Background: Effect of cervical and lumbar tractions on the reduction in the angle of curvature and the effect of a correction exercise or a general traction method on balance, muscle strength, pain, and body alignment, however insufficient research has been undertaken on self traction exercises targeting patients with scoliosis. Purpose: To determine the effect of cervical and lumbar tractions on the reduction in the angle of curvature and the effect of a correction exercise or a general traction method on balance, muscle strength, pain, and body alignment. Design: Randomized controlled clinical trial (single blinded) Methods: Twelve adults(20s) with scoliosis were included in this study and performed a traction program that was composed of a 5-min warm-up exercise, a 15-min main exercise, and a 5-min cool-down exercise (25 minutes in total), three times a week for four weeks. The Chiro traction machine was used for the self-traction exercise. Vertebral alignment, muscle strength, and flexibility were compared before and after the intervention using the paired T-test. Results: The scoliosis angle, pelvic torsion, and lumbar extensor were significantly changed by intervention; however, there was no significant difference in flexibility. Conclusion: The results revealed that self-traction exercise activated blood flow through the extension and contraction of muscles, effectively increasing the function of the muscles around the vertebrae.
Low intensity laser irradiation is potential physical agent that triggers the muscle regeneration by previous study. In muscle regeneration, a number of growth factors also promotes that is triggered in response to muscle damage. The transforming growth factor(TGF)-$\beta$ is involved in the activation of cell proliferation and the inhibition of cell differentiation in muscle regeneration. This is secreted not only autocrine system but also paracrine and endocrine. Therefore, We investigated that effects of Gallium aluminum arsenide(GaAlAs) diode laser for the expression of TGF-$\beta$ on lumbar spinal cord after extensor digitorum muscle crush injury. After laser irradiation, the immunoreactivity of TGF-$\beta$ was increased bilaterally in gray mater of spinal cord. Especially, in 1 day, experimental group was highed than control, and in 3 day, lateral motor nucleus were storong immunoreactivy of TGF-$\beta$. Also, in 1 and 2 day, TGF-$\beta$ was showed in white mater as well as gray mater, but in 3 day, only showed in gray mater. These data may suggests to the establishment of laser irradiation on spinal cord for skeletal muscle injury.
Objects: This study is designed to analysis improvement and maintaince of trunk stability targeting patients who need post operative rehabilitation exercise after undergoing opened microscopic laser discectomy(OMLD) due to HNP. Method: Between March 2004 and February 2005 a total sixty patients who underwent OMLD due to $L_4/L_5$, $L_5/S_1$ HNP and who agreed to the experiment were subject for this study. Experimental group consisted of 18 subjects, and they underwent 45 minutes dynamic lumbar stabilization exercise And control group consisted of 18 subjects who conducted conservative physical therapy based on the use of physical modality for 45 minutes except to exercise. Results: Experimental group that was lumbar extensor's isometric peak torque, weight distribution of both leg, trunk muscle balance and Oswestry LBP disability index increased during 12 weeks in a statistically significant manner compared to before exercise (p<.05). When re-test was tried, isometric peak torque (p>.05) and Oswestry LBP disability index(p<.05) maintained 12th week level or improved. Weight distribution rate of both leg and trunk muscle balance maintained the level of 8th week of exercise execution(p<.05). Control group that was lumbar extensor's isometric peak torque, weight distribution rate of both leg and trunk muscle balance aggravated during 12 weeks compared to pre-test(p>.05) But, Oswestry LBP disability index increased in a statistically significant level compared to pre-test(p<.05). When re-test was tried, isometric peak torque increased slightly compared to pre-test, but decreased when at least $60^{\circ}$ flexion angle(p>.05). Weight distribution rate of both leg and trunk muscle balance also aggravated compared to pre-test(p>.05), and Oswestry LBP disability index was similar to the 4th week of physical therapy execution(p>.05). Conclusion: Successive postoperative Especially, Application of dynamic lumbar stabilization exercise program is important than traditional lumbar strengthening exercise program in the maintaince of spinal stability.
