Resection of Pulmonary Tuberculosis An Analysis of 100 Cases (폐결핵 잔류병변에 대한 폐늑막 절제술 100례)

  • During the period of seven years from Jan. 1976 to Jan. 1983, one hundred cases of pulmonary tuberculous residual lesions were resected at the Department of Thoracic Surgery, Paik Hospital in Seoul, Korea. During the period of this study, 1764 patients were admitted with the diagnosis of pulmonary and/or pleural tuberculosis in the medical and surgical department as a primary or associated conditions. Among these 1764 patients, one hundred selective cases were operated. The results were as follows; l. Extents of the disease by the predominant clinical pictures were: totally destroyed lung; 18, destroyed lobe; 6, cavitary lesion with or without positive sputum; 35, bronchiectasis; 7, bronchostenosis with atelectasis; 2, empyema with or without BPF; 20, pleural thickening; 4, tuberculoma; 3, bullous cyst with tuberculosis; 5 cases, or per cent [Table 1]. 2. Male and female ratio was 1.2:1 or 55 and 45 per cent. Age distribution ranged 15 and 55 with average of 33 years [Table 2]. 3. Type of procedures were: pleuropneumonectomy; 15, pneumonectomy; 25, lobectomy; 37, bilobectomy; 6, lobectomy plus segmentectomy; 3, pleurectomy; 14 cases, or percent, Site of resections were: right; 58 and left; 42 cases, or per cent [Table 3]. 4. Incidence of complications were 10 per cent and the mortality was 4 per cent. The causes of morbidity were analyzed. The main causes of death were pulmonary insufficiency; 2, cardiac arrhythmia; 1, and hepatic insufficiency; 1 case or per cent [Table 4]. 5. Pathologic examinations of the resected pulmonary and pleuropulmonary lesions were observed by gross specimen, correlating with the pre-operative indications of the disease [Fig. 1, 2, 3, 4, 5, 6].>br> 6. Anti-tuberculous chemotherapy was done for 6 to 18 months, post-operatively, in 80 patients. Of these 49 cases were need medication for 12 months [Table 5]. Except the four operative mortality and a case of post-operative recurrent buberculosis under medication, all the other 95 cases are well in activity and free from the disease at the moment.

  • This survey reports on the extensive examination of the qualify of 14 jogging and tennis shoes separately, availavable in the domestic market, Both jogging and tennis shoes were catagorised in two groups, namely the general goods(less expensive ones) and the exclusive goods(expensive models). The detailed classification are shown below; The domestic products meet the general purpose just as that of the foreign products. Due to the adhesion technology, the domestic tennis shoes consist of rubber sole with natural leather upper;In fact, the desirable combination, as foreign products, must be polyurethane/natural leather. The expensive models were heavier than the less expensive ones. The greater weight difference of right and left shoes are shown in the expensive models than that of the less expensive ones. The moisture absorption ratio of the expensive model is far superior in comparison with the less expensive model. The reason is identified by the material supplyed to produce the expensive model consisted of the natural leather and proper parts. The abrasion resistance of the expensive models were excellent that of the less expensive ones, because the rubber soles were showed the properties better than polyurethane sole which was used in the less expensive model.

Generating a Stereoscopic Image from a Monoscopic Camera (단안 카메라를 이용한 입체영상 생성)

  • In this paper, we propose a method of producing a stereoscopic image from multiple images captured from a monoscopic camera. By translating a camera in the horizontal direction, left and right images are chosen among N captured images. For this, image edges are extracted and a rotational angle is estimated from edge orientation. Also, a translational vector is also estimated from the correlation of projected image data. Then, two optimal images are chosen and subsequently compensated using the rotational angle as well as the translational vector in order to make a satisfactory stereoscopic image. The proposed method was performed on thirty-two test image set. The subjective visual fatigue test was carried out to validate the 3D quality of stereoscopic images. In terms of visual fatigue, the 3D satisfaction ratio reached approximately 84%.

Super-resolution Reconstruction Method for Plenoptic Images based on Reliability of Disparity (시차의 신뢰도를 이용한 플렌옵틱 영상의 초고해상도 복원 방법)

  • In this paper, we propose a super-resolution reconstruction algorithm for plenoptic images based on the reliability of disparity. The subperture image generated by the Flanoptic camera image is used for disparity estimation and reconstruction of super-resolution image based on TV_L1 algorithm. In particular, the proposed image reconstruction method is effective in the boundary region where disparity may be relatively inaccurate. The determination of reliability of disparity vector is based on the upper, lower, left and right positional relationship of the sub-aperture image. In our method, the unreliable vectors are excluded in reconstruction. The performance of the proposed method was evaluated by comparing to a bicubic interpolation method, a conventional disparity based method and dictionary based method. The experimental results show that the proposed method provides the best performance in terms of PSNR(Peak Signal to noise ratio), SSIM(Structural Similarity).

