A Review on Clinical Research Trend in Treatment of Chai-Hu-Shu-Gan-San (Sihosogan-San) for Insomnia (불면증에 대한 시호소간산 치료의 임상연구 동향)

  • Objectives: This review aimed to investigate the effectiveness of Chai-Hu-Shu-Gan-San (CHSGS) for treatment of insomnia. Methods: 9 databases were comprehensively searched from their inception dates until Jan 2020 using keywords like Insomnia, Chai-Hu-Shu-Gan-San and Sihosogan-san. We included randomized controlled trials (RCTs) using CHSGS as the main intervention for insomnia patients. We assessed the quality of RCTs using the Cochrane risk of bias tool and Jadad scale. Results: 5 RCTs were included. The Chinese classification of mental disorders -3 (CCMD-3) was the most frequently used diagnostic criteria. The effective rate was the most frequently used as outcome. All the 5 RCTs that reported CHSGS and CHSGS adjunctive therapy on insomnia were more effective compared to western medication. Only one study reported adverse effects. The methodological quality of the included RCTs were generally poor. Conclusions: This review found that CHSGS may be effective to treating insomnia. However, because of the small number, heterogeneity and very low methodological quality of included studies, we could not draw any definitive conclusions. Therefore, more rigorous randomized control studies are warranted.

  • Consequence-Based Engineering (CBE) is a new paradigm proposed by the Mid-America Earthquake Center (MAE) to guide evaluation and rehabilitation of building structures and networks in areas of low probability - high consequence earthquakes such as the central region of the U.S. The principal objective of CBE is to minimize consequences by prescribing appropriate intervention procedures for a broad range of structures and systems, in consultation with key decision makers. One possible intervention option for rehabilitating unreinforced masonry (URM) buildings, widely used for essential facilities in Mid-America, is passive energy dissipation (PED). After the CBE process is described, its application in the rehabilitation of vulnerable URM building construction in Mid-America is illustrated through the use of PED devices attached to flexible timber floor diaphragms. It is shown that PED's can be applied to URM buildings in situations where floor diaphragm flexibility can be controlled to reduce both out-of-plane and in-plane wall responses and damage. Reductions as high as 48% in roof displacement and acceleration can be achieved as demonstrated in studies reported below.

Renal Cell Carcinoma is More Aggressive in Turkish Patients with the Metabolic Syndrome

  • Background: Metabolic syndrome (MetS) is a multifactorial disease characterized by impaired glucose tolerance/diabetes, obesity, high triglyceride levels, low HDL levels, and hypertension. In this study we evaluate the relationship between tumor size and grade, and presence of the metabolic syndrome in patients with renal cell carcinoma. Materials and Methods: Between 2007-2013, radical nephrectomy was performed for 310 patients with renal tumors in our clinic and those with pathology reported renal cell carcinoma were enrolled and divided into two groups, with and without metabolic syndrome diagnosed on the basis of the National Cholesterol Education Program (NCEP) Adult Treatment Panel III (ATP III) criteria. The relationship between tumor size and grade of the two groups (Fuhrman nuclear degree) was evaluated statistically. Results: The metabolic syndrome was found in 70 patients, with a mean age of 65.5 (40-87), as compared to 58.8 (31-84) years in the non-metabolic syndrome group. Tumor size over 7 cm was found in 54% and 33%, respectively, and tumor grade over Fuhrman 3 in 56% and 32% of patients. Patients with metabolic syndrome had significantly higher tumor size and grade (p<0.05). In the presence of hypertension, diabetes and high triglyceride levels, significant assocations were again observed (p<0.05). Tumor size and degree also increased with increasing body mass index but this was not statistically significant (p>0.05). Conclusions: Renal cancer is more aggressive in patients with metabolic syndrome. Lifestyle and risk factors were revealed to be significant influences in renal cancer patients.

Association between household food insecurity and nutritional outcomes among children in Northeastern of Peninsular Malaysia

