간질환(肝疾患)과 혈청효소(血淸酵素) (Serum Enzyme in Hepatic Disease)

  • Liver is an oragan which has various functions. For this functions in liver disease change variously, many methods of examination of them are invented. Each liver disease impairs these functions diversly rather than uniformly, thus it is possible to differentiate each disease by using various methods of testing it. Liver has high reproductive capacity and despite partial impairment of its function localized disease may not show any abnormality in liver function test due to compensation of remaining intact tissue of the liver. Thus the result of liver function test should be interpreted under adequate clinical investigation with other test, not depending on the liver function test only. This thesis deals with the composition, distribution in living body and clinical significance of liberating enzyme, GOT, GPT, and LDH which are included in serum enzyme that show abnormality in liver disease.

산삼약침의 간 기능 개선효과에 대한 증례보고 (Case report on the Improvement of Liver Functions by Mountain Cultivated Wild Ginseng Pharmacopuncture)

  • Mountain cultivated wild ginseng(MCWG) pharmacopuncture, which is known to have anti-cancer and anti-oxidation actions, was administered for clinical usage and changes in manifestations were observed on three patients suffering from pain attributed from liver dysfunctions. In two cases of hepatitis due to poisoning by drugs and one case of liver cirrhosis, long-term administration of MCWG pharmacopuncture yielded significant changes in AST and ALT, which are important indices for liver functions. Improved liver functions after the administration of MCWG pharmacopuncture suggest a need for investigation on future usage of MCWG pharmacopuncture on liver dysfunctions, hepatitis, liver cirrhosis, or liver cancer. Further studies on treating aforementioned disorders by MCWG pharmacopuncture are needed immediately.

  • Lidocaine and galactose loading tests were performed on a bioartificial liver (BAL), an extracorporeal medical device incorporating living hepatocytes in a cartridge without a transport barrier across the membranes. The concentration changes were analyzed using pharmacokinetic equations to evaluate the efficacy and limitation of the proposed method. Lidocaine and galactose were found to be suitable drugs for a quantitative evaluation of the BAL functions, as they did not interact with the plasma proteins or blood vessels, making their concentrations easy to determine. The drug concentration changes after drug loading were easily analyzed using pharmacokinetic equations, and the BAL functions quantitatively expressed by pharmacokinetic parameters, such as the clearance (CL) and galactose elimination capacity (GEC). In addition, these two drugs have already been used in clinical tests to evaluate human liver functions over long periods, and lidocaine CL values and GEC values reported for a normal human liver. Thus, a comparison of the CL and GEC values for the BAL and a natural liver revealed what proportion of normal liver functions could be replaced by the BAL.

한.양약 복용이 간기능에 미치는 영향-피부과치료 한약복용 환자를 중심으로- (Effects of Liver Function in Blood of Drug Users(Herbal and Western) in Koreans)

