Long-term Outcomes of Conjunctivo-limbal Autograft Alone and Additional Widening of Limbal Incision in Recurrent Pterygia (군날개 재발에 있어서 자가윤부결막이식술 단독 및 추가 윤부절개술의 장기 결과)

  • Purpose: We report the clinical outcomes of patients undergoing additional widening of the limbal incision to prevent disease recurrence after conjunctivo-limbal autograft combined with pterygial excision. Methods: We retrospectively compared 95 eyes with primary and 12 eyes with recurrent pterygia randomized to treatment via two surgical methods: conjunctivo-limbal autograft alone and combined with widening of the limbal incision to allow for pterygial removal. We widened the limbal incisions by 1 mm on both the superior and inferior limbal margins and removed the pterygia. The outcomes were compared between 39 eyes undergoing conjunctivo-limbal autograft alone and 68 eyes undergoing conjunctivo-limbal autograft with widening of the limbal incision. Results: The mean overall postoperative follow-up period was $29.6{\pm}10.5months$ and conjunctivo-limbal autograft alone group was $27.4{\pm}11.5months$, combined with widening of the limbal incision group was $30.7{\pm}9.7months$. Six pterygia (15.4%; four primary and two recurrent) developed in those undergoing conjunctivo-limbal autograft alone and two (2.9%; one primary and one recurrent) in those undergoing additional widening of the limbal incision; the recurrence rate differed significantly between the two groups (p < 0.05). In the group treated with conjunctivo-limbal autograft alone, the mean time to development of a new primary pterygium was $6.3{\pm}3.4months$ and that to development of a recurrent pterygium $4.3{\pm}2.5months$; the respective values for the group undergoing additional widening of the limbal incision were $12.1{\pm}2.6$ and $8.4{\pm}4.6months$; the recurrence rates differed significantly (p < 0.05). Conclusions: Conjunctivo-limbal autograft with additional widening of the limbal incision used to treat both new primary and recurrent pterygia was more effective in terms of reducing pterygial recurrence than conjunctivo-limbal aAutograft alone.

Malignant Anterior Uveal Melanoma Masquerading as Limbal Melanoma in a Shih-Tzu Dog with Recurrences of Hyphema

  • A 14-year-old neutered-male Shih-Tzu was referred for glaucoma and hyphema accompanied by systemic hypertension. After topical corticosteroid and antiglaucoma medications, the left eye (OS) progressed phthisically, but ocular hypertension redevelop one-year-and-seven-months later. Suspected limbal melanoma developed in peripheral cornea/limbal sclera with heavily pigmented cornea OS. Ultrasound biomicroscopy and ocular ultrasonography differentiated ciliary body origin-heterogeneous mass from limbal mass. Persistent hyphema and pigmented cornea might obscure intraocular lesion and advanced diagnostic methods played a crucial role. Unlike the generally benign limbal melanoma, aggressive treatment was recommended for uveal tumor. Histopathology of enucleated OS confirmed malignant uveal melanoma with chronic hypertensive vasculopathy.

Limbal Lensectomy with or without Anterior Vitrectomy for the Management of Lens Subluxation (비외상성 수정체이탈 환자에서 시행한 윤부 수정체절제술)

  • Purpose : This study was conducted to assess the surgical outcomes of limbal lensectomy with or without anterior vitrectomy for the management of lens subluxation. Materials and Methods : The medical records of 20 consecutive patients (33 eyes) with lens subluxation who had undergone limbal lensectomy with or without anterior vitrectomy from February 1999 to January 2004 were retrospectively reviewed. Results : All the patients, except one high axial myopic patient, were implanted with scleral sutured posterior chamber intraocular lens. We evaluated the preoperative, postoperative visual acuity and postoperative complications and compared the results in group I (limbal lensectomy with anterior vitrectomy, 27 eyes) to those in group II (limbal lensectomy without anterior vitrectomy, 6 eyes). The preoperative best-corrected visual acuity was 0.21 and postoperative best-corrected visual acuity was improved by 2 lines or more in all 27 eyes in group I, and in 5 eyes in group II (p>0.05). The most frequent postoperative complication was intraocular lens dislocation in four eyes (14.8%) in group I alone. No retinal detachment occurred in either group, even in patients with high myopia. Conclusion : Limbal lensectomy without anterior vitrectomy improved visual acuity similarly to limbal lensectomy with anterior vitrectomy without significant increase of postoperative complications. This results of this study suggest that anterior vitrectomy is not necessarily required for the management of lens subluxation.

