PET/CT검사에서 $^{18}F$-FDG가 가장 널리 이용되며, 장비의 물리적 특성에 따라 환자 주입 $^{18}F$-FDG량이 다르게 권고되고 있다. 또한, 검사 특성상 방사선종사자와 환자의 접촉으로 인하여 방사선의 피폭이 불가피하기에, 본 연구에서는 각기 다른 PET/CT 장비를 대상으로 환자에게 주입되는 $^{18}F$-FDG가 방사선 종사자에게 미치는 피폭선량과의 관계를 분석하였다. 총 3대의 각각 다른 PET/CT (Scanner 1 (S1) : 0.15 mCi/kg, Scanner 2 (S2) : 0.17 mCi/kg, Scanner 3 (S3) : 0.12 mCi/kg)를 대상으로 각 장비에 숙련도를 고려하여 총 6명의 방사선종사자를 5개월간 순환근무 하였고, 하루에 검사하는 환자수를 일정하게 유지하였다. 또한, 검사 진행 방법을 유사하게 유지하고, 방사선종사자의 개인피폭선량계인 열형광유리선량계(TLD)를 매월 판독 하여 분석하였다. 개인의 월별 평균 피폭선량은 장비에 따라 S1은 0.76 mSv, S2는 0.93 mSv, S3는 0.47 mSv였다. 피폭선량은 개인 최대 1.12 mSv, 최저 0.42 mSv로 숙련도와 경험에 따라 유의한 차이를 보였고, 또한 각 주입량에 따른 PET/CT의 종류에 따라 피폭선량은 유의한 상관관계를 나타냈다. 본 연구를 통하여 주입 $^{18}F$-FDG가 적을수록 방사선종사자의 피폭선량이 낮았다. 또한, 개인숙련도에 따라 피폭선량이 감소하였으나, 장비의 특성에 따라 적은 방사성의약품 주입량의 영향이 방사선종사자의 피폭선량을 현저하게 감소할 수 있기에 이에 대한 연구가 보다 활성화 되어야 할 것이다.
소아 두부 CT 검사 시 방사선 피폭 최소화와 진단적 가치가 높은 영상을 확보하기 위하여 Volume Axial Mode, High Pitch Mode, Helical Mode 간 피폭 선량과 영상의 비교 분석을 통하여 Volume Axial Mode의 유용성을 평가하였다. 1세 미만의 소아 두부 CT Helical Mode, High Pitch Mode, Volume Axial Mode에서 70, 80, 100 kVp의 관전압에 따라 영상평가와 선량평가를 시행하였다. 영상평가는 각 영상에 ROI를 설정해 SNR과 CNR을 산출하여 영상평가를 진행하였으며 일원배치 분산분석(ONE-WAY ANOVA)을 사용하여 통계적 유의성을 평가하였고, 선량평가는 검사 후 장비에서 직접 제공하는 Dose Report에 표시되는 DLP값을 이용하여 비교 분석하였다. Volume Axial Mode를 사용하여 검사하였을 때, DLP 값은 전체적으로 낮게 나왔고, SNR과 CNR은 각각 ROI와 kVp별 차이를 가졌다. Volume Axial Mode가 다른 Scan Mode와 비교하여 영상의 질을 평가하였을 때, 그 차이 값이 균일하지 않아 특정 Mode가 우수하다고 볼 수 없었으나 선량 측면에서는 가장 피폭선량을 줄일 수 있었다. 또한 Volume Axial Mode의 짧은 스캔 시간과 Table의 이동 없이 제자리에서 검사 할 수 있는 점은 방사선 감수성이 높은 1세 미만 소아 CT검사에서 유용하게 적용된다.
