Effects of Coordinative Locomotor Training Program on Low Extremity Strength, Balance and Quality of Life in Patients with Cancer: Single-Subject Design (CLT프로그램이 암 환자의 하지 근력, 균형 및 삶의 질에 미치는 영향: 단일사례설계)

  • PURPOSE: This study was to identify the effect of a coordinative locomotor training (CLT) programs on lower extremity strength, balance, and quality of life of a cancer patients. METHODS: Subjects were instructed in CLT programs. The CLT programs consisted of sprinter and skater pattern movements performed for 30 minute three day per week for two weeks. CLT programs performed the arms and legs alternately and step up the steps on the stairs. The timed sit to stand test (TSTS) and one leg standing test (OLST) were used to test lower extremity strength. The star excursion balance test (SEBT) was used to measure balance; and the functional assessment of cancer therapy - general (FACT-G) for measuring the quality of life throughout the sessions. RESULTS: Participants showed enhanced strength compared to first baseline, as measured by TSTS (14.58%). Balance was improved compared to the first baseline as measure by OLST with open eyes (30.3%), OLST with closed eyes (114.81%), SEBT (43.76%). The increase of strength and balance were maintained in a second baseline session. There was no change for quality of life between first baseline and intervention period. CONCLUSION: These findings indicate that the CLT program is a useful exercise program that can help improve the physical activity of cancer patients who have a reluctance to move by positively affecting cancer patients' balance and leg strength.

Comparison of Lower-Limbs Muscle Activity according to the Abdominal Co-contractive Activation (복부 동시수축 형태에 따른 하지 근활성도 비교)

  • The purpose of this study was to investigated the muscle activity difference of lower extremity by abdominal co-contractive activation. 30 healthy women without pain in the low back and lower extremity were conducted. Muscle activities of lower extremity were measured with the active straight leg raise (ASLR) in 20cm raised from the floor without bending the knee at rest, abdominal drawing-in maneuver (ADIM) and abdominal bracing maneuver (ABM), respectively. Hip adductor longus (HadL), rectus femoris (RF), and biceps femoris (BF) muscle activity in the Abdominal bracing maneuver were statistically significantly higher, also gluteus medius (Gmed) and tibialis anterior (TA) muscle activity in the ADIM were higher. We provide the evidence to the positive effects of individual abdominal co-contractive activation training which can enhance postural stability and lower extremity strengthening with volitional preemptive abdominal contraction.

The Effect of Rehabilitation Training Programs on the Kinetic and Kinematic Parameters During Sit-To-Stand in Chronic Stroke Patients (만성편마비 환자의 재활 운동 유형이 일어서기 동작의 운동학 및 운동역학적 변인에 미치는 영향)

  • The purpose of this study was to analyze the effect of different types of rehabilitation training program on the kinetic and kinematic parameters during sit-to-stand movement(STS) in chronic stroke patients. Two groups of hemiparetic patients, experimental and control, participated in the study. The experimental group participated in a 10-week training program (three sessions/wk, $1{\sim}1.5\;hr/session$) consisting of a warm-up, aerobic exercises, lower extremity strengthening. and a cool-down. The control group participated in an aerobic exercise. Three dimensional kinematic analysis and force platform; were used to analyze the duration of STS, lower extremity angle, and weight bearing ability. The experimental group which had more strength of lower extremity displayed decrease in duration of STS. However, the control group showed increases in duration during sit-to-stand movement. The control group flexed their trunk more than the group did Therefore, it took more time to extend their trunk during STS. The duration in sit-to-stand was affected by the strength of lower extremity and the angle of trunk movement. The angles of ankle and knee joint had an influenced on duration of STS. The post experimental group performed with their feet near the front leg of the chair during sit-to-stand, therefore the duration was decreased. The repetitive sit-to-stand movements as a resistance exercise was effective to hemiparetic patients in learning mechanism of sit-to-stand. The control group showed decreased differences in the vertical ground reaction forces between paretic and non-paretic limbs. Their training program included strengthening exercise that may help improving weight bearing ability. The control group showed increases in the center of pressure in the anteroposterior and mediolateral displacement. This means that the stability of movement was low in the control group. Their training program which combined aerobic and strengthening exercises that are more effective to improve the stability of movement.

