Education in an Academy of Chinese Medicine L$\"{u}$shantang(侶山堂) and the Qiantang Medical School(錢塘醫派) (중의서원(中醫書院) '여산당(侶山堂)' 강학(講學)과 '전당의파(錢塘醫派)')

  • This study aims to discuss the establishment and development of the Qiantang Medical School(錢塘醫派) represented by Zhang Zhicong(張志聰) via focusing on an academy of Chinese medicine named L$\"{u}$shantang(侶山堂). The teaching method in L$\"{u}$shantang(侶山堂) displays a representative feature of the Qiantang Medical School(錢塘醫派), but the foundation of the method consisting of lectures and discussions had already been laid by Lu Zhiyi(盧之頤) during the Ming-Qing transition period. The tradition was succeeded by Zhong Xuelu(仲學輅) in the Hangyuan Medical Board(杭垣醫局) even after L$\"{u}$shantang(侶山堂) was burnt down during a war taken place under the Qianlong(乾隆) period. That the function and the role of the local Confucian academies, which had been focused on the discussion of Confucian classics and local issues, were changed to adopt the education of medicine which had been treated as a lesser subject may be interpreted as a gradual change in the social perception of medicine. The change in the function of the Confucian academies combined with the tendency in which the literati elites of the period left the Confucian philosophy for medicine presents one of many examples showing the changes occurred during the Ming-Qing transition period. The education of medicine provided by the Confucian academies is regarded historically significant in that it was offered by ordinary civilians rather than the government before the formal school education system was established in the modern period. This educational tradition played an important role in bridging the Chinese medicine in the medieval times with that in the modern period.

  • Purpose - Local social commerce market has grown up remarkably. And, Coupang has shown new delivery strategy of rocket delivery. Making new paradigm at local market, Coupang has expanded market scale. This study investigated state of local social commerce market, weight and promotion strategy of Coupang market to find out competitiveness edge of Coupang. Research design, data, and methodology - The study investigated state and concept of social commerce to find out state, problems and competitiveness of social commerce. New distribution service was short of precedent studies. Statistical analysis and experimental analysis were not used, and interview was done to investigate three of social commerce businesses. Results - CRM construction is insufficient to have poor system, Local delivery system could not be made enough at overnight delivery and customers were dissatisfied with ties with another company. Promotion shall be done by delivery system for increase of profitability, funding for more investment, chatbot to build new customer control system, and new delivery system to produce profit. Conclusions - Coupang and others have grown up rapidly to worsen profit and to jeopardize survival. Excessive initial investment has threatened the businesses, for instance, low sales of Amazon, excessive expenses, bench marking of logistics system, and others.

The Association between the Subjective Perception of the Regional Healthcare Environment and Unmet Medical Needs (지역의 의료서비스 환경에 대한 주관적 인식과 연간 미충족의료 발생 간의 연관성)

  • Based on the basic ideology of health care, this study realized the seriousness of annual unmet medical need and conducted a study to confirm the relationship between the subjective perception of regional healthcare environment and unmet medical needs. The independent variable was classified into attitude 'satisfaction' and 'unsatisfaction' toward regional healthcare, and the dependent variable was classified as whether unmet medical needs occurred annually. Based on previous studies, the control variables were selected as demographic and socioeconomic characteristics that can affect the occurrence of unmet medical care annually and characteristics related to health behavior. Descriptive statistics were conducted for each variable on the extracted sample, and multivariate survey logistic regression analysis was conducted to confirm the association between variables. As a result, more unmet medical needs occurred annually than those who were satisfied with the medical services in the area where they lived. In addition, more unmet medical need occurred annually in "unsatisfied" households compared to households "satisfied" with local medical services. In residential areas, women live in "metropolis" and "rural areas" compared to "urban," women live in men, lower education levels, and poor subjective health levels, and less satisfied with local medical services. As such, the impact of environmental factors in the community on the use of health and medical services is one of the major areas of interest in the field of health science, such as health policy and social dynamics. Therefore, hospitals in each region need to make efforts in terms of hospital management to increase the overall satisfaction of medical services in the region by continuously monitoring the attitude of residents to achieve universal health security, and policymakers should also be interested and propose new policies.

