The purpose of this study was to verify the effects of developed assisting wear which maximize trunk(back) muscle's stretch-shortening effect during backswing and downswing for increasing golf ball carrying distance. Design and fabrication of assistive wear were performed based on the results of analyzed data of trunk EMG activity from the golf drive swings of elite professional male golfers during back swing and downswing phases. After the prototype of wear was produced, surface EMG and Flight scope tests were conducted to verify the effectiveness of the wear for increasing distance to the professional golfers. Results indicated that wearing trial showed significant longer carry distance than the non-wearing trial(p<.001). The carry distance of wearing trial showed an average 229 m compared to the non-wearing trial, the average 225 m. The swing with wearing also produced significantly faster ball speed than the trial without wearing(p<.05). Average 245 Km/h and 244 Km/h were produced for the swing with and without wearing trails, respectively. EMG results also indicated that the muscle activity of left psoas was significantly increased for wearing trial during downswing and near the impact. Thus, this may affect positively to increase club head speed. The activity of the left latissimus dorsi was dramatically increased during the final stage of swing. This generates elongation effect for longer follow-through and increased impulse between club and ball so eventually valid assistance to increase carry distance. Therefore, the developed assisting wear was proved to be effective tool for increasing golf ball carry distance with maximizing trunk(back) muscle's stretch-shortening effect during backswing and downswing.
Among many kinds of introduced free flaps, scapular freeflap is one of the most popularly using modalities in fasciocutaneous defect coverage with minimal donor defect and easier procedure and constant vascular patterns of the donor. Many surgeons who had experience of this flap pointed out deficit of the reliable sensation of the transplanted flap is the main shortcoming of the scapular free flap. If we can subjugate that point, scapular free flap is the most excellent procedure in such a cases as heel pad reconstruction and hand reconstruction which are relatively important to have skin with protective sensation. Author performed anatomical literature review, 10 cadaveric dissections and 12 clinical dissections. In surgical anatomical aspect, the upper six dorsal rami of the thoracic nerves have medial branches which pierce Longissimus thoracis and Multifidus muscle with small cutaneous twigs which pierce Latissimus dorsi and Trapezius muscle. Among that cutaneous twigs, several twigs distribute to the skin of the back from midline to lateral aspect which territory is identical to scapular free flap. We analysed clinical experiences of that sensory bearing scapular free flap surgical anatomy and one year follow-up studies with several results. 1) Two to three cutaneous twigs which pierced from the Trapezius muscle over the scapular free flap region. 2) Each twigs has two to four nerve fascicles with small artery. 3) The nerve distributed to the ordinary scapular free flap and large enough size and pedicle length to neurorrhapy with various recipient site nerves. 4) The inconvenience of this procedure is the vascular pedicle and nerve pedicle have opposite directions, vascular pedicle of that comes from lateral direction from subscapular vessels, but nerve pedicle comes from medial direction from trapezius muscle. Author can found constant cutaneous nerve branches which come from piercing the Trapezius. This nerves are helpful for protective sensation in transplanted scapular free flap. We can't had enough follow-up and evaluation of the nerve function of this procedure, we need continuous research works to application of this procedure. The in conveniences come from directional differences of pedicle can solve with longer harvest neural pedicle and change direction of the neural pedicle.

