지역 모바일 앱 1인 창조기업의 성공요인에 관한 실증분석 : 사회적 자본의 조절효과를 중심으로 (An Empirical Study on Successful Factor of Local Mobile App One-Person Creating Company : The Moderating Effects of Social Capital)

  • 대한민국은 실물경제에서 지식경제로, 그리고 창의성과 상상력을 중심으로 하는 창조경제로 패러다임이 변화하고 있다. 창조경제의 핵심인 창조산업은 기술과 정보를 갖춘 개인이 중요한 역할을 수행하는 산업이다. 대한민국의 높은 청년실업률과 경제 양극화 문제를 해결하는 창조산업 중 모바일 앱 산업의 역할을 인식하고, 범국가적인 차원에서 모바일 앱 산업 분야의 1인 창조기업을 지원하는 다양한 정책이 추진되고 있다. 이러한 지원 정책이 모바일 앱 1인 창조기업의 성공적인 창업에 기여할 수 있도록 지역 모바일 앱 1인 창조기업을 대상으로 성공요인에 대한 실증조사를 수행하고 이를 통해 보다 효과적이고 효율적인 지원 방안을 모색하고자 본 연구를 수행하였다. 이 연구에서는 문헌 고찰을 통해 1인 창조기업의 성공요인에 관한 4개의 연구가설을 도출하고, 지역 모바일 앱 1인 창조기업의 실증자료를 기반으로 분석을 하였다. 분석 결과 첫째, 모바일 앱 관련 기술의 발전 속도가 빠르고 이에 따른 새로운 경쟁 제품이 출현할 경우 기업의 매출액과 성장성이 높아지는 경향을 발견할 수 있었다. 둘째, 창업자가 산업 경험을 갖추고 기업 경영과 제품 개발에 필요한 자금을 확보할 경우 정량적인 성과가 높아지는 경향을 발견할 수 있었다. 셋째, 목표 시장에 맞춘 경영 전략을 수행할 경우 성장성이 높아지는 경향을 발견할 수 있었다. 넷째, 이들 환경 동태성, 기업 특성, 경영 전략과 사업 성과의 관계에 네트워크의 강도와 신뢰도가 조절효과로 일부 작용하는 것을 발견할 수 있었다. 이러한 결과는 1인 창조기업의 창업가에게 경험을 축적할 수 있는 방안과 함께 필요한 자금을 적시에 제공할 수 있어야 하고, 다양하고 신뢰할 수 있는 네트워크와 만날 수 있는 기회를 지속적으로 제공해야함을 시사한다. 본 연구를 통해 모바일 앱 분야와 같은 지식 기반 1인 창조기업들을 지원하는 기관에서 기업의 성공 가능성을 높일 수 있는 지원 시책을 수립하는데 활용되길 바란다.

지역창업 활성화를 위한 청년창업 애로 요인에 관한 연구 (A Study on Difficulty Factors of Youth Startups for Activating Local Startups)

  • 중앙정부는 일자리 창출을 위한 중요한 대안으로 청년 창업지원 정책을 확대하고 있다. 이에 지방정부에서도 청년창업 활성화를 위한 많은 노력에도 불구하고 창업에 다양한 애로요인이 존재한다. 이에 본 연구에서는 지역 청년 창업가들이 현장에서 느끼는 실질적인 애로요인이 무엇인지를 파악하였다. 그리고 지역 청년창업의 애로 요인을 극복하기 위한 대안과 청년창업 활성화를 위한 정책적 시사점을 도출하는데 목적이 있다. 따라서 선행연구 참고, 문헌 조사, 브레인스토밍을 통해 청년창업의 애로사항에 대한 평가요인 5개 항목을 도출하고, 세부 애로요인으로 25개 항목을 도출하였다. 주요 애로 평가요인을 AHP 기법을 활용해 군산지역 청년(19~39세), 남녀, 계층별(대학생, 예비창업자, 기창업자)로 실증분석을 진행하였다. 분석결과 지역 청년들이 느끼는 창업 애로요인은 비즈니스 모델, 경영관리, 창업자금, 창업제도개선, 창업 인식개선 순으로 나타났다. 무엇보다 지역의 청년들은 창업자금보다 자신의 아이디어를 비즈니스 모델 사업화 과정에서 많은 어려움을 느끼고 있었다. 세부요인으로 시장정보획득, 기술사업화, 사업 타당성, 기술개발, 판로개척 순으로 25개 창업 애로 요인이 도출되었다. 성별 비교에서 남성은 여성보다 비즈니스 모델 사업화에서 상대적으로 높은 애로요인이었다. 반면 여성은 비즈니스 모델 애로 요인을 제외한 모든(경영관리, 창업자금, 창업 제도개선, 창업인식개선) 요인에서 남성보다 높게 나타났다. 또한, 청년 계층(대학생, 예비창업자, 기창업자)별로 창업 애로요인이 상대적으로 다르게 나타났다. 결론적으로 지역에서 청년창업 활성화를 위해서 창업자금보다 비즈니스 모델 사업화 애로요인을 적극적으로 해결할 필요가 있다는 시사점을 도출하였다. 또한 일반적인 해결책 제시가 아닌 성별, 계층별 애로요인이 다르기 때문에 맞춤형 창업지원 및 상황별 행정서비스를 전략적으로 지원해야 한다. 본 연구는 지역 청년들이 직면한 창업 애로 요인의 실질적인 우선순위와 도출방법을 제시하였고, 향후 지역 청년창업 활성화를 위한 방안과 개선점을 제시하였다.

