Strategy for Strengthening Community-Based Public Health Policy (지역사회기반 공중보건정책 강화방안)

  • Public health system for more prevention-oriented health promotion rather than hospital-based curative service, focusing population rather than individual, and comprehensive health management in the local community strongly needs to be constructed to solve major issues on efficiencies and equity problems which Korean healthcare system is facing nowadays. Public health promotes and protects the health of people and the communities where they live, learn, work, and play. Medical care tries to cure those who have diseases, but public health tries not to become ill and not to be injured. Debates on how we build or rebuild public health system, which is contrasted with medical care system, are needed in Korea, focusing how needs for healthy community and right to health are fulfilled. Public health specialists for practising population health at local community level should be systematically recruited, the function of public health centers should be strengthened, and new government organization should be established for place-based health management.

The Type of Medical Service Desired by Those Communities Which at Present have None (일부(一部) 무의면 주민(住民)들이 원(願)하는 의료시설의 형태(形態)에 관(關)한 조사(調査))

  • To try to determine the type of medical service wanted by the rural population, in Chungnam Province, a survey amongst the populations of 6 counties was conducted; Within the 6 counties, 2 local communities, which had no access to local medical services, were surveyed. The 12 communities were actively involved in Sae-maul activities, and total number of households surveyed, was 822. The survey was conducted over a 1 month period, from July 16th, 1976, thru August 20th, and the followings are the results, summarised. 1. The largest number of respondents desired a combination of Public Health Center and Country Hospital, followed in order by Private Clinic and Modern Medical Facility. 2. The respondents, aged under 40 years, desired the Private Clinic type medical service, whilst those over 40 years of age, wanted the County Hospital, and as the numbers in this age bracket, were larger, so the ratio was much higher. 3. Sex, educational background, and occupation did not play any particular emphasis in the decisions. 4. Monthly income affected the responses to the survey. These in the lower-income bracket, wanted the County Hospital, and the ratio was high. These in the high income bracket desired the Modern Medical Service, accordingly. Those with an income of 50,000 won or less, amongst the low-income bracket, favored the Public Health Sub-center type of service. The ratio for this service was very high.

Effects of Personal Characteristics and Public Hospitals Awareness by Community Residents on use-experience of Public Hospitals (지역주민 특성과 인지도가 공공병원 이용경험에 미치는 영향)

  • Purpose: This study aims to explore the effects of personal characteristics, satisfaction, quality of care, role performance, image, awareness level of public hospitals perceived by community residents on use-experience of public hospitals. Methods: A cross-sectional survey was conducted with 2,100 community residents around 39 district public hospitals. The questionnaire was designed to collected information on personal characteristics and community awareness of public hospitals. The community awareness consists of 5 factors and 15 items. The data were collected utilizing call-interview by a survey company. Results: The personal characteristic and community awareness were shown significant differences between having use-experience and not-having use-experience of public hospitals (p<.001), except gender. As the results of multiple logistic regression, the significant variables of use-experience of public hospitals were satisfaction (OR=1.06 95%CI=1.010-1.116), quality of care (OR=1.07, 95%CI=1.016-1.134, level of awareness to public hospitals (OR=1.50, 95%CI=1.378-1.632), age (OR=0.43, 95%CI=0.236-0.785), education (OR=1.62, 95%CI=1.013-2.590), type of medical security (OR=0.37, 95%CI=0.142-0.945). Conclusions: Public hospitals have to effort to improve community awareness through providing quality of care, and role performance. It is possible to support them by the Central and Local Government.

