의사결정나무기법을 활용한 장기요양 복지용구 권고모형 개발 (A recommendation system for assisting devices in long-term care insurance)

  • 노인의 신체기능에 부합하는 복지용구를 제공하는 것은 노인이 가능한 한 오랫동안 자신의 집과 지역사회에서 자립하여 생활할 수 있도록 돕기 위해 매우 중요하다. 본 연구는 수급자의 신체 및 인지 기능 상태를 고려하여 개개인에게 적합한 복지용구 품목을 권고할 수 있는 과학적인 복지용구 표준급여모형 알고리즘을 개발하고자 수행되었다. 모형개발에는 데이터마이닝기법인 의사결정나무를 활용하였다. 수급자 8,084명의 장기요양인정조사자료와 파워어세서가 작성한 표준급여계획, 수급자 특성 자료를 이용하여 데이터를 구축하였고, 15개 복지용구 품목별로 표준급여모형을 개발하였다. 본 연구는 노인장기요양보험의 복지용구 급여계획의 객관성 및 과학성을 확보하고 수급자의 자립생활과 안전을 향상시키는 데에 기여할 것으로 기대된다.

조건부가치측정법을 이용한 노인장기요양보험에 대한 지불의사금액 추정 (Estimation of Willingness to Pay for Long-Term Care Insurance Using the Contingent Valuation Method)

  • According to rapid increase of the population of senior citizens, there has been growing concern of Long-Tenn Care(LTC) services recently. Long-Tenn Care services, however, haven't been established systematically in Korea and the supply of LTC services is not sufficient despite the increase in the current social demand. This study aims to estimate the 'Willingness to Pay(WTP)' for LTC insurance which the government plans to introduce by means of social insurance, using Contingent Valuation Method(CYM). In addition, this study analyzes the factors affecting WTP for LTC insurance. An interview survey was carried out to derive WTP for LTC from 450 people who lived in Seoul aged 20 and above during the period from 16th to 21st of June 2003. Double-Bounded Dichotomous Choice Method was applied among several CVMs available to estimate both use value and no-use value of goods. There was pilot survey carried out prior to the main survey. The results show that the average monthly. WTP for LTC provided in home and residential setting is 18,192Won and 19,293Won, respectively. In the case of home care, WTP goes higher depending on reliability of LTC insurance policy and need for LTC insurance, as well as marital status, education and average monthly income. On the contrary, WTP is conversely affected by higher age and higher bids. In the case of institutional care, the factors affecting WTP are similar to those of home care, except age. This study followed NOAA's suggestions generally and the value derived through survey could be reliable. However, there can be the least bias in the process of survey because the CVM should be used under the supposed circumstances. Despite those limitations, it can be concluded that the amount the citizens are willing to pay for LTC is high enough to meet the costs needed to provide LTC.

  • Korea currently provides long-term care benefits for the elderly with poor functionality, but most of the service providers are private businesses. This is the time when quality management of care services is required, which is just around the corner of the super-aged era. In this study, we would like to look at the case in which 'A company', which operates a long-term care institution, attempted to make voluntary changes ahead of social demands. The company tried to transform the social needs of quality management by judging them as opportunities, not threats, and establishing an integrated database of centers. First, the company processed data and built a cloud-based database system. Second, the company automatically linked data from existing systems for the efficiency of data utilization. Third, the company pursued visualization for the convenience of data utilization. This allowed the company to make data-driven strategic decisions internally. This is expected to increase sales as it will soon lead to securing new customers and pioneering new markets. It is also significant in that it can provide best practices for the long-term care industry.

노인장기요양보험 재가서비스 이용자의 시설서비스 이용 결정요인 (A Study of Determinants on Institutionalization of Elderly using Home Care Services)