Interspecies comparative morphological evaluation of the corneal epithelial stem cell niche: a pilot observational study

  • Background: The corneal and limbal morphology relevant to corneal epithelial maintenance in ten different species was examined using histological methods. Objectives: The presence of a Bowman's layer, limbal epithelial cell, and superficial stromal morphology was examined in the following species to evaluate the differences in corneal thickness and epithelium: Java sparrows, frogs, macaws, spoonbills, red pandas, penguins, horses, Dobermans, orangutans, and humans. Methods: Corneal sections (4 ㎛) were obtained from ten ocular globes from three different animal classes: Aves, Amphibia, and Mammalia. All sections were stained with hematoxylin and eosin and periodic acid-Schiff reaction. After microscopy, all stained slides were photographed and analyzed. Results: Significant morphological differences in the corneal and limbal epithelia and their underlying stroma between species were observed. The number of corneal epithelial cell layers and the overall corneal epithelial thickness varied significantly among the species. The presence of a Bowman's layer was only observed in primates (orangutans and humans). Presumed supranuclear melanin caps were noted in four species (orangutans, macaws, red pandas, and horses) in the limbal basal epithelial layer (putative site of corneal epithelial stem cells). The melanin granules covered the apex of the cell nucleus. Conclusions: Supranuclear melanin capping has been described as a process within the epidermis to reduce the concentration of ultraviolet-induced DNA photoproducts. Similarly, there may be a relationship between limbal stem cell melanin capping as a protective mechanism against ultra-violet radiation.

Ocular Filariasis in Canine Anterior Chamber (개 전안방 안구사상충증)

  • An one-year-old, female pointer dog with conjunctival hyperemia, corneal opaque and ocular pain in right eye was referred to Veterinary Medical Teaching Hospital, Chungbuk National University. In the ophthalmic examination the worm was observed in anterior chamber, The heart worm antigen test was positive. A modified Knott's test was negative. The values of complete blood count and serum chemistry (TP, BUN, creatinine, AST, ALT, ALP, albumin, globuline) showed normal ranges. Any abnormality was not detected in the heart using a thoracic radiography and cardiac sonography. Therefore, this dog was diagnosed as ocular filariasis. The worm was removed by surgical incision through a limbus of cornea. The closure of limbal incision was sutured in a simple interrupted suture pattern with 6-0 silk and the eye was reinflated with sterile saline solution. Antibiotics and dexamethasone ophthalmic solution were applied to right eye every 24 hours for 7 days. To prevent latent filariasis, ivermectin was also administered on day 14 of operation. The heart worm antigen test on day 60 was negative. The dog was successfully cured.

  • The corneal tissue consists of three layers : epithelium, stroma, and endothelium. Central cornea is a highly differentiated tissue whereas the limbus contains the epithelial stem cell. In the present study. we report the engineering of the three-dimensional reconstructed cornea derived from rabbit limbal epithelial and stromal cells. The differentiation degree of corneal stem cells were assessed in serum concentration and inoculation density of stromal cells. Optimal condition differentiation of corneal stem cells is achieved when 5% FBS was supplemented to culture medium and $1-2{\times}10^5$ cells/ml inoculation density of stromal cells.

Diurnal Change of Intraocular Pressures and Comparison of $Tono-pen^{XL}$ and Schi$\emptyset$tz Tonometer in Normal Dogs (정상견 안압의 일변동과 안압계간 측정치의 비교)

  • To investigate the diurnal change of intraocular pressures(IOP) and compare of $Tono-pen^{XL}$ and Schi$\emptyset$tz tonometer, we measured the IOP in clinically normal eyes of dogs. To estimate the diurnal change of IOP, the measurements of IOP were taken with $Tono-pen^{XL}$ in 44 normal canine eyes. Two physicians measured the IOP through the central cornea and limbal cornea. To compare of two tonometers, a physician measured the IOP with both $Tono-pen^{XL}$ and Schi$\emptyset$tz (7.5 g) tonometer in 23 clinically normal eyes of dogs. Mean of lOP was 26.51 $\pm$ 6.91 mmHg and IOP was highest as 30.65 mmHg at a.m. 10, but not significantly different within physicians and corneal regions (p > 0.05). Correlation coefficient within two tonometers was too low.
