Venereal Squamous Papilloma in a Male Dog

  • A dog (Maltese, 4-year-old, intact male) was referred to the hospital because of the multiple cabbage-shaped nodular masses on penis, preputial fornix and inner layer of prepuce with the free-roaming history. Those appearance was similar with the characteristic of transmissible venereal tumor (TVT). As a result, it was tentatively diagnosed as TVT by the veterinarian of a local clinic and treated with vincristine sulfate. However, the lesion did not regress. Histologically, the mass was consisted of fibrovascular connective tissue stalks and keratinized stratified squamous epithelium, and koilocytosis and intranuclear inclusion body were not shown in the epithelial layer. In addition, inflammatory changes were minimal in the tissue of mass. Based on these findings, this case was diagnosed as venereal squamous papilloma. As the treatments of TVT and papilloma differs, practitioners must be cautious with the diagnosis.

Maxillary Sinusitis by Staphylococcus aureus Infection in a Thoroughbred Gelding: Case Report

  • A 4-year-old gelding Thoroughbred racehorse, which had been undergoing antibiotic therapy at a local veterinary clinic, was referred to the KRA veterinary center with a 20-day history of continuous right nasal discharge. Patient's history, endoscopic examination, and radiographic examination revealed primary maxillary sinusitis. Under sedation, surgical intervention was performed to collect samples and remove the accumulated mucopurulent exudate in the sinus. Swab samples were collected from the sinus during surgery for cytology and antimicrobial susceptibility testing. Only one type of bacteria was cultured, and molecular analyses of 16S ribosomal RNA gene sequences identified it as Staphylococcus aureus (S. aureus). The isolate was resistant to multiple antibiotics, which are frequently used in equine practice. Trimethoprim-sulfamethoxazole was chosen based on antibiotic susceptibility test, trephination, and sinus lavage using saline were applied to treat bacterial sinusitis. The clinical signs improved after 1 month and the patient resumed training. This report describes S. aureus isolated from bacterial maxillary sinusitis in a horse and its antibiotic susceptibility.

A STUDY ON THE CHANGES OF PATIENT DISTRIBUTION AND TREATMENT PATTERN FOR THE LAST 5 YEARS IN PEDIATRIC DENTAL PRACTICE OF GWANGJU (최근 5년간 광주지역 소아치과에 내원한 신환의 분포와 진료 내용에 대한 조사)

  • The treatment pattern of pediatric dentistry has been changing recently, because of so many socioeconomic changes such as decrease of birth rates and prevalence of caries. The purpose of this study is to recognize the changing patterns in pediatric dental treatment and to help predict future direction for pediatric dentistry. Patients distribution and treatment pattern were examined in all new patients of the department of pediatric dentistry, Chosun University Dental Hospital and two pediatric local clinics in Gwang-ju, from 2005 to 2010. The number of new patients in recent 5 years has been increasing. There was a higher ratio of male patients. Age distribution has shown the percentage of 3~4 age group was highest. The hospital visit rate to Chosun University Dental Hospital was high, whereas the visit rate for local clinics was low. Dental caries showed the highest percentage in chief complaints, oral examinations have increased. The percentage of restoration treatment was highest, fluoride treatment had increased. In restorative treatment, the percentage of resin and GIC restoration was highest and amalgam restoration has decreased. The percentage of sedation treatment was decreased a little, Chosun University Dental Hospital showed a higher rate than local clinics.

A study on the state of inservice education for dental hygienists and their relevant awareness (치과위생사의 보수교육 실태 및 인식에 관한 연구)