The purpose of this study was to find the effects of the lumbar stabilizing exercise on the isometric lumbar extension strength and the range of motion of lumbar extension. 16 healthy subjects were recruited (9 males, 7 females, mean age 25${\pm}$3.2). They performed 5 types of exercises 5 times per week for 4 weeks. We evaluated isometric trunk extension strength (0, 12, 24, 36, 48, 60, $72^{\circ}$) by MedX(Ocala, FL) and measured range of motion of lumbar extension. All measurements were measured at pre-exercise and 4 weeks post-exercise. The results of this study were summarized as follows; After 4 weeks of exercise, isometric lumbar extension strength was generally increased but there were only significant differences at $12^{\circ},\;24^{\circ},\;48^{\circ}$. The range of motion of lumbar extension was increased (p<.05). The isometric lumbar extension strength correlated with the range of motion of lumbar extension. In conclusion, our results have shown that lumbar stabilizing exercise influenced both lumbar extension strength and range of motion of lumbar extension.
Main cause of low back pain is a poor posture. Most low back pains are due to a poor posture. The poor posture induces muscle tension and finaliy low back pain. In general, maintaining good posture during working and sleeping hours are the first line of defence against back pam. (1) Supine posture is the easiest posture that relaxes and fixes muscles. However, supine posture can increase the pressure in ribcage posture aspect and disphragram due to visceral oragans. (2) Sitting in one position for a long time results in fatigue and relaxation of spinal muscies. Finally, body strength is weakened and sitting posture will become poor. If this poor posture continues for a longer time, pain will be accompanied due to overelongation of muscle ligaments. The habitual poor posture could induce intervertebral disc distortion. If the intervertebral disc is damaged, sitting in one position or movement causes pain. (3) Abnormal lumbar curve induces the tention of abdominal muscle and paravertebral muscle groups as well as tention of lower limb muscle group connected to pelvis. For a person with weak body strength, muscle relaxation increases curvature in lumbar, chest and cervical regions. This will induce a pelvic anterior tilting of the imaginary line between A. S. I. S. and P. S. I. S. Hip joint extensor muscle acts on releasing the pelvic anterior tilting. Contrections of hamstring muscle and femoral muscle recover the imaginary line between A.S.LS. and P.S.I.S. from pelvic anterior tilting. thus, contraction of rectus abdominis muscle are required to maintain the normal lumbar curve.
Purpose: The aim of this study was to investigate correlations between the Functional Movement Screen (FMS), pain, and performance ability in professional fencing players. Methods: Fifty-six athletes participated in this study. The pain group included those who had a score on a pain-related Visual Analogue Scale (VAS) of ${\geq}$20 and an Oswestry Disability Index (ODI) score ${\geq}$10). In the non-pain group, these scores were: VAS(<20), ODI(<10). The VAS and ODI were used to measure pain throughout the study. Performance ability included motor function of the lower extremities (as assessed by a Modified Functional Index Questionnaire, MFIQ), dynamic balance (Balance system, BS and Posture med, PM), flexor and extensor muscle strength of the lumbar region was recorded as maximal isometric strength. Results: Among athletes who had pain, 5 of 15(33.33%) showed impaired functional movement. Conversely, only 2 of 41(4.88%) of those who had no pain showed such impairment (FMS ${\leq}$14score). The athletes who had pain and who had an FMS score above 14 (10/56; 17.86%) showed a significantly higher score for extensor muscle strength of the lumbar compared with those with pain and an FMS score below 14 (5/56; 8.93%) were significant correlations between the FMS and pain (r=-0.40 to -0.42, p<0.01), the MFIQ (r=-0.33, p<0.05), dynamic balance (r=-0.27 to -0.40, p<0.05-0.01), muscle strength of the lumbar (r=0.27 to 0.29, p<0.05). Stepwise multiple regression analysis showed that the dynamic balance score (${\beta}{\beta}$=-0.41) had slightly more power in predicting FMS score than pain, motor function of lower extremity, or muscle strength. Conclusion: The FMS was significantly associated with values of pain, motor function of the lower extremities, dynamic balance, and muscle strength of the lumbar. However the FMS appears to lack relevance and reasonable evidence to suggest that it is an acceptable measurement tool for functional movement analysis.

