  • An anterior cruciate ligament (ACL) avulsion fracture is an uncommon injury that occurs predominantly in the pediatric and adolescent population. Accurate reduction and fixation of an avulsed fragment are necessary to restore and maintain the length of the ACL and normal knee biomechanics. Several techniques are available to repair an ACL avulsion fracture. On the other hand, treatment is controversial in skeletally immature patients due to risk of physeal injury. This paper reports a case of an ACL avulsed fracture in a skeletally immature patient treated with arthroscopic all-inside suture bridge repair, in which an excellent result and firm stability were obtained without physeal injury.

Clinically Correlated Anatomical Basis of Cricothyrotomy and Tracheostomy

  • Objective : Cricothyrotomy and tracheostomy are performed by physicians in various disciplines. It is important to know the comprehensive anatomy of the laryngotracheal region. Hemorrhage, esophageal injury, recurrent laryngeal nerve injury, pneumothorax, hemothorax, false passage of the tube and tracheal stenosis after decannulation are well known complications of the cricothyrotomy and tracheostomy. Cricothyrotomy and tracheostomy should be performed without complications and as quickly as possible with regards the patients' clinical condition. Methods : A total of 40 cadaver necks were dissected in this study. The trachea and larynx and the relationship between the trachea and larynx and the surrounding structures was investigated. The tracheal cartilages and annular ligaments were counted and the relationship between tracheal cartilages and the thyroid gland and vascular structures was investigated. We performed cricothyrotomy and tracheostomy in eleven cadavers while simulating intensive care unit conditions to determine the duration of those procedures. Results : There were 11 tracheal cartilages and 10 annular ligaments between the cricoid cartilage and sternal notch. The average length of trachea between the cricoid cartilage and the suprasternal notch was 6.9 to 8.2 cm. The cricothyroid muscle and cricothyroid ligament were observed and dissected and no vital anatomic structure detected. The average length and width of the cricothyroid ligament was 8 to 12 mm and 8 to 10 mm, respectively. There was a statistically significant difference between the surgical time required for cricothyrotomy and tracheostomy (p < 0.0001). Conclusion : Tracheostomy and cricothyrotomy have a low complication rate if the person performing the procedure has thorough knowledge of the neck anatomy. The choice of tracheostomy or cricothyrotomy to establish an airway depends on the patients' clinical condition, for instance; cricothyrotomy should be preferred in patients with cervicothoracal injury or dislocation who suffer from respiratory dysfunction. Furthermore; if a patient is under risk of hypoxia or anoxia due to a difficult airway, cricothyrotomy should be preferred rather than tracheostomy.

  • Purpose: Previous transtibial double bundle posterior cruciate ligament (PCL) reconstruction methods have several problems in graft length and tibial fixation. We introduce new surgical method that is less restrictive by graft length and is more stable with single tibial fixation. Operative technique: After diagnostic arthroscopy, we prepare the graft, ream the tibial tunnel and perform the procedure for TransFix tibial fixation. Femoral 2 tunnel is made and graft is passed via anteromedial (AM) portal. Tibial fixation is done and femoral 2 graft is fixed sequentially at each knee position. Conclusion: TtransFix tibial single fixation method in double bundle PCL reconstruction provides more stable fixation, more free graft selection and prevents graft damage by passing the graft via AM portal.

  • A coupled damage-viscoplasticity model is presented for the analysis of localisation and size effects. On one hand, viscosity helps to avoid mesh sensitivity because of the introduction of a length scale in the model and, on the other hand, enables to represent size effects. Size effects were analysed by means of three-point bending tests. Correlation between the fracture energy parameter measured experimentally and the density fracture energy modelling parameter is discussed. It has been shown that the dependence of nominal strength and fracture energy on size is determined by the ligament length in comparison with the width of the fracture process zone.

