left Bronchial Rupture Following Endobronchial Intubation - One case report -)

  • 이중관 튜브(double-lumen tube)로 기관 삽관한 후 발생하는 기관지 파열은 극히 드문 합병증이다. 우리 는 기관 삽관후에 발생한 좌측 기관지 파열을 치험하였다. 58세 여자 환자로 수술전 검사상 우하엽에 병 기 IIB의 분화가 잘된 악성 선암으로 진단되었다. 환자는 우하엽절제술을 위하여 로버트쇼 튜브 (Robertshaw tube)로 기관 삽관하였고 수술중 종격동 기종과 환기 장애로 인하여 좌측 주기관지의 파열을 발견하였다. 즉시 좌측 개흉술을 시행하여 기관지 파열부위를 흡수성 봉합사(PDS)로 봉합하였으며 수술 후 별다른 문제없이 회복되었다.
