We treated a 53 year-old male patient. He had alcoholic liver cirrhosis and diabetes. The main symptoms were fatigue, ascites, edema, anorexia, indigestion, constipation, and abdominal pain. We diagnosed him as 'the retention of pathogenic damp-heat in the liver and gall bladder’ and adminstered Saenggangeonbi-tang three times a day. We observed clinical symptoms, liver function test(AST, ALT, ${\gamma}-GT$, ALP, total bilirubin), nutritional state(total protein, albumin) and capillary blood glucose. After treatment, all the main symptoms disappeared and the patient's nutritional state has improved. Child's grade using the improvement index in liver cirrhosis has changed from B to A. Although the very mechanism of Saenggangeonbi-tang was not proven, we could suggest that Saenggangeonbi-tang is not toxic to the liver and has a good effect on alcoholic liver cirrhosis with diabetes mellitus.
A child with five delay, five ankylosis, five weakening needs to take herbal medicine for a long term. So safety, efficacy inspection for the long term usage of herbal medicine and mixed dose is needed because other medication or functional health foods, etc could be taken during this process. Operated liver function test before and after dosing herbal medicine to one hundred and sixty children who aided private oriental medicine located in Seoul, and measured the efficacy by comparing sleeping hours, morbidity, stamina, sanguineness, personality, digestion with the state before taking herbal medicine based on questionnaire done by the parents. Every children were under the limits, and there were no differences between boys and girls, and no notable differences according to family histories. There were positive results in efficacy inspection ordered as improvement in personality, stamina, sanguineness, decrease in morbidity, increase in appetite, digestion. No signs of disorder in liver function has been found during the dose of herbal medicine or simultaneous medicine, and shown promotion of health.
Objectives : The aim of this study is to investigate the preventive effect of Puerariae Flos ethanol extract (PE) on methionin and choline deficient (MCD)-diet-induced Nonalcoholic fatty liver disease (NAFLD) in C57BL/6J mice. Methods : In the in vivo experiments, C57BL/6J mice were divided into 4 groups; Normal group, Control group, MCD+PE 100 group, and MCD+PE 300 group. After 4 weeks, body weight, liver weight, biochemical parameters for liver function test, histological changes, reverse transcription polymerase chain reaction (RT-PCR), and Western blot were assessed. Results : Mice lost body weight with the MCD-diet and the MCD+PE 100 group and MCD+PE 300 groups lost less than the control group, though showed no statistical significance. Liver weights were decreased by the MCD diet, but MCD+PE 300 groups were increased significantly. In the liver function test, all the values were decreased with the MCD-diet, MCD+PE 100 group and MCD+PE 300 groups were increased significance. In histological findings of the livers, MCD-diet induced severe fatty accumulation in the livers, but this fatty change was reduced in the MCD+PE 100 group and MCD+PE 300 groups was inhibited respectively. In lipid accumulation factors (such as SREBP-1c, $C/EBP{\alpha}$, PPAR-${\gamma}$), MCD+PE 100 group and MCD+PE 300 groups showed inhibitory effect on liver lipogenesis by reducing associated gene expressions caused by MCD diet. Conclusions : We were able to know that Puerariae Flos ethanol extract (PE) shown hepatoprotective effects via a decrease on the hepatic lipogenesis factors in the experimental NAFLD Models.
Objectives: The aim of this study was to report a clinical case of decompensated liver cirrhosis in a patient with jaundice who showed improvement with Korean medicine treatment. Methods: A patient with decompensated liver cirrhosis with jaundice was treated with herbal medicine (Injinoryung-tang, Galgeunjowi-tang), acupuncture, cupping, and moxibustion. Changes in jaundice, fatigue, and dysuria were measured with subjective parameters. Liver function was checked by performing a blood test 8 times. The levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), gamma glutamyl transpeptidase (GGT), total bilirubin (TB), albumin, and platelets (Plt), prothrombin time (PT), and international normalized ratio (INR) were recorded. Results: The Korean medicine treatment improved jaundice, fatigue, and dysuria and lowered the AST, ALT, ALP, GGT, and TB levels. Conclusions: Korean medicine treatment containing Galgeunjowi-tang positively improves clinical symptoms and liver function.
Purpose : The purpose of this study is to report the effect of oriental treatments to hyperemesis gravidarum in disturbed liver function. Methods : Hyperemesis gravidarum is a severe and intractable form of nausea and vomiting in pregnancy. It is a diagnosis of exclusion and may result in weight loss; nutritional deficiencies; and abnormalities in fluids, electrolyte levels, and acid-base balance. The peak incidence is at 4, 8-16 weeks of pregnancy. Interestingly, nausea and vomiting of pregnancy is generally associated with a lower rate of miscarriage. However disturbed diver dunction with Hyperemesis gravidarium is rare very and dangerous. Traditionally, oriental medical therapy has been used to patients with hyperemesis gravidarum and showed effective result. We treated 1 patient who had hyperemesis gravidarum in disturbed diver dunction at Se-Myung university Oriental Hospital in affiliation by Herbal medication, acupuncture and moxa therapy and got good result from them. Results : As a result, symptoms are remarkably alleviated and liver function test are improved. Conclusion : Therefore we would like to report that it is effective to cure the hyperemesis gravidarum in disturbed liver function through the herb-medication and acupuncture treatment.