본 연구에서는 저선량 흉부 CT 영상을 이용하여 VGGNet을 학습시키고 폐기종 검출 모델을 구현하고 성능을 확인하고자 한다. 연구에 사용된 저선량 흉부 CT 영상은 정상 진단 8000장, 폐기종 진단 3189장이며, 모델 학습을 위해 정상 데이터와 폐기종 데이터를 train, validation, test dataset으로 각각 60%, 24%, 16%로 무작위 추출하여 구분하였다. 학습을 위한 인공신경망은 VGGNet 중 VGG16과 VGG19를 사용하였으며, 학습이 완료된 모델 평가를 위해 정확도, 손실율, 오차 행렬, 정밀도, 재현율, 특이도, F1-score의 평가지표를 사용하였다. 폐기종 검출 정확도와 손실율은 VGG16과 VGG19 각각 92.35%, 95.88%, 0.21%, 0.09%, 정밀도는 91.60%, 96.55%, 재현율은 98.36%, 97.39%, 특이도는 77.08%, 92.72%, F1-score는 94.86%, 96.97%였다. 위의 평가지표를 통해 VGG19 모델의 폐기종 검출 성능이 VGG16 모델에 비해 우수하다고 판단된다. 본 연구를 통해 VGGNet과 인공신경망을 이용한 폐기종 검출 모델 연구에 기초자료로 사용할 수 있을 것으로 사료된다.

Objective: The purpose of this study was to assess whether a deep learning (DL) algorithm could enable simultaneous noise reduction and edge sharpening in low-dose lumbar spine CT. Materials and Methods: This retrospective study included 52 patients (26 male and 26 female; median age, 60.5 years) who had undergone CT-guided lumbar bone biopsy between October 2015 and April 2020. Initial 100-mAs survey images and 50-mAs intraprocedural images were reconstructed by filtered back projection. Denoising was performed using a vendor-agnostic DL model (ClariCT.AITM, ClariPI) for the 50-mAS images, and the 50-mAs, denoised 50-mAs, and 100-mAs CT images were compared. Noise, signal-to-noise ratio (SNR), and edge rise distance (ERD) for image sharpness were measured. The data were summarized as the mean ± standard deviation for these parameters. Two musculoskeletal radiologists assessed the visibility of the normal anatomical structures. Results: Noise was lower in the denoised 50-mAs images (36.38 ± 7.03 Hounsfield unit [HU]) than the 50-mAs (93.33 ± 25.36 HU) and 100-mAs (63.33 ± 16.09 HU) images (p < 0.001). The SNRs for the images in descending order were as follows: denoised 50-mAs (1.46 ± 0.54), 100-mAs (0.99 ± 0.34), and 50-mAs (0.58 ± 0.18) images (p < 0.001). The denoised 50-mAs images had better edge sharpness than the 100-mAs images at the vertebral body (ERD; 0.94 ± 0.2 mm vs. 1.05 ± 0.24 mm, p = 0.036) and the psoas (ERD; 0.42 ± 0.09 mm vs. 0.50 ± 0.12 mm, p = 0.002). The denoised 50-mAs images significantly improved the visualization of the normal anatomical structures (p < 0.001). Conclusion: DL-based reconstruction may enable simultaneous noise reduction and improvement in image quality with the preservation of edge sharpness on low-dose lumbar spine CT. Investigations on further radiation dose reduction and the clinical applicability of this technique are warranted.
The purpose in this study is to investigate CT number difference between conventional CT and CT simulator. It shows good correlation in CT number on the muscle, bone, and air. However, in the liver, lungs and water, the low correlation was detected. This result can become the good index for the direction of the distribution of dose difference research between CT equipment for using the computerized radiation therapy planning system.
Purpose : Fixed way of mAs previously Low Extremity Computed Tomography Angiography(LECTA) examination were used. Automatic Current Selection(ACS) to use for the purpose of reducing the dose when Low Extremity Computed Tomography Angiography examining patients. Materials and methods : Were analyzed from July 2011 to July 2012 MDCT examination of Dose Length Product(DLP) LECTA 116 Case. It was defined as previous inspection methods(Old protocol). CT workstation is set to 100 mAs and 150 mAs protocol based on the patient's weight 70kg examined by LECTA. We defined as 'New protocol' that applies to ACS. The data collection period are 76 cases from October 2012 to January 2013 Results : 1. Average Total DLP of 'Old protocol' is 3602.943 $mGy^*cm$. 2. Average Total DLP of 'New protocol' is 1762.977 $mGy^*cm$. 3. Due to the 'New Protocol' use of Total DLP was reduced by approximately 51 %. Phase-specific dose reduction is as follows. Pre(33.62 %), Artery(64.63 %), Delay(49.0 %) 4. Using One way ANOVA Analysis of fluctuations obtained DLP is as follows. 'Old protocol', 'New protocol' a value of P < 0.001, P = 0.882 values were obtained. Conclusions : Dose reduction of 51 % is a useful study that proves. The results obtained using the ACS, the effects of a dose reduction of 51 % was obtained. Therefore, it has been proven to be a useful way. Statistics using SPSS version came out of the 'Old protocol' P-value P < 0.0001. This result means that the DLP a large difference values. On the other hand, The results of the 'New protocol' was P = 0.882. These results means to that small and regularly was fluctuations of the dose. The use of ACS, you can get a reduction of the dose and will able to get the effect of reducing the dose errors.