  • The purpose of this study was to examine the reality of inservice education provided to members of Korean Dental Hygienists Association, the state of relevant academic conferences, and the perception of the members about inservice education and academic conference. It's basically meant to help boost their participation in inservice education and their satisfaction with it, and to show some of the right directions for that. The subjects in this study were dental hygienists who attended a symposium on July 1, 2006. After a survey was conducted, the answer sheets from 489 participants were analyzed, and the findings of the study were as follows: 1. General hospitals and university hospitals made up the largest group(91.4%) that gave a monthly leave of absence, and the second largest group was dental hospitals(75.4%), followed by dental clinics(58.3%) and public dental clinics(48.0%). The most common closing time in dental clinics and dental hospitals was 5 p.m., and that was 12 p.m. in general hospitals and university hospitals. The dental hygienists in public dental clinics didn't work on Saturdays. By type of workplace, treatment was the most common duty for the dental hygienists in dental clinics and dental hospitals to perform, and those who worked at general hospitals, university hospitals and public health clinics were in charge of extensive range of jobs. 2. The rates of the dental hygienists who took that education stood at 94.9% in public dental clinics, 78.7% in dental hospitals and 75.3% in dental clinics, general hospitals and university hospitals. Regarding how many marks they got on an yearly basis, those who got eight marks or more made up the largest group(55.6%), followed by four marks or more(11.8%), six marks or more(3.4%), and two marks or more(1.5%). As for the usefulness of inservice education for their job performance, the largest number of the dental hygienists(40.8%) found it to be helpful, and the second greatest group(37.5%) considered its effectiveness to be so-so. The third largest group(8.4%) found it to be of great use, and the fourth biggest group(4.2%) considered it to be of no service. The fifth biggest group(l.3%) thought it was absolutely useless. By type of workplace, the workers in dental clinics, dental hospitals, general hospitals and university hospitals wanted the most to learn how to take care of clinical work(acquisition of up-to-date technology), and those in public health clinics hoped the most to learn about public dental health. By type of workplace, the workers in dental clinics had their sight set on self-development the most, and the dental hygienists in dental hospitals, general hospitals, university hospitals and public health clinics were most in pursuit of acquiring new knowledge. By type of workplace, the specific given conditions at work were most singled out by the dental clinic workers as the reason, and the dental hospital employees pointed out time constraints the most. The dental hygienists in general hospitals and university hospitals cited time constraints and financial burden the most, and the public health clinic personnels mentioned inaccessibility of a place for inservice education as the reason. 3. The public health clinic workers participated in academic conferences the most(90.8%), followed by the general and university hospital personnels(68.8%), dental hospital employees(65.6%) and dental clinic workers(65.5%). By type of workplace, the public health clinic workers(73.5%) expressed the most satisfaction, followed by the general and university hospital employees(67.7%), dental clinic workers(62.3%) and dental hospital personnels(54.1%). By type of workplace, the employees of dental clinics, dental hospitals, general hospitals and university hospitals preferred Saturdays, and the public health clinic workers had a liking for weekdays. As for a favored place, hotels were most preferred, followed by university hospitals, general hospitals, college lecture rooms, district halls and local public institutions. Hotels were most favored regardless of the type of workplace. 4. Regarding outlook on inservice education, they had the highest opinion on the facilities and given conditions of lecture rooms($3.41{\pm}0.83$), followed by the professionalism of lecturers($3.34{\pm}0.83$), procedures of receipt and attendance confirmation($3.34{\pm}0.83$) and class size($3.13{\pm}0.89$). On the contrary, they took the most dismal view of the inaccessibility of a place of inservice education($2.08{\pm}0.92$), followed by limited opportunity and limited date for that education($2.51{\pm}0.99$), extra financial burden($2.53{\pm}1.18$) and high tuition fee($2.57{\pm}0.96$).

  • Stellate ganglion block and cervical epidural nerve block are frequently practiced in pain clinics because of simple procedure and good effect. Nerve block at head and neck may produce serious complication such as loss of consciousness and cardiac arrest. Blood supply is rich in neck and inadvertent arterial injection of local anesthetics may enter directly into brain. We experienced convulsion and respiratory arrest during SGB and cervical epidural block. The patients were resuscitated successfully and recovered without any adverse effects.

Pain Management of Post-herpetic Neuralgia (대상포진 후 신경통의 통증관리)

  • Postherpetic neuralgia, the most feared complication of acute herpes zoster, may agonize the pain clinician because the appropriate management of intractable pain can fail inspite of various and prolonged therapeutic techniques. Of all patients with herpes zoster, approximately 5~10% will develop postherpetic neuralgia. Postherpetic neuralgia is very rare in young patient but very common in patients over 60. In other words, the older, the higher incidence. In our pain clinics, 13 postherpetic neuralgic patients were treated with sympathetic blocks, local infiltration with local anesthetics and steroids, TENS, Laser and various drugs including antiderpressant. The results of management of pain in 13 patents were as follows. 1. Excellent pain control :4(30.8%) 2. Good pain control :3(23.0%) 3. Fair pain control :2(15.4%) 4. No effect :4(30.8%).

Work-Related Stress among Physical Therapists (물리치료사의 업무관련성 스트레스)

  • This study was carried out to measure the degree of work-related stress and to identify factors influencing the excessive stress among clinical physical therapists. One thousand and five physical therapists surveyed nationwide met the established criteria for inclusion in the study. The respondents were an average age of 27.1 years, $34.5\%$ were married, and $61.2\%$ were female. Most of the respondent $(48.1\%)$ worked in local clinics. The average stress score was 50.9. Analysis-of-variance test demonstrated significant difference in the degree of stress among treatment facilities. There was a significant relationship between treatment facilities (general hospital, local hospital, clinic), number of patients treated per day, job satisfaction, low back pain and work-related stress, respectively. Work-related stress was unrelated to matters of sex, marital status, education. Hospital administrators, physical therapydirectors, directors, and individual therapists should work within their organizations to reduce work stress and its detrimental effects. Further research is necessary to investigate preventive measures to decrease the work-related stress.