  • We have conducted questionnaire and measured serum liver function tests on 153 patients whom have received a treatment at a local Oriental medicine clinic in Seoul from Sept. 1, 2002 to Dec. 31, 2002. Patients were categorized into the groups of control(10), decoction group(41), herbal group(5), western medicine group(45), and the group taking both herbal and western medicines(55) concurrently or separately. This study was conducted to investigate how these treatment methods can affect the liver functions. Following results were obtained : 1) Most of the research subjects were male(l03 individuals) living in large urban area, with drinking habit, highly educated, and with normal marriage. For the treatment history, 45 individuals received western medicine treatment(28.85%), 46 with Oriental medicine (29.49%), and 55 patients(35.26%) experienced both forms of treatment. 2) Measuring SD of serum liver functions for all the research subjects, T-protein was 6.95(0.73)g/dl, 4.07(0.51)g/dl for albumin, 0.44(0.19)mg/dl for T-bilirubin, 23.18(18.68)U/L for ALT, 27.42(27.43)U/L for AST, and 22l.27(64.58)mg/dl for LDH. 3) Among the research subjects with the liver functions outside of normal range were 0.64% for T-protein, 3.21% for T-bilirubin and ALT, 36.62% for AST, 0.64% for LDH, and none for Albumin. 4) Confusion variables which may affect the liver functions such as age, smoking, drinking, occupation, and residence were eliminated in calculation and no significant differences were witnessed between the control and experiment groups. 5) In measuring affects of treatment duration on the liver functions, no statistical significances were shown in multiple regression's ,${\beta}$(SE) values. 6) The values of odd ratio in the experiment groups compared to the control were in the range of $0.26(0.22){\sim}0.62(1.23)$. but T-bilirubin of the group receiving both Oriental and western medicine was 1.47(1.90) which showed statistical increase(p<0.001). 7) Factors affecting the liver functions are drinking volume per round duration of alcohol intake, western medicine treatment, gender, and age difference caused significant influences. 8) 57 individuals with serum liver functions out of normal range were mostly male, living in large urban area, median age of 38.89, with normal marriage, highly educated drinking less than once a week with less than 2 bottles per round, and with prior treatment in either western or Oriental medicine. Summarizing above results, we can carefully deduce that taking herbal medicine (both decoction and pills) or western medicine doesn't cause much affect in the liver functions. But taking two forms of medication simultaneously or altering dosage affect significant increase in the T-bilirubin level, thus one needs to take precaution when taking both. Further research is needed in this field to verify this evidence.

황함유 아미노산의 간기능 보호 작용: 간세포암 예방의 가능성 (Hepatoprotective Functions of Sulfur Containing Amino Acids: Possibilities of Hepatocellular Carcinoma Prevention)

  • While it is known that sulfur containing amino acids (SCAA) are very important in regulating hepatocyte growth and preventing liver-diseases, the fundamental molecular mechanisms of how they exert their hepatoprotective functions are not well known. Since it is widely understood that the hepatic concentrations of S-adenosylmethionine (SAMe) in chronic liver disease patients are severely decreased, the pathophysiological importance of SAMe and its downstream antioxidant, glutathione should be discussed in order to see a big picture of relationship between SCAA and liver diseases. Chronic SAMe deficient mice have shown spontaneous hepatocellular carcinoma development due to impaired mitochondria functions with low levels of prohibitin1 protein, and through deficiency in many genes which are known to ameliorate genetic instability, such as APEX1 and DUSP1, the functions of which are recovered by SAMe treatment. In this review, current knowledge of the basic concepts of the mechanisms through which SCAAs protect the liver will be discussed in detail. Also, a possible tumor suppressor in livers, prohibitin1, and its functional relationship with SAMe will be discussed.

HEPATOTOXICITY OF PHOTOTOXIC DRUGS USED IN SYSTEMIC PHOTOCHEMOTHERAPY

  • Psoralens are metabolized in the liver and thus can produce liver damage in laboratory animals when given in excessive doses. However, in humans, reports of the effects of psoralens on liver functions during photochemotherapy have been contradictory. We studied 311 patients t o observe the effects of various phototoxic drugs: 8-methoxypsoralen (8-MOP), 5-methoxypsoralen (5-MOP), 4,5,8-trimethylpsoralen (TMP)-on liver function during photochemotherapy. Of the 311 patients, only seven had transient elevations of transaminases. Incidence of hepatotoxicity of 8-MOP, TMP and 5-MOP showed 0.6%, 6% and 3%, respectively. Although the effects of psoralens on liver functions during photochemotherapy have been contradictory in humans and there have been few cases showing transient elevated values of liver transaminases during photochemotherapy, it is advisable to obtain serial liver function tests before and during photochemotherapy.

