Diplopia after inferior alveolar nerve block: case report and related physiology

  • Although inferior alveolar nerve block is one of the most common procedures performed at dental clinics, complications or adverse effects can still occur. On rare occasions, ocular disturbances, such as diplopia, blurred vision, amaurosis, mydriasis, abnormal pupillary light reflex, retrobulbar pain, miosis, and enophthalmos, have also been reported after maxillary and mandibular anesthesia. Generally, these symptoms are temporary but they can be rather distressing to both patients and dental practitioners. Herein, we describe a case of diplopia caused by routine inferior alveolar nerve anesthesia, its related physiology, and management.

  • A 2-year-old, castrated male, Scottish fold cat was referred to Veterinary Medical Teaching Hospital of Seoul National University (VMTH-SNU) for evaluation of acute bilateral blindness after general anesthesia. For dental prophylaxis in local animal hospital, general anesthesia had been induced with intravenous acepromazine and ketamine, and maintained with isoflurane after intubation. At VMTH-SNU on next day, complete blood count, electrolytes and serum chemistry values were within normal ranges. On neurologic examination, visual placing and postural reactions like as hopping, hemiwalking and wheelborrowing were reduced on right hindlimb. On ophthalmic examination, menace responses were absent on both eyes and pupillary light reflex (PLR) reduced on right eye, but other reflex and fundus were normal. Prednisolone (2 mg/kg sid for 3 days) was administrated orally and tapered. Visual placing was possible on 2nd day, and postural reactions were recovered on 4th day after dental prophylaxis. Based on the process and recovery, this case was considered as postoperative visual loss (POVL) after general anesthesia.

Effects of confinement on physiological and psychological responses and expression of interleukin 6 and brain derived neurotrophic factor mRNA in primiparous and multiparous weaning sows

  • Objective: The present study aimed to investigate whether the long-lasting, recurrent restricting of sows leads to the physiological and psychological reaction of discomfort. Methods: Sows (Large White) that had experienced restricting for about 0.5 or 3 years and agematched sows kept in a group housing system (loose sows) were compared. Pupillary light reflex parameters were measured at the weaning stage. Immediately after slaughter, blood samples were taken to measure serum cortisol levels, and the brain was dissected, gene expression in the hippo-campus, frontal cortex and hypothalamus was analyzed. Results: The serum cortisol levels were higher in the confined sows than in the loose sows. The full maturity, but not the young adolescent, confined sows had longer latency time in the onset of pupil constriction than their loose counterparts. Real-time polymerase chain reaction analyses revealed an increased expression of interleukin 6 mRNA in the hippocampus and decreased expression of brain derived neurotrophic factor mRNA in hippocampus and hypothalamus and to a lesser extent in the frontal cortex of the full maturity confined sows, compared with the full maturity loose sows. Conclusion: Taken together, these data indicated that recurrent restricting stress in full maturity sows leads to the physiological and psychological reaction of discomfort.

Study on Prognostic Factors using Computerized Tomographic Findings and Ability in daily Life(ADL) Evaluation in Patients with Hypertensive Intracerebral Hemorrhage (고혈압성(高血壓性) 뇌출혈(腦出血) 환자(患者)의 Brain-CT소견과 일상생활능력(ADL)평가를 통한 예후인자에 대한 고찰)

  • Clinical Observation was made on 29 cases of Hypertensive intracerebral hemorrhage patients in the ICU of In-Chon Oriental Medical Hosptital of Dongguk University from October in 1994 to June in 1996. The observation are ability in daily life(ADL) of patients by Location and Type of Hemorrhage, Amounts of Hematoma, Graeb's Score, Intraventricular Hemorrhage, States 4th Ventricle, Surrounding Edema around the Hematoma, Middle Line Shift, Age, Level of Consciousness. Pupillary Light Reflex and Treatment Modalities. Our conclusions on Prognostic Factors using Computerized Tomographic Findings and Ability in daily Life(ADL) Evaluation in patients with Hypertensive Intracerebral Hemorrhage Patients are as follows. A variety of prognostic factors that influence ADL5+6(%) were observed. 1. ADL5+6($\%$) of total cases was 34.9%. The prognosis were unfavorable when high Graeb score(P<0.05), dilated 4th ventricle(P<0.01), much surrounding edema around the hematoma (P<0.05), unilateral unreactive or both unreactive pupillary light reflex(P<0.05). 2. There was no difference of ADL5+6(%) in both hypertensive basal ganglionic and thalamic intracerebral hemorrhage. 3. The prognosis gets poorer as the volume of hematoma is more than 16cc. But there was no difference of ADL5+6(%) in each group. 4. The prognosis gets poorer in cases with IVH than without IVH. But there was no difference of ADL5+6(%) in each group. 5. The prognosis gets poorer as the middle line shift is more than 6mm. But there was no. difference of ADL5+6(%) in each group. 6. The prognosis gets poorer as the level of consciousness is more than drowsy. But there' was no difference of ADL5+6(%) in each group.