Purpose: To evaluate the effects of low-dose intravenous N-acetylcysteine on the prevention of contrast-induced nephropathy (CIN) in patients undergoing computed tomography (CT). Methods: All patients presenting to our emergency department and undergoing CT with intravenous contrast media between August 2014 and April 2016 were retrospectively enrolled. We included hospitalized patients with renal dysfunction [estimated glomerular filtration rate (GFR) between 30 and $89mL/min/1.73m^2$]. A 600-mg injection of N-acetylcysteine was given to patients once before and once immediately after CT, depending on the preference of physician. The primary outcome was CIN defined as an increase in creatinine level of ${\geq}25%$ or ${\geq}0.5mg/dL$ from the baseline within 48 to 72 hours after CT. A trained person blindly reviewed all medical records. Results: Of the 1903 admitted patients, CIN occurred in 9.8% of patients who received 1200 mg intravenous N-acetylcysteine (24/244) and 6.8% of patients who did not (113/1659, p=0.090). In a multivariable regression analysis, N-acetylcystine was not relevant to the prevention of CIN (odds ratio=1.42 [95% CI, 0.90-2.26]). Even in the stratified analysis using the propensity score matching, N-acetylcysteine was irrelevant (GFR 30-59: odds ratio=1.06 [95% CI, 0.43-2.60]; GFR 60-89: odds ratio=1.76 [95% CI, 0.75-4.14]). After adjustment, crystalloids were significantly associated with the reduction in CIN compared with dextrose water (odds ratio=0.60 [95% CI, 0.37-0.97]). Conclusion: No effect was found when low-dose intravenous N-acetylcysteine was used to prevent CIN. However, there seems to be an association between crystalloids and reduction in CIN.
In an ordinary CT scan, a large number of projections with full field-of-view (FFOV) are necessary to reconstruct high resolution images. However, excessive x-ray dosage is a great concern in FFOV scan. Region-of-interest (ROI) CT or sparse-view CT is considered to be a solution to reduce x-ray dosage in CT scanning, but it suffers from bright-band artifacts or streak artifacts giving contrast anomaly in the reconstructed image. In this study, we propose an image reconstruction method to eliminate the bright-band artifacts and the streak artifacts simultaneously. In addition to the ROI scan for the interior projection data with relatively high sampling rate in the view direction, we get sparse-view exterior projection data with much lower sampling rate. Then, we reconstruct images by solving a constrained total variation (TV) minimization problem for the interior projection data, which is assisted by the exterior projection data in the compressed sensing (CS) framework. For the interior image reconstruction assisted by the exterior projection data, we implemented the proposed method which enforces dual data fidelity terms and a TV term. The proposed method has effectively suppressed the bright-band artifacts around the ROI boundary and the streak artifacts in the ROI image. We expect the proposed method can be used for low-dose CT scans based on limited x-ray exposure to a small ROI in the human body.