Impact of Screw Type on Kyphotic Deformity Correction after Spine Fracture Fixation: Cannulated versus Solid Pedicle Screw

  • Study Design: Retrospective review. Purpose: To detect the effect of cannulated (poly-axial head) and solid (mono-axial head) screws on the local kyphotic angle, vertebral body height, and superior and inferior angles between the screw and the rod in the surgical management of thoracolumbar fractures. Overview of Literature: Biomechanics studies showed that the ultimate load, yield strength, and cycles to failure were significantly lower with cannulated (poly-axial head) pedicle comparing to solid core (mono-axial head). Methods: The medical charts of patients with thoracolumbar fractures who underwent pedicle screw fixation with cannulated or solid pedicle screws were retrospectively reviewed; the subjects were followed up from January 2011 to December 2015. Results: Total 178 patients (average age, $36.1{\pm}12.4years$; men, 142 [84.3%]; women, 28 [15.7%]) with thoracolumbar fractures who underwent surgery and were followed up at Hamad Medical Corporation were classified, based on the screw type as those with cannulated screws and those with solid screws. The most commonly affected level was L1, followed by L2 and D12. Surgical correction of the local kyphotic angle was significantly different in the groups; however, there was no significant difference in the loss of correction of the local kyphotic angle of the groups. Surgical correction of the reduction in the vertebral body height showed statistical significance, while the average loss of correction in the reduction of the vertebral body height was not significantly different. The measurement of the angles made by the screws on the rods was not significantly different between the cannulated (poly-axial head) and solid (mono-axial head) screw groups. Conclusions: Solid screws were superior in terms of providing increased correction of the kyphotic angle and height of the fractured vertebra than the cannulated screws; however, no difference was noted between the screws in the maintenance of the superior and inferior angles of the screw with the rod.

  • The Qiantang Medical School(錢塘醫派) formed on the basis of the education provided by L$\ddot{u}$shantang(侶山堂) in the specific area of Hangzhou(杭州) displays clear differences from other medical schools in that it combined medicine with educational and research activities. The Qiantang Medical School(錢塘醫派) played an important part in the history of the traditional Chinese medicine via providing the education through Confucian academies and the researches conducted to find appropriate treatment methods based on the climate and natural environment of the southern part of China. The tradition of joint researches on the classical works of Chinese medicine is equally important in terms of the traditional Chinese medicine.

Radiological Recurrence Patterns after Bevacizumab Treatment of Recurrent High-Grade Glioma: A Systematic Review and Meta-Analysis

  • Objective: To categorize the radiological patterns of recurrence after bevacizumab treatment and to derive the pooled proportions of patients with recurrent malignant glioma showing the different radiological patterns. Materials and Methods: A systematic literature search in the Ovid-MEDLINE and EMBASE databases was performed to identify studies reporting radiological recurrence patterns in patients with recurrent malignant glioma after bevacizumab treatment failure until April 10, 2019. The pooled proportions according to radiological recurrence patterns (geographically local versus non-local recurrence) and predominant tumor portions (enhancing tumor versus non-enhancing tumor) after bevacizumab treatment were calculated. Subgroup and meta-regression analyses were also performed. Results: The systematic review and meta-analysis included 17 articles. The pooled proportions were 38.3% (95% confidence interval [CI], 30.6-46.1%) for a geographical radiologic pattern of non-local recurrence and 34.2% (95% CI, 27.3-41.5%) for a non-enhancing tumor-predominant recurrence pattern. In the subgroup analysis, the pooled proportion of non-local recurrence in the patients treated with bevacizumab only was slightly higher than that in patients treated with the combination with cytotoxic chemotherapy (34.9% [95% CI, 22.8-49.4%] versus 22.5% [95% CI, 9.5-44.6%]). Conclusion: A substantial proportion of high-grade glioma patients show non-local or non-enhancing radiologic patterns of recurrence after bevacizumab treatment, which may provide insight into surrogate endpoints for treatment failure in clinical trials of recurrent high-grade glioma.