Case Report of Oculomotor Nerve Palsy with Mydriasis Improved by Sa-Am Acupuncture (사암침으로 호전된 동공산대를 동반한 동안신경마비 1례)

  • This study aims to report a case of oculomotor nerve palsy with mydriasis improved by Sa-Am acupuncture (大腸正格). The patient visited our clinic due to Right oculomotor nerve palsy with symptoms such as periocular pain, diplopia, vertigo, ptosis, eye adduction disorder, and mydriasis on November 11th, 2019. For the treatment, Sa-Am acupuncture (大腸正格) which used to give a significant effect on eye diseases and headaches and to remove toxins from the body was performed every time the patient visited the clinic. About a month after the treatment, ptosis disappeared, and eye adduction disorder also changed from -2 to 2 points, showing a significant improvement. Subjective symptoms such as vertigo, diplopia, and eye pain also significantly decreased from 13 to 4 points. At the second month of treatment, the eye adduction disorder improved to a normal level, and subjective ocular discomforts disappeared. The pupil size decreased from 5 mm to 2.5 mm. After 3-4 months from the start of treatment, most of the symptoms including the light reflex returned to normal. After a total of 32 acupunctures treatment, subjective symptoms such as periocular pain, diplopia and vertigo, as well as ptosis and eye adduction disorder disappeared, and the pupil size, including the light reflex, also improved to the normal level.

Inherited retinal degeneration in a Bengal cat

  • A 2-year-old intact female Bengal cat was presented with a 6-month history of visual impairment. The cat manifested bilateral negative menace responses and dazzle reflexes and sluggish pupillary light reflexes. Bilateral fundus changes included generalized tapetal hyperreflectivity, advanced retinal vascular attenuation, and increased pallor of the optic disc. A diagnosis of bilateral retinal degeneration was made. The clinical findings suggest that the investigated Bengal cat was most likely to have an inherited retinal degeneration. Further studies of the Bengal cat breed are needed to determine the prevalence of inherited retinal degeneration in this breed in Korea.

  • Compact system cameras (CSCs) are commonly used nowadays and feature enhanced video functions and thin yet light interchangeable lenses. They differ from digital single-lens reflex (DSLR) cameras in their lack of mirror boxes. CSCs, however, have autofocus (AF) speeds lower than those of conventional DSLRs, requiring weight reduction of their AF groups. To ensure the marketability of large telephoto zoom lenses with fixed f/2.8 regardless of field angle variation, in particular, light weight AF groups are essential. In this paper, we introduce a paraxial optical design method and present a new, large, telephoto zoom lens with f/2.8 regardless of the field angle variation, plus a lightweight AF group consisting of only one lens. Using the basic paraxial optical design and optimization methods, we fabricated a new and lighter zoom lens system, including a single-lens, lightweight AF group with almost the same performance.

A Model of Pupil's Change with Luminance (Luminance에 의한 Pupil의 변화에 대한 모델)

  • The size of pupil with pupillary light reflex is determined by the amount of a luminance. and it is dependent with the distribution function of the retinal illuminance which is the amount of transmittance for the external light due to the size of pupil, and the detector of cone and rod due to the amount of the luminance. The change of the pupil size with the luminance can be expressed with the mathematical model $$y(x)={\alpha}+{\beta}\frac{1}{1+{e}{x}{p}(x-x_0)/{\theta}}$$ where ${\alpha}$ is the size of the pupil diameter in a maximum value of the luminance, ${\beta}$ is the deviation of the pupil's diameter between maximum and minimum, ${\theta}$ is the parameter showed the degree of a sensitivity. Comparing with the experimental value of P.Moon et al, We known that the equation of the model is very compatible.

  • Central anticholinergic syndrome occurs when an anticholinergic substance works in vivo or as a result of the insufficient release of acetylcholine. Its symptoms include confusion, agitation, behavioral change, hallucination, blurred vision, and dysarthria. Occasionally, these symptoms occur with the use of a scopolamine patch. A 54-year-old female complained of behavioral change and confused mentality. She attached a scopolamine patch at the postauricular area in the morning of the day before her hospital visit. Neurological examination revealed bilateral symmetric mydriasis without light reflex. The brain MRI was normal, and electroencephalography showed nonspecific abnormalities. The patient recovered completely after the removal of the scopolamine patch.

  • 본 논문은 스마트폰 카메라를 이용하여 동공 반응 검사를 할 수 있는 스마트폰용 애플리케이션을 제안한다. 스마트폰의 카메라에 접사 렌즈를 부착한 후, 카메라 렌즈를 실험자의 눈 앞에 위치시켰으며, 빛 자극에 따른 전안부 영상을 촬영하였다. 스마트폰으로 얻은 전안부 영상을 HSV 컬러 모델로 변환 후 전처리와 필터를 걸쳐 동공을 검출하였다. 실험은 5명의 젊은 남성을 대상으로 수행되었으며 실험결과 71.2%의 동공 검출률을 얻었다.
