  • Background: Carcinoid crisis is a life-threating syndrome of neuroendocrine tumors (NETs) characterized by dramatic blood pressure fluctuation, arrhythmias, and bronchospasm. In the era of booming anti-tumor therapeutics, this has become more important since associated stresses can trigger carcinoid crisis. Somatostatin analogues (SSTA) have been recommended for prophylactic administration before intervention procedures for functioning NETs. However, the efficacy is still controversial. The aim of this article is to review efficacy of SSTA for preventing carcinoid crisis. Materials and Methods: PubMed, Cochrane Controlled trials Register, and EMBASE were searched using 'carcinoid crisis' as a search term combining terms with 'somatostatin'; 'octreotide'; 'lanreotide' and 'pasireotide' until December 2013. Results: Twenty-eight articles were retrieved with a total of fifty-three unique patients identified for carcinoid crisis. The most common primary sites of NETs were the small intestine and respiratory tract. The triggering factors for carcinoid crisis included anesthesia/surgery (63.5%), interventional therapy (11.5%), radionuclide therapy (9.6%), examination (7.7%), medication (3.8%), biopsy (2%) and spontaneous (2%). No randomized controlled trials (RCTs) were identified and two case-control studies were included to assess the efficacy of SSTA for preventing carcinoid crisis by meta-analysis. The overall pooled risk of perioperative carcinoid crisis was similar despite the prophylactic administration of SSTA (OR 0.44, 95% CI: 0.14 to 1.35, p=0.15). Conclusions: SSTA wasnot helpful for preventing carcinoid crisis based on a meta-analysis of retrospective studies. Attentive monitoring and careful intervention are essential. Future studies with better quality are needed to clarify any effect of SSTA for preventing carcinoid crisis.

The data were taken from 1996 and 1999 Korean Household Panel Studies, and 1,293 households were selected for the final analysis. Methods of analysis included frequencies, percentages, Chi-square tests, F-tests, and t-tests. Major findings are as follows: 1. In the midst of the financial market changes during the period of the IMF crisis, consumers tended to manage their household portfolio differently according to their household characteristics. 2. The changes of household portfolio can be classified into two different types: the changed type (44.4%) and the unchanged type(55.6%). There are significant differences in the level of wealth, family life cycle stage, housing tenure, and the household head's job, between the changed type and the unchanged type. The family members of the unchanged type are more likely to be older and relatively wealthy compared with the families in the changed type. 3. The changes of household portfolio can be further classified into six different types: the unchanged-liquidity type (21%), the unchanged-multiplication type (24.6%), the unchanged-insurance type (9.8%), the changed-to-liquidity type (13.9%), the changed-to-multiplication type (13.0%), and the changed-to-insurance type (17.5%). There are significant differences in income level, wealth level, family life cycle stage, housing tenure, and the job of household head among the six types of changes.

  • This questionnaire study examined the processes linking complexity of self-construal to self-positivity conceptualized as taking multiple perspectives, self-acceptance and self-regulation that, in turn, affect psychological well-being among people with an experience of life-crisis during adulthood due to physical disability or divorce. Data were drawn from 359 adult Koreans residing in Seoul metropolitan area, including the physically disabled, the divorced, and their counterparts without a life-crisis experience during adulthood for comparison. On average, the life-crisis experience group(LEG) had a higher independent selfconstrual and a lower interdependent self-construal than the comparison group(CG). As predicted, complexity of self-construal contributed to taking multiple perspectives that helps searching for meaning, and this, in turn, was found to foster self-acceptance and self-regulation. Through these variables except self-regulation, complex self-construal contributed to psychological well-being as manifested by a higher level of positive emotion and a lower level of depression. As expected, while taking multiple perspectives fostered positive emotion both directly and indirectly through self-acceptance among LEG and CG, only in LEG it directly reduced depression. The comparative approach employed in this study enabled to discern that taking multiple perspectives promotes psychological well-being in LEG by contributing to both meaning making and positive attitude, whereas it promotes psychological well-being in CG primarily by fostering positive attitude. Finally implications for this study were discussed and directions for future research were suggested.

  • The purpose of this study is to analyze the records of 8 people with mental disabilities who had participated in Research and Mutual Help of Bethel's House and to investigate the occurrence of mental crisis and process of self-help by using the grounded theory method. As a result of axial coding, the central phenomenon of crisis experience of the people with mental disabilities was derived as 'being overwhelmed by hardships in the life related to their symptoms'. In the self-help process of overcoming the crisis, Action-interaction strategies were derived as 'talking about the hardships', 'researching and practicing with peers', 'participating in various groups', etc. through the intervening conditions such as 'understanding patterns and meanings of hardships led by the person' and 'finding ways to overcome the hardships with the person'. These strategies were analyzed as a process in which the person with mental disabilities leads his/her life reassuming control over his/her crisis. The result of this study shows that it is necessary to expand the political crisis concept in the support system for the mental crisis of people with mental disabilities, and the self-help approach in which the persons participate autonomously can be useful.
