전방머리자세에서 목근육 근활성도에 대한 맥켄지 신장운동과 어깨 안정화운동의 효과 비교 (Comparison between McKenzie Stretch Exercise and Scapula Stability Exercise on Neck Muscle Activation in the Forward Head Posture)

  • Purpose : The purpose of this study was to compare the effect of exercise on the neck muscles activities between scapular stability and McKenzie stretch exercise in the forward head posture subjects. Method : After measuring cervical alignment using the Global Posture System, 20 volunteers with forward head posture posture were selected and divided into two groups. The experimental group A(n=15) and Experimental group B(n=15) participated in respectively and McKenzie neck stretching exercise, three times per week for 4 weeks. The scapular stabilization exercise program was comprised middle and lower trapezius strength exercises and the stretching exercise program was comprised levator scapulae and upper trapezius stretching exercise. The activities of the muscles of the posterior neck was then measured using electromyography. Result : After the intervention, there was significant difference of a electromyography activity changes between the pre-test and post-test in the experimental group. Conclusion : This study showed that both scapular stabilization and McKenzie neck stretching exercises are more effective for reducing neck muscles activities.

어깨 손상에 따른 골프 드라이버 스윙의 근활성도 분석 (Muscle Activity Analysis According to Shoulder Injury of Golf Drive Swing)

  • Objective : The purpose of this study was to describe the correlation between the activity of the muscles of upper and lower limbs of a golf driver swing according to shoulder injury. Method : The subjects were 18 professional golfers (7 male in KPGA and 11 female in KLPGA). Using surface electromyography, we evaluated muscle activities during the golf drive swing. Surface electrodes were used to record the level of muscle activity in the left Deltoid(MD), Triceps Brachii(TB), Pectoralis(PM), Trapezius(UT), Rectus Femoris(RF), Vastus Medialis Obilique(VMO), Biceps Femoris(BF), Gastrocnemius(GCM) muscles during the golfer's swing. The golf swing was divided into five phases: take away, forward swing, acceleration, early follow through, late follow through. Results : The results can be summarized as follows: MD, VMO had statistically significant difference in take away phases, GCM had statistically significant difference in acceleration phases, MD, PM, VMO had statistically significant difference in early follow through phases, PM had statistically significant difference in late follow through phases, Conclusion : Muscle activity of the 8 muscles(Deltoid, Triceps Brachii, Pectoralis, Trapezius, Rectus Femoris, Vastus Medialis Obilique, Biceps Femoris, Gastrocnemius) along the shoulder damage or absence of has shown that they are organically connected to each of the phases.

  • The pressure pain thresholds of head and neck muscles of patients suffering from tensiontype headache220, all female, ages ranged from 13 to 50 years (28.4$\pm$9.6) and 39 healthy controls, all female, ages ranged from 14 to 46 years (24.4$\pm$9.2) were recorded by the electronic algometer (Electyronic Algometer Type I, Somedic, Stockholm, Sweden). And the obtained results were as follows : 1. The pressure pain thresholds of patient group were lower than those of controls in superior sternocleidomastoid muscle, middle sternocleidomastoid muscle, and trapezius insertion muscle (P<0.001) 2. The pressure pain thresholds of patient group were not different from those of controls in anterior temporal, middle temporal, posterior temporal, deep masseter, anterior masseter, inferior masseter, medial pterygoid, posterior digastric, splenius capitus and upper trapezius muscle (P>0.05). 3. Seventy-one percent of tension-type headache patients had more than one muscle, of whicb pressure pain threshold was lowered significantly (less than mean of control - 1.5SD). 4. The pressure pain thresholds of head and neck muscles should be considered as a criterion for the diagnosis of tension-type headache.

Comparison of Activity of the Muscles around the Shoulders during Push Up and Push Up Plus Exercises under Diverse Stabilization Conditions

  • This study aimed to examine the most effective exercise method for shoulder joint rehabilitation by comparatively observing activity of shoulder stabilizer muscles during push up and push up plus exercises under diverse stable conditions. The subjects were 20 healthy adults students who went to M university. While the subjects conducted push up and push up plus exercises under diverse stable conditions(a stable surface, a support of 25cm height, a support of 30 cm height and a balance pad), activities of the upper trapezius muscle, pectoralis major muscle, serratus anterior muscle, and triceps brachii muscle were recorded. During push up and push up plus exercises, activities of the stabilizer muscles were higher when the stable condition was changed rather than on the stable surface. In particular, when the support of 30cm height and balance pads were applied, activity of the shoulder stabilizer was highest. There were significant differences in the upper trapezius muscle and triceps brachii muscle during the push up exercise(p<.05) and in the serratus anterior muscle during the push up plus exercise(p<.05). Activities of the shoulder stabilizers were higher when the upper and lower limbs' surface stable conditions were changed than the stable surface. Therefore, when programs for rehabilitation of shoulder joints are applied, provision of diverse stable conditions according to patients' conditions will be effective methods.