한국의 그린 비즈니스/IT 실태분석을 통한 추진전략 우선순위 도출에 관한 연구 (Development of Korean Green Business/IT Strategies Based on Priority Analysis)

  • Recently, the CO2 emission and energy consumption have become critical global issues to decide the future of nations. Especially, the spread of IT products and the increased use of internet and web applications result in the energy consumption and CO2 emission of IT industry though information technologies drive global economic growth. EU, the United States, Japan and other developed countries are using IT related environmental regulations such as WEEE(Waste Electrical and Electronic Equipment), RoHS(Restriction of the use of Certain Hazardous Substance), REACH(Registration, Evaluation, Authorization and Restriction of CHemicals) and EuP(Energy using Product), and have established systematic green business/IT strategies to enhance the competitiveness of IT industry. For example, the Japan government proposed the "Green IT initiative" for being compatible with economic growth and environmental protection. Not only energy saving technologies but energy saving systems have been developed for accomplishing sustainable development. Korea's CO2 emission and energy consumption continuously have grown at comparatively high rates. They are related to its industrial structure depending on high energy-consuming industries such as iron and steel Industry, automotive industry, shipbuilding industry, semiconductor industry, and so on. In particular, export proportion of IT manufacturing is quite high in Korea. For example, the global market share of the semiconductor such as DRAM was about 80% in 2008. Accordingly, Korea needs to establish a systematic strategy to respond to the global environmental regulations and to maintain competitiveness in the IT industry. However, green competitiveness of Korea ranked 11th among 15 major countries and R&D budget for green technology is not large enough to develop energy-saving technologies for infrastructure and value chain of low-carbon society though that grows at high rates. Moreover, there are no concrete action plans in Korea. This research aims to deduce the priorities of the Korean green business/IT strategies to use multi attribute weighted average method. We selected a panel of 19 experts who work at the green business related firms such as HP, IBM, Fujitsu and so on, and selected six assessment indices such as the urgency of the technology development, the technology gap between Korea and the developed countries, the effect of import substitution, the spillover effect of technology, the market growth, and the export potential of the package or stand-alone products by existing literature review. We submitted questionnaires at approximately weekly intervals to them for priorities of the green business/IT strategies. The strategies broadly classify as follows. The first strategy which consists of the green business/IT policy and standardization, process and performance management and IT industry and legislative alignment relates to government's role in the green economy. The second strategy relates to IT to support environment sustainability such as the travel and ways of working management, printer output and recycling, intelligent building, printer rationalization and collaboration and connectivity. The last strategy relates to green IT systems, services and usage such as the data center consolidation and energy management, hardware recycle decommission, server and storage virtualization, device power management, and service supplier management. All the questionnaires were assessed via a five-point Likert scale ranging from "very little" to "very large." Our findings show that the IT to support environment sustainability is prior to the other strategies. In detail, the green business /IT policy and standardization is the most important in the government's role. The strategies of intelligent building and the travel and ways of working management are prior to the others for supporting environment sustainability. Finally, the strategies for the data center consolidation and energy management and server and storage virtualization have the huge influence for green IT systems, services and usage This research results the following implications. The amount of energy consumption and CO2 emissions of IT equipment including electrical business equipment will need to be clearly indicated in order to manage the effect of green business/IT strategy. And it is necessary to develop tools that measure the performance of green business/IT by each step. Additionally, intelligent building could grow up in energy-saving, growth of low carbon and related industries together. It is necessary to expand the affect of virtualization though adjusting and controlling the relationship between the management teams.