Analysis of Productivity by Environmental Factors in Regional Base Public Hospitals (지역거점 공공병원의 환경적 요인에 따른 생산성 분석)

  • The purpose of this study is to analyze the difference of productivity according to environmental factors among 25 Regional base public hospitals. Also this study is to propose a method to improve the productivity of Regional base public hospitals in the future by improving the public performance and stable management performance by studying the productivity variables affecting profitability. The survey period was based on the last three years, and 25 Regional base public hospitals were selected for the survey. The dependent variable is the total capital medical marginal profitability and the medical profit marginal profitability which are the indicators of profitability. The independent variable, productivity, is classified into three indicators: capital productivity, labor productivity, and value added productivity. The ANOVA analysis method was used to analyze the productivity difference according to the frequency factor and the environmental factors of the Regional base public hospitals. Finally, we conducted a hierarchical regression analysis to examine the productivity variables affecting profitability. The results of this study showed that there were differences in productivity due to environmental factors such as hospital size, competition in the local medical market, and differences in management performance. The difference in productivity and profitability depending on the environmental factors suggests that it is difficult for Regional base public hospitals in each regional base to perform a balanced public service. In order to overcome this, it is necessary to provide balanced medical services such as government financial support expansion, regional medical demand forecasting and facility infrastructure construction.

Effects of job satisfaction and patients satisfaction on medical profit at public hospitals (공공병원 직원의 직무만족도 및 환자 만족도가 의료수익에 미치는 영향)

  • Purpose: This study was conducted to examine the factors associated with medical profit per bed related job satisfaction and patients satisfaction in public hospitals. Method: A cross-sectional survey was conducted through staff and patients in 39 hospitals (34 local province hospitals, and 5 red-cross hospitals) from September to October in 2011. A questionnaire was designed to collect information on job satisfaction, inpatients and outpatients satisfaction. The data of medical profit per bed was collected in 2011 annual financial reports from each 39 hospitals. Finally, data from 5,521 staff, 1,730 in-patients and 1,730 out-patients were analyzed. Multiple logistic regression analysis was performed to determine factors associated with medical profit per bed related job satisfaction and patients satisfaction in public hospitals. Results: The medical profit was positively correlated with job satisfaction of staff. As the results of multiple logistic regression, the significant variables of medical profit per bed, were communication satisfaction of staff(OR=0.48, 95% CI=0.25-0.91), hospital environment satisfaction of in-patients(OR=1.53, 95% CI=1.05-2.22). Conclusion: The satisfaction of patient and staff were significantly effect to medical profit in public hospitals. The Government and administrators have make an effort to improve satisfaction of staff and patients in public hospitals.

Practical Examples of the Comprehensive Strategy of Japanese Dementia Policy: Kumamoto Model by Kumamoto Province (일본 인지증 정책 종합 추진전략의 실천 사례: 쿠마모토현의 쿠마모토 모델)

  • The purpose of this research is to introduce the best practice of the Japanese national dementia strategy and explore implications to the Korean national dementia strategy. Interview was conducted among professions those who is in charge of Kumamoto dementia care practice in Kumamoto province, upon review of related literature and public documents. The Kumamoto model is implemented by the department of neuropsychiatry in public university hospitals, which can offer dementia-specialized medical services. Medical centers for dementia in public university hospitals play a leading role for managing practice and training local dementia centers specialist, coordinating medical services among medical institutions and community welfare facilities. In reference to the Kumamoto model, the Korean national dementia strategy can find implications in the direction of current system, specifically its approaches toward policy governance.

The Development Directions of Health Tourism in Gangwon Province by Participation of Local Governments and Health Care Industry (자치단체와 병.의원 협력에 의한 강원도 의료관광 발전방향)

  • Gangwon province has been chosen as a health tourism site by one of the nation level projects from 2008. This study surveyed local governments in the province in 2009, in order to develop suitable approaches to health tourism for the province. From the result, all respondents supported the health tourism in their city. Furthermore, an appropriate type of health tourism for the province identified as health promotion whereby the natural environment rather than advanced medical care. To respond to this, cooperation between local governments and hospitals is important, thus Gangwon province should seek the strategies for this.

