The Comparative analysis of X-ray film on Lumbar Scoliosis and HNP by Barge Method (Barge 방식에 의한 요추부 척추측만증과 추간판 탈출증의 X-ray 비교분석)

  • Objective: The purpose of this study is to evaluate the disc block subluxation for lumbar scoliosis and herniation of nucleus pulpous (HNP). Methods: We used AP & Lateral view X-ray for patients. And we measured of disc wedge angle, vertebral body rotation to evaluate the typical and atypical disc block subluxation. Results: On the analysis of the lateral view X-ray, 4th lumbar intervertebral disc angle (I.V.D angle) showed $4^{\circ}$, $5^{\circ}$, $3^{\circ}$, $0^{\circ}$ in the cases. On the 4th lumbar analysis of the AP view X-ray, lumbar scoliosis showed right disc wedge angle was $11^{\circ}$ (case 1), $17^{\circ}$ (case 2) and left vertebral body rotation was 13mm, 6mm. Lumbar HNP showed left disc wedge angle was $5^{\circ}$ (case 3), $4^{\circ}$ (case 4) and left vertebral body rotation was 2mm, 4mm. Conclusions: Disc block subluxation has been in lumbar scoliosis, but not been in lumbar HNP.

The relationship between Width of Lumbar Disc Space and Severity of Herniated Intervertebral Disc in Patients Diagnosis Disc space narrowing (요추 추간판 간격 좁아짐 진단 환자에서 요추 추간판 간격과 요추 추간판 탈출증 정도와의 상관성)

  • Objective : The purpose of this study was to suggest (diagnostic) criteria for lumbar disc space and see the correlation between lumbar disc space narrowing and the level of HIVD. Methods : We looked at the 113 patients diagnosed disc space narrowing with lumbar x-ray and MRI films who have visited Jaseng Hospital of Oriental Medicine with low back pain in October,2013. The width of lumbar disc space and the severity of Herniated Intervertebral Disc(HIVD) on MRI films were collected and stastically analyzed using one-way ANOVA. Results : In this study, measured value(%) for lumbar disc space of the group diagnosed serious HIVD, such as protrusion, extrusion and migration was significantly decresed compared to the normal group and the bulging group. Conclusion : There was a significant correlation between lumbar disc space narrowing and the level of HIVD.

  • Lumbar discs are one of the first parts of the musculoskeletal system to age in the human body. Lumbar disc for females start to age at twenty years of age and completely age between fifty and sixty. The number of lumbar herniated intervertebral disc patients are increasing rapidly; however, studies on protective inner wear are insufficient. This study was conducted to develop protective inner wear equipment that retains an aesthetic appreciation of the body. The methods and procedures of this study are as follows. First, foreign and domestic lumbar pads were researched and analyzed by visiting medical shops and internet shopping malls that sell lumbar pads. Second, the survey was conducted on forty patients with disc disease in hospitals located in Seoul and surrounding suburbs. The following are the study results and observations. First, the lumbar pads selling on the market had a variety of designs, textiles, and fabrics. The result of the research was to develop additional inner protective equipment for lumbar herniated intervertebral disc patients. Second, the survey results of the forty patients with disc disease were that housewives were the most vulnerable. All patients had experience purchasing lumbar pads. The prevention of pain and stress were the most selected responses for the motivation to buy lumbar pads. Most individuals said that they used lumbar pads for less than 1 year. Dissatisfaction came from fabric, activity and design. 'Being uncomfortable to wear' was the most common complaint. 'Protection and how easy the clothes were to wear' were the most selected for the necessity of developments. The Inner wear shape was the most selected in shape. Except for six respondents, all favored the lumbar protective inner wear design.

Two Cases on Tortipelvis caused by Lumbar Disc Herniation - Using Mckenzie Method Treatment (요추 추간판 탈출증에 의한 골반경사 치험 2례 - Mckenzie 수기요법을 이용)

  • Objectives : We evaluated the effect of Mckenzie method treatment by participating two patients suffering from Tortipelvis by lumbar disc herniation. Methods : Two patients were diagnosed as Tortipelvis by lumbar disc herniation through CT scan and X-ray examination. We used Mckenzie method and measured VAS(Visual analogue scale). Results and Conclusions : After using Mckenzie method treatment, We found out a recovery from both patients suffering from Tortipelvis by lumbar disc herniation. The result, through our study, suggests that Mckenzie method treatment was effective to cure the Tortipelvis by lumbar disc herniation. It also was effective to reduce the low back pain and to stabilize the lumbar disc herniation.

Loss of Disc Height after Spontaneous Regression of a Herniated Lumbar Disc - A Case Report - (탈출된 요추 추간판 자연 소실 후 발생한 추간판 간격 감소 - 증례 보고 -)

  • Study Design: Case report. Objectives: We report 3 cases of loss of disc height after spontaneous regression of a herniated lumbar disc. Summary of Literature Review: Reports of spontaneous regression of a herniated lumbar disc were identified. Materials and Methods: We conservatively treated 3 patients who were diagnosed with a herniated lumbar disc. During outpatient follow-up, radiating pain improved in all patients, but they complained of chronic lower back pain. Magnetic resonance imaging (MRI) was performed for diagnostic purposes. Results: On MRI, spontaneous regression of the herniated lumbar discs was observed, but loss of disc height was also found. Conclusions: A herniated lumbar disc may be a risk factor for loss of disc height. It is important to recognize that a patient with a herniated lumbar disc can struggle with chronic lower back pain even if spontaneous regression of the herniated lumbar disc occurs.