가상 공동체 사용자의 전자상거래 수용에 대한 연구 (A Study on EC Acceptance of Virtual Community Users)

  • Virtual community(VC) will increasingly be organized as commercial enterprises, with the objective of earning an attractive financial return by providing members with valuable resources and environment. For example, Cyworld.com in Korea uses several community services to enable customers of Cyworld to take control of their own value as potential purchasers of products and services. Although initial adoption is important for online network service success, it does not necessarily result in the desired managerial performance unless the initial usage is continuously related to the continuous usage and purchase. Particularly, the customer who receives relevant online services and is well equipped with online network services, will trust the online service provider and perceive less risk and experience more activities such as continuous usage and purchase. Thus, how to promote continued online service usage or, alternatively, how to prevent discontinuance is a critical issue for VC service providers to consider. By aggregating a wide range of information and online environments for customers and providing trust to its members, the service providers of virtual communities help to reduce the perceived risk of continuous usage and purchase. Drill down, online service managers realize that achieving strong and sustained customers who continuously use online service and purchase on it is crucial. Therefore, the research into this online service continuance will identify the relationship between the initial usage and the continuous usage and purchase. The research of continuous usage or post adoption has recently emerged as an important issue in the IS literature. Individuals' information systems(IS) continuous usage decisions are congruent with consumers' repeat purchase decisions. The TAM(Technology Acceptance Model) paradigm has been strongly confirmed across a wide range from product purchase on EC to online service usage contexts. The analysis of IS usage based on TAM has proven to be successful across almost online service contexts. However, most of previous studies have focused on only an area (i.e., VC or EC). Just little research has tried to analyze the relationship between VC and EC. The effect of some factors on user intention, captured through several theories such as TAM, has been demonstrated. Yet, few studies have explored the salient relationships of VC users' EC acceptance. To fill this gap between VC and EC research, this paper attempts to develop a research model that extends the TAM perspective in view of the additional contributions of trust in the service provider and trust in members on some factors that affect EC and VC adoption. In this extension, we applied the TAM-to-TAM(T2T) model, and analyzed the transfer effect of trust between these two TAMs. The research model was empirically tested on the context of a social network service. The model was to extend TAM with the trust concept for the virtual community environment from the perspective of tasks. By building an extended model of TAM and examining the relationships between trust and the existing variables of TAM, it is aimed to explain a user's continuous intention to use VC and purchase on EC. The unit of analysis in this paper is an individual user of a virtual community. The population of interest is the individual with the experiences in virtual community. The data for this paper was made available via a Web survey of VC users. In total, 281 cases were gathered for about one week, but there were some missing values in the sample and there were some inappropriate cases. Thus, only 248 cases were finally analyzed. We chose the structural equation analysis to test the hypotheses and it is better suited for explaining complex relationships than the other methods. In this test, AMOS was used to test the Structural Equation Model (SEM). Noticeable results have been found in the T2T model regarding the factors affecting the intention to use of virtual community and loyalty. Our result showed that trust transfer plays a key role in forming the two adoption beliefs. Overall, this study preliminarily confirms the salience of trust transfer in online service.

검색 효과성에 영향을 미치는 시맨틱웹 검색시스템 품질요인에 관한 연구 (Quality Dimensions Affecting the Effectiveness of a Semantic-Web Search Engine)

