We present a case of left ventricular pseudoaneurysm, which is a very rare and fatal complication of cardiac procedures such as mitral valve replacement. A 55-year-old woman presented to the Department of Thoracic and Cardiovascular Surgery at Hanyang University Seoul Hospital with chest pain. Ten years prior, the patient had undergone double valve replacement due to aortic regurgitation and mitral steno-insufficiency. Surgical repair was successfully performed using a prosthetic pericardial patch via a left lateral thoracotomy.

We report a case of pseudo-pseudoaneurysm, which is a very rare complication of myocardial infarction. A 69-year-old man was admitted to our clinic with chest tightness and dyspnea. He had undergone aortic valve replacement with a pericardial bioprosthetic valve, ring mitral annuloplasty, and reconstruction of an aortic annular defect due to infective endocarditis with bovine pericardium 4 years prior. Echocardiography and computed tomography showed pericardial effusion and a 16-mm cavity at the anterolateral wall of the left ventricle. Magnetic resonance imaging suggested either pseudo-pseudoaneurysm or myocardial abscess. We successfully repaired the myocardial defect using a patch made from a vascular graft with pledgeted horizontal mattress sutures under cardiopulmonary bypass.

좌심실 가성류로 내원한 39세 남자 환자에 대한 수술 치험 예를 보고하고자 한다. 환자는 4년 전 판막륜 농양을 동반한 감염성 심내막염으로 기계판막을 이용한 대동맥판막 및 승모판막 치환술과 농양 제거수술을 받았다. 수술 후 시행한 심초음파 소견상 좌심실과 농양이 있던 공동 사이에 교통이 있음이 관찰되었고, 추적 관찰 심초음파 검사에서 점차 공동의 크기가 증가하였다. 수술은 심페바이패스와 심정지하에 대동맥판막-승모판막 섬유연속부에 위치한 가성류와 좌심실 사이의 결손을 첩포폐쇄하였다. 수술 후 경과는 양호하였으며 수술 후 9일째에 합병증 없이 퇴원하였다.

심근 경색 후 발생하는 심실 중격 결손은 드문 질환이다. 그러나 심실 중격 결손이 발생한 경우 서둘러 수술을 하지 않으면 90% 이상의 환자가 사망에 이르는 매우 위험한 질환이다. 심근 경색 후 심실 중격 결손에 대한 수술 방법은 여러 가지가 알려져 있는데 이는 약해진 경색 조직의 봉합부위가 다시 파열되어 중격 결손이 재발하는 것을 방지 하지 위하여 다양한 방법들이 소개되어 왔기 때문이다. 저자들은 관상동맥 조영술상 이상 소견을 보이지 않는 Prinzmetal형 협심증에 의한 심실 중격 결손과 가성 좌심실류의 파열 소견을 보인 환자를 "Sandwich technique"을 이용해 성공적으로 치료하였기에 보고하고자 한다.