Objective To elucidate the effect of a 12-week horizontal vibration exercise (HVE) in chronic low back pain (CLBP) patients as compared to vertical vibration exercise (VVE). Methods Twenty-eight CLBP patients were randomly assigned to either the HVE or VVE group. All participants performed the exercise for 30 minutes each day, three times a week, for a total of 12 weeks. Altered pain and functional ability were evaluated using the visual analog scale (VAS) and Oswestry Disability Index (ODI), respectively. Changes in lumbar muscle strength, transverse abdominis (TrA) and multifidus muscle thicknesses, and standing balance were measured using an isokinetic dynamometer, ultrasonography, and balance parameters, respectively. These assessments were evaluated prior to treatment, 6 weeks and 12 weeks after the first treatment, and 4 weeks after the end of treatment (that is, 16 weeks after the first treatment). Results According to the repeated-measures analysis of variance, there were significant improvements with time on VAS, ODI, standing balance score, lumbar flexor, and extensor muscle strength (all p<0.001 in both groups) without any significant changes in TrA (p=0.153 in HVE, p=0.561 in VVE group) or multifidus (p=0.737 in HVE, p=0.380 in VVE group) muscle thickness. Further, there were no significant differences between groups according to time in any of the assessments. No adverse events were noticed during treatment in either group. Conclusion HVE is as effective as VVE in reducing pain, strengthening the lumbar muscle, and improving the balance and functional abilities of CLBP patients. Vibrational exercise increases muscle strength without inducing muscle hypertrophy.
Back extension exercises have been used for rehabilitation of the injured low back, prevention of injury, and fitness training programs. However, excessive loading on low back can exacerbate existing structural weakness. The purpose of this study was to compare muscle activity of low back muscles during back extension exercises. Twenty healthy male subject s were evaluated. Electromyographic (EMG) activities of low back muscles at L1 and L5 level were recorded during seven different back extension exercises and two reference tasks by surface EMG and saved for data analysis. Reference tasks of lifting 20% and 40% of their body weight were included for comparison. The result were as follows: 1) Single-arm extension and single-leg extension exercises on quadruped position appeared to constitute a low-risk exercise for initial extensor strengthening. 2) When arm extension was combined with contralateral leg extension on quadruped position, EMG activities of low back muscles were increased. 3) EMG activity of low back muscles was highest during the trunk extension exercises on prone position. 4) EMG activities of low back muscles during arm and leg extension exercises on quadruped position were less than those of reference task of lifting 40% of their body weight. These result s have important implications for progressive back extensor muscle strengthening exercises in patients with back pain.
Background: This study was conducted in order to see if muscle strength is different according to fall experience in female elders by comparing their muscle strength of waist and knee, which is one of factors inducing fall. Method: For this study, 22 female elders aged over 65 sampled from senior centers in D City were surveyed on their fall experiences using a questionnaire asking clinical history. Then, 8 elders with fall experience (FE) and another 8 without (NFE) were selected and participated in the research. Isokinetic muscle strength was measured with isokinetic equipment BIODEX SYSTEM 3 PRO, and the peak torque was obtained from the highest of 9 measurements at angular velocity $60^{\circ}/sec$ according to the method recommended in the manual. Measured items were waist flexion and extension, and knee flexion and extension. Results: The FE group showed higher extensor and flexor muscle strength of the waist, but the difference was not significant (p<.05). In addition, no difference was observed in extensor muscle strength of the knee (p<.05). In flexor muscle strength, however, the FE group was significantly higher than the NFE group (p<.05). Conclusion: The application of regular exercise programs for improving muscle strength, endurance, etc. to those with fall experience will be effective in preventing falls among them.
PURPOSE: This study aims to determine the optimal knee joint angle and hip joint angle for minimizing the cervical muscle tension and maximizing the muscle activity of the trunk during the bridging exercise for trunk stabilization. METHODS: The bridging exercise in this study included seven forms of exercise: having a knee joint flexion angle of $120^{\circ}$, $90^{\circ}$, $60^{\circ}$, $45^{\circ}$ and hip joint abduction angle of $15^{\circ}$, $10^{\circ}$, $5^{\circ}$. The posture of the bridging exercise was as follows. To prevent the increase of hyper lumbar lordosis during the bridging exercise, the exercise was practiced after maintaining the lumbar neutral position through the pelvic posterior tilting exercise. RESULTS: The abduction angles did not result in statistically significant effects on the cervical erector, external oblique, rectus abdominis and erector spinae muscles. However, in relation to the knee joint angles, during the bridging exercise, statistically significant results were exhibited. CONCLUSION: The knee joint angle affected the muscle activity of the neck muscle. The greater the knee joint angle, the lower the load placed on the neck muscle. In contrast, the load increased as the knee joint angle decreased. In addition, the muscle activity of the neck muscle and trunk muscle increased as the knee joint angle decreased.