  • This paper empirically examines factors that potentially influence the success of a Web-based semantic search engine. A research model has been proposed that shows the impact of quality-related factors upon the effectiveness of a semantic search engine, based on DeLone and McLean's(2003) information systems success model. An empirical study has been conducted to test hypotheses formulated around the research model, and statistical methods were applied to analyze gathered data and draw conclusions. Implications for academics and practitioners are offered based on the findings of the study. The proposed model includes three quality dimensions of a Web-based semantic search engine-namely, information quality, system quality and service quality. These three dimensions each have measures designed to collectively assess the respective dimension. The model is intended to examine the relationship between measures of these quality dimensions and measures of two dependent constructs, including individuals' net benefit and user satisfaction. Individuals' net benefit was measured by the extent to which the user's information needs were adequately met, whereas user satisfaction was measured by a combination of the perceived satisfaction with search results and the perceived satisfaction with the overall system. A total of 23 hypotheses have been formulated around the model, and a questionnaire survey has been conducted using a functional semantic search website created by KT and Hakia, so as to collect data to validate the model. Copies of a questionnaire form were handed out in person to 160 research associates and employees working in the area of designing and developing semantic search engines. Those who received the form, 148 respondents returned valid responses. The survey form asked respondents to use the given website to answer questions concerning the system. The results of the empirical study have indicated that, of the three quality dimensions, information quality was found to have the strongest association with the effectiveness of a Web-based semantic search engine. This finding is consistent with the observation in the literature that the aspects of the information quality should serve as a basis for evaluating the search outcomes from a semantic search engine. Measures under the information quality dimension that have a positive effect on informational gratification and user satisfaction were found to be recall and currency. Under the system quality dimension, response time and interactivity, were positively related to informational gratification. On the other hand, only one measure under the service quality dimension, reliability was found to have a positive relationship with user satisfaction. The results were based on the seven hypotheses that have been accepted. One may wonder why 15 out of the 23 hypotheses have been rejected and question the theoretical soundness of the model. However, the correlations between independent variables and dependent variables came out to be fairly high. This suggests that the structural equation model yielded results inconsistent with those of coefficient analysis, because the structural equation model intends to examine the relationship among independent variables as well as the relationship between independent variables and dependent variables. The findings offer some useful implications for owners of a semantic search engine, as far as the design and maintenance of the website is concerned. First, the system should be designed to respond to the user's query as fast as possible. Also it should be designed to support the search process by recommending, revising, and choosing a search query, so as to maximize users' interactions with the system. Second, the system should present search results with maximum recall and currency to effectively meet the users' expectations. Third, it should be capable of providing online services in a reliable and trustworthy manner. Finally, effective increase in user satisfaction requires the improvement of quality factors associated with a semantic search engine, which would in turn help increase the informational gratification for users. The proposed model can serve as a useful framework for measuring the success of a Web-based semantic search engine. Applying the search engine success framework to the measurement of search engine effectiveness has the potential to provide an outline of what areas of a semantic search engine needs improvement, in order to better meet information needs of users. Further research will be needed to make this idea a reality.

뇌졸중 환자를 대상으로 한 병원 환경에서의 작업 기반 양측성 상지 훈련 프로토콜 개발 (Development of a Occupation-Based Bilateral Upper Extremity Training Protocol in a Medical Setting for Stroke Patients)

  • 목적 : 임상 적용에 있어 효율적 적용이 가능한 작업 기반 양측성 상지 훈련을 위해 작업기반 중재의 활동 선정과 양측성 상지훈련의 주요 전략을 기본으로 한 프로토콜을 개발하는 것이었다. 연구 방법 : 델파이 기법을 활용한 연구의 절차는 총 3단계의 연구 과정으로 진행하였다. 첫 단계는 개방형 설문 개발 단계로 문헌고찰을 통해 1차 초안을 마련하고 초안을 바탕으로 한 예비연구를 통해 개방형 질문을 개발하였다. 두 번째 단계는 델파이 조사로, 개방형 델파이 설문을 통해 얻은 전문가들의 응답을 바탕으로 훈련 프로토콜로서의 적절성을 리커트 5점 척도로 제시하였다. 설문조사를 통해 얻은 평균 및 표준편차, 안정도, 수렴도, 합의도, 내용 타당도 비율 분석을 통해 항목의 수정 및 삭제 등의 과정을 거쳐 항목을 도출하여 재구성하여 실시하였다. 세 번째 단계는 프로토콜의 완성 단계로 연구자 간 토의를 거쳐 최종 도출된 프로토콜 내용에 대하여 재 조직화를 통해 병원 기반의 작업 기반 양측성 상지 훈련 프로토콜을 완성하였다. 결과 : 작업 기반 중재 선정 영역에서 3영역, 9항목, 양측성 상지 훈련 영역에서 4영역, 81항목의 프로토콜이 완성되었다. 결론 : 본 연구를 통해 병원 기반으로 이루어지고 있는 현재 상황에서 작업 기반 활동을 중재로 사용할 수 있도록 작업치료 전문가들의 의견을 종합한 근거 기반의 방법을 제시하며, 양측성 상지 훈련 또한 구체적인 방법 제시를 통해 임상에서의 효율적으로 적용할 수 있도록 할 수 있다는 점에서 의미가 있다.