Introducing a New Risk Factor for Lumbar Disc Herniation in Females : Vertical Angle of the Sacral Curvature

  • Objective : To characterize the importance of the vertical angle of the sacral curvature (VASC) in lumbar disc herniations. Methods : Morphological data derived from lumbar sagittal MRI imaging. The statistical significance of the findings are discussed. The angles of 60 female patients with lumbar disc herniations (LDH) were compared with the 34 female patients without LDH. Results : 128 of the 185 patients met our inclusion criteria. The vertical angle of sacral curvature is statistically significantly bigger in females with lumbar disc herniations when compared to subjects in control group, 28.32 and 25.4, respectively. (p=0.034<0.05). Same difference was not seen in males. Conclusion : The vertical angle of sagittal sacral curvature may be another risk factor in females with lumbar disc herniations.

Lumbar Epidural Varix Mimicking Disc Herniation

  • Lumbar radiculopathy is generally caused by such well-recognized entity as lumbar disc herniation in neurosurgical practice; however rare pathologies such as thrombosed epidural varix may mimic them by causing radicular symptoms. In this case report, we present a 26-year-old man with the complaint of back and right leg pain who was operated for right L4-5 disc herniation. The lesion interpreted as an extruded disc herniation preoperatively was found to be a thrombosed epidural varix compressing the nerve root preoperatively. The nerve root was decompressed by shrinking the lesion with bipolar thermocoagulation and excision. The patient's complaints disappeared in the postoperative period. Thrombosed lumbar epidural varices may mimic lumbar disc herniations both radiologically and clinically. Therefore, must be kept in mind in the differential diagnosis of lumbar disc herniations. Microsurgical techniques are mandatory for the treatment of these pathologies and decompression with thermocoagulation and excision is an efficient method.

A Study of the Relationship between Straight Leg Raising, Valsalva Test and Size, Position of Lumbar Disc Herniation (요추부 추간판 탈출 정도와 SLR, valsalva test의 관계)

  • Objectives : This study investigated the relationship between straight leg raising(SLR), valsalva test and size, position of lumbar disc herniation. Methods : We took SLR and valsalva test on 105 patients with lumbar disc herniation. According to the result of MRI findings, this study classified three groups of 105 patients with lumbar disc herniation, bulging, protrusion and extrusion. According to the position of lumbar disc herniation, 72 patients that were diagnosed protrusion and extrsuion were sorted 4 groups, lateral, lateral postero-lateral, central postero-lateral, central. The association size, position of lumbar disc herniation and SLR, valsalva test were analysed. Results : The bigger size of disc herniation, the more positive result of SLR and valsalva test, the lower angle of SLR test. There was not significant association between the position of lumbar disc herniation and the angle of SLR test. Conclusions : The SLR and valsalva test is an useful physical examination to speculate about the degree of lumbar disc herniation.

  • The lumbar disc is one of the first parts of the human body to age. Female discs start to age at twenty and completely age between fifty and sixty. The number of lumbar herniated intervertebral disc patients is increasing rapidly; however, studies on protective inner wear are insufficient. This study develops protective inner wear equipment for bodies. The following were the study methods and procedures. First, the protective design, pattern and making of the inner wear were based on the analysis of collected data. Second, one subject was selected, then human body parts were measured to create the inner protective pattern. The inner pattern was made by the basic size of the subject. The inner protective equipment was made for lumbar disc disease patients after a wearing test and after correcting the pattern. Third, inner protective equipment was thoroughly tested to verify the compatibility of lumbar pads. The following were the study results and observations. First, lumbar pads who had inner protection were easily worn and the design was very ladyish in contrast to other lumbar pads on the market. Second, the pattern was completed by referring to, Lee Hyoung Sook's, torso and her lumbar pad shape. The fabric was mesh (for good ventilation) and poly spandex (for elasticity). Third, lumbar inner protective equipment was verified through a subject assessment and a major assessment. This study showed high scores on appearance and functional satisfaction versus existing disc lumbar pads in the market. This study predicts that new waist protective inner wear can help the female patient's health and social life because it has great functionality and can maintain outer line patterns.

Spontaneous Regression of a Large Lumbar Disc Extrusion

  • Although the spontaneous disappearance or decrease in size of a herniated disc is well known, that of a large extruded disc has rarely been reported. This paper reports a case of a spontaneous regression of a large lumbar disc extrusion. The disc regressed spontaneously with clinical improvement and was documented on a follow up MRI study 6 months later. The literature is reviewed and the possible mechanisms of spontaneous disc regression are discussed.
