The Effect of a Hand Massage Program on Anxiety and Immune Function in Clients with Cataract Surgery under Local Anesthesia (손마사지 프로그램이 백내장 수술환자의 불안과 면역기능에 미치는 영향)

  • This study was designed to investigate the effect of a hand massage program on anxiety and immune function in patients during cataract surgery. The hand massage program, in this study, consisted of hand massage and hand-holding. The subjects were sixty-three patients, thirty for the experimental and thirty-three for the control group, who were admitted at Kang Nam St. Mary's Hospital for cataract surgery. This study was carried out from December 10, 1997 to February 26, 1998. The level of anxiety as measured by the Visual Analogue Scale, systolic blood pressure, diastolic blood pressure, and pulse rate were measured before, after hand massage, and after hand-holding. Epinephrine, norepinephrine, cortisol, blood sugar levels, neutrophil, lymphocyte, and natural killer cell percentages also were measured before hand massage and five minutes before the end of the operation. Data were analyzed by t-tests, ANCOVA, repeated measures ANOVA, and Bonferroni multiple comparisons. The results were as follows : 1) After hand massage, psychological anxiety levels decreased significantly compared with before hand massage in the experimental group, not in the control group. After hand holding, there were significant decrease in both groups. 2) There were not significant differences on systolic blood pressure, diastolic blood pressure, and pulse rates in both groups. 3) The hand massage program decreased epinephrine, norepinephrine, and cortisol significantly in the experimental group, and increased epinephrine, norepinephrine, and cortisol in the control group. 4) There were no differences in blood sugar levels, neutrophil and lymphocyte percentages in white blood cells after the hand massage program. However, natural killer cells in lymphocytes were significantly increased in the experimental group. These findings indicate that a hand massage program could be a effective nursing intervention in decreasing the psychological and physiological anxiety levels and improving immune function in clients having cataract surgery under local anesthesia.

  • We studied the effect of barrier rib height variation using ray-optics code incorporated with three-dimensional plasma simulation to analyze the effects of cell geometry for varying pressure conditions. The optimal barrier rib height decreased as the Xe partial pressure increased which resulted in due to the formation of local, strong sheath under high Xe partial pressure condition.

  • A new algorithm, CBIR (Correlation Based Image Registration) was proposed to improve the resolution of image registration for PSP (Pressure Sensitive Paint). The local displacement vectors were obtained by finding the displacement which maximizes the cross-correlation between two interrogation windows of 'wind-off' and 'wind-on' images. A recursive multigrid processing was employed to increase the non-linear spatial resolutions. The variations of image were precisely measured without identifying the control points.

Effect of aerodynamic modifications on the surface pressure patterns of buildings using proper orthogonal decomposition

  • This study analyzed the pressure patterns and local pressure of tall buildings with corner modifications (recessed and chamfered corner) using wind tunnel tests and proper orthogonal decomposition (POD). POD can distinguish pressure patterns by POD mode and more dominant pressure patterns can be found according to the order of POD modes. Results show that both recessed and chamfered corners effectively reduced wind-induced responses. Additionally, unique effects were observed depending on the ratio of corner modification. Tall building models with recessed corners showed fluctuations in the approaching wind flow in the first POD mode and vortex shedding effects in the second POD mode. With large corner modification, energy distribution became small in the first POD mode, which shows that the effect of the first POD mode reduced. Among building models with chamfered corners, vortex shedding effects appeared in the first POD mode, except for the model with the highest ratio of corner modifications. The POD confirmed that both recessed and chamfered corners play a role in reducing vortex shedding effects, and the normalized power spectral density peak value of modes showing vortex shedding was smaller than that of the building model with a square section. Vortex shedding effects were observed on the front corner surfaces resulting from corner modification, as with the side surface. For buildings with recessed corners, the local pressure on corner surfaces was larger than that of side surfaces. Moreover, the average wind pressure was effectively reduced to 88.42% and 92.40% in RE1 on the windward surface and CH1 on the side surface, respectively.

  • Effects of aeration intensity on local fouling were investigated in submerged membrane modules. Higher liquid velocities were observed at the section with the lower fiber packing density. The liquid velocity is increased with increasing the gas-liquid injection factor. The high shear stress coincided with the high liquid velocity. The shear stress increases with the increasing of gas-liquid injection factor and the liquid velocity improves with the increasing of gas-liquid injection factor. Irreversible fouling resistance ($R_{ir}$) of the fiber position is significant in a local region of high suction pressure near the suction point of the fiber (position 1). The ratio of $R_{ir}/R_m$ and $R_{ir}/R_r$ of position 1 was highest compared to the position 2 and 3. Irreversible fouling resistances results confirmed the preferential deposition of foulants near the suction part of the fiber where the local suction pressure is the highest and correspondingly, more particles are accumulated to the membrane surface. The effects of local fouling along the fiber length are significant factors to optimize the design of submerged modules.