기름의 양과 지지는 시간에 따른 화전의 관능적 및 물리적 특성 (Sensory and Physical Characteristics of Hwajeun Depended on the Various Levels of Oil Amounts and frying Time)

  • 사용하는 기름의 양과 지지는 시간을 달리하여 제조한 화전의 기계적 특성 및 관능적 특성을 통해 품질 평가를 하였다. 기름 사용량을 달리한 경우, 수분 함량은 기름의 양이 증가함에 따라 유의적(p<0.001)으로 감소하는 것으로 나타났으며. 지지는 시간을 달리한 경우 가장 지짐 시간이 길었던 S7의 수분함량이 47.59%로 다른 처리구에 비하여 유의 적으로 낮은 값을 보였다(p<0.01). 이것은 지짐시간이 길어짐에 따른 수분 손실에 의한 것으로 생각되었다. 흡유율은 기름 사용량을 달리한 경우, S1이 87.5%로 다른 두 처리구에 비하여 유의적(p<0.001)으로 가장 높은 값을 나타내었고 지지는 시간을 달리한 경우 지짐 시간이 길어짐에 따라 흡유율도 유의적(p<0.001)으로 높아짐을 알 수 있었다. 색도는 기름의 첨가량이 16g으로 가장 많은 S3의 L값이 낮고 b값은 13.9로서 높게 나타나 기름량이 많아질수록 화전을 지질 때 충분한 열전도가 이루어져 골고루 잘 지져졌기 때문인 것으로 생각되었다. 한편, 지짐 시간을 달리하여 만든 화전의 색도변화는 다른 두 처리구에 비하여 S6과 S7이 전반적으로 b값이 높은 것은 지지는 시간이 길어짐에 따라 잘 지져졌기 때문인 것으로 생각되었다. Two bite compression test에 의한 텍스쳐 특성치를 살펴보면, 기름 사용량에 따른 처리구의 경우 Hardness는 기름의 사용량이 가장 적은 S1이 1484.6으로 유의적으로(p<0.001) 높은 값을 나타내었고 지짐시간에 따라서는 Hardness가 지짐시간 2분까지는 유의적으로 그 값이 감소하다가 지짐 시간 2분 30초인 S7은 다시 값이 증가하였다. 관능검사 결과에서는 항목별로 유의적인 차이를 나타냈는데, 기름의 사용량에 따른 처리구에서는 S3가 기름이 충분하여 색이 노릇노릇하게 잘 나타났고 적당한 기름의 향미와 부드러우면서도 바삭바삭한 질감으로서 선호도가 가장 높게 나타났다. 한편, 지짐 시간을 달리한 처리구의 경우, S6가 color, flavor, chewiness 및 taste 등의 도든 항목에서 유의적으로 높은 값을 보여 전반적인 기호도 7.2(p<0.01)로 가장 관능적 품질이 우수한 것으로 나타났다. 따라서 기름의 양은 화전 10개 분량(200g) 기준으로 16g을 사용하고 지짐시간은 앞, 뒷면 각각 2분씩 지지는 것이 바람직할 것으로 보인다.

베이지안 네트워크와 AHP (Analytic Hierarchy Process)를 활용한 쉴드 TBM 터널 리스크 분석 (Overall risk analysis of shield TBM tunnelling using Bayesian Networks (BN) and Analytic Hierarchy Process (AHP))