Treatment of Surgical Site Infection and Delayed Union in Fetlock Arthrodesis of a Mare

  • A 6-year-old Thoroughbred mare presented to the Korea Racing Authority Equine Hospital with dropping of the left front fetlock due to an injury sustained while racing. Radiographic examination revealed a comminuted fracture of both proximal sesamoid bones of the affected fetlock. Arthrodesis of the fetlock joint using a broad dynamic compression plate with a tension band wire was performed as a salvage procedure for the future use as a broodmare. After surgery, however, a delayed union of the bones and surgical site infection was present for a prolonged period. Staphylococcus aureus was persistently identified from the surgical site, and antimicrobial therapies were based on antibiotic sensitivity tests, including regional perfusions. The removal and replacement of surgical implants associated with seropurulent discharge was based on coordinating the development of fetlock ankylosis and infection control over 13 months. Firstly, seven screws associated with surgical drainage were replaced and bone morphogenetic protein-2 (BMP-2) and local antibiotics were placed into the surgical site to accelerate bone fusion at postoperative month 7. Further six screws, along with drainage, were removed at postoperative month 10. The plate and screws were removed from the limb due to the progression of bone fusion at postoperative month 13; BMP-2 and local antibiotics were also used. Delayed healing of arthrodesis due to surgical site infection and implant instability were treated by implant removals and antibiotic therapies, and the horse eventually showed improved weight-bearing ability of the affected limb.

Evaluation for the Purpose and Relevance of Obesity Treatment in Local Clinics (외래 비만 클리닉에서 행해지는 비만 치료의 목적 및 타당성에 관한 평가)

  • Metabolic syndrome is increasing nowadays and one of causes is obesity. Therefore prevention and treatment of obesity is very important to decrease of Metabolic syndrome. However currently many pharmacotherapy for obesity are popular in Korea and it may be dangerous. The aim of this study is to evaluate the purpose and relevance of obese treatment in two areas (capital & local) of Korea and suggest to guide for the right directions of pharmacotherapy in obesity treatment. We selected and surveyed 90 patients (88 females, 2 males, age of $23{\sim}60$) at random who received a prescription after consulting with a doctor at several clinics of Cheongju-area and Seoul-area, from May thru June, 2007. And we evaluated their prescriptions. In results, only 26.7% were obese (13.3%, 2325), most 73.3% patients were normal (65.6%, 18.5$m^2$). The purpose of treatment was for appearance (38.9%), health (41.1%) or both (18.9%, p<0.01). 94% patients had the past history of obese treatment. Prescriptions were consisted of CNS stimulant, anticonvulsants, antidepressant, laxatives, diuretics, or antianxietics. Obese treatment is very important for health to prevent disease such as Metabolic Syndrome. However it should not be abused. In conclusion, some patients of obese treatment is not relevant to obesity and pharmacist role is important to guide them for the right directions in obesity treatment.

Exogenous Estrogen Toxicity in a Dog (개의 외인성 에스트로겐 중독 증례)

  • A 2-year-old female mongrel dog was referred to the Veterinary Medical Center of Chungbuk National University due to anorexia, depression and weight loss after the administration of exogenous estrogen for contraception in the local hospital. Although the dog was medicated with antibiotics and fluid for two weeks in the local hospital, there was a deterioration in the patient's general condition. Therefore the dog was referred to our Veterinary Medical Center. Hematological and biochemical analysis showed non-regenerative anemia, thrombocytopenia, severely immature leukocytosis, hyperbilirubinemia, hyperglycemia and elevated serum alkaline phosphatase, gamma-glutamyltransferase, and lactate dehydrogenase. Abdominal ultrasonography showed double line of gallbladder and irregular hypertrophic wall of uterus. On the basis of these examination's results and history taking, final diagnosis was exogenous estrogen-induced myelotoxicosis with cholecystitis and endometritis. The symptomatic therapy for estrogen toxicosis was performed for 26 days after the final diagnosis. However, the symptom had no response to therapy except endometritis and anemia. On day 135 after administration of estrogen, platelet and neutrophil count were normalized, but monocytic leukocytosis was occurred.