AN EXPERIMENTAL STUDY ON THE STRESS DISTRIBUTION IN THE PERIODONTAL LIGAMENT (치주인대의 응력 분포 양상에 관한 실험 연구)

  • In order to achieve a desirable tooth movement, it is of great importance to control the M/F ratio and to know the location of the center of resistance. The purpose of this study was to locate the center of resistance and the axis of rotation, and to estimate the stress distribution in the periodontal ligament with experimental model. After preparing a model of an upper canine with a simulated periodontal ligament and alveolar bone, the force and moment were applied. The tooth movement was traced using measuring device with LVDTs(Linear variable differential transformers) that can measure three dimensional tooth movement in real time. The results were as follows. 1. The location of center of resistance by transverse force was $29\%$ of root length measured from alveolar crest to apex regardless of force magnitude. The position of the center of resistance is more coronal than that of two-dimensional model($42\%$). 2. The center of resistance and the axis of rotation coincide when couple moment was applied. 3. As the magnitude of moment increases, tooth tends to extrude irrespective of the direction of the moment. 4. The relationship between location of force and axis of rotation (a x b = $49.6\;mm^2$) was obtained. A tooth movement can be predicted through this formula. 5. The centers of rotation by transverse force were plotted linearly.

LEAK-BEFORE-BREAK ANALYSIS OF THERMALLY AGED NUCLEAR PIPE UNDER DIFFERENT BENDING MOMENTS

  • Cast duplex stainless steels are susceptible to thermal aging during long-term service at temperatures ranging from $280^{\circ}C$ to $450^{\circ}C$. To analyze the effect of thermal aging on leak-before-break (LBB) behavior, three-dimensional finite element analysis models were built for circumferentially cracked pipes. Based on the elasticeplastic fracture mechanics theory, the detectable leakage crack length calculation and J-integral stability assessment diagram approach were carried out under different bending moments. The LBB curves and LBB assessment diagrams for unaged and thermally aged pipes were constructed. The results show that the detectable leakage crack length for thermally aged pipes increases with increasing bending moments, whereas the critical crack length decreases. The ligament instability line and critical crack length line for thermally aged pipes move downward and to the left, respectively, and unsafe LBB assessment results will be produced if thermal aging is not considered. If the applied bending moment is increased, the degree of safety decreases in the LBB assessment.

Biometry of Genitalia, Incidence of Gynecological Disorders and Pregnancy Loss in Black Bengal Goat : An Abattoir Study

  • This study was aimed to determine the biometry of genital organs, incidence of gynecological disorders and pregnancy loss in Black Bengal goat (Capra hircus). Genitalia of 118 does were collected from local abattoirs. Biometric parameters of genital organs were measured and gross and histopathological examinations were carried out for detection of abnormalities. For gravid uterus, age of the fetus was determined by measuring crown-rump length. There was no significant difference in the length, width and weight of right and left ovaries (P>0.05). However, the number of follicles between left ($5.3{\pm}2.3$) and right ovaries ($7.4{\pm}2.7$) varied significantly (P<0.05). The mean length of right fallopian tube and uterine horn were not varied with those of left fallopian tube and uterine horn. The length of uterine body, cervix and vagina were $1.3{\pm}0.1cm$, $3.3{\pm}0.5cm$ and $6.8{\pm}1.3cm$, respectively. Overall, 29 (24.6%) genitalia had abnormalities. Fifteen genitalia (12.7%) had ovarian abnormalities including ovaro-bursal adhesions (6.8%), parovarian cyst (5.1%) and follicular cyst (0.9%). Uterine abnormalities were found in 12 genitalia (10.2%) and predominant uterine lesion was endometritis (6.8%) followed by adenomyosis (1.7%), hemorrhagic lesion on endometrial surface (0.9%) and cyst in broad ligament (0.9%). In addition, cyst in fallopian tube (0.9%) and vagina (0.9%) were recorded. The proportion of slaughtered pregnant goats was 15.3% (18/118). The pregnancy wastage was highest in the first month (50.0%) followed by second (33.3%) and third (16.7%) month. It can be concluded that ovaro-bursal adhesions, parovarian cyst and endometritis are the gynecological disorders of major concern in Black Bengal goat.