  • 본 논문에서는 쉴드 TBM 시공 시 발생 가능한 사건 및 원인의 규명, 리스크 발생의 인과관계 규명, 리스크의 위험도 판별, 리스크의 저감대책 제시를 통한 쉴드 TBM의 전반적인 시공 리스크 관리에 대한 연구를 수행하였다. 이를 위해서 쉴드 TBM의 사고 사례에 대한 문헌조사, 설계 및 시공 전문가 인터뷰를 수행하였다. 리스크 사건은 절삭량 저하, 막장면 붕괴, 지반 침하, 지반 융기, 이수 분출, 배토 불능, 굴착 불가, 지하수 누수의 8개의 그룹으로 나뉘어졌다. 리스크의 원인은 지질 원인, 설계 원인, 시공관리 원인의 3가지 그룹으로 나뉘어졌다. 리스크 원인과 사건간의 인과관계를 체계적으로 분석하기 위하여 베이지안 네트워크를 이용한 도식적인 관계도를 작성하였다. 리스크의 위험도를 산정하기 위하여 리스크가 발생하였을 때 이를 복구하기 위한 다운타임 및 비용을 기준으로 전문가를 대상으로 Analytic Hierarchy Process (AHP)를 수행하였으며, 위험도 결과에 기반하여 리스크 대응단계를 제시하고 이를 실제 리스크 발생사례와 비교하여 검증하였다. 또한 발생 가능한 리스크에 대응하기 위하여 설계 및 시공단계에서의 리스크 저감대책을 제안하였다. 제안된 연구는 TBM 설계자 및 시공자가 현장의 조건을 고려하여 리스크 원인을 선정하고 이로 인해 발생 가능한 리스크를 체계적으로 분석하여 파악할 수 있게 해주며, 리스크의 위험도의 판별 및 그에 대한 설계 및 시공단계에서의 저감대책을 통해 체계적인 쉴드 TBM 리스크 관리에 도움을 줄 수 있다.

조산사의 라마즈 산전교육에 대한 중요성 인식과 수행정도 및 저해요인에 관한 연구 (Midwives' Perceptions of the Importance of Teaching the Lamaze Method of Childbirth Preparation, Their Practice of it and Inhibiting Factors.)

  • This study was done to investigate how importantly midwives thought of Lamaze Method of Childbirth, how satisfactorily they performed their Lamaze Method of Childbirth and what inhibited their activities from being performed if not done satisfactorily. The subjects of this study were 76midwives at midwifery in Pusan, Deagu, Kyungnam and Kyungbok. Data were collected by using questionaire from Sep. 1 to Sep.30, 1991. The measurement tools was maded by the researcher based on Kim's scale for prenatal nursing activities. The statistical analysis was done with based statistical values likes frequencies, means, standard deviations and advanced methods such as Pearson's correlation coefficient, 1-test, F-test, F-test by using SPSS package program. The results of this study can be summarized as follows ; 1. The results of Investigating perception according to importance degree and performance degree of Lamaze Method of Childbring showed that midwives were slightly below both perception according to importance(2.17) and performance(2.16) of Lamaze Method of Childbirth 2. There appears to be a positive relation between perception according to importance degree and performance degree of Lamaze Method of Childbirth (r=0.69, P<0.01). There appears to be a negative relation between perception according to importance degree and inhibit factors of performance (r=-0.38, P<0.01). There appears to be a negative relation between performance and inhibit factors of performance (r=-0.22. P<0.01). 4. The item of highest perception defree in prenatal education is 1st, Consultation about sexual life (2.53), End, illustration of the importance of a bath and perinal cleanliness and Avoidance of excessive respiration method and also its comfortable application during uterine contractility(2.45). The lowest one is leaching the pregnant woman and her family about the role of husband and her family and also teaching the couple to learn exercise and respiration method (1.84). Though importance perception degree and performance degree is low. Midwives who wert subject in this research shoves that they perform general items concerning prenatal education well. 5. In the relationship between perception according to importance degree of Lamaze Method of Childbirth and the general characteristics of subjects, the differences in mean number of visiting pregnacies for 1 month. 6. In the relationship between performance degree of Lamaze Method of Childbirth and the general characteristics of subjects, the differences in age, marriage, and mean number of visiting pregnancies for 1 month. With the above information we see the importance of midwife prenatal care education and high correlation between performance degree and perception degree. When Lamaze prenatal education is well performed the number of pregnant woman who have follow-up, check-ups increase. Therefore in this research we can validity that there is a relation between Lamaze prenatal education and patient follow-up. This research showes in a situation where if one does not maximally perform a prenatal education there is a possibility that the popular use of midwife activities may encounter obstacles so the education to save new knowledge and training for prenatal education is needed as a function of Lamaze prenatal education, when a special Lamaze education is well performed for the pregnant woman, who follows-up at amidwife clinic. With the above conclusion we can suggest : 1. Continued research which minimized obstacles to Lamaze prenatal education is needed. 2. The official method of midwives is necessary.