  • 다른 사람의 도움을 받아야 하는 노인도 적절한 재가서비스를 받으면서 가능한 오래 가정에 머물 수 있다면 그들의 삶의 질을 향상시키고 비용을 절감할 수 있다고 알려져 있다. 이 연구는 노인장기요양보험제도를 시행한 후, 재가서비스 이용자가 시설서비스로 전환하여 입소하는 것에 대한 결정요인을 살펴보고자 한 종적 연구이다. 이 연구는 장기요양급여 이용자와 주수발자를 대상으로 한 '2009년 노인장기요양서비스 만족도 조사' 자료를 활용하였다. 이 연구의 분석대상자는 총 1,230명으로, 이들은 시도별, 장기요양등급별, 의료보장형태별로 비례할당으로 추출된 것이다. 종속변수는 연구대상자가 재가서비스를 이용하다가 시설로 입소하는 데까지 걸린 시간이고, 이것은 노인장기요양급여DB를 연계하여 파악하였다. 또한 이 연구는 Cox Proportional Hazard Model을 이용하여 hazard ratio를 산출하였다. 연구결과에 의하면, 다른 변수들을 보정한 후에도 이용자가 골절을 앓고 있는 경우에는 시설에 입소할 확률이 유의하게 높은 것으로 나타났다. 또한 통계적으로 유의하지는 않았지만 저하된 인지기능의 항목수가 많을수록 시설에 입소할 확률이 높은 것으로 나타났다. 이러한 영향요인들을 감안하여 향후에는 이용자의 욕구를 충족시키는 재가서비스 이용체계를 구축하고 보완하는 정책개발이 필요하다.

  • 노인장기요양보험에서 가장 중요한 이슈는 급여대상자의 희망, 건강 및 기능상태에 따라 어떤 급여를 제공할 것인가 이다. 이를 해결하고자 노인장기요양보험의 보험자인 국민건강보험 공단은 급여대상자에게 '표준장기요양이용계획서'를 제공하고 있다. 본 연구에서는 표준장기요양이용계획 작성의 효율화 방안을 마련하고자 노인장기요양보험 3차 시범사업 표준이용계획 자료를 활용하여 노인장기요양급여 권고모형을 개발하였다. 모형개발에는 데이터마이닝의 의사결정나무모형, 로지스틱회귀모형, 앙상블 모형의 배깅과 부스팅 기법을 사용하였고, 이 중 실무자가 이해하기 쉬운 의사결정나무를 채택하여 권고모형을 설명 하였다. 본 연구는 노인장기요양보험 제도의 이용계획 수립의 객관성 및 과학성을 확보하고 이용계획 업무를 효율화하는 데에 기여할 것으로 기대된다.

이중차이분석을 활용한 노인장기요양보험제도의 방문간호서비스 효과 (The Effects of Visiting Nursing Services in Long-term Care Insurance: A Difference-in-Difference Analysis)

  • Purpose: The purpose of this study was to demonstrate the effects of visiting nursing services in long-term care (LTC) insurance on changes in health status and physical functions among the elderly. Methods: We analyzed survey data on the living status and welfare needs of the elderly from 2008 and 2011 using the difference-in-difference method with propensity score matching. Results: The subjects were 76 elders including 38 visiting nursing service users(intervention group) and 38 home-based LTC service users (control group). Results from DID analysis confirmed that the subjective health status level of the intervention group was 2.2 points higher (p=.044) and the depression level was 0.3 point lower (p=.039) than the control group. In addition, the intervention group's ADL score was 1.9 (p=.027) and IADL 3.9 (p=.030), showing that their health was deteriorated less. Conclusion: Visiting nursing service in LTC insurance was associated with delayed deterioration of subjective health status, depression, ADL and IADL. These findings suggest the need of rebuilding visiting nursing service programs focusing on prevention services, which will be more contributive to elderly health care and the reduction of social costs.

뇌졸중 환자들의 노인전문병원 및 노인요양시설 선택요인과 이용만족도 (A Study of Preference and Satisfaction Factors between Senior Specialized Hospitals and Senior Care Facilities for Senior Stroke Patients)

  • This study compares the degree of satisfaction and the causes of selecting facilities for stroke patients in the senior specialized hospitals and other senior care facilities. The research results are followed. First, The patients who held the level of senior long-term care used senior specialized hospitals, while the patients who had the level of 2 or 3 degree used senior care facilities. The patients helped by cooperative care service used senior care facilities, and the patients helped by private service or family service used senior specialized hospitals. Second, The patients in senior specialized hospitals had affirmative attitude for their service system, while the patients in senior care facilities preferred their various service systems. In the satisfaction of the facilities, the patients in senior care facilities felt more satisfaction to staff, environment, service, and other factors than the patients in hospitals did. Third, in the result of logistic analysis, the patients had an affirmative attitude in case that they had spouse, experience of senior specialized hospitals or senior care facilities, without senior long-term care insurance. They also valued the service standard and the staff quality. As this study points out, the overall preference is higher in the senior care facilities. So, the stroke patients recognized the new role between the hospitals and the facilities after the establishment of long-term senior medicare system. This research had some limitation for the research areas and numbers. So the data analysis for the types of facility and the responses may not be generalized. However, the standard of choosing facility and satisfaction will be a guideline for establishing a new future role between hospitals and facilities. This result will be used as a basic data for the renovation of long-term senior medicare insurance.

