Surgically Induced Right Bundle Branch Block with Left Anterior Hemiblock in Postoperative Tetralogy of Fallot (활로씨 4증후군 완전교정후 발생한 좌전각차단)

  • The electrocardiographic changes produced at surgery in all patients undergoing total correction of tetralogy of Fallot at the Seoul National University Hospital from 1961 through August 1979, were reviewed. Particular attention was directed to the presence of postoperative right bundle branch block combined with left anterior hemiblock and its prognosis. 14 patients [9.4%] out of 149 who were survived for 30 days postoperatively, showed right bundle branch block with left anterior hemiblock pattern in electrocardiography. But no complete heart block was occurred in these l 4 patients during the follow up periods of 2-56 months.

  • Takotsubo or reverse Takotsubo cardiomyopathy is a well-known cardiac complication of subarachnoid hemorrhage (SAH) that shows transient left ventricular wall motion abnormalities with electrocardiogram (ECG) changes. ST change followed by T inversion is a common ECG finding complicated with these disorders, left bundle branch block (LBBB) may be a potential ECG pattern which is seen. In this case, we describe the clinical profile and outcomes of a patient with LBBB and reverse Takotsubo cardiomyopathy after anesthetic induction, which was scheduled as an emergent external ventricular drainage after SAH. This is the first report of an LBBB pattern in reverse Takotsubo cardiomyopathy.

  • Arrhythmias are complications of tunneled cuffed hemodialysis catheter insertion. Most complications associated with arrhythmias occur during guide-wire access, where the guide wire can cause traumatic damage to the conduction system of the heart. Conducting system injury in tunneled cuffed hemodialysis catheter insertion often involves the right bundle, causing right bundle branch block (RBBB). Transient RBBB with sinus rhythm is not usually accompanied by abnormal vital signs. However if patients already have left bundle branch block (LBBB), new onset RBBB can cause complete atrioventricular block (AVB), which can lead to fatal complications requiring invasive treatment. We report on a patient with LBBB who developed complete AVB during hemodialysis catheter insertion.

  • Through the use of a dual chamber (DDD) pacemaker, we achieved a cardiac resynchronization effect in a 51-year-old female patient who was transferred to our hospital from another hospital for an operation for three-vessel coronary artery disease. Her electrocardiogram showed a left bundle branch block (LBBB) and a prolonged QRS interval of 166 milliseconds. Severe left ventricle (LV) dysfunction was diagnosed via echocardiography. Coronary artery bypass grafting (CABG) was then performed. In order to accelerate left atrial activation and reduce the conduction defect, DDD pacing using right atrial and left and right ventricular pacing wires was initiated postoperatively. The cardiac output was measured immediately, and one and twelve hours after arrival in the intensive care unit. The cardiac output changed from 2.8, 2.4, and 3.6 L/min without pacing to 3.5, 3.4, and 3.5 L/min on initiation of pacing. The biventricular synchronization using DDD pacing was turned off 18 hours after surgery. She was transferred to a general ward with a cardiac output of 3.9 L/min. In patients with coronary artery disease, severe LV dysfunction, and LBBB, cardiac resynchronization therapy can be achieved through DDD pacing after CABG.

  • In this case report regarding one patient with complete left bundle branch block and mild hypokinesia of left ventricle, who had cebebellar infarction and therefore showed the symptoms of cerabellar dyskinesia followed by dyspnea, chest discomfortness, insomnia and dry cough. From the point of oriental diagnostic criteria, the patient s clinical conditions were all classified as $^{\circ}AEdeprivation$ of kidney essence' and treated accordingly. He showed no change in EKG monitoring but above symptoms were removed markedly after following treatment. Further elaboration of oriental diagnostic classification could possibly lead to the fundamental treatment.

