Two Treatment Cases of Combined Korean Medicine for Patients with Dermatological Diseases Exacerbated by COVID-19 (COVID-19로 악화된 피부과 질환 환자의 한의 치료 2례에 대한 증례보고)

  • Objectives : The purpose of this study is to report the clinical effect of herbal medicine decoction on COVID-19 symptoms remaining after administering Western medicine and worsening eczema during the COVID-19 prevalence period in patients with chronic eczema. Methods : The two chronic eczema patients suffering from COVID-19 symptoms and worsening eczema received a series of herbal medicine decoction for treatment. The effectiveness of treatment was evaluated by VAS, NRS, EASI, PASI, DLQI, photography and subjective evaluation of the patient's improvement of symptoms. Results : After treatment, all symptoms of COVID-19 disappered and aggravated eczema lesions were remarkably improved in both cases. Compared to its peak, the first case shows that PASI improved by 88% and DLQI by 44%. According to photo and symptom changes, the size of the affected area of facial acne and lip eczema was decreased. The second case shows that EASI improved by 43% and DLQI by 65%. According to photo and symptom changes, redness, rash were stopped and pruritus was decreased. Conclusion : This study suggests that herbal medicine could induce effectiveness on both COVID-19 symptoms as well as chronic eczema.

FACIAL ASYMMETRY OF UNILATERAL CLEFT LIP AND PALATE PATIENTS (편측성 순구개열자의 안면비대칭에 관한 연구)

  • Craniofacial asymmetry was analyzed in 39 patients with complete unilateral cleft lip and palate(UCLP). The samples are devided into three groups nine below 9 years, twenty three from 9 years 1 month to 14 years and seven over 14 years group. Seventeen measurements were obtained from the tracing of PA X-ray cephalometric headfilms to evaluate the asymmetric characteristics and changes accdording to aging in UCLP. The obtained results were as follows. 1. Facial asymmetry in UCLP is variable(1.22-3.47 $mm/^{\circ}$) and the length from midsagitta1 reference line to maxillary 1st molar, to upper central incisor and the length of mandibular ramus showed significant asymmetry 2. Nasal septum and anterior nasal spine were deviated In the cleft side and the lower border of nasal cavity was 1ower in cleft side. 3. The deviation of nasal septum was continued significantly till after 14 years old.

THE EFFECT AND RELAPSE PATTERN OF FACEMASK THERAPY FOR CLASS III MALOCCLUSION CHILDREN (유치열기 3급 부정교합 환아에서 facemask의 효과와 재발 양상)

  • The purpose of this study was to evaluate skeletal and soft tissue changes that occur after using a facemask for treatment of skeletal class III malocclusion, and to assess the relapse pattern when no retention appliance was used. Fifteen skeletal class III malocclusion patients were treated with a facemask for an average of 12 months. No retention appliance was used during the 1 year follow-up period. Cephalograms were taken during pretreatment, posttreatment, and the 1 year follow-up. Cephalograms were traced, analyzed, and the results were compared between cephalograms. All patients showed significant sagittal skeletal changes after treatment, but they also showed a significant relapse during the 1 year follow-up period when no retention appliance was used. Despite the relapse, the sagittal skeletal changes that remained were still significant. Vertical skeletal change was also significant after treatment, but the total change was not significant after a 1 year follow-up due to relapse. In soft tissue changes, facial convexity and upper lip position improved after treatment and this change remained significant after the 1 year follow-up period. Facemask therapy is therefore an effective method for treatment of skeletal class III malocclusion, however, retention is imperative to maintain the treatment effect.

A STUDY ON PROFILE CHANGE OF SKELETAL CLASS III MALOCCLUSION PATIENTS AFTER WEARING PROTRACTION HEAD GEAR (골격성 제III급 부정교합 환자에서 상악골 전방견인장치 사용후 측모 변화에 대한 연구)

  • Cause of skeletal Class III malocclusion in growing patients can be classified into maxillary deficiency, mandibular overgrowth, and combination of the two. Use of Protraction Head Gear(P.H.G.) has been recommended for treatment of growing Class III malocclusion patients, for it results in forward & downward movement of maxilla and backward & downward rotation of mandible. Numerous animal experiments were performed and clinical study data have been reported ; nevertheless, studies on soft tissue profile change and comparison of treatment effects among the patients who had undergone treatment are considered to be somewhat insufficient. The author selected 93 patients, who had been diagnosed as skeletal Class III malocclusion with maxillary deficiency and then treated with P.H.G. ; the sample group was divided according to sex, treatment beginning age, palatal suture opening(intraoral appliance), and facial growth pattern. For each group, changing patterns of hard and soft tissue profile observed, and comparision with 20 normal group(Angle's Class I) patients of statistical significance in amount of growth and treatment of hard and soft tissue was done. The following results were obtained. 1. Skeletal, dental, and soft tissue measurements indicated that more growth changes was induced in the sample group that used P.H.G. compared to the growth amount of normal group. 2. No statistical significance was observed in the amounts of maxillary forward movement and mandibular backward & downward rotation depending on treatment beginning age in both sex group. 3. R.P.E. showed more significant maxillary forward movement and less protrusion of upper incisor than La-Li. 4. There was no statistical significance in the amount of maxillary forward movement depending on facial growth pattern. On the other hand, measurements indicating mandibular downward & backward rotation indicated greater change in counterclockwise growth pattern group than the clockwise. 5. Changes in upper and lower lip thicknesses showed a close relationship with positional changes in underlying bone tissue and upper and lower teeth, and upper lip height and nasolabial angle increased and mentolabial angle decreased.