  • Objective : Morphometric data for the lumbar posterior longitudinal ligament (PLL) was investigated to identify whether there is a difference in the morphometry of the PLL of the lumbar spine at each level with respect to the pattern of intervertebral disc displacement. Methods : In 14 formalin-fixed adult cadavers (12 males and 2 females), from L1 to L5, the authors measured the width and height of the PLL and compared them with other landmarks such as the disc and the pedicle. Results : Horizontally, at the upper margin of the disc, the central portion of the superficial PLL covered 17.8-36.9% of the disc width and the fan-like portion of the PLL covered 63.9-76.7% of the disc width. At the level of the median portion of the disc, the PLL covered 69.1-74.5% of the disc width. Vertically, at the level of the medial margin of the pedicle, the fan-like portion of the PLL covered 23.5-29.9% of the disc height. In general, a significant difference in length was not found in the right-left and male-female comparisons. Conclusion : This study presents the morphometric data on the pattern of intervertebral disc displacement and helps to improve the knowledge of the surgical anatomy of the lumbar PLL.

THREE-DIMENSIONAL FINITE ELEMENT STRESS ANALYSIS OF THE JAWS AT THE SIMULATED BILATERAL AND UNILATERAL CLENCHINGS (양측성 및 편측성 이악물기시 상하악골 응력변화 및 변위에 관한 3차원 유한요소법적 연구)

  • This study is to analyze the stress and displacement on the jaws during the bilateral and unilateral clenching task on three dimensional finite element model of the dentated skull. For this study, the computed tomography(G.E.8800 Quick, USA) was used to scan the total length of human skull in the frontal plane at 1.9mm intervals. The CAD data were extracted from the tomograms through digitizer(Summa Sketch III, USA) and then reconstructed by means of the spline method in the CAD program. In this project, a commercial software I-DEAS(Master Series ver-sion 3.0, SDRC Inc, USA) was used for three-dimensional stress analysis on the finite element model. which consists of articular disc, maxilla, mandible, teeth, periodontal ligament and cranium. The results are as follows. ; 1. During the bilateral clenching, each major muscle forces caused high stresses on various areas of skull: masseter muscle on articular disc and teeth ; temporal muscle on mandible and periodontal ligament ; medial pterygoid muscle on the temporomandibular joint. During the unilateral clenching, masseter muscle induced the maximum stress ; medial pterygoid muscle the minimum stress. 2. During the bilateral clenching, higher compressive stresses on articular disc were generated by the masseter muscle and higher deformation occurred on the most front outer sites. And during the unilateral clenching, temporal muscle and medial pterygoid muscle exerted their forces to twist temporomandibular joint area of the balancing side and induced a higher compressive stresses on the front outer sites of articular disc. 3. During the bilateral clenching, the masseter muscle bended the mandible outwardly, and then caused tensile stresses on the lingual surface of mandibular symphysis. And the medial pterygoid muscle caused tensile stresses on the labial surface of mandibular symphysis. 4. When each muscles were simultaneously applied on jaws, a high stress and displacement took place on mandible rather than on the maxilla. Also, a high stress and displacement took place during the unilateral clenching rather than during the bilateral clenching.

  • The present study has numerically and experimentally investigated the spray behavior of liquid jet injected in subsonic cross-flow. The corresponding spray characteristics are correlated with jet operating parameters. The spray dynamics are known to be distinctly different in the three regimes: the column, the ligament and the droplet regimes. The behaviors of column, penetration and breakup of liquid jet have been studied. Numerical and physical models are base on a modified KIVA code. The primary atomization is represented by a wave model base on the KH(Kelvin-Helmholtz) instability that is generated by a high interface relative velocity between the liquid and gas flows. In odor to capture the spray trajectory, CCD camera has been utilized. Numerical and experimental results indicate that the breakup point is delayed by increasing gas momentum ratio and the penetration decreases by increasing Weber number.