  • BACKGROUND/OBJECTIVES: The purpose of the study was to investigate the relationship between household food insecurity and nutritional status of children in low-income households. A cross sectional study involved a survey of households (n = 223) receiving the financial assistance. SUBJECTS/METHODS: Eligible mothers that fulfilled the inclusion criteria such as non-pregnant, non-lactating mothers, aged 18 to 55 years with their youngest children aged 2 to 12 years, were purposively selected. The Radimer/Cornell hunger and food-insecurity instrument was administered and children's height and weight were measured. RESULTS: About 16.1% of the households were food secure, while 83.9% experienced some kind of food insecurity. Out of food insecure category, 29.6% households were food insecure, 19.3% women were individual food insecure and 35.0% fell into the child hunger category. Education of the mother (P = 0.047), household size (P = 0.024), number of children (P = 0.024), number of children going to school (P = 0.048), total monthly income (P < 0.001), income per capital (P < 0.001), number of household members contributing to the income (P = 0.018) and food expenditure (P = 0.006) were significant risk factors for household food insecurity. The prevalence of underweight, stunting and wasting in children were 61.0%, 61.4% and 30.6% respectively. Based on multinomial logistic regression, children in food-insecure households were 2.15 times more likely to be underweight and three times to be stunted than children in the food-secure households. CONCLUSIONS: The findings suggest that household food insecurity is associated with the nutritional status of the children in the rural area of Northeastern Peninsular Malaysia.

Emission Estimation and Exposure to Hazardous Gaseous Pollutants Associated with Use of Air Fresheners Indoors (실내 방향제 사용에 의한 유해 가스상 오염물질 배출 산정 및 노출 평가)

  • This study quantitatively investigated the emissions of indoor air pollutants associated with the utilization of air fresheners indoors, and evaluated individual exposure to five specified indoor air pollutants, which were chosen on the basis of selection criteria. An electrically-polished stainless steel chamber (50L) was employed to achieve this purpose. Test air fresheners were selected through three steps: first, on the basis of market sales; second, on the basis on a preliminary head-space study; and lastly, on the basis of emissions of toxic compounds (benzene, ethyl benzene, limonene, toluene, and xylene). The empirical mathematical model fitted well with the time-series concentrations in the environmental chamber (in most cases, determination coefficient, $R^2{\gtrsim}$0.9), thereby suggesting that the empirical model was suitable for testing emissions. The concentration equilibrium appeared 180 min after the introduction of sample air fresheners into the chamber. Both the chamber concentrations of emission rates or factors varied greatly according to air freshener type. It is noteworthy that although benzene, ethyl benzene, toluene, and xylene were emitted from all test air fresheners, their exposure levels were not significant enough to result in any significant health risk. However, certain type of air fresheners were observed to emit significant amount of limonene, which is potentially reactive with ozone to generate secondary pollutants with oxidants such as ozone, hydroxyl radicals, and nitrogen oxides. The exposure levels to limonene associated with the utilization of three air fresheners were estimated to be 13 to 175 times higher than that of other air fresheners. This information can help consumers to select low-pollutant-emitting air fresheners.

Is catechol-o-methyltransferase gene polymorphism a risk factor in the development of premenstrual syndrome?

  • Objective: The objective of this study was to investigate whether there was a correlation between catechol-o-methyltransferase (COMT) gene polymorphism, which is believed to play a role in the etiology of psychotic disorders, and premenstrual syndrome (PMS). Methods: Fifty-three women with regular menstrual cycles, aged between 18 and 46 years and diagnosed with PMS according to the American Congress of Obstetrics and Gynecology criteria were included in this study as the study group, and 53 healthy women having no health problems were selected as the controls. Venous blood was collected from all patients included in the study and kept at $-18^{\circ}C$ prior to analysis. Results: There was no significant difference between the groups in terms of demographic features such as age, body mass index, number of pregnancies, parity, and number of children. No statistically significant difference was observed in terms of COMT gene polymorphism (p=0.61) between women in the PMS and the control groups. However, a significant difference was found between arthralgia, which is an indicator of PMS, and low-enzyme activity COMT gene (Met/Met) polymorphism (p=0.04). Conclusion: These results suggested that there was no significant relationship between PMS and COMT gene polymorphism. Since we could not find a direct correlation between the COMT gene polymorphism and PMS, further studies including alternative neurotransmitter pathways are needed to find an effective treatment for this disease.

Frequency of Chromosomal Abnormalities in Pakistani Adults with Acute Lymphoblastic Leukemia