Nose Changes after Maxillary Advancement Surgery in Skeletal Class III Malocclusion (골격성 III급 부정교합자에서 상악골 전방 이동술 후 코의 변화에 관한 연구)

  • The purpose of this study was to evaluate the amount and interrelationship of the soft tissue of nose and maxillary changes and to identify the nasal morphologic features that indicate susceptibility to nasal deflection in such a manner that they would be useful in presurgical prediction of nasal changes after maxillary advancement surgery in skeletal Class III malocclusion. The sample consisted of 25 adult patients (13 males and 12 females) who had severe anteroposterior skeletal discrepancy. The patients had received presurgical orthodontic treatment. They underwent a Le Fort I advancement osteotomy, rigid internal fixation, alar cinch suture and V-Y advancement lip closure. The presurgical and postsurgical lateral cephalograms and lateral and frontal facial photographs were evaluated. The computerized statistical analysis was carried out. Soft tissue of nose change to h point change ratios were calculated by regression equations. The results were as follows 1. The correlation of maxillary hard tissue horizontal changes and nasal soft tissue vortical changes were high and the ${\beta}_0$ for soft tissue to ADV were 0.228 at ANt, 0.257 at SNt. 2. The correlation of maxillary hard tissue and nasal soft tissue horizontal changes were high and the ${\beta}_0$ for soft tissue to ADV were 0.484 at ANt, 0.431 at SNt, 0.806 at Sn. 3. The correlation of maxillary hard tissue horizontal changes and width changes of ala of nose were high and the ${\beta}_0$ lot alar base width ratio to ADV were 0.002. 4. The DRI, Prominence of nose, Pre-Op CA is not a quantitative measure that can be used clinically to improve the predictability of vertical and horizontal nasal tip deflection. In this study, increases in nasal tip projection and anterosuperior rotation occur when there is an anterior vector of maxillary movement. These nasal changes were Quantitatively correlated to magnitude of maxillary(A point) movement.

  • The purpose of this study is to offer basic data to adolescents' cosmetic culture by investigating and analysing the choices made by female students who have been becoming principle consumers of cosmetics these days. Firstly, social and cultural backgrounds and concepts of N generation, and then characteristics and influencing powers of female students were reviewed. And this study also included questionnaire surveys of 500 female students in middle and high schools located in Seoul. Data were processed using a SPSS$^+$ program and analyzed by using frequency, percentage and the $X^2$-test. The major findings run as follows: Female students have the effects of spreading their words quickly and abilities of making a decision what to buy. And they accept consuming behaviors itself as a part of cultural lives. Therefore, industries have acknowledged them as new principal consumers with a powerful influence in the market. They also play an important role in active consumer as emotional generation who has distinct personalities and prefer fashion trends, changes and innovations. They show conformities with their peer groups and they also want to identify themselves with characters or stars. The cosmetic behaviors of female students relating to demographic characteristics showed a significant relation to grades, majors, school groups and places of residence. Female students have a lot of interest and knowledge about cosmetics. And they use various cosmetic products. This study demonstrated the age for using make-up for the first time has been becoming younger more and more. Recently, female adolescents tend to use face powder, lip gloss, mascara and eye shadow. It is assumed that cosmetic purchases of female students can be considered as an impulsive aspects. However, before purchases, the proportion of students having any particular cosmetic brand in mind previously has been increasing. They take into consideration skin safety, the quality of product and color, when purchasing cosmetic products.

A Numerical Model for Bed Elevation Change and Bed Material Sorting in the Channel of Non-uniform Sediment (혼합사로 구성된 하천에서 하상변동 및 유사의 입도분포 계산을 위한 수치모형 개발)

  • A computer model was proposed to simulate channel changes and bed material sorting of the meandering channels with different grain size in time and space simultaneously. The bed at the outside of the meandering channel with mixed sediments was scoured deeply and composed of coarser materials, and at the inside was aggradated and composed of finer materials. The sorting process started at the upstream inflection point and was finished at the downstream inflection point. At the natural with complicated boundaries and non-uniform grain sizes, the bed near the outside at the bend and narrow width was scoured deeper with coarse materials than in the channel with uniform grain sizes. The point bars showed lip at the inside near the bend and the bed materials were finer The bed at the outside near the bend and in the narrow width was scoured deeply with the coarser materials.