  • The entry of aging society and the coming of (super) aged society need overall the elderly welfare policy and budget for quantitative expansion and qualitative increase to the elderly welfare services. However, financial limit to the elderly welfare and increase to elderly welfare services in local government rely on central government or private sector. This study is discussed the gap between demands and supplies of the elderly welfare services in Busan Metropolitan City and policy implications suggested by these results. The major findings of the study are summarized as follows. First, the rate of the poor elderly under minimum living standard and the elderly housing facilities in demand of the elderly welfare services is reduced, but other measuring indicators are generally increased. Second, a per 10,000 elderly welfare service officials and a per 10,000 elderly medical welfare facilities in supply of the elderly welfare services are generally increased, but other measuring indicators are reduced. The policy implications of the study is to reduce the poor elderly under minimum living standard, to expend the elderly welfare budget, and adequately to supply a variety of the elderly welfare facilities for reduce demand of the elderly welfare services.

Patterns of recurrence after radiation therapy for high-risk neuroblastoma

  • Purpose: To investigate the patterns of recurrence in patients with neuroblastoma treated with radiation therapy to the primary tumor site. Materials and Methods: We retrospectively analyzed patients with high-risk neuroblastoma managed with definitive treatment with radiation therapy to the primary tumor site between January 2003 and June 2017. These patients underwent three-dimensional conformal radiation therapy or intensity-modulated radiation therapy. A total of 14-36 Gy was delivered to the planning target volume, which included the primary tumor bed and the selected metastatic site. The disease stage was determined according to the International Neuroblastoma Staging System (INSS). We evaluated the recurrence pattern (i.e., local or systemic), progression-free survival, and overall survival. Results: A total of 40 patients with high-risk neuroblastoma were included in this study. The median patient age was 4 years (range, 1 to 11 years). Thirty patients (75%) had INSS stage 4 neuroblastoma. At the median follow-up of 58 months, there were 6 cases of local recurrence and 10 cases of systemic recurrence. Among the 6 local failure cases, 4 relapsed adjacent to the radiation field. The other 2 relapsed in the radiation field (i.e., para-aortic and retroperitoneal areas). The main sites of distant metastasis were the bone, lymph nodes, and bone marrow. The 5-year progression-free survival was 70.9% and the 5-year overall survival was 74.3%. Conclusion: Radiation therapy directed at the primary tumor site provides good local control. It seems to be adequate for disease control in patients with high-risk neuroblastoma after chemotherapy and surgical resection.

  • Health care that is considered to be one of the major factors for the quality of life is nowadays receiving a great deal of attention, and thus there is a growing need in Korea to identify the efficiency of national medical service and enhance the competitiveness. Although there exist studies on the medical service efficiency about general hospitals and local hospitals, they mostly deal with the efficiency problems from a domestic and regional perspective. In response, this paper analyzes the competitive efficiency of national medical service with respect to 16 OECD countries, by exploiting Data Envelopment Analysis (DEA) and Malmquist Productivity Index (MPI). Building on the DEA and MPI analysis results, this paper identifies the competitive position of Korean national medical service and suggests implications for the medical service improvement.

Review of Literature on Chi Acupunture(Carboxytherapy) (경피기주입술(Carboxytherapy)에 대한 문헌적 고찰)

  • Objective : This study was conducted to offer basic materials for the clinical study of Chi acupuncture (carboxytherapy). Methods : Recent clinical studies were reviewed for carboxytherapy (carbon dioxide therapy) and the ancient and present texts were reviewed for Chi acupuncture. Results and Conclusions : Chi acupuncture is oriental medical therapeutic treatment using carboxytherapy. Chi acupuncture refers to the transcutaneous administration of CO2 gas for therapeutical purposes relating to obesity, cellulites, local circulation and skin irregularity. Recent studies have demonstrated the effects of this therapy on adipose tissue by showing vasomotor effects, increasing local circulation and inducing a partial increase in tcPO2. Chi acupuncture influences the microcirculation and likely has a positive effect upon the physiological oxidative lipolytic process.