  • 하지전신계측검사(low extremity teleography)에서 자세 변화에 따른 중요장기의 방사선량을 측정하고 영상을 비교 분석하여 검사방법에 따른 유용성을 알아보고자 하였다. 대상은 하지전신계측검사를 시행한 성인남자 10명을 대상하였고 촬영조건은 관전압 73 kVp, 관전류량 32 mAs, SID 180 cm로 설정하였다. 방사선량 측정은 란도 팬텀을 이용하여 수정체, 갑상선, 생식선 부위에 유리선량계(ion chamber)를 부착한 후 전후방향자세와 후전방향자세를 각각 5번씩 시행하여 부위별로 방사선량을 측정한 후 Paired T-test로 비교 분석 하였다. 영상평가는 전후방향자세와 후전방향자세를 시행한 영상을 blind test를 실시하여 5점 척도로 평가하였다. 결과적으로 전후방향자세검사에 비해 후전방향자세검사가 수정체 약 6%, 갑상선 약 6%, 생식선에 미치는 방사선량을 약 27% 감소시킬 수 있으며 영상평가에서도 두 그룹 간에 큰 차이가 없어, 하지전신계측검사에서 전후방향자세검사보다 후전방향자세검사가 유용할 것으로 사료된다.

Availability of the Pendulum Test Using NK Table for Spasticity Measurement of Low Extremity (하지 강직 평가에 있어 NK테이블을 이용한 진자검사의 유용성)

  • PURPOSE: The purpose of this study was to investigate the clinical availability of the pendulum test (through reliability and validity) using a NK table attached electrogoniometer for spasticity measurement in patients with brain lesions. METHODS: Thirty-one stroke and traumatic brain injury subjects participated in the study. Intraclass correlation coefficient (ICC) was used to verify the test-retest reliability of spasticity measures of the pendulum test. Pearson's product correlation coefficient was used to examine the validity of the pendulum test through the amplitude of the deep tendon reflex (DTR) test known for objective and quantitative measure of spasticity. RESULTS: In these results, the test-retest reliability was showed significantly high correlation between pendulum tests (ICCs=.95~.97, p<.01). There were significant negative correlations between the amplitude of the DTR test and all measures of spasticity of the pendulum test(r=-.77~-.85, p<.01). CONCLUSION: Thus, it is possible to use the pendulum test using a NK table as an objective measure of spasticity, rather than other expensive equipment, which is more complicated to use. Further studies are needed to explore the therapeutic effects of spasticity using a newly designed pendulum test equipment in this study.

Unilateral Horner's Syndrome and Upper Extremity Paralysis following Lumbar Epidural Block in a Obstetric Patient (산모에서 요부 경막외 차단후 발생한 편측 호너 증후군과 상지마비 -증례 보고-)

  • Horner's syndrome is a well-recognized complication of regional analgesia of neck and shoulder region, and not often a complication of lumbar or low thoracic epidural block. Recently we experienced right Horner's syndrome accompanying paralysis of right upper extremity following lumbar epidural block in for an obstetric patient. Epidurography and MRI was performed to clarify the cause of unilateral high epidural block and cervical sympathetic block. Radiologic study demonstrated a loop formation of the epidural catheter and tip of catheter was located in right anterior epidural spaced(L1-2). The initial epidurogram revealed unilateral spreading of dye in the cervical region in right epidural space. A second epidurogram, 10 minutes following, showed dye filling in left epidural space, however spread of dye in left side was limited to lumbar and low thoracic region. We concluded the most probable cause of this unilateral high epidural block was due to misplacement of the catheter into the anterior epidural space.