  • The method that exists in Photodynamic Therapy uses Photosensibility drug strongly Influencing tumour accumulation together with photochemical laser effect and makes the structure of tumour be localized and become extinct. The intracavity transformation of the Nd :YAP main radiation 1079 nm was Raman converted in barium nitrate crystal and the Stokes frequency (1216 nm) was doubled using KTP or RTA crystals. The LiF or Cr:YAG crystals are used for the Q-switch. The radiation Parameters were obtained at 100 Hz pump repetition frequency. The average power at 608 nm radiation with LiF and KTP was 700 mW at multi-mode generation. The 3-6 single 10-15 ns pulses were generated during one cycle of pumping. The doubling efficiency with RTA was two times more than with KTP. The cells of Ehrlich adenocarcinoma (0.1 ml) were i.m. implanted in hind thighs of ICR white non-imbred mice. The cells were preliminarily diluted in medium 199 in the ratio of 1 to 5. HpD was intravenous administered in a dose of 10 mg/kg. The left clean-shaven hind leg was irradiated with laser light 21-27 hours after the administration of the preparation. The right non-Irradiated leg of each animal served as a control. The animals with the transplanted tumor that were not injected with HpD sewed as a control to estimate the complex effect (HpD+ irradiation). Before the administration of HpD and on 3 and 4 days after irradiation the tumor size was measured and the percent of the tumor growth inhibition was calculated. The results of animal treatments has shown high efficiency of PDT method for cancer treatment by means 0.608 m high power pulse solid state laser.

Clinical Analysis of Suspected Airway Foreign Bodies in Infants and Children (의증 유소아 기도이물의 임상적 고찰)

  • The bronchoscopy is usually performed in the patients who present symptoms of wheezing, hemoptysis, signs of airway obstruction, unexplained long-term chronic cough with or without radiologic features of pneumonia, emphysema or atelectasis. The symptoms and signs of cases of suspected airway foreign body are of often cofused with those of asthma pneumonia and other respiratory tract disease. The ventilating bronchoscopy will be helpful for removal of certain foreign bodies as well as alleviating medically-unresponsive bronchopulmonary conditions via removing abnormal secretions or merely improving ventilation. The authors have performed clinical analysis of 59 patients who were suspected of airway foreign bodies and treated by ventilating bronchoscopy from September 1985 to February 1995. The results were as follows: 1) Among the 59 cases of bronchoscopy, 51 cases(97%) were under the age of 3. The ratio of male and female was 2.69:1 2) Most common presenting symptom was coughing(84.7%) followed by dyspnea, fever, wheezing and anterior chest pain. 3) Among 39 cases of identified foreign bodies, abnormal findings in the chest X-ray films were found in 33 cases(84.6%) and most common abnormal radiologic feature was pneumonia in 10 cases(25.6%) followed by combination of pneumonia and emphysema, or emphysema alone. All the 20 cases of absent foreign body presented abnormal radiologic features and common findings were pneumonia and atelectasis. 4) Vegetable foreign bodies were the most common form of removed airway foreign bodies(69.2%) followed by metallic foreign bodies(12.8%) and plastic foreign bodies(7.7%). 5) By distribution of the location of foreign bodies, 29 cases(74.4%) were located in the main bronchus( 16 cases of right side and 13 cases of left side), 8 cases were in the trachea and 2 cases were found at the cordal level. 6) By duration of foreign body lodgement in the airway, 9 cases(23.1%) were removed within 7 to 30 days, 10 cases(5.6%) within 3 days and 5 cases(12.8%) were enlodged over 30 days. 7) In the cases of absent foreign bodies, common bronchoscopic findings were mucopurulent or thick mucoid discharge in the main bronchi, erythema or edema of bronchial mucosa and all the cases were improved after ventilating bronchoscopy with concomitant medical management.

Time Course of Ventricular Remodeling after Atrial Septal Defect Closure in Adult Patients

  • Background: Atrial septal defect (ASD) is the most common congenital heart disease. However, the details of cardiac chamber remodeling after surgery are not well known, although this is an important issue that should be analyzed to understand long-term outcomes. Methods: Between November 2017 and January 2019, cardiac magnetic resonance imaging was performed preoperatively, at a 1-month postoperative follow-up, and at a 1-year postoperative follow-up. Cardiac chamber volume, valve regurgitation volume, and ejection fraction were measured as functions of time. Results: Thirteen patients (10 men and 3 women) were included. The median age at surgery was 51.4 years. The preoperative median ratio of flow in the pulmonary and systemic circulation was 2.3. The preoperative mean right ventricular (RV) end-diastolic volume index (EDVi) and RV end-systolic volume index (ESVi) had significantly decreased at the 1-month postoperative follow-up (p<0.001, p=0.001, respectively). The decrease in the RVEDVi (p=0.085) and RVESVi (p=0.023) continued until the postoperative 1-year follow-up, although the rate of decrease was slower. Tricuspid valve regurgitation had also decreased at the 1-month postoperative follow-up (p=0.022), and continued to decrease at a reduced rate (p=0.129). Although the RVEDVi and RVESVi improved after ASD closure, the RV volume parameters were still larger than the left ventricular (LV) volume parameters at the 1-year follow-up (RVEDVi vs. LVEDVi: p=0.016; RVESVi vs. LVESVi: p=0.001). Conclusion: Cardiac remodeling after ASD closure is common and mainly occurs in the early postoperative period. However, complete normalization does not occur.

