Red scabream, Pagrus major , is a valuable aquaculture species in Korea , but spontaneous eye abnormality occurred in cultured individuals . The incidence of eye abnormality was 4% in the group of cultured red scebream. The abnormality was characterized by unilateral and bilateral exophthalmos, opacity and lens pathology. Lense prolapse was found in two cases. Lenses in diseased fishes was considerably small in diameter and eyes were deformed as a whole. In the group of induced hybrid red scabream Pagrus major (♀) x black seabrearn Acanthopagrus schlegeli (♂), the incidence of eye abnormality wa~ the same 4%, but only opacity was registered. Opacity was also found in two of sixteen examined Wild-caught red seabrcam. Histopathological changes of lenses in cultured red seabreem included vecuolated cytoplasm of lens fibers, necrosis of fibers in central part of lens, folding and increase in thickness of lens capsule, and epithelial proliferation beneath the anterior lens capsule. In affected eyes no parasites or gas bubbles were found.
These studies were performed to investigate the complications affecting the vision after extracapsular lens extraction(ECE), the effects of an I/A (irrigation & aspiration)device and a viscoelastic material used on the vision, the occurrence of complications and the effective corneal incision method to reduce the corneal opacity in dogs. ECE was performed bilaterally with 3 different methods using clinically normal twele mixed dogs; the method in which I/A device and viscoelastic material were not used, the method in which I/A device was used but viscoelastic material not, and the method in which I/A device and viscoelastic material were used. Postoperative complications were observed as followed; conjunctival injection, uveitis, corneal opacity due to endothelial cell loss, hyphemia, remnants of lens cortex, vitreous loss, synechia and capsular opacity. Preservation rate of vision was lower significantly in the cases showing signs of synechia, capsular opacity, or remnants of lens cortex than the cases not showing the above signs(p<0.01). There were significant reduction of the complications such as corneal opacity, clot in anterior chamber in the group using I/A device compared to the group in which I/A device was not used(p<0.01). Groups using I/A device showed slightly higher vision than the group not using I/A device (75%; 42%). There were no significant differences in the occurrence rate of complications and the preservative rate of vision between the groups with and without viscoelastic material. The present study indicated that the postoperative complications of posterior synechia, capsular opacity, uveitis and vistreous loss were important factors affecting the vision and that I/A device was applicable to extract the lens cortex and effective to elevate the success rate after ECE in dogs.

Diabetic cataract is a major complication of diabetes mellitus. Excess accumulation of sorbitol plays an important role in the pathogenesis of diabetic complications such as cataract formation. In this study, we investigated the inhibitory effect of the extract of the aerial parts of Aster koraiensis (AK) on diabetic cataractogenesis. To examine this further, we evaluated sorbitol accumulation during cataract development using streptozotocin-induced diabetic rat, an animal model of type 1 diabetes. Diabetic rats were treated orally with AK (100 mg/kg and 200 mg/kg body weight) once a day orally for 9 weeks. In vehicle-treated diabetic rats, lens opacity was increased, and lens fiber swelling and membrane rupture were observed. In addition, sorbitol accumulation in diabetic lens was markedly enhanced. However, AK treatment delayed the progression of diabetic cataract through the inhibition of sorbitol accumulation, and prevented lens fiber degeneration in a dose-dependent manner. These observations suggest that AK treatment can delay the progression of lens opacification in the diabetic rats during the early diabetic cataractogenesis.
Purpose: This study calculated the prevalence of cataract in Korean and examined corrected eyesight with different types of opacification of crystalline lens. Methods: Using the data of the national health and nutrition examination survey (NHANES), population-based and cross-sectional research were designed in this study. After dividing total 4,977 persons into different groups by age, from 40 to 95 and by sex, the prevalence of cataract with different types of opacification was investigated. The average age of the total subjects was $58.28{\pm}12.07$, and the number of men and women were 2,142(43.0%) and 2,835(57.0%), repectively. Eyesight was calculated when it was less than 0.63. Results: Among total 4,977 subjects, 40.6% (men 17.5%, women 23.1%) had cataract. Different prevalence rates were found with different types of cataract; nuclear cataract (56.3%), cortical cataract (23.4%), mixed cataract (16.2%), anterior subcapsular(3.0%), and posterior subcapsular (1.1%) in order. There was no difference among men and women by the types. And those in their 40's showed a higher prevalence of cortical cataract than those in the other age groups. Most of subjects whose corrected eyesight is less than 0.63 had anterior subcapsular cataract. Conclusions: The type of cataract having the highest prevalence was nuclear opacity. Moreover, epidemiologic investigation and prevention policy with different type of lens opacity seems to be required.

