2 Cases of Pneumosinus Dilatans (기부비동 확장증(Pneumosinus dilatans) 2례)

  • Pneumosinus Dilatans consists of an abnormal dilatation of the paranasal sinuses which contain air only and lined by normal mucosa. It is a rare condition, the etiology of which is unclear. A 16 year old male complained frontal bossing which developed slowly. Simple X-ray and CT showed abnormal distension of frontal sinus. Another 19 year old male complained slowly growing left cheek mass. Simple X-ray and CT showed abnormal distension of anteromedial wall of maxillary sinus. In view of the cosmetic appearance, operations were performed. Distended sinuses walls were removed and reconstructed using $Medpore^{(R)}$. I report 2 cases of pneumosinus dilatans which developed in frontal and maxillary sinuses and the literature reviewed.

  • Merkel cell carcinoma (MCC) is a relatively rare and aggressive cutaneous neuroendocrine malignancy. It is characterized by high rates of recurrence and metastasis, both to regional lymph nodes and to distant locations. Its characteristic clinical manifestation is a single, painless, hard, erythematous nodule on a sun-exposed area, particularly in older men. Surgical management of both the primary site and the sentinel lymph node is the standard of care. In this article, we describe the diagnosis and treatment of a case of MCC in the left cheek.

  • Salivary gland tumors usually appear in solitary mass in single salivary gland. The coexistence of tumors with different histological types occurring within a unilateral parotid gland is an extremely rare event. We experienced a case which two different types of malignant tumors developed at different time points in same gland; metachronous tumors. The second tumor was excised widely and reconstruction was performed by free tissue transfer. Sensory and motor nerve to the left cheek appeared to be intact, and circulation was adequate. This rare case was presented in this article.

  • Trigeminal neuralgia is a unique disease entity to be dealt with at pain clinic due to it's severe pain attack. A 33-year old male patient had severe pain on left cheek area usually initiating from first upper molar tooth area for three years. We successfully treated this patient with maxillary nerve block using pure alcohol by a lateral approach. Three months after maxillary block the patient is still pain free.

  • The objective of this study was to observe the healing of skin and mucosal grafts, comparing with the control sites. Six adult dogs were divided into two groups of three. Mucosal defects were created in all four quadrants, and the left side was used as a control. In the Group I animals, a split-thickness skin from the chest was grafted, and in the group II animals, a split-thickness mucosa from the buccal cheek was grafted into the mouth. Gross and microscopic examination was done at 3, 7, 14, 28, 42 days. The results were as follows. 1. Wound epithelization was accelerated in the supraperiosteal grafts. 2. After healing, the grafted skin was unnatural to the oral cavity, showing grayish-white color, hair, and sebaceous gland. But the mucosal grafts could not be discriminated from the adjacent mucosa. 3. the grafting on the cortical bone failed.

  • Embryonal rhabdomyosarcoma of the head and neck is very rare in adults. We report a case of embryonal rhabdomyosarcoma in the nasal cavity, occuring in a 24-year-old male patient presenting with left nasal obstruction, ipsilateral eye and cheek pain and multiple neck nodules. Rhabdomyosarcoma should be considered in the differential diagnosis of poorly defined soft tissue mass of the nasal cavity not only in children, but also in adults.

  • 최근 들어 건강에 대한 관심이 증가하면서 건강관리 및 유지 방법으로 식이요법이 널리 사용되고 있다. 또한 IT 융합 기술이 다양한 분야에 적용되면서 체질과 식품과의 연관성 등을 객관적으로 입증하기 위한 식품 IT 기술이 연구되고 있다. 따라서 본 논문에서는 한의학적 진단 이론과 IT 영상처리 기술을 연계하여 칡이 간과 위장에 미치는 영향을 분석하는 실험을 수행하였다. 이를 위해 20대 남성10명을 대상으로 칡즙을 복용하고 간과 위장에 관련된 얼굴 부위인 좌측뺨과 콧등 영역의 색상 변화를 측정하여 칡과 간, 위장과의 상관성을 분석하는 연구를 수행하였다.

  • Pneumoparotid is a rare diagnosis to consider when faced with unexplained parotid swelling. It is caused by an excessive increase of intraoral pressure and reflux of air through the Stensen duct and its glandular branches. It is more likely to occur in persons who have regularly raised intraoral pressure, for example glass and balloon blowers and wind instrument players. It can also be self-induced by habitual tic. Herein, we report a rare case of pneumoparotid with initial presentation of painless swelling in left parotid region. It was caused by habitual cheek inflating and improved with conservative management.

Intramasseteric schwannoma treated with facelift incision and retrograde facial nerve dissection

  • Schwannoma is a slow-growing, well-demarcated, benign soft tissue tumor of the peripheral nerve sheath. It commonly develops in the head and neck region, usually in the parapharyngeal space. In this case, a 42-year-old woman visited the outpatient department to manage a painless mass on her left cheek. She had no history of concern and no neurological symptoms were observed. In the enhanced computed tomography scan, a 2.8×2.8×1.8 cm, heterogeneously enhanced tumor was detected in the left masseter muscle. A tumor resection under general anesthesia was planned. For the resection, a facelift incision was chosen; branches of the facial nerve were identified and retrogradely dissected. A well-marginated, yellowish, solid mass was found in the left masseter muscle. The mass was excised and given a histopathological diagnosis of schwannoma. A definite diagnosis of schwannoma, originating in the masseter muscle, is difficult to arrive at with radiographic findings alone; it is often misdiagnosed as intramuscular hemangioma. Histopathological examinations, including fine-needle aspiration or histological biopsy after surgery, are necessary. Using a facelift incision with retrograde facial nerve dissection, tumor resection in an intramasseteric lesion can be performed efficiently, without nerve damage, or leaving conspicuous scars on the face.

Treatment of Lymphangioma combined with Facial Bone Deformity (안면골 변형을 동반한 림프관종의 치험례)

  • Lymphangioma is a benign, growth of lymphatic tissue that is present at birth or develops in early childhood, which may cause serious alterations in growth and developmemt. The problems with facial lymphangioma is usually releated directly to their size and to the area of the face which is involved. The lesions themselves may range from small, localized blemishes to huge facial masses involving both soft tissue and underlying bone and causing great distortion and asymmetry. The facial bones are seldom involved, but the natutal evolution of an individual lesion often cannot be accurately predicted when the child is first seen. Any changes in the underlying facial bone could be due either to a direct growth of the lesion into the bone, or secondary to pressure of the lesion growing outside the bone itself. A case of cystic lymphangioma extending from the neck to the tongue is reported. A six-year-old female was admitted because of swelling of the tongue. At that time, the tongue reportedly reached the extraoral size of 7x5x2.5cm and a soft, diffuse swelling of left anterior neck was revealed. The removal of cystic mass including left neck dissection and partial glossectomy were undertaken. The another case of lymphangioma is located on mandibular cheek. A twenty nine-year-old male was admitted because of palpable mass of the left mandibular area and fissure of palate. The radical excision of mass with mandibulectomy of body were undertuken. Thus we reported such a rare case and reviewed the lymphangioma.