A LONGITUDINAL STUDY OF SOFT-TISSUE FACIAL PROFILE CHANGES IN KOREAN CHILDREN (한국인아동의 연조직측모의 성장변화에 관한 누년적 연구)

  • A serial cephalometric study was undertaken to define the growth of the soft tissue facial profile in Korean children. The sample was composed of 25 males and 15 females for whom yearly cephalometric records were taken from the ages of 6 to 13 years. From the tracings, points on skeletal and soft tissue profiles were located and recorded on magnetic tape utilizing a Calcomp Talos RP660 X-Y digitizer. Linear and angular measurements of soft tissues were made directly from tape in a Cyber 174-16 computer after cephalometric enlargement had been corrected. A statistical evaluation was made of the data and the average profile diagrams in male and female were described by a Calcomp 960 pen plotter. On the basis of the findings of this study, the following trends were established. 1. The most prominent growth in soft tissue facial profile thickness was the nose and the least was the forehead. 2. The general growth direction of the soft facial tissue to the cranium described the downward and forward. 3. The degree of soft tissue facial convexity was decidely more than that exhibited earlier in life even though the soft tissue chin had protruded to the cranium. 4. The measurements indicated a general tendency for males to have larger nose and more convex and long soft tissue facial profile than did females. 5. Males showed significantly more growth than females in base of the upper lip and height of the upper anterior facial profile. 6. There was a difference between males and females in the rates of soft tissue facial profile growth. 7. Korean children showed less convex in the soft tissue profile convexity than did American children.

Comparison of treatment effects between four premolar extraction and total arch distalization using the modified C-palatal plate

  • Objective: The purpose of this study was to compare the skeletal, dental, and soft-tissue treatment effects of nonextraction therapy using the modified C-palatal plate (MCPP) to those of premolar extraction (PE) treatment in adult patients with Class II malocclusion. Methods: Pretreatment and posttreatment lateral cephalographs of 40 adult patients with Class II malocclusion were retrospectively analyzed. The MCPP group comprised 20 patients treated with total arch distalization of the maxillary arch while the PE group comprised 20 patients treated with four PE. Fifty-eight linear and angular measurements were analyzed to assess the changes before and after treatment. Descriptive statistics, paired t-test, and multivariate analysis of variance were performed to evaluate the treatment effects within and between the two groups. Results: The MCPP group presented 3.4 mm of retraction, 1.0 mm of extrusion, and $7.3^{\circ}$ lingual inclination of the maxillary central incisor. In comparison, the PE group displayed greater amount of maxillary central incisor retraction and retroclination, mandibular incisor retraction, and upper lip retraction (5.3 mm, $14.8^{\circ}$, 5.1 mm, and 2.0 mm, respectively; p < 0.001 for all). In addition, the MCPP group showed 4.0 mm of distalization and 1.3 mm of intrusion with $2.9^{\circ}$ distal tipping of the maxillary first molars. Conclusions: These findings suggest the MCPP is an effective distalization appliance in the maxillary arch. The amount of incisor retraction, however, was significantly higher in the PE group. Therefore, four PE may be recommended when greater improvement of incisor position and soft-tissue profile is required.

Substitution of Glycine 275 by Glutamate (G275E) in Lipase of Bacillus stearothermophilus Affects Its Catalytic Activity and Enantio- and Chain Length Specificity

  • The lipase gene(lip) from Bacillus stearothermophilus was recombined in vitro by utilizing the DNA shuffling technique. After four rounds of shuffling, transformation, and screening based on the initial rate of clear zone formation on a tricaprylin plate, a clone (M10) was isolated, the cell extract of which showed about 2.8-fold increased lipase activity. The DNA sequence of the mutant lipase gene (m10) showed 3 base changes, resulting in two cryptic mutations and one amino acid substitution: S113($AGC{\rightarrow}AGT$), L252 ($TTG{\rightarrow}TTA$), and G275E ($GGA{\rightarrow}GAA$). SDS-PAGE analysis revealed that the increased enzyme activity observed in M10 was partly caused by high expression of the m10 lipase gene. The amount of the expressed G275E lipase was estimated to comprise as much as 41% of the total soluble proteins of the cell. The maximum velocity ($V_{max}$) of the purified mutant enzyme for the hydrolysis of olive oil was measured to be 3,200 U/mg, which was 10% higher than that of the parental (WT) lipase (2,900 U/mg). Its optimum temperature for the hydrolysis of olive oil was $68^{\circ}C$ and it showed a typical $Ca^{2+}$-dependent thermostability, properties fo which were the same as those of the WT lipase. However, the mutant enzyme exhibited a high enantiospecificity towards (S)-naproxen compared with the WT lipase. In addition, it showed increased hydrolytic activity towards triolein, tricaprin, tricaprylin, and tricaproin.