Naturally occurring coumarin compounds have received substantial attention due to their pharmaceutical effects. Esculetin is a coumarin derivative and a polyphenol compound that is used in a variety of therapeutic and pharmacological strategies. However, its effect on aldose reductase activity remains poorly understood. In this study, the potential beneficial effects of esculetin on lenticular aldose reductase were investigated in galactose-fed (GAL) rats, an animal model of sugar cataracts. Cataracts were induced in Sprague-Dawley (SD) rats via a 50% galactose diet for 2 weeks, and groups of GAL rats were orally treated with esculetin (10 or 50 mg/kg body weight). In vehicle-treated GAL rats, lens opacification was observed, and swelling and membrane rupture of the lens fiber cells were increased. Additionally, aldose reductase was highly expressed in the lens epithelium and superficial cortical fibers during cataract development in the GAL rats. Esculetin reduced rat lens aldose reductase (RLAR) activity in vitro, and esculetin treatment significantly inhibited lens opacity, as well as morphological alterations, such as swelling, vacuolation and liquefaction of lens fibers, via the inhibition of aldose reductase in the GAL rats. These results indicate that esculetin is a useful treatment for galactose-induced cataracts.
Cataracts are the leading cause of blindness worldwide and are characterized by increased opacity of the lens that significantly diminishes visual acuity. It has been suggested that increased risk of lens opacities are associated with age, exposure to sunlight, diabetes, smoking, and poor nutrition. Antioxidant nutrients have born demonstrated to protect the lens membrane and protein against damage due to oxidative stress. The purpose of this study was to investigate the antioxidant system in the blood of cataract patients. The status of the blood antioxidant system was evaluated based on the levels of antioxidant vitamins and minerals as well as glutathione peroxidase (GSH-Px) and malondialdehyde (M7A) activity in 34 patients with cataracts (17 male and 17 female) and 45 control subjects (20 male and 25 female). After adjusting for age, the results showed significantly lower levels of antioxidant vitamins such as lycopene (M : p < 0.05, F: p < 0.01), zeaxanthin (F: p < 0.01), ${\gamma}$-tocopherol (F: p < 0.01) and ascorbic arid (M: p < 0.05) in the cataract patients than in the control subjects. In contrast, the concentration of cryptoxanthin (F : p < 0.07) showed a significantly higher value in the cataract patients. The serum level of the antioxidant mineral Zn (M : p < 0.01) was found to be significantly lower in the cataract patients while the ratio of cu/zn appeared significantly higher (M : p < 0.05). Significantly higher (M : p < 0.01, F: p < 0.05) concentrations of MDA in serum was found in the cataract patients as compared to the control subjects. GSH-Px activity was significantly lower (F: p < 0.05) in 71e cataract patients. In conclusion, the antioxidant system may play an important roll in cataract creation. Further studies are needed to clarify the mechanisms underlying these findings and to establish preventive measures with an emphasis on antioxidant nutrition for cataract patients.