Monopsony Power of General Hospitals in Nurse Labor Market (간호사 노동시장의 수요독점에 대한 연구 - 종합병원을 중심으로 -)

  • Nurses are medical personnel, who play a key role in supporting patient care, so it is important to supply them adequately in balance with ever increasing medical demand. But there appears severe shortage of nurses in some hospitals because of their uneven distribution, especially in small sized-hospitals and rural-hospitals. As nationwide distorted distribution of nurses in Korea is just like what monopsony model(a kind of market structure model) tells us, it is attempted to explain this situation of nurse labor market in Korea on the basis of monopsony model and presented in this paper. Specifically, determinants of nurse wage and the level of their relative employment were examined, and monopsony impact on their wage and the level of relative employment controlling those determinants were studied. Major results of this study arc as follows. The most important determinant of nurse wage level in this study was the wage level of a local community where each hospital located Hospital owner's characteristics an educational function of each hospital were also important factors. With these factor controlled, it was found that monopsony power of each hospital was negativel associated with nurse wage level as expected. 1% increase in monopsony power of hospital(measured by Herfindah-Hirschman Index) reduced nurse wage by $5,674{\sim}19,19$ won(in Korean currency). With regard to the level of relative employment, the most important determinant wa the capacity for supplying nurses of the local community. Again, hospital owner characteristics and educational function of each hospital were also important. With these factors controlled, it was found that monopsony power of each hospital was negative associated with the number of nurses per bed, as expected. 1% increase in monopsony power of each hospital(again measured by Herfindah-Hirschman Index) reduced the number of nurses per 100 bed as much as $0.46{\sim}0.67$. In conclusion. structural factors of nurse labor market influence the instability of nurse labor supply in Korea. Further consideration for these market structural characteristics needed for policy making related to nurse resource allocation.

A Study on the Operation and Use of Physiotherapy room at Public Health Service Center (보건소내 물리치료실 운영실태와 물리치료사의 배치방안)

  • The public health service center and its branch are, as one of public institution, responsible for providing non-profitable medical service to local residents, which can't be addressed by private medical institutes. In addition, as a planning coordinator, they need to manage local medical resources effectively to make sure local residents can be offered good-quality health service fairly and impartially. The purpose of this study was to highlight the importance of physical therapy in public health service center's successful role performance, one of treatment to which local residents had a direct access. For attaining the purpose, an investigation was made into the actual facilities of public health service center and its branches in North Cholla province, the extent of use of physicaltherapy room, and the general characteristics of physical therapists. And literatures concerned were referred to. As a result, the dominant opinion was that the current physicaltherapy room shoulder be more reinforced(38.9%) or that it should he kept as it was(46.5%). Besides, the physical therapists investigated still were serving in a poor working condition. To elevate the quality and quantity of medical service, the current way to employ expert personnels temporarily or not to guarantee the position of physiotherapists should be removed immediately. Instead, physicaltherapists should be employed as regular public official. As, in home-visit project, physiotherapist are expected to play a bigger role in rehabilitation care or the management of patients with chronic disease, the physicaltherapy room should be increased and more physicaltherapists should be provided to health service institutions.

APPRAISAL OF MAINTENANCE MANAGEMENT STRATEGIES USED IN PUBLIC HOSPITAL BUILDINGS IN LAGOS STATE, NIGERIA

  • The study focused on the evaluation of maintenance management strategies used in public hospital buildings in Lagos state. It also assessed the labour composition for maintenance operations. In achieving these objectives, opinions of maintenance officers of ten (10) different hospitals in different local government areas of the state were sampled through well structured questionnaires. The data collected were analyzed using descriptive and inferential statistics. From the analysis, the study revealed that majority of those public hospitals do not have specific budget for maintenance programmes, maintenance policies, maintenance log book and maintenance manual to guide the operatives. About 98% of them do not understand the type of maintenance strategy being used for their maintenance operations. 78% of the maintenance work are only executed when there is a breakdown or in response to user's request. For labour composition, the cleaning of interior and exterior of the building, inspection of building elements, repairs and replacements of building elements are mainly carried out by in-house staff, while the repair and replacement of equipment is by outsourcing. The study also revealed that using in- house staff, reduces costs and provides a higher security while outsourcing provides more flexibility in staffing reduces equipment expenditures and provide better access to special skills. The study recommended proactive measures to provide necessary training and support for maintenance staff and users of these facilities and a means of securing sufficient funds for maintenance programmes.
