  • Hospice palliative care refers to holistic care provided by an interdisciplinary team aimed at improving the quality of life of patients suffering from life-threatening diseases and their families. Among interdisciplinary team members, hospice advanced practice nurses (APNs) trained as master's-level advanced nursing professionals are leaders who play an important role in providing patient-centered care and improving the quality of services. The Medical Service Act revised in 2018 requires the scope of practice of APNs in each field to be specified in the Ordinance of the Ministry of Health and Welfare. Accordingly, discussions on the role and scope of practice of hospice APNs are actively underway. In this review, the curriculum of hospice APNs, their work responsibilities and roles, and their current status are reviewed, and the future direction of the hospice APN system is also discussed.

  • Many terminally ill cancer patients grapple with a range of physical, psychological, and social challenges. Therefore, it is critical to offer effective psychological interventions to assist them in managing these issues and enhancing their quality of life. This brief communication provides a concise overview of acceptance and commitment therapy (ACT), along with empirical evidence of its application for patients, caregivers, and healthcare professionals in hospice and palliative care settings and an overview of future directions of ACT interventions in South Korea. ACT, a third-wave type of cognitive behavioral therapy, is a model of psychological flexibility that promotes personal growth and empowerment across all life areas. Currently, there is substantial evidence from overseas supporting the effectiveness of ACT on health-related outcomes among patients with various diseases, caregivers, and healthcare professionals. The necessity and significance of conducting ACT-based empirical research in hospice and palliative care settings in South Korea are discussed.

  • Objectives : The growing elderly population and social changes have fueled a rapid increase in demand for elderly care facilities, but health care services for the elderly, especially oral health services, have long been overlooked. The purpose of this study was to examine the oral health care of elderly residents in elderly care facilities in an effort to provide some information on the elderly's oral health care and the development of dental care programs geared toward institutionalized elderly people. Methods : The subjects in this study were 230 elderly people who were in elderly care facilities in Jeolla Province. An individual interview was held, and they got a dental checkup. As for data analysis, t-test and ANOVA were utilized, and Scheffe post-hoc test was employed. Results : 1. In relation to the subfactors of the quality of life related to oral health, the elderly people investigated got a mean of 4.58, 4.47, 4.38, 4.09, 3.94, 3.91 and 3.76 respectively in activity disorder, mental disorder, social disorder, mental inconvenience, functional disorder, physical pain and physical disorder. 2. Concerning the overall quality of life related to oral health, there were statistically significant gaps in this aspect according to gender, age, presence or absence of systemic disorder, presence or absence of eating difficulties, subjective health status, subjective oral health state and oral health concern. 3. The presence or absence of root caries had a significant relationship to physical pain and social disorder among the subfactors of the OHIP-14, but that had nothing to do with functional disorder, mental inconvenience, physical disorder, mental disorder and activity disorder. Conclusions : The oral health indexes of the institutionalized elderly people in Jeolla Province were measured, and what factors affected the subfactors of oral health was checked. As a result, there appeared a close relation ship between oral health and the quality of life. Therefore effective oral health plans that cater to the elderly should be carried out to improve elderly people's quality of life related to oral health who stay in long-term elderly care facilities. In the future, prolonged research should be implemented from diverse angles for the sake of institutionalized elderly people.

  • This study has been performed to explore conveniences and inconveniences of daily lives in rural residential care facilities among the elderly residents. In this study, five males and seven females aged 68 to 78 were asked about socio-familial and physical aspects of their daily lives in the facilities such as meals, social activities, family visits, recreation, and health care. The answers of the qualitative interview were drawn as follow; first, the residents perceived well planned regular meals and snacks, free visits of friends, regularly supervised daily sanitary activities, periodic outdoor activities, education programs provided by religious experts, and pastoral farming lives as most convenient aspects of the retirement facilities. In addition, some felt that facility life made them free of familial conflicts, while others appealed fear of being forgotten because of distant location. On the other hand, they perceived the supervised group activities and meals as major inconveniences. They also felt persistently depressed when closely observing serious illness or deaths of their co-residents. Therefore in this study, suggestions were made as follows: first, combination of normal family life and retirement facility should be developed in the mixed form of community welfare center and shared home. Second, intensive medical care facilities should be in collaboration with the retirement facilities, so that the residents are relieved from stressful contacts with extremely ill patients in the residential area.

The results of the study can be used as basic materials for developing comprehensive intervention methods of care givers of CRF patients. Method: The research used van Manen's hermeneutic and phenomenological research methods in order to describe the lived experience and to understand its meaning. It concentrates on the understanding the essence of experience and consists of existential survey, hermeneutic and phenomenological reflection and hermeneutic writings. Participants in this research were five women care givers of CRF patients who had hemolysis at C university hospital in a metropolitan city, the period of data collection was from July 27 to Sep. 4, 2004 and major data of results in the following 5 essential themes. "sole responsibility for the patient enduring everything", "creating their own field", "heavy and painful life without hope of their private life", "wishing not to be inherited and consoling each other". Conclusion: The above findings point out that the experience of care givers of CRF patient affected and changed all parts of an individual life and his or her family life. • Objectives: The purpose of this study was to identify the predictors of quality of life (QOL) for terminal cancer patients admitted into the community health center, and to establish a hypothetical model to explain and verify causative relationships among the variables. Methods: Data were collected from January 2015 to June 2016. Participants were 237 registered patients in Busan Metropolitan City hospice & palliative care center. The descriptive and correlation statistics were analyzed using the SPSS/WIN 24.0, and the structural equation modeling procedure was performed using the AMOS 24.0 program. Results: The results of this study showed that the physical symptoms of terminal cancer patients were the most direct factors affecting the QOL, and satisfaction with health care services has a direct effect on the QOL. Conclusions: The study contributes to drawing up measures to improve QOL for terminally ill cancer patients who are living in the end-of-life section of the community by revealing the causal relationship to the QOL for terminal cancer patients.