Five hundred seventeen farmers(214 males and 303 females), over age 30 and living in Suh San County, Unsan township, were examined to determine the prevalence of peasants syndrome. Using the health interview questionnaire, the farmers were asked whether they had any of eight subjective symptoms during the past one month before the survey, October 1979. Those symptoms reported were scored based on a pre-determined point system. The farmers with high symptom scores(7 points or more) were further examined by the liver function test. In addition, all the sample(517 cases) were subjected to the examination of urinalysis and blood pressure. The liver function test is also undertaken for those farmers with high symptome scores(7 points or more) and those with abnormal findings in urinalysis and blood pressure. The results are summarized below: 1) 156 out of 517 farmers(30.2%) have had seven points or more of the symptom scores. Among the eight major symptoms, 72% of the farmers complained of lumbago. 2) The prevalence of peasant's syndrome were higher among females and the older age group. The symptom scores of this syndrome were slightly higher among women who have had many children (e. g. 4 or more). 3) The prevalence of peasant's syndrome was also related to the level of one's education-the lower education groups were found to have higher symptom scores of this syndrome. This may be due to the fact that the lower education groups are more exposed to manual labor. 4) The points of the peasants syndrome were higher in hypertensive farmers and those with abnormal findings of urinaly sis. 5) It was reported, in Japan, that those with high scores of the syndrome are more likely to have an abnormal findings in the liver function test. However, in this study it was observed that there was no significant difference between those with high scores of the syndrome (7 points or more) and those with low score of the syndrome(6 points or less) in the prevalence of abnormal liver function. As this study indicates that the point system given for the peasant's syndrome is closely related to the high bleod pressure and abnormal urinalysis(and to some extent to the abnormal liver function), there is a need to study peasant's syndrome more extensively. Such an effort will serve to detect varonic disease conditions among farmer's towards better management of farmer's health, which is vitally important for rural development in Korea.
Objectives: This study investigated the effect of SM-2015 on blood alcohol clearance and hangover. We undertook this study to test whether SM-2015 is effective in decreasing blood alcohol concentration and preventing the symptoms of alcohol-induced hangover. Methods: Twenty healthy volunteers participated in this randomized crossover study. All participants were classified between an SM-2015 intake group (test group) and a non-intake group (control group). The primary outcome measure was the difference in blood alcohol concentration and hangover severity scores between the test and control groups. The secondary outcome measure was the difference in a liver function test (LFT) between the test and control groups. Results: After alcohol exposure, the sensitivity scores of blood alcohol concentration and hangover symptoms (sleepiness, dizziness, nausea, weakness, stomach pain, diarrhea, and concentration disorder) were significantly decreased in the SM-2015 intake group compared with the non-intake group. There were no differences in the LFT results between the SM-2015 intake group and the non-intake group. Conclusions: SM-2015 is effective in decreasing blood alcohol concentration and preventing the symptoms of alcohol-induced hangover. Larger studies are required to confirm these findings.

Methods : A/J mice were divided into 4 groups: Normal group (normal diet without any treatment). Control group (MCD diet only), YJCGT group (MCD diet with YJCGT), and YJSRS group (MCD diet with YJSRS). After 5 weeks, body weight, liver weight, biochemical parameters for liver function test, histological changes, and real-time PCR were assessed. Results : Mice lost body weight with the MCD diet and the YJCGT and YJSRS groups lost less than the control group, though showed no statistical significance. Liver weights were decreased by the MCD diet, but not significantly. In the liver function test, all the values were increased with the MCD diet, though some did not show significance. Alp and ALT levels were significantly less increased by YJCGT compared to the normal (p<0.05). All values were decreased or increased compared to the control by treatment though showed no significance possibly due to insufficient sample numbers. In histological findings of the livers. MCD-diet induced severe fatty change and collagen accumulation in the livers, but this fatty change was reduced in the YJCGT and YJYRS groups and fibrogenesis was inhibited significantly with p<0.05 and p<0.01, respectively. In real-time PCR analysis, YJCGT and YJYRS showed inhibitory effect on liver fibrogenesis by reducing associated gene expressions caused by MCD diet. Conclusion : YJCGT and YJSRS are considered to be possible candidates for the treatment of patients with NASH and/or liver fibrosis.
간 섬유화는 임상에서 흔히 접하는 질환으로, 간세포 염증 및 괴사가 수년간 지속 및 반복되는 질환으로 간경변이 진행된다. 따라서, 만성 간질환 환자에서 간경변 및 간세포암으로의 진행을 막음으로써 간경변의 합병증 및 간세포암에 의한 사망률 및 유병률을 감소시키는 것이 주요한 임상 과제라 할 수 있다. 이에 본 연구에서는 미만성 간질환 환자에게서 초음파 영상, 혈액검사, 간탄력도와의 상관관계를 수치화 하고자 하였다. 본 연구에서는 P사의 초음파 진단기 (IU-22)로 영상을 획득한 환자와 Fibrosccan을 시행 받은 141명을 대상으로 시행하였고, 혈액 검사는 초음파 영상과 간섬유 스캔을 시행한 시점의 검사 결과로 기초자료를 획득하였다. 각 자료에 대한 통계 분석은 집단 간 차이 검증을 위해 일원분산분석(One-way, ANOVA)을 시행하여 검증하였다. 탄성 초음파의 수치는 정상, 만성간질환, 간경변 순으로 증가하였다. 판독 결과에 따라 ALT, Albumin은 대상군 간 통계학적 차이를 보이지 않았으며, 나이, AST, ALP, Bilirubin, PLT, PT, kPa에서 초음파 판독 결과에 따라서 차이가 있고, 통계적으로 유의(p<0.05)하였다. 그리고 다른 연구에서 만성 간질환 탄성 초음파 수치값이 12.5kPa 이상이라고만 제시하였으나, 본 연구에서는 질환별 평균 kPa임계값을 제시하여, 정량적으로 진단이 가능하게 수치화를 하였다. 또한, 진단결과의 상관관계를 제시하여 질환별 만성 간질환 환자의 진단에 일차적인 도구로 사용될 수 있으리라 사료된다.