Survival Effect of Complete Multimodal Therapy in Malignant Pleural Mesothelioma

  • Background: Malignant pleural mesothelioma (MPM) is an aggressive pleural malignancy, and despite all multimodal treatment modalities, the 5-year overall survival rate of patients with MPM is less than 20%. In the present study, we aimed to analyze the surgical and prognostic outcomes of patients with MPM who received multimodal treatment. Methods: In this retrospective, single-center study, the records of patients who underwent surgery for MPM between January 2010 and December 2020 at our department were reviewed retrospectively. Results: Sixty-four patients were included in the study, of whom 23 (35.9%) were women and 41 (64.1%) were men. Extrapleural pneumonectomy, pleurectomy/decortication, and extended pleurectomy/decortication procedures were performed in 34.4%, 45.3%, and 20.3% of patients, respectively. The median survival of patients was 21 months, and the 5-year survival rate was 20.2%. Advanced tumor stage (hazard ratio [HR], 1.8; p=0.04), right-sided extrapleural pneumonectomy (HR, 3.1; p=0.02), lymph node metastasis (HR, 1.8; p=0.04), and incomplete multimodal therapy (HR, 1.9; p=0.03) were poor prognostic factors. There was no significant survival difference according to surgical type or histopathological subtype. Conclusion: Multimodal therapy can offer an acceptable survival rate in patients with MPM. Despite its poor reputation in the literature, the survival rate after extrapleural pneumonectomy, especially left-sided, was not as poor as might be expected.

Hot Water Extract of Triticum aestivum L. (Common Wheat) Ameliorates Renal Injury by Inhibiting Apoptosis in a Rat Model of Ischemia/Reperfusion

  • Objectives : Interruption and subsequent restoration of blood flow into the kidney result in renal injury. As an approach to preventing the renal injury, we determined the optimal conditions and the underlying mechanisms by which supernatant of hot water extract of ground Triticum aestivum L. (extract) attenuated ischemia/reperfusion (I/R) injury. Methods : One hour after administration of the extract (400 mg/kg) by intraperitoneal injection, renal I/R injury was generated by clamping the left renal artery in rats after surgical removal of the right kidney, followed by reperfusion. The maximal difference between the vehicle-treated and the extract-treated group under ketamine/xylazine or enflurane anesthetization was assessed at varying periods of ischemia (30-45 min) and reperfusion (3-48 hr), based on the renal function assessed with serum creatinine levels, tissue injury with hematoxylin/eosin staining, and apoptosis with terminal deoxynucleotidyltransferase-mediated dUTP nick-end labeling staining. Results : Enflurane anesthetization with 40 min of ischemia and 24 hr of reperfusion was identified to be the optimal condition, under which condition serum creatinine levels and tubular damage in the extract-treated group were significantly reduced compared with those in the vehicle-treated group ($1.3{\pm}0.2$ versus $2.7{\pm}0.3$ mg/dL, P < 0.01, and average score $1.8{\pm}0.1$ versus $3.5{\pm}0.3$, P < 0.01, respectively). These beneficial effects were mediated by inhibition of apoptotic cascades through attenuation of renal tissue malondialdehyde levels, Bax/Bcl-2 ratio and caspase-3 levels. Conclusions : The extract conferred renal protection against ischemia/reperfusion injury in rats by scavenging reactive oxygen species and consequently blocking apoptotic cascades, plausibly augmented by enflurane protection.

Development and performance evaluation of lateral control simulation-based multi-body dynamics model for autonomous agricultural tractor

  • In this study, we developed a dynamic model and steering controller model for an autonomous tractor and evaluated their performance. The traction force was measured using a 6-component load cell, and the rotational speed of the wheels was monitored using proximity sensors installed on the axles. Torque sensors were employed to measure the axle torque. The PI (proportional integral) controller's coefficients were determined using the trial-error method. The coefficient of the P varied in the range of 0.1 - 0.5 and the I coefficient was determined in 3 increments of 0.01, 0.05, and 0.1. To validate the simulation model, we conducted RMS (root mean square) comparisons between the measured data of axle torque and the simulation results. The performance of the steering controller model was evaluated by analyzing the damping ratio calculated with the first and second overshoots. The average front and rear axle torque ranged from 3.29 - 3.44 and 6.98 - 7.41 kNm, respectively. The average rotational speed of the wheel ranged from 29.21 - 30.55 rpm at the front, and from 21.46 - 21.63 rpm at the rear. The steering controller model exhibited the most stable control performance when the coefficients of P and I were set at 0.5 and 0.01, respectively. The RMS analysis of the axle torque results indicated that the left and right wheel errors were approximately 1.52% and 2.61% (at front) and 7.45% and 7.28% (at rear), respectively.