3가지의 운동프로그램이 작은가슴근의 길이와 머리전방자세에 미치는 영향 (Effects of Three Exercise Programs on the Length of Pectoralis Minor and Forward Head Posture)

  • Purpose: The purpose of this study was to investigate the effects of three exercise programs on forward head posture. Methods: Ninety-five individuals were recruited to assess the of the pectoralis minor muscle. Of these, 36 individuals with pectoralis minor muscles were selected and randomly divided into three groups: a functional massage group, a stretching group, and a stretching and muscle strengthening group. The exercise program consisted of sessions four times per week for two weeks. The length of pectoralis minor and the activity of the trapezius muscles were measured using electromyography. Forward head posture was assessed using the craniovertebral angle (CVA) and the cranial rotation angle (CRA) pre-, during and post-treatment. Result: Significant improvements were observed in pectoralis minor length, CRA, and CVA post-treatment in all groups (p<0.05, p<0.01, p<0.001). However, only the activity of the lower trapezius demonstrated a statistically significant difference post-treatment. There were no significant differences between the groups. Conclusion: The findings of this study suggest that all three exercise programs were effective in improving forward head posture and the length of pectoralis minor post-treatment.

The Effect of Shoulder Stabilization Exercise through Visit Rehabilitation on Muscle Activity and Postural Alignment, Self-Efficacy in Rural Elderly People with Round Shoulders

  • Purpose: This study covered the effect of shoulder stabilization exercise on muscle activity, postural alignment and self-efficacy of the elderly people with round shoulders by visiting village hall in rural area. Methods: 40 elderly people with round shoulders were recruited for this study (experimental group: 20 subjects and control group: 20 subjects). The experimental group was asked to perform shoulder stabilization exercise 60 minutes per one time for 8 weeks and three times in a week and the control group was asked to receive education related to pain management for the initial one time. Before and after the experiment, we measured the subjects' muscle activity of upper trapezius, serratus anterior, infraspinatus, pectoralis major and the lower trapezius, then postural alignment of craniovertebral angle and round shoulder posture. In addition, self-efficacy is measured through the questionnaire. Results: Compared to the control group, the experimental group showed significant differences in the muscle activity of all muscles, posture alignment of the craniovertebral angle. round shoulder posture, and improvement of self-efficacy. Conclusion: We found that shoulder stabilization exercise is effective in muscle activity, postural alignment and self-efficacy of the elderly people through visit-rehabilitation service. This study can be used for improving the physical and mental abilities through active visit-rehabilitation service for areas where have poor medical benefits. It is considered that systematic visit rehabilitation service should have institutional framework ultimately.

독립화 비율과 정량화 비율을 사용한 대각선 패턴의 어깨운동 간의 근활성도 비교 (Comparison of Muscle Activities between the Diagonal Pattern of Shoulder Exercises Using the %Normalization and %Isolation Method)

  • Purpose: The present study aims to investigate the effects of a diagonal exercise pattern on selective activation of the upper extremity muscles using both normalization and isolation methods. Methods: In total, 17 asymptomatic subjects participated in this study. During the two diagonal patterns of exercise (diagonals 1 and 2), muscular activities of the upper trapezius (UT), lower trapezius (LT), serratus anterior (SA), anterior deltoid (AD), and infraspinatus (IS) were measured. The collected data were analyzed in two ways, according to the dominance of muscle activities (%Isolation) and according to normalized activities (%MVIC). Results: There were significant differences in LT, SA, AD, and IS between %MVIC and %Isolation (p<0.05), and the diagonal 1 pattern of exercise showed significantly more LT activities compared with the diagonal 2 pattern (p<0.05). Further, except for LT, there were no significant differences in muscle activities between the diagonal 1 and 2 exercises. Conclusion: The present study suggests that a diagonal pattern of exercise is advantageous for strengthening shoulder muscles, but caution is needed when applying to patients requiring selective strengthening. Regarding both the concentric and eccentric phases of exercise, there was no significant difference in muscular activation, except in LT, between the two diagonal patterns of exercises.