  • In this paper, ECG based cardiac disease diagnosis models are developed. Conventionally, ECG monitoring equipments can only measure and store ECG signals and they always require medical doctor's diagnosis actions which are not desirable for continuous ambulatory monitoring and diagnosis healthcare systems. In this paper, two kinds of neural based self cardiac disease diagnosis engines are developed and tested for four kinds of diseases, sinus bradycardia, sinus tachycardia, left bundle branch block and right bundle branch block. For diagnosis engines, error backpropagation neural network (BP) and probabilistic neural network (PNN) were applied. Five signal features including heart rate, QRS interval, PR interval, QT interval, and T wave types were selected for diagnosis characteristics. To show the validity of proposed diagnosis engine, MIT-BIH database were used to test. Test results showed that BP based diagnosis engine has 71% of diagnosis accuracy which is superior to accuracy of PNN based diagnosis engine. However, PNN based diagnosis engine showed superior diagnosis accuracy for complex-disease diagnoses than BP based diagnosis engine.

Feature Extraction of ECG Signal for Heart Diseases Diagnoses (심장질환진단을 위한 ECG파형의 특징추출)

  • ECG limb lead II signal widely used to diagnosis heart diseases and it is essential to detect ECG events (onsets, offsets and peaks of the QRS complex P wave and T wave) and extract them from ECG signal for heart diseases diagnoses. However, it is very difficult to develop standardized feature extraction formulas since ECG signals are varying on patients and disease types. In this paper, simple feature extraction method from normal and abnormal types of ECG signals is proposed. As a signal features, heart rate, PR interval, QRS interval, QT interval, interval between S wave and baseline, and T wave types are extracted. To show the validity of proposed method, Right Bundle Branch Block (RBBB), Left Bundle Branch Block (LBBB), Sinus Bradycardia, and Sinus Tachycardia data from MIT-BIH arrhythmia database are used for feature extraction and the extraction results showed higher extraction capability compare to conventional formula based extraction method.

  • 본 논문에서는 부정맥 판별을 위한 전처리 과정으로 PCA, LDA, ICA를 바탕으로 하여 정확도를 비교하여 보았다. 각각의 전처리는 고유의 특성을 가지고 있으며 본 논문의 목적은 부정맥 판별상 어떤 전처리가 더욱 정확성의 면에서 효과적인지를 알아보는 것이다. 본 논문의 데이터는 MIT-BIH에 기반하고 있으며, Beat의 분류는 정상(Normal), 좌각차단(Left Bundle Branch Block, LBBB), 우각차단(Right Bundle Branch Block, RBBB), 조기심실수축(Premature Ventricular Contraction, PVC), 조기심방수축(Atrial Premature Beat, APB), paced Beat, 심실보충수축(Ventricular Escape Beat)로 나누었다. 실험적 결과는 PCA-BPNN의 경우 95.53%, ICA-BPNN의 경우 93.95%, LDA-BPNN의 경우 96.42%로 LDA가 가장 ECG 부정맥 판별 응용에 있어 가장 효율적인 방법으로 나타났다.

Electrocardiographic Findings in School Children (국민학생 및 중학생의 심전도 소견)

  • Mass electrocardiographic (ECG) examination was performed on 13,801 children (male 7,526 and female 6,275) of elementary and middle school in Taegu from May 1. 1986. to April 30. 1987. We read their ECG according to the "Pediatric Electrocardiography." The results were as following; The Incidence of ECG abnormality was 1.05%(male 1.3% and female 0.75%). Fifty eight children (0.42%) had atrial and ventricular hypertrophy; two right atrial hypertrophy, five left atrial hypertrophy, thirty five fight ventricular hypertrophy and sixteen left ventricular hypertrophy respectively. Ectopic beats occurred in 25 children (0.18%) ; They were atrial in 12 children, ventricular in 8 children and junctional in 5 children. There were 62 children (0.45%) of conduction disturbance ; They were first degree atrioventricular (A-V) block in 21 children, type I second degree A-V block in 1 child, A-V dissociation in 1 child, right bundle branch block in 36 children, left bundle branch block in 1 child and WPW syndrome in 2 children. Nonspecific ST, T changes and sinus tachycardia were found in 3 and one children respectively.