재가요양보호사의 직무스트레스와 이직의도의 관계에서 직업의식 매개효과 검증 (The Mediating Effort of Vacational Consciousness on the Relationship between Job Stress and Turnover Intention in Long-Term Care Worker)

  • 본 연구는 재가요양보호사의 직무스트레스와 이직의도와의 관계에서 직업의식 매개효과를 검증하고, 그 결과를 기초하여 재가요양보호사의 이직의도를 감소시키려는 목적으로 수행하게 되었다. 연구대상은 서울·경기지역에서 일하고 있는 재가요양보호사 447명을 대상으로 하였다. 연구목적을 위해 자료분석은 SPSS 통계패키지를 활용하여 기술분석과 상관관계 분석 및 다중공선성 확인 후 매개효과를 검증하였다. 분석결과 첫째, 재가요양보호사가 연령이 낮고, 월가구소득이 높을수록 이직의도가 높을 것으로 나타났다. 둘째, 재가요양보호사가 직무스트레스를 높게 인식할수록 이직의도가 높아지는 것으로 나타났다. 셋째, 직업의식이 높을수록 이직의도는 낮게 나타났다. 넷째, 재가요양보호사의 직무스트레스는 직업의식을 부분 매개하여 이직의도에 통계적으로 유의미한 영향을 주는 것으로 나타났다. 이상의 결과를 토대로 재가요양보호사의 이직을 예방하고 노인돌봄 서비스의 질을 향상시키기 위한 임상적·정책적 실천에 대한 제언을 하였다.

지역별 노인 만성기 의료 및 요양·돌봄 공급체계 유형화 (Categorization of Regional Delivery System for the Elderly Chronic Health Care and Long-Term Care)

  • Background: By applying the suggested criteria for needs-based chronic medical care and long-term care delivery system for the elderly, the current status of delivery system was identified and regional delivery systems were categorized according to quantity and quality of delivery system. Methods: National claims data were used for this study. All claims data of medical and long-term care uses by the elderly and all claims data from long-term care hospitals and nursing homes in 2016 were analyzed to categorize the regional medical and long-term care delivery system. The current status of the delivery system with a high possibility of transition to a needs-based appropriate delivery system was identified. The necessary and actual amount of regional supply was calculated based on their needs, and the structure of delivery systems was evaluated in terms of the needs-based quality of the system. Finally, all regions were categorized into 15 types of medical and care delivery systems for the elderly. Results: Of the total 55 regions, 89.1% of regions had an oversupply of elderly medical and care services compared to the necessary supply based on their needs. However, 69.1% of regions met the criteria for less than two types of needs groups, and 21.8% of regions were identified as regions where the numbers of institutions or regions with a high possibility of transition to an appropriate delivery system were below the average levels for all four needs groups. Conclusion: In order to establish an appropriate community-based integrated elderly care system, it is necessary to analyze the characteristics of the regional delivery system categories and to plan a needs-based delivery system regionally.

스마트 장기요양 애플리케이션의 사용자 만족도 개선방안 도출: 심층 인터뷰와 토픽 모델링 활용 (User Satisfaction Enhancement of 'Smart Long-Term Care' Mobile Application: In-depth Interview and Topic Modeling)

  • Two dominant phenomena in modern world; population ageing and digitalization, have led public sector organizations to heavily rely on B2C(Business-to-Consumer) mobile applications. Yet, fatal concerns and complaints have often been raised by the mobile application users, notably from social welfare sector. With the continual expansion of digital landscape as well as the growth of life expectancy, usage of mobile applications has become prevalent across the stakeholders involved in social welfare sector. 'Smart Long-Term Care (SLTC)', inter alia, is a primary example of such mobile applications, designed to support Long-Term Residential Care (LTRC) service. The main goal of SLTC is to serve more convenient and practical LTRC service for both caregivers and care receivers. To examine user satisfaction of SLTC mobile application, this study investigates existing challenges and means to improve user satisfaction. Hence, we conducted this study using two methods: in-depth interview and topic modeling. Interestingly, two research outcomes commonly indicated that 5 factors (stability, accessibility, usefulness, responsiveness, and ease of use) were found significant in affecting user satisfaction of SLTC. Our findings suggest that the aforementioned factors can be seen as potential causes of the genuinely low user satisfaction. Eventually, this work will be a stepping-stone to elevate the overall quality level of LTRC service along with the user satisfaction degree of SLTC mobile application.
