This study is aimed both to define a conception of Health Care Systems and to suggest Desirable Reorganization Directions in Korea. The Desirable Reorganization Directions of Health Care System in Korea is as follows ; 1. The Health Care System of Free Market System has to reorganize step by step for the Directions of National Health System. 2. The Health Care System has to reorganize with local socite as the center of local community. Especially, Health Sub-Center should be reorganized to provide Compresensive Health Care, so that the Sub-Center consist at least 15 members of health workers including a chief of governmental office. 3. The Health Care System has to reorganize for the Directions responding problems of the Elder, New Medical Technology Development, and Health Information System.
The purposes of this study were to investigate the perception of community nutrition programs and the conditions to carry them out, for 32 directors of health centers where public health nutritionists are not employed. The results of this study can be summarized as follows. Nutrition intervention programs were not carried out actively except those for infants but the health center directors strongly felt the necessity of carrying out various nutrition programs. The factors that affect the selecting and priotizing of nutrition programs were the need of community residents, the perception of the local congressmen, and the head of the local government. The most urgent and important problem to be solved in order to intervene nutrition programs was employing public health nutritionists on a tenure basis. Other were securing necessary revenue, precise guidance, political support, hardwares, etc. These results showed the importance of employing public health nutritionists for intervening appropriate nutrition programs and improving the perception of nutrition services for health center directors, local congressmen and personnels in charge of formulating nutrition policies.
Due to development of modern medical services and economics, people raised expectation and demand about medical services from previous disease treatment to comprehensive health care covering prevention and health care. Responses of each medical facility to these social needs and the evolution of concept of medicine rapidly occur. The health examination centers are being operated with the purpose of health examination and this trend is reflected on several aspects such as the size of the facilities, function and configuration of space in health examination centers. Thus, health examination centers consisting of various space systems appear, but this trend and interpretations are lacking. Therefore, the purpose of this study is to draw trends of system through analysis of types and its evolved space systematic analysis and establish it. Analysis targets were classified into small, medium and large groups by sizes based on number of space and a total of 12 health examination centers in four for each category were selected. As research methods, functional relationship of space was examined through analysis of type in which segmentalized type tools were applied in local units. The flow diagram was established based on direction turning point and was classified into sub-flow and main-flow in local units and the systems between local units were derived. Finally, the results of this study can be summarized as the following three results. 1) The space connection system of health examination center showed four systems such as circulation, independence, continuation, and network. 2) Local type indicators and global type indicators which were evolved more from limitation of type analysis tools in existing research were derived so that more systematic analysis could be made. 3) Network system is distributed approach system and space for each function is formed around public space.

The most common cause of treatment failure in oral cavity cancer is when it is found to have local recurrence, usually occurring in the ipsilateral cervical lymph node. On the contrary, it is extremely rare to find local recurrence in soft tissue metastasis (STM) in the contralateral neck. Furthermore, lung cancer and malignant lymphoma are most commonly confined to their primary sites. The poor general condition increases the likelihood of STM, which indicates bad prognosis. A 72-year-old man with a hard and fixed mass on the right submandibular space visited our clinic. He had received a wide excision with local flapreconstruction for squamous cell carcinoma in the left corner of lower lip 18 months ago. We performed the wide excision with bilateral selective neck dissection (I-III), and he was finally diagnosed as STM from contralateral lip cancer. We report this unique and rare disease entity with a literature review.
Recently, two main changes were observed in relation to activities of District Health Centers. One is the rapid increase in the quantity and variety of people's demands for health services due to improvement of income level and the other is an introduction of autonomy in local administration. Unitl recently, the District Health Services were mostly depended on the instructions and orders given by Central Governments, and it would be necessary to prepare for the future implementation of autonomous local administration. Especially, the urban health centers may need more attention than those in rural areas because of their complexity and it would be more difficult in choosing and implementing a most suitable health programs for urban communities. This study was made to find out a feasibl developmental model which could be applicable for the urban bealth centers in Kyeongnam- do area. The most studies relating to health centers activities in the past were made by saking questions to current health center workers, but this study has made an effort to find out the opinions of those who are representing the people in the community. For this study, therefore, the Members of Gity Parliament and Village Headmen from 10 cities is Kyeongnam- do were interviewed for the study and the results obtained were compared to those of health officers. It was found that both Parliament Members and Village Headmen were well aware of the need of health centers, however, they tend to put lower priority for health services in compare with other community activities, and pointed out poor quality of services and lack of public understanding as the problems to be overcome. As to the desirable body for policy decision and implementation of health programs, the Members of Local Parliament think the Local Parliament as ideal, where as Village Headmen prefer health experts, and health officenr prefer District Health Center respectively. The most urgent problems of the District health Centers are mentioned as lack of health manpower. As the results of his study, the followings are suggested for improvement of District Health Services; First : reinforcement of professional health workers, Second : establishment of Community Health Council, Third : strengthening of public health education and support, Fourth : flexibility in Local Autonomy and target system relating to health activities.