  • In this paper, a stress-strain model for high-strength confined concrete is proposed using compressive fracture energy. In the compression test performed by author in Reference [6], an acrylic bar with strain gauges was embedded in the center of the specimen to measure the local strain distribution. It was found from the test that the local strain measurement by this acrylic rod is very effective. The local fracture zone length was defined based on the local strain distribution measured by the acrylic rod. Specifically, it was defined as the length where the local strain increases more than twice of the strain corresponding to maximum stress. In addition, the stress-strain relationship of confined concrete with compressive fracture energy is proposed on the assumption that the amount of energy absorbed by the compressive members subjected to the given lateral confining pressure is constant regardless of the aspect ratio and size. The proposed model predicts even results from other researchers accurately.

Numerical Investigation on the Self-Ignition of High-pressure Hydrogen in a Tube Influenced by Burst Diaphragm Shape (튜브 내 고압 수소의 파열막 형상에 따른 자발 점화 현상에 대한 수치해석)

  • Numerical simulations are conducted to investigate the feature of spontaneous ignition of hydrogen within a certain length of downstream tube released by the failure of pressure boundaries of various geometric assumption. The results show that the ignition feature can be varied with the shape of pressure boundary. The ignition at the contact region are developed at the spherical pressure boundaries due to multi-dimensional shock interactions, whereas the local ignition is developed in limited area such as boundary layer at the planar pressure boundary conditions. The spontaneous ignition inside the tube can be generated from the reaction region of only boundary layer regardless of existence of the reaction of core region.

  • The APAM's quantitative effectiveness and comparative study in preventing and treating pressure ulcer has not been sufficiently evaluated mainly because of uncertainty of pressure load input and lack of interpretation of dynamic perfusion recovery characteristics of soft tissue. The purpose of this paper was to quantify and analyze the alternating pressure characteristics of APAM as a preventive measure for pressure ulcers. To quantify the alternating load to human body, we introduced alternating pressure profile concept and developed parametric model of the profile. Regarding pressure level and cycle time, 3 global and 7 local periodic parameters were used to define the profile such as light, standard, typical and heavy duty profile shape. Pressure impulse ratio of light duty is the lowest but pressure fluctuation is significantly high. For the same duty shape, contact conditions are changed with alternating cycle time and more dramatically in shorter alternating cycle time conditions. We can conclude that if we use shorter alternating cycle time on APAM's operation we can get more positive effects regarding to inflated contact time condition. We proposed the quantitative methods on tissue viability study of external loading by simultaneous measurement of interface pressure and tissue perfusion with proper alternating pressure profile conditions.

The Effects of the Hand-holding Program as an Independent Nursing Intervention for the Patients with Anxiety in Local Anesthesia (국소마취 백내장 수술환자의 불안에 대한 직접간호중재로서의 손잡아주기 프로그램 효과)

  • Operation usually brings about psychological stress as well as physical stress to the patient and it greatly influences the maintenance of the homeostasis. Until now, most of the nursing research concerning the anxiety of the patient undergoing an operation has been done while the patient was under general anesthesia. Under local anesthesia, the anxiety of the patient who undergone operation is short. However, there is much demand of the operational nurses to mediate between the patient who is undergoing operation when fully conscious, as they are aware of the patient's anxiety and to decrease the patient's anxiety of this local anesthesia. Therefore, this research tries to verify the effects of the Hand-holding program on the anxiety of the operational patient undergoing local anesthesia and affirms the role of the nursing intervention. As an unequivalent control group non-synchronized designs, this research selects out 20 control groups and 21 experimental groups at random who have received cataract operation while under local anesthesia and after implementing the Hand-holding program to the experimental groups while undergoing the operation, the effect of the experiment was observed. By observing the Epinephrine, Cortisol, blood pressure, and pulse for the physical indexes before and after the enforcement of the hand-holding, and using the measurement of Speilberger(1972) for the psychological indicators for the anxiety of the patient before and after hand-holding, the results were analyzed through SAS program. As a results, intervention of hand-holding had a remarkable effect to decrease the anxiety felt by the patient and the diastolic blood pressure also was effectively decreased. On the other hand, through the decrease of the systolic blood pressure was also observed, it was not statistically significant. In the Epinephrine and Cortisol indicators, the intervention of hand-holding was revealed to have an insignificant effect. The Hand-holding was revealed to have an indicators, the intervention of Hand-holding was revealed to have an insignificant effect. The Hand-holding program may be said to have an effect which decreases the psychological anxiety of the patient undergoing local anesthesia.

Failure Mode and Fracture Behavior Evaluation of Pipes with Local Wall Thinning Subjected to Bending Load (감육배관의 굽힘하중에 의한 손상모드와 파괴거동 평가)

  • Fracture behaviors of pipes with local wall thinning are very important for the integrity of nuclear Power Plant. In Pipes of energy Plants, sometimes, the local wall thinning may result from severe erosion-corrosion (E/C) damage. However, the effects of local wall thinning on strength and fracture behaviors of piping system were not well studied. In this paper, the monotonic bending tests were performed of full-scale carbon steel pipes with local wall thinning. A monotonic bending load was applied to straight pipe specimens by four-point loading at ambient temperature without internal pressure. From the tests, fracture behaviors and fracture strength of locally thinned pipe were manifested systematically. The observed failure modes were divided into four types; ovalization. crack initiation/growth after ovalization, local buckling and crack initiation/growth after local buckling. Also, the strength and the allowable limit of piping system with local wall thinning were evaluated.