  • Background: The difference in prognosis of adult and childhood acute lymphoblastic leukemia (ALL) can be attributed largely to variation in cytogenetic abnormalities with age groups. Cytogenetic analysis in acute leukemia is now routinely used to assist patient management, particularly in terms of diagnosis, disease monitoring, prognosis and risk stratification. Knowing about cytogenetic profile at the time of diagnosis is important in order to take critical decisions in management of the patients. Aim and Objectives: To determine the frequency of cytogenetic abnormalities in Pakistani adult patients with ALL in order to have insights regarding behavior of the disease. Materials and Methods: A retrospective analysis of all the cases of ALL (${\geq}15$years old) diagnosed at Aga Khan University from January 2006 to June 2014 was performed. Phenotype (B/T lineage) was confirmed in all cases by flow cytometry. Cytogenetic analysis was made for all cases using the trypsin-Giemsa banding technique. Karyotypes were interpreted using the International System for Human Cytogenetic Nomenclature (ISCN) criteria. Results: A total of 166 patients were diagnosed as ALL during the study period, of which 151 samples successfully yielded metaphase chromosomes. The male to female ratio was 3.4:1. The majority (n=120, 72.3%) had a B-cell phenotype. A normal karyotype was present in 51% (n=77) of the cases whereas 49% (n=74) had an abnormal karyotype. Of the abnormal cases, 10% showed Philadelphia chromosome; t(9;22)(q34;q11.2). Other poor prognostic cytogenetic subgroups were t(4;11)(q21;q23), hypodiploidy (35-45 chromosomes) and complex karyotype. Hyperdiploidy (47-57 chromosomes) occurred in 6.6%; all of whom were younger than 30 years. Conclusions: This study showed a relatively low prevalence of Philadelphia chromosome in Pakistani adults with ALL with an increase in frequency with age (p=0.003). The cumulative prevalence of Philadelphianegative poor cytogenetic aberrations in different age groups was not significant (p=0.6).

Review of Moxibustion Treatment for Hypertension in Clinical Trials (임상연구에서 나타난 고혈압 뜸치료법에 대한 고찰)

  • Objectives : The aim of this study is to review clinical trials using moxibustion on hypertension and to assess their methodology and results. Methods : Electronic literature searches for clinical trials (randomized trial, non-randomized trial, before-after study) of moxibustion were performed in nine electronic databases (four international databases and five Korean databases) and handsearch. English, Korean or Chinese articles were included. Laboratory or animal studies were excluded. Results : A total of twelve studies met the inclusion criteria. Seven randomized controlled trial, three non-randomized trials and two before-after studies were included. Seven studies used direct moxibustion(two are scarring one) and five used indirect one. Five of twelve studies used moxibustion treatment just one time, except for that most frequency was once daily or 2-3 times per week for 1-2 months. Of ten randomized or non-randomized studies, three used antihypertension drug control and another three used waiting list control. Compare to baseline, change of blood pressure after moxibustion treatment was significant in all studies. However, the results of effect in blood pressure between moxibustion and controlled were not consistent. Methodological quality of clinical trials included in this review was low and has risk of bias, especially in blinding of parcitipant. Conclusions : There are little high-quality clinical trials of moxibustion for hypertension. To evaluate the effect of moxibustion, more rigorous trials are warranted. Also, methodology of clinical trials have to be descripted in detail.

Daoyin Exercise Therapy for Stroke : A Systematic Review (뇌졸중에 대한 도인 운동요법의 효과: 체계적 문헌 고찰)

  • Objectives : This study aimed to review the effects of Daoyin exercise on stroke patients in clinical studies. Methods : In this study, we reviewed 11 electronic databases (CAJ, EMBASE, Cochrane Library, Web of Science, Pubmed, RISS, Dbpia, NDSL, KISTI, KISS, KMBASE) on October 27, 2019. We included only randomized controlled trials (RCTs) which evaluated the effect of Daoyin exercise on stroke patients. The methodological quality of the included studies was checked using the Cochrane risk of bias tool. Results : After screening the papers, eleven RCTs fulfilled the inclusion criteria. The results of the meta-analysis showed that Daoyin exercise improves the measurement value on the Fugl-Meyer Assessment, Modified Barthel Index and National Institutes of Health Stroke Scale in stroke patients. Conclusions : We concluded that Daoyin exercise has therapeutic effects in functional recovery and in enhancing the independence of daily living activities for stroke patients. However, the quality of the original articles was low and the number of papers included were few. Thus, to confirm these results, further well-designed RCTs should be conducted.

  • Information system development projects, have a mechanism for many of the costs generated by a variety of risk factors. In general, the probability that the software project of the information system is carried out successfully in the delivery time is very low. This prediction of a formal cost is needed as the most important factor since it can prevent the project from being failed. However, objectivity of most of the project scale calculation during the calculation criteria is insufficient. Further, it is the actual situation that the management of the base line is not properly made during the project. Therefore, in this paper, we propose a model to calculate the number of steps it takes to develop on the basis of a methodology in an attempt to overcome the limitation of being unpractical in the early stage of the information system development project. It is expected to be a tool to estimate the effort and cost required by the information system development business through these convergence proposals model.