A study on the upper body type and size of men aged 30-44 for men jacket pattern design (남성 재킷 패턴 설계를 위한 30-44세 남성의 상반신 체형 및 유형별 사이즈 연구)

  • This study aimed to analyze adult men's body sizes and shapes and suggest size specifications to provide preliminary data to academia and industries. A total of 814 adult men aged 30-44 were selected from the 7th Size Korea data, and 55 direct upper body measurement and calculation items were analyzed using SPSS 25.0. In individual Individual differences, thickness, circumference, and width were high, and height and length were low. Height above the waist base line and shoulder dimension decreased in early 40s age group, while height below the waist base line declined as age increased. In addition, buttocks shape changes were found in early 40s age group. According to factor analysis, 'upper body and upper-extremity horizontal size', 'torso height and upper extremity length', 'shoulder dimension', 'upper body length' and 'shoulder angle' were derived. Using clustering analysis, four different body types were classified: i) big abdomen with flat chest, ii) slender with big, raised shoulders, iii) dwarfish with small, droopy shoulders, and iv) obese with large shoulders. 'Slender with big, raised shoulders' was a typical body shape among men aged 30-44. In older participants, the 'big abdomen with flat chest' ratio was low, while 'obese with large shoulders' was more common. This study proposed size specifications by body type considering the above characteristics.

  • This study is an ergonomic study on the function of sleeves related with the expansion and contraction of the upper Extremity skin surface by various movememts. RESULTS : 1. According to the plane figure, a. The change of form is like fig. 3. b. In the changing rates of the expansion and contraction of skin surface by various movements, that of arm hole girth shows an extremely big discrepancy and that of Elbow Girth shows a relative low one. c. According to the rate of the expansion and contraction of each block, the inside of the upper arm area expands most in all the blocks measured. 2. According to the rate of expansion and contraction of skin surface by somatometry, inside lehgth of arm in M\sub 2\ and outside length of the upper arm in M\sub 4\ expand significantly8and also elbow girth in M\sub 6\, M\sub 7\, M\sub 8\ expands significantly.

Relationship of Physical Impairment, function and Insulin Resistance in stroke patients

  • Insulin resistance is a risk factor for stroke or recurrent stroke. Sedentary behavior increases insulin resistance. This study aimed to identify the relationship between physical impairments and functions and insulin resistance, examining which physical impairments specifically influence insulin resistance the most. The subjects of this study were 63 stroke patients. The subject's insulin resistance and physical impairments and functions were measured using the Chedoke-McMaster Stroke Assessment (CMSA) and Stroke Impairment Assessment Set (SIAS). The study results exhibited that insulin resistance is statistically significantly related to the variable of foot according to the CMSA(r=.95, p<.05) and to the variable of lower extremity sensory function (touch) in relation to the SIAS(r=.91, p<.05). This study also revealed close correlations between insulin resistance and the variables of ankle control(${\beta}=-1.05$, p<.05) and low extremity tactile sensations(${\beta}=-1.82$, p<.05).

Investigation of High-Sensitivity C-reactive Protein and Erythrocyte Sedimentation Rate in Low Back Pain Patients

  • Background: Chronic low back pain can be a manifestation of lumbar degenerative disease, herniation of intervertebral discs, arthritis, or lumbar stenosis. When nerve roots are compromised, low back pain, with or without lower extremity involvement, may occur. Local inflammatory processes play an important role in patients with acute lumbosciatic pain. The purpose of this study was to assess the value of erythrocyte sedimentation rate (ESR) and high sensitivity C-reactive protein (hsCRP) measurements in patients with chronic low back pain or radiculopathy. Methods: ESR and hsCRP were measured in 273 blood samples from male and female subjects with low back pain and/or radiculopathy due to herniated lumbar disc, spinal stenosis, facet syndrome, and other diseases. The hsCRP and ESR were measured prior to lumbar epidural steroid injection. Results: The mean ESR was 18.8 mm/h and mean hsCRP was 1.1 mg/L. ESR had a correlation with age. Conclusions: A significant systemic inflammatory reaction did not appear to arise in patients with chronic low back pain.