Objectives : This study investigates the determinants of contingent workers' ratio in public health centers. Since the economic crisis in 1997, there have been many studies on contingent workers in Korea. But, previous studies have been not conducted focusing on public health center. Methods : This study used 253 public health centers, installed and operated since December 31, 2008. in Korea as units of analysis. To examine the determinants of contingent workers' ratio, this study uses Pearson correlation and multiple regression analysis. Results : The following appeared as significant variable affecting contingent workers' ratio in public health centers; degree of the local government's financial independence(p<0.001), rate of increase/decrease in ages 65 and over(p<0.001), rate of increase/decrease in basic livelihood security recipients(p<0.01) and rate of increase/decrease in registered disabled persons(p<0.01). In contrast, internal organizational environment characteristics related variables were not statistically significant. Conclusions : Contingent workers' ratio in public health center is significantly affected by financial vulnerability of the local government and increase in demand of health care services.
Objective : The aim of this study was to examine the relationship between occupational stress and depression, as it is seen to be corresponding with the work characteristics of local government officials. Methods : The data of 671 local government officials who participated in this study from July to December 2015 as a part of an occupational stress management program, were analyzed retrospectively in this study. Likewise, the participants completed questionnaires, including a short form of the Korean Occupational Stress Scale (KOSS), and the Beck Depression Inventory-II (BDI-II) the results of which were evaluated and reviewed. Results : The study noted that the subscales of occupational stress related to depression were varied, according to the work characteristics of the participants, after adjusting for age and job grade. It was found that the participant's variables of job insecurity, lack of reward, organizational system and occupational climate were significantly related to the development of a depressed mood in the participant in a general sense. Notably, the factor of interpersonal conflict was significantly related to depressed mood among the general administrative group; meanwhile, lack of reward in the welfare group and insufficient job control in the technical group were significantly associated with the likelihood of developing a depressed mood in participants reviewed. In the operation and maintenance of facilities group, job insecurity was significantly related with depressed mood, whereas job demand was related more with the incidence of depression in the field work group. Conclusion : This study revealed that the influence of occupational stress on depression varied according to the occupational characteristics experienced by the participants in the work environment. These findings may be used to enhance the occupational stress management program for local government officials according to their work characteristics, to bring awareness to this issue.
Decentralization to local governments and amending of Health Center Law are to promote the efforts of health planning at the level of local agencies. In the health facility planning, it is important to take into account that what to be built, where to be located, how far should be service area and so forth, because health facilities are immovable, and require capital as well as personnel and consumable supplies. The aim of our study, answering to the question of 'where to be located?', is to determine the best location of urban health sub-center. At the local level, planning is the matter of finding the best location of specific facilitiy, in relation to population needs. We confine the accessibility, which is basic to location planning, to geographic one. Location-Allocation Model is used to solve the problem where the location is to maximize geographic accessibility. To minimize the weighted travel distance, objective function, $R_k=\sum{\sum}a_{ij}w_{i}d_{ij}$ is used. Distances are measured indirectly by map measure-meter with 1:25,000 Suwon map, and each potential sites, 10 administrative Dongs in Kwonson Gu, Suwon, are weighted by each number of households, total population, maternal age group, child age group, old age group, Relief for the livelihood, and population/primary health clinics. We find that Kuwoon-Dong, Seodun-Dong, Seryu3-Dong, according the descending orders, are best sites which can minimize the weighted distance, and conclude that it is reasonable to determine the location of urban health sub-center among those sites.
We investigated dissatisfaction of elements of local exhaust system, needs for local exhaust system, and information networks for local exhaust system from June 1998 to September 1999 using the questionnaire structured. It contained questions concerning general characteristics of factory and local exhaust system, troubles and dissatisfaction of elements of local exhaust system, and information networks for local exhaust system. The collected data were analyzed by descriptive statistics analysis. Information networks for local exhaust system were analyzed by multidimensional scaling using path distance of network analysis and by graph analysis using Krackplot. Among complaints of local exhaust system, that of duct has show the highest percentage of complaint. In the information network for local exhaust system, Seoul is positioned in the center of network with mediating role.
Objective : We proposed fundmental rules of prospective on legal and institutional position and role of Korean medicine doctors working at public health center. Methods : By the result of this research on the current situation, the grade and allowance given to the Korean medicine doctors working at public health center were different every self-governing body. Results : The reason the Korean Medicine Doctor can't serve as a regular order of 5th grade is that the 'The Enforcement Regulation about Administrative Organization and the Standard of Pixed Number of person of Self-Governing Body(지방자치단체의 행정기구와 정원기준등에 관한 규정 시행규칙)' prescribes the number of regular order of 5th grade is regulated within 7% among the number of regular order officials. But not appointing to office as the regular order of 5th grade infringes on the Constitution, the highest law. The reason the Korean Medicine Doctors can't be appointed to office as the regular order officials by the self-governing body is that 'The Enforcement Order of the Law of Preservation of good health of Local Area(지역보건법시행령)' prescribes the Korean Medicine Doctors are not indispensable to Public Health Center. But in fact, the Korean Medicine Doctors can execute many kinds of work such as medical examination or instructing house nursing. Conclusion : The Korean Medicine Doctors working at Public Health Center serve at low positions as daily use or common use, not receiving a regular order. All laws including the Constitution(헌법), the Medical Services Law(의료법), the Law of Preservation of good health of Local Area(지역보건법), the National Public Service Law(국가공무원법), the Local Public Service Law(지방공무원법) and the Law of Higher Education Law(고등교육법) describe that the Korean Medicine Doctors and the Western Medicine Doctors are equal to their position and right.