방사선에 조사된 쥐 간에서 블루베리의 방사선방호효과 (Radioprotective Effects of Blueberry on the Liver of Radiation Irradiated Rats)

  • 방사선은 간 기능에 심각한 장애를 준다. 블루베리는 Vit A, Vit C, Vit E, 엽산, ${\beta}$-카로티노이드 및 안토시아닌을 포함하고 있는 과일이다. 본 연구의 목적은 블루베리의 간 기능의 보호 효과를 평가하는 것이다. 4 Gy (Linac 6 Mev)를 X 선 장치를 사용하여 조사하였고, 각 실험군 당 7 마리로 쥐를 사용하였다. 간기능을 보기 위해 TP, ALB, GLOB, ALT, ALKP 그리고 CHOL 등을 측정하였다. 본 실험을 통해 블루베리가 방사선으로 인한 간 기능의 손상에 대해 방호효과가 없다는 것을 관찰 할 수 있었다. 그러나 통계적으로 ALB (P> 0.01)와 ALT (P> 0.1)는 유의성이 있다고 나타났다. 결론적으로, 블루베리는 방사선으로 인해 유발되어지는 간 기능의 손상에 대해 방호효과가 없다.

퍼지이론을 이용한 임상검사 자동분석에 관한 연구 - 간기능검사 결과 자동분석시스템 - (Automated Clinical best Result Analysis System - Application to liver function test -)

  • Automated system to analyze liver function test results is presented based on fuzzy logic knowledge. Clinician's knowledge and experience was first expressed in linguistic terms fol- lowed by conversion to numerical values to create membership functions of disease possibility for each test item and liver disease. Membership functions were then compensated for different relative importances of test items. Liver diseases considered were acute viral hepatitis (AVH), chronic persistent hepatitis(CPH), chronic active hepatitis(CAH), and liver cirrhosis(LC), Liver function test results of alanine aminotransferase(ALT), aspartate amino- transferase(AST) , glutamate dehydrogenase(GDH), ornithine carbamyltransferase(OCT) , ALT/AST, and 10* GDH/ALT in 218 patients were analyzed by the present system, welch resulted in 80% accuracy. AVH and CAH showed the highest 93 % and the lowest 58% ac- curacies, respectively, which was similar to the clinician's expectation. The simple mathemat- ical formulation of the present system would enable an easy implementation in commercial analysis instruments. Also, the identical fuzzy logic can be applied to similar diagnostic envi- ronments in general.

Development of Hepatocyte Spheroids Immobilization Technique Using Alternative Encapsulation Method

  • Primary hepatocytes of small animals such as rat and rabbit were often used for the study of extracorporeal liver support systems. Freshly isolated rat hepatocytes form spheroids within tow days when cultivated as suspension in spinner vessels. These spheroids showed enhanced liver specific functions and more differentiated morphology compared to hepatocytes cultured as monolayers However, shear stress caused by continuous agitation deteriorated spheroids gradually. In this work we immobilized spheroids to prolong liver specific activities. First, hepatocyte spheroids were suspended in collagen solution containing calcium chloride and then dropped into alginate solution. A thin layer of calcium alginate was formed around the droplet and then was removed after the inner collagen was gelled by treatment of sodium citrate buffer. Spheroids embedded in collagen-gel bead maintained liver specific functions such as albumin secretion rate longer than hepatocyte spheroids exposed to shear stress. Therefore, we suggest that this immobilization technique may offer an effective long-term hepatocyte cultivation and facilitase the development of a bioartificial liver support device.

Interpretation of the Five Viscera's Ascending Kidney-Water and Descending Heart-Yang

  • According to the principle of ascending water and descending fire, water has the property of wetting downward, which is the opposite of fire, which has the property of blazing upward. Thus, they work differently according to their innate properties. Nature and the human body maintain harmony through the interaction of ascending water and descending fire. When applied to the human body, the heart and kidney are the center of this principle. In other words, the heart above is the fire and the kidney downward is water. When the heart-fire harmonizes downward, the kidney becomes warm, enabling genuine vital functions to be active. When the kidney yin moves upward, the heart receives the nourishing yin to harmonize nutrients and blood. Thereby, physiological functions become normal throughout the blood meridians. However, in the ascending kidneywater and descending heart-yang of the heart and the kidney, the liver and lung are the major functional organs. In other words, the liver through the dispersing and raising yang functions moves water, which is the vital essence of the kidney, upward. And the lung, through the astriction?clearing of the lung and descending Qi?dispersing functions, moves the heart-fire downward. These functions are deeply related with changing seasons; thus, these functions can be explained with the ascending kidney-water and descending heart-yang of the five viscera.
