Effects of the APACHEIII Score, Hypermetabolic Score on the Nutrition Status and Clinical Outcome of the Patients Administered with Total Parenteral Nutrition and Enteral Nutrition (경정영양과 중심정맥영양을 공급받는 환자에서 질병의 상태(APACHEIII Score), 과대사 정도가 영양상태 및 임상적 결과에 미치는 영향)

  • The aim of this study is to evaluate the clinical outcome. Between January 1,2002 to September 30, 2002, we prospectively and retrospectively recruited III hospitalized patients who received Enteral Nutrition (EN group n = 52) and Total Parenteral Nutrition (TPNgroup n = 59) for more than seven days. The factors of clinical outcomes are costs, incidences of infection, lengths of hospital stay, and changes in weight. The characteristics of patients were investigated, which included nutritional status, disease severity CAP ACHE III score) and hypermetabolic severity Chypermetabolic score). Hypermeta-bolic scores were determined by high fever (> $38^{\circ}C$), rapid breathing (> 30 breaths/min) , rapid pulse rate (> 100 beats/min), leukocytosis (WBC > 12000 $mm^{3}$), leukocytopenia (WBC > 3000 $mm^{3}$), status of infection, inflammatory bowel disease, surgery and trauma. There was a positive correlation between hypermetabolic score and length of hospital stay (ICU), medical cost, weight loss, antibiotics adjusted by age while APACHEIII score did not show correlation to clinical outcome. Medical cost was higher by $18.2\%$ in the TPN group than the EN group. In conclusion, there was a strong negative correlation between the clinical outcome (cost, incidence of infection, hospital stay) and hypermetabolic score. Higher metabolic stress caused more malnutrition and complications. For nutritional management of patients with malnutrition, multiple factors, including nutritional assessment, and evaluation of hypermetabolic severity are needed to provide nutritional support for critically ill patients.

  • This paper is 1) to analysis how attitude of cross-cultural and personal traits affects on acculturation for foreigner students living in seoul, and 2) to suggest some guideline that can be helpful to fast adaption in college life and Korea living and also to utilize personal resource in organization. The findings are showed up as followed; The first, 'cultural distance' factor had high significant on 'social custom' in acculturation and also, two factors which are 'flexible culture' and 'recognition of cross-culture' affect on 'human relations' in acculturation. The second, 'flexible culture' factor and 'recognition of cross-culture' factor had influence on 'living satisfaction' in Korea. The third, foreign students who have length of stay less than 4 years had high recognized on 'social custom' and people who have length of stay 3 years come under 'traditional culture' factor. The fourth, an extrovert and female students had high recognized on 'human relationship' factor, and an oriental had high recognized on 'living culture' factor.

  • The study aims to examine the determinants that affect satisfaction, revisit intention, and recommendation intention with foreign tourists who visited Korea despite the threat of COVID-19. This study employs the survey data collected by the Korea Tourism Organization from 8,135 foreign tourists who visited Korea in 2020. As the survey data contains a mixture of continuous and categorical variables, decision tree analysis can ensure analytical validity for the research. According to the analytical results, the determinants affecting satisfaction are the purpose of the visit and acceptance of self-quarantine during their stay. The factors influencing revisit intention are the purpose of the visit, frequency of the visit, and acceptance of self-quarantine during their stay. The determinants affecting recommendation intention are the purpose of the visit, length of stay, and gender. Based on the results of this analysis, this study not only explains the relationship between these determinants and tourism satisfaction, revisit intention, and recommendation intention, but also suggests implications for revitalizing tourism activities.