Inherited metabolic disorders (IMD) are a large group of rare disorders affecting normal biochemical pathways. The ophthalmic involvement can be very varied affecting any part of the eye, including abnormalities of cornea, lens dislocation and cataracts, retina and the optic nerve, and extraocular muscles. Eye disorders can be initial symptoms of some IMD and can be clue for diagnosis of IMD. However, eye disorders can evolve later in the natural history of an already diagnosed metabolic disorder. Awareness of IMDs is important to facilitate early diagnosis and in some cases instigate early treatment if a patient presents with eye involvement suggestive of a metabolic disorder. Ophthalmological interventions are also an important component of the multisystem holistic approach to treating patients with metabolic disorders.
Aldose reductase (AR), the principal enzyme of the polyol pathway, has been shown to play an important role in the diabetic complications, including diabetic cataract. The inhibitors of AR, therefore, would be potential agents for prevention of diabetic complications. As part of our ongoing project directed toward the discovery of preventive and/or treatment for diabetic complications, 48 Korean herbal medicines have been investigated with an in vitro evaluation system using aldose reductase inhibitory activities. Of these, 12 herbal medicines exhibited a significant inhibitory activity against aldose reductase. Particularly, seven herbal medicines, i.e., Eurya japonica, Chrysanthemum indicum, Akebia quinata, Saururus chinensis, Catalpa bignonioides, Lonicera japonica, Vitex rotundifolia showed two times more potent inhibitory activity than the positive control 3.3-tetramethyleneglutaric acid (TMG). In addition, Catalpa bignonioides showed the retardation of cataract-opacification of the lens of the eye under diabetic condition by xylose. Therefore, this result may provide a potential therapeutic approach for preventing and treating diabetic cataracts.

Purpose: We report a case of postoperative endophthalmitis in the cataract patient, associated with removal of an intralenticular foreign body that had remained in place without symptoms for 20 years. Case summary: A 45-year-old male visited our outpatient clinic complaining of gradual visual loss in his right eye over the past 3 months. In slit-lamp examinations, anterior capsular opacification, nuclear sclerosis, and posterior subcapsular opacity were observed in the right eye. Twenty years before, a tiny metallic projectile had hit his right eye, but slit-lamp examination at the time of injury did not reveal any intraocular foreign body. We decided to undergo cataract surgery. During phacoemulsification, a metallic foreign body was found in the lens and safely removed; then an intraocular lens was implanted. As hypopyon was evident 3 days later, we injected intravitreal antibiotics and applied fortified antibiotic eye drops to the right eye. The anterior chamber inflammation improved and the best-corrected visual acuity recovered to 1.0. Conclusions: Surgeon should be aware of that endophthalmitis could develop after cataract surgery with removal of an intralenticular foreign body that had been in place for 20 years. But did not trigger inflammation or cause any symptoms as the cataract progressed.

To investigate the minimum near-infrared ray intensity required (quantifiable threshold value) for consistent measurements of axial length (AL) using partial coherence interferometry (PCI), we attached two polarizing lenses (PL) to two types of PCI (IOLmaster, ALscan). The near-infrared ray intensity of PCI was modified by rotating the axis of one PL at intervals of 5 degrees. The right eye of each volunteer was measured three times and the AL and signal-to-noise ratio (SNR) was recorded five times for each measurement. Reduction of light intensity was theoretically estimated using Malus' Law. AL was measured consistently with both IOLmaster and ALscan until they reached 55 degrees (1.33 % of intensity) and 60 degrees (0.77%), respectively (P = 0.343, Log-rank test). In contrast, SNR decreased as light intensity decreased. In addition, to analyze media opacities that precluded measurement of AL, we retrospectively reviewed the medical records of patients unmeasurable by PCI (ALscan) from May to November 2013. Thirty-eight of 473 eyes (8.0%) could not be measured using ALscan due to media opacities, such as severe posterior subcapsular cataract (PSC, 11 eyes), hypermature cataract (9 eyes), and vitreous hemorrhage (18 eyes). The mean grades of vitreous haze and PSC were $7.72{\pm}0.96$ and $4.45{\pm}1.04$, respectively. In conclusion, up to 0.77-1.33% of near-infrared rays decreased, and AL could be measured consistently.