자궁적출술 환자를 위한 critical pathway 개발과 적용효과 (Critical Pathway Development for the Hysterectomy Patients and its applied Effect)

  • At present in the medical care, the study and effort for producing health service to consider efficiency, effectiveness, and quality are urgently called for because of the difficulty in the keen competition according to the inter- nationalization and opening, the operation in the medical institution service testing system, the change in the medical policy of KDRGs, and the lack of the health care cost increasing rate. As an alternative, the case management for the new management system is introduced in the U.S., and the Critical Pathway that is the method designing the contents of activity and its result has been developed and applied in order to anticipate and manage the patient-outcome for the realization of the cost-effective case-management. Thus, this study intended to analyze the effectiveness to obtain by developing the Critical Pathway presented as the method to improve the quality-betterment and cost effectiveness through the continuous and consistent patient management for the hysterectomy patient and applying it to the real practice. As a study method, this author formed a conceptual framework through considering five Critical Pathway used in the current U.S. and three Critical Pathway presented in the literature to develop the Critical Pathway for the hysterectomy patient, and made out the preliminary Critical Pathway through reviewing the old chart. This author made the verified the validity of the expert group about the developed Critical Pathway, and to confirm the possibility of practice application, completed and settled the final Critical Pathway after using the Critical Pathway to the hysterectomy patient from March 1st to 15th, 1997. Finally, to analyze the application-effect of the developed Critical Pathway, this author offered health care service applying the Critical Pathway to the hysterectomy patient from April 15th to August 31th, 1997. The guide for the Critical Pathway was carried out in advance by outpatient setting nurse for outpatient setting visit before the operation, and after hospitalization the primary nurse monitored the execution degree on the every duty. After discharge this author surveyed the complication through phone visiting, and one month after discharge surveyed the patient's reaction about the offered service when outpatient setting visit and analyzed the result. The source for health care cost was obtained by the statistics about the hospital charge which was offered by the General Business Department. The results were as follows. 1. It was decided that the vertical line of the Critical Pathway was made up of eight items such as monitoring/assessment, treatment, line/drains, activity, medication, lab test, diet, patient teaching, and the horizontal line of the Critical Pathway was made up of from hospitalization to discharge. 2. After the analysis of service contents through reviewing the old chart, it was decided that the horizontal line of the preliminary Critical Pathway was made up of from hopitalization to fourth postoperative day, and the vertical line of it was divided into eight items which were the contents to occur with the time frame of the horizontal line. 3. After the verifying the validity of the expert group about the preliminary Critical Pathway, the horizontal line was amended from hopitalization to third postoperative day, and taking their consensus, some contents of the horizontal line was amended and deleted. 4. From March 1st to 15th, 1997, to confirm the clinical suitability, this author offered eight hysterectomy patients the medical service through the Critical Pathway. The result was that three of them could be discharged at the expected discharge day, and the others later than that day. Supplementing the preliminary Critical Pathway through analyzing the cause of that delay- case, this author developed the final Critical Pathway. 5. There were no significant differences between the experimental and the control group in the incidence of complication(P > 0.05). 6. The 92.4% of experimental group was satisfied with the Critical Pathway service. 7. The length of hospital stay of the experimental group offered with the Critical Pathway service was 4.6 days and there was a significant difference that it was 1.3 days shorter than that of the control group(t=-29.514, P=0.000). 8. There wsa a significant difference that the mean medical charge per one patient of the experimental group offered the Critical Pathway service was cheaper \124,150 than that of the control group(t=-9.826, P=0.000). 9. The result that the author assumed and analyzed hospital income with the rate of turning bed was assumed that the increase of hospital income was \63,245,072 for that study, and the income increase was expected with \68,704,864 for a year. The result that this author applied the Critical Pathway to the hysterectomy patient have no differences in the incidence of complication, high satisfaction with that service, and the length of hospital stay decreased in the experimental group, and the mean hospital charge per one patient decreased, but hospital income increased. Suggestions for further study and nursing practice are as follows. 1. The study to apply the Critical Pathway for a year, verify the validity, and measure the effect repeatedly is needed. 2. To apply and manage the Critical Pathway effectively, the study to computerize it is needed. 3. The study to develop hospital-based Critical Pathway about other diseases or procedure, and measure the effect is needed.