Influence of late removal after treatment on the removal torque of microimplants

  • Objective: To compare the removal torque of microimplants upon post-use removal and post-retention removal and to assess the influencing factors. Methods: The sample group included 241 patients (age, 30.25 ± 12.2 years) with 568 microimplants. They were divided into the post-use (microimplants removed immediately after use or treatment) and post-retention (microimplants removed during the retention period) removal groups. The removal torque in both groups was assessed according to sex, age, placement site and method, and microimplant size. Pearson correlation and multiple linear regression analyses were performed for evaluating variables influencing the removal torque. Results: The mean period of total in-bone stay of microimplants in the post-retention removal group (1,237 days) was approximately two times longer than that in the post-use removal group (656.28 days). The removal torques in the post-retention removal group (range, 4-5 N cm) were also higher than those in the post-use removal group. The mandible and pre-drilling groups demonstrated higher placement and removal torques than did the maxilla and no-drilling groups, respectively. In the no-drilling post-use removal group, the placement torque and microimplant length positively correlated with the removal torque. In the post-retention removal group, unloading in-bone stay period and microimplant diameter positively correlated with the removal torque in the no-drilling and pre-drilling methods, respectively. Conclusions: The removal torques differed according to the orthodontic loading and removal time of microimplants. With prolonged retention of microimplants inserted using the no-drilling method, the removal torque was clinically acceptable and positively correlated with the unloading in-bone stay period.

Construction Method of Seohae Grand Bridge (서해대교 시공 공법 소개)

  • Since 1993, Seohae grand bridge has been continued construction for 7 years and will be completed late this year. The bridge is a part of west sea castal highway and consists of 3 types of bridge including precast segmental method, free cantilever method and cable stayed bridge. A cable stayed bridge is the core of this bridge and it consists of 5 span, symetrical cable-stayed bridge with a total length of 990 m. The main span between two H-shaped pylons extending approximately 180 M above massive foundation of a cable stayed bridge is 470 m long and an approach span of that is 260 m long respectively. The circular cofferdam with 16 ea of 25 m diameter flat type sheet pile had been applied to construct foundation. The slipform method had been applied for forming of con'c of two H-shaped pylons with 3 cross beams respectively which is varied horizontally and vertically. The deck has been erected with balanced cantilever method using movable derrick crane. The stay cables is a bundle of parallel individually protected, 7 wire high tensile strands. The strands is hot deep galvanized and sheathed with a tight high density polyethylene coating. A petroleum wax fills all the inter-wire voids. The bundle of strands to prevent from deterioration due to the ambient problem covered with high density polyethylene pipe. The Isotension method has been applied for the stressing of cable strands to ensure uniformity of force in all the strands of a syay and such works has been performed on the stay specially provided in the pylon.

Functional Status and Health Care Utilization among Elders with Hip Fracture Surgery from a Fall (낙상후 대퇴골절 수술 노인의 신체기능, 의료이용 및 낙상의 두려움)

  • Purpose: The purposes of this study were to examine 1) functional status at 2 months after hip fracture surgery 2) health care utilization after a fall episode and 3) fear of falling experienced during first 2 months after a fall episode. Method: With a convenient sample of 99 elderly from six university or general hospitals with hip fracture from a fall, data were collected at 2-3 days before discharge and at 2 months after hip fracture surgery. Result: 1) At 2 months after hip fracture from a fall, significant proportion (25.3%) of elderly was not able to walk indoors. 2) Average length of hospital stay was 27.6 days with a range of 8 to 86 days. About 51% subjects received physical therapy during hospital stay, and only 6.1% subjects received physical therapy following discharge from the hospital. 3) Significant proportion (72.7%) had fear of falling after the fall episode. About 51% reported that they restricted their activities because they had fear of falling. Conclusion: Fall is a dreaded event which result in loss of independence and restriction of activity. Development and application of fall prevention program is critical especially for those with risk factors of fall.