분진에 노출된 집단의 흉부 디지털영상에서 진폐 소견에 따라 흉부 저선량 CT영상에서 발견된 소견을 비교 분석하였다. 2007년 4월부터 2008년 8월 사이 일개 진폐정밀건강진단기관에 내원한 진폐정밀건강진단 대상자 남자 328명에 대해 저선량 CT촬영을 실시하였고, 진폐 판독경험이 많은 1명의 영상의학과 전문의가 판독하였다. 흉부 디지털영상의 진폐 소견은 국제노동기구 흉부 영상 분류법(ILO, 2000)따라 아날로그 표준 사진을 참조하여 2명의 영상의학과 전문의가 합의 판독한 결과로부터 얻었고, 소음영의 병형에 따라 진폐 소견이 없는군(87명, 26.5%)과 있는군(241명, 73.5%)으로 분류하였다. 진폐 소견이 있는군에서 나이(60.9 vs. 65.0, p<0.001)와 분진노출기간(17.0 vs. 19.2, p=0.024)이 통계학적으로 유의하게 높았지만, 흡연력은 차이가 없었다. 전체 대상자 328명 중 245명(74.7%)이 저선량 CT영상에서 13 종류의 소견이 발견되었다. 진폐 소견이 없는군 보다 있는군에서 관상동맥 석회화 소견이 통계학적으로 의미 있게 높은 빈도를 보였고(25.3% vs. 36.9%, p=0.049), 벌집모양 소견도 높은 빈도를 보였다(1.2% vs. 6.2%, p=0.079). 하지만, 저선량 CT영상에서 진폐 소음영을 시사하는 작은 결절은 진폐 소견이 없는 군에서 더 높은 빈도를 보여 흉부 디지털영상이 민감도가 낮았다. 이상의 결과에서 분진 노출에 의한 진폐 소견이 저선량 CT영상의 관상동맥 석회화 소견과 유의한 관련성을 보였지만, 독립적인 위험요인임을 규명하기 위해서는 관상동맥 혈관 조영술을 이용한 전향적인 추가 연구가 필요할 것으로 사료된다.
간 역동적 CT(Liver Dynamic Computed Tomography; LDCT) 검사에서 일반적으로 사용하는 프로토콜과 관전압을 낮게 설정한 후 프로토콜을 변화시켰을 때 방사선량과 영상의 질을 비교하여 영상의 질을 유지하면서 방사선량을 감소시킬 수 있는 방안을 알아보고자 하였다. LDCT를 시행한 환자 중 신체질량지수(body mass index; BMI)가 18.5~24인 환자 40명을 대상으로 일반적인 복부 CT 검사 프로토콜을 적용한 A그룹 20명(관전압: 120 kVp, SAFIRE strength1)과 관전압을 낮게 설정한 B그룹 20명(관전압: 100 kVp, SAFIRE strength 0~5 적용)이었다. 영상의 질 평가는 동맥기의 간 실질 조직, 대동맥, 상장간막동맥, 복강동맥, 내장지방 그리고 백그라운드에 관심영역(region of interest; ROI)을 설정해 잡음(noise), 신호대 잡음비(signal to noise ratio; SNR), 대조도 대 잡음비(contrast to noise ratio; CNR), CT number를 측정 비교하였다. 또한 정성적 평가는 경험이 풍부한 영상의학과 전문의 2명이 0~3점까지로 평가하였다. 방사선량은 총 DLP(dose length product)와 유효선량, CTDIvol(volume computed tomography dose index)을 비교하였다. 관전압 100 kVp에서 SAFIRE가 높을수록 잡음은 감소하고, CT number는 증가하였다. 따라서 SNR과 CNR은 SAFIRE 단계가 높을수록 증가하였다. 관전압 120 kVp와 비교하여 잡음, SNR, CNR이 SAFIRE strength 2, 3에서 가장 유사하였다. 정성적 평가는 SAFIRE strength 2가 가장 많았고 관전압이 100 kVp일 때 영상의 질이 더 좋다고 평가한 경우는 SAFIRE 1이었다. 방사선량은 120 kVp에 비해 100 kVp에서 21.69% 감소하였다. BMI가 비교적 높지 않은 LDCT 검사의 경우 공장에서 출고될 당시에 관전압이 높게 설정되어 있어 불필요한 방사선피폭이 우려되고 있는 현실을 고려하면, 본 연구 결과에 따라 관전압을 낮게 설정하고 SAFIRE strength를 2로 조정하면 영상의 질 저하 없이 방사선량도 감소시킬 수 있을 것으로 사료된다.

제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