상지의 대각선 운동형태와 수축형태에 따른 근활성도 비교연구 (Comparison Among Shoulder Diagonal Pattern of Exercises Considering Phase of Contraction Based on Muscle Activity)

  • Purpose: The present study investigated how variations in diagonal exercises affect shoulder muscle activity, examining changes in neuromuscular activation during scapular and shoulder diagonal patterns of exercises. Furthermore, we explored whether the exercise phase affects muscle activation. Methods: Sixteen asymptomatic male participants were recruited. Four diagonal pattern exercises (scapular anterior elevation, posterior elevation, flexion-adduction-external rotation, flexion, abduction-external rotation), and two exercise phases (concentric and eccentric) were administered. Surface electromyography data were collected. Results: Upper trapezius activity was significantly higher during the concentric phase of scapular posterior elevation exercises compared with the others (p<0.05). The serratus anterior, anterior deltoid, and infraspinatus activities were significantly higher during shoulder diagonal pattern exercises compared with the scapular diagonal pattern exercises (p<0.05). Except for the lower trapezius, muscular activities during the concentric phase were significantly greater compared with the eccentric phase (p<0.05). Conclusion: The current study suggests that the diagonal pattern of exercise may contribute to selective strengthening of the shoulder complex muscles and that the form of exercise should be tailored to the subject. It also suggests that diagonal exercises with concentric contractions tend to be more beneficial than eccentric contractions for overall muscle recruitment, but the effects vary for specific diagonal patterns.

날개 어깨뼈 대상자들에게 어깨 내밈 운동시 벌림 각도와 외부 무게에 따른 앞톱니근, 큰가슴근, 위 등세모근의 활성도 및 비율에 미치는 영향 (Effect on the Activity and Ratio of the Serratus Anterior, Pectoralis Major, and Upper Trapezius according to the Angle of Abduction and External Weight During Shoulder Protraction Exercise for Winged Scapular Subjects)

  • Background: Winged scapular (WS) causes muscle imbalance with abnormal patterns when moving the arm. In particular, the over-activation of the upper trapezius (UT) and decrease in activity of the lower trapezius (LT) and serratus anterior (SA) produce abnormal scapulohumeral rhythm. Therefore, the SA requires special attention in all shoulder rehabilitation programs. In fact, many previous studies have been devoted to the SA muscle strength training needed for WS correction. Objects: The purpose of this study was to investigate the effect of shoulder girdle muscle and ratio according to the angle of shoulder abduction and external weight in supine position. Methods: Twenty three WS patients participated in this experiment. They performed scapular protraction exercise in supine position with the weights of 0 kg, 1 kg, 1.5 kg, and 2 kg at shoulder abduction angles of $0^{\circ}$, $30^{\circ}$, $60^{\circ}$, and $90^{\circ}$. The angle and weight applications were randomized. Surface electromyography (EMG) was used to collect the EMG data of the SA, pectoralis major (PM), and UT during the exercise. The ratio of PM/SA and UT/SA was confirmed. Two-way repeated analyses of variance were used to determine the statistical significance of SA, PM, and UT and the ratios of PM/SA and UT/SA. Results: There was a significant difference in SA according to angle (p<.05). Significant differences were also identified depending on the angle and weight (p<.05). The angle of abduction at $0^{\circ}$, $30^{\circ}$ and weight of 2 kg showed the highest SA activity. However, there was no significant difference between PM and UT (p>.05). There was a significant difference between PM/SA and UT/SA in ratio of muscle activity according to angle (p<.05). Significant differences were found at PM/SA angles of $30^{\circ}$, $60^{\circ}$ and $90^{\circ}$ (p<.05). For UT/SA, significant difference was only observed at $90^{\circ}$ (p<.05). Conclusion: Based on the results of this study, in order to strengthen the SA, it was found to be most effective to use 1 and 1.5 kg weights with abduction angles of $0^{\circ}$ and $30^{\circ}$ at shoulder protraction in supine position.

Comparison of the Upper and Lower Trapezius and Serratus Anterior during Push-up Plus Exercise with and without an Elastic Shoulder Band

  • PURPOSE: Scapular stability is very important to arm function during activities of daily living. The push-up plus exercise is commonly used to stabilize the shoulder muscles. In the present study, we designed an elastic shoulder band (ESB) that could be used with the push-up plus exercise and studied muscle activities of the serratus anterior (SA) and the lower and the upper trapezius (LT and UT) on stable and unstable support. METHODS: Fifteen healthy people participated in this study. Three muscles were investigated using surface EMG with and without the ESB on stable and unstable support. All subjects performed each trial three times for 7 seconds per set and rested for 30 seconds. Repeated one-way ANOVA was used for statistical analyses. RESULTS: The EMG activity of the SA was significantly different during the push-up plus exercise on stable support with the ESB compared with that on unstable support without the ESB (p<.05). However, there were no differences in the activities of UT and LT on stable and unstable support (p>.05). CONCLUSION: The ESB was effective for the activity of SA during the push-up plus exercise on stable support; therefore, it has the potential for use by people during push-up plus exercises. However, further studies are required to investigate the rehabilitation tools for patients with shoulder instability.