Medication Use as a Risk Factor for Falls among Hospitalized Stroke Patients (노인전문병원에 입원한 뇌졸중환자의 복용약물과 관련된 낙상 위험요인)

  • Purpose: To identify the relationship between medication use and falls among hospitalized stroke patients. Method: The medical records of 472 patients with strokes were reviewed using a questionnaire on falling developed by the authors. Frequencies, percentages, means, standard deviations, and t-test and ${\chi}^2$-test, multiple logistic regression analysis were done using the SAS program. Results: The rate for falls by the patients during their stay in the hospital was 14.0%. The length of stay was longer and the morbidity duration of stroke shorter in the fall group than in the non-fall group. The use of sedatives, laxatives, and antidepressants was a significant predictor of falls and was associated with increase likelihood of falling(1.82, 1.81, 1.75 times respectively). Conclusion: In hospitalized stroke patients, there was a significant association between the use of sedatives, laxatives, antidepressants and falls. The number and kinds of ingested drugs was also associated with falls. It is necessary to further analyze the causes of falls based on results of the present study.

Image Making As a Planning/Design Principle: A Case Study of Andong Municipal Museum Complex (AMMC)

  • This study addressing the underlying strategies for Andong municipal museum complex development is in timely view that Andong has obtained a worldwide reputation as a treasury of traditional Korean Confucian culture. Thus far, there has been a tendency that various local museums are proposed to meet architectural aspirations architects and users commonly hold. Overall, though, the major role they play in making overall city image has not been considered in a systematic manner. Based on Lee's (2001) two previous studies, this study summarized the utility of cognitive distance and cognitive map concepts, which are proposed by Kevin Lunch (1976) to evaluate city image, in planning Andong municipal museum complex (AMMC). Sample is stratified into city residents and outsiders, and also into the general public and design-related professionals to see if there is any group difference in constructing their mental image. Three major findings are obtained. First, familiarity, so-called the degree of knowing, is the function of the length of stay in a designated area. That is, the longer people stay in Andong, the more likely they are familiar with its overall environmental aspects. Second, mental proximity of Andong municipal museum complex relative to existing cultural landmarks is closely related to the degree of how people value those landmarks in terms of their significance. Dosan Seowon and Hahoe folk village are most highly valued, which means higher proximity. Third, functional diversity turned out to be the most important design dimension, while display mechanism are least valued. Cognitive simulations of this sort are meaningful in that projected composite image might be a rough first approximation of true public image.

An analysis of unplanned reoperation ('계획에 없던 재수술' 의 분석)

  • Background: Clinical indicators are objective measures of process or outcome of patient care in quantitative terms. This study aims to review the medical records of patients who 'return to operating room during the same admission', which is one of the critical clinical outcomes, and describe the result by unplanned reoperation rate. Methods: Computerized patient registry was used for selecting subject conditions. For medical records retrieved, two nurse evaluators identified the presence of explicit reoperation planning in medical records. Results: Overall reoperation rate was 2.8% and unplanned reoperation rate 1.3%. The main category of reoperation cause was the postoperative bleeding. Duration of stay from previous operation to reoperation of the unplanned group, 12.7 days, was shorter than that of the planned(p< .05). The differences did not reach statistical significance in age, sex and length of stay. Conclusion: Results suggested that unplanned reoperation rate was lower than 'threshold' level other institutions had established. However, this result could become comparable only after management of medical records would be improved and risk adjusted.

Factors Associated with Length of Stay in Elderly Inpatients in a General Hospital in Seoul (서울소재 3차 의료기관에 재원 중인 노인 환자의 재원기간에 영향을 미치는 관련 요인)

  • To identify characteristics of elderly inpatients who had long term hospitalization in a general hospital, this study categorized into two groups who were hospitalized for less than 30 days and a group hospitalized for more than 30 days. To compare the groups the independent variables were the sociodemographical characteristics, the medical care utility characteristics, and the disease characteristics and finally analyzed factors affecting the hospitalization period. The data of 18,727 inpatients who were older than 65 years of age by the year 2005 were used and the data were analyzed using SPSS for Windows 12.0. With the results, it is necessary to provide intensive and positive management to elderly inpatients who belong to the more than 30 days group and also necessary to share roles and functions of hospital by medical network with local hospitals and clinics in order to manage long-term elderly inpatients and offer continuous post-management to discharged patients by encouraging them to use a long-term care center or by implementing an early discharge program. This study should provide many studies on how to manage the period of hospitalization efficiently on long-stay elderly inpatients in the future.