The purpose of this study was to determine the comparison of upper arm, trunk and core muscle activities according to different performance in high Plank exercise using weightless exercise equipment. Six males(age, 23.00±0.73 years; height, 172.95±2.05 cm; body mass, 66.83±2.75 kg; and BMI, 22.33±0.72 kg/m2) completed this study as the subjects. Four type's high Plank exercises using weightless exercise equipment were performed(high plank, HP; high plank with air walker, HPAW; high plank with surf board, HPSB; high plank with waist trainer, HPWT). For the EMG analysis, we measured the body muscle activities of right side on the deltoid(DT), triceps brachii(TB), latissimus dorsi(LD), and external oblique(EO). This research's results were as follows. DT, TB, LD, & EO muscle activities were greatest during HPSB(p=.000). Therefore, these results are expected to serve as basic data for high Plank exercise using weightless exercise equipment performance applications in effective exercise programs.
This study was performed to measure the posture correction effect of wearing ergonomic experimental clothing. Two types of experimental clothing (A, B) were developed. Both of them reinforced the muscles surrounding the spine. Experimental clothing A was developed by reflecting the shape and position of the pectoralis major muscle, abdominal muscle, and latissimus dorsi muscle. Experiment clothing B was developed by reflecting the integrated form of those muscles. Subjects were males in their 20s~30s (n=32). They were divided into two groups based on their acromion distance (AD): the turtle neck syndrome group (n=16, AD=3.81cm) and the normal group (n=16, AD=1.27cm). The correction of body posture was detected by three index values: change of the cervical curvature angle, horizontal distance between the seventh cervical vertebra and tragion, and AD. The comfort and tightness of the clothing were also measured. The results showed that both types of experimental clothing corrected body posture and that turtle neck syndrome can be prevented by wearing these types of posture correcting clothing. Two index values were decreased: cervical curvature angle ($0.31{\sim}1.32^{\circ}$) and horizontal distance between seventh cervical vertebra and tragion (0.22~0.31cm). The chest was also stretched. The comfort was rated as good for both types of experimental clothing. These results indicate that the experimental clothing had a posture correction effect without any discomfort for daily living.
Purpose: Reconstruction of chest wall has always been a challenging problem. Muscle flaps for chest wall reconstruction have been helpful in controling infection, filling dead space and covering the prosthetic material in this challenge. However, when we use muscle flaps, functional and cosmetic donor site morbidities could occur. The authors applied and revised various partial muscle flaps and combination use of them to cover the prosthetic material for the chest wall reconstruction and evaluated the usefulness of partial muscle flaps. Methods: This study included 7 patients who underwent chest wall reconstruction using partial muscle flap to cover prosthetic material from 2004 to 2008. The pectoralis major muscle was used in anterior 2/3 parts of it leaving lateral 1/3 parts of it. The anterior 2/3 parts of the pectoralis major muscle were used while lateral 1/3 parts were left. In case of the rectus abdominis muscle flap, we used upper half of it, or we dissected it around its origin and then advanced to cover the site. The latissimus dorsi muscle flap was elevated with lateral portion of it along the descending branch of the thoracodorsal artery. If single partial muscle flap could not cover whole prosthetic material, it would be covered with combination of various partial muscle flaps adjacent to the coverage site. Results: Flap coverage of the prosthetic material and chest wall reconstructions were successfully done. There occurred no immediate and delayed post operative complications such as surgical site infection, seroma, deformity of donor site and functional impairment. Conclusion: When we use the muscle flaps to cover prosthetic material for chest wall reconstruction, use of the partial muscle flaps could be a good way to reduce donor site morbidity. Combination of multiple partial flaps could be a valuable and good alternative way to overcome the disadvantages of partial muscle flaps such as limitation of volume and size as well as flap mobility.
Free flap transplantation demands meticulous microsurgical technique to cover the exposed vital structures which is important to restore and maintain functions of the extremities. From July 1992 through December 2000, 99 patients were received reconstructive microsurgery in the upper and lower extremity at Department of Orthopedic Surgery, Chonbuk National University Hospital. The most common cause in the upper extremity was industrial accident, 8 cases of total 15 cases and in the lower extremity was traffic accident, 66 cases of total 84 cases. The most commonly involved site was thumb and finger, 8 cases of total 15 cases and in the lower extremity was leg, 65 cases of total 84 cases. In upper extremity, the wrap around free flap was carried out in 4 cases(4.0%), first dorsal metatarsal artery flap and lateral arm flap were 3 cases(3.0%) each in 15 cases and in lower extremity, latissimus dorsi myocutaneous flap were 23 cases(23.2%), gracilis 20cases(20.2%), and rectus abdominis muscle flap 18(18.2%) in 84 cases. Overall 89 cases(89.9%) of 99 cases were survived and maintained and revealed good cosmetic results.

Purpose: Scoliosis is a multifactorial disorder caused by genetic, biochemical, developmental, neuromuscular factors and causes complex deformities which include skeletal deformity, pain, cardiovascular dysfunction, motor function disorder. Until now, the treatment of scoliosis have been focused on orthopedic correction, preservation of cardiopulmonary and neurologic function. But recently, as aesthetic demands increases, the needs for the correction or improvement of the trunk and extremity contour does. So, the correction of soft tissue contour deformity can be a new concept for the treatment of scoliosis. Methods: We corrected a deformed contour with prefabricated silicone implant in a scoliosis patient who had been operated for orthopedic correction previously. Submuscular pocket was made under trapezius and latissimus dorsi muscle. Silicone implant was placed in the pocket and fixed to thoracolumbar fascia with sutures. Results: We had a satisfactory results for the correction of contour deformity. There was no significant complication. Conclusion: Silicone implant is a new trial for the correction of scoliosis contour deformity. This method is simple, safe and brings on satisfactory results.

Purpose: To report the case of recurred malignant proliferating trichilemmal tumor on scalp Methods: Case report and literature review Results: A 40-year-old man presented with a recurred malignant proliferating trichilemmal tumor, which developed on the occipital area. It was $8{\times}9{\times}4.5cm$ in size, protruded, firm and non-tender mass. This tumor was widely excised including normal skin margin and pericranium. Immediate reconstruction using free latissimus dorsi muscle flap and skin graft were done. Histopathologic examination revealed abrubt, compact trichilemmal keratinization in the central area of lobular epithelial proliferation. Conclusion: There has been no recurrence of tumor during follow-up period of nine months without adjuvant chemotherapy or radiotherapy.
This research was conducted to biomechanically analyze Tippelt motion in parallel bars, and establish technical understanding. To achieve that goal, the performances of the Tippelt acts carried out by five world top-class national gymnasts in the parallel bars 3-dimensional cinematographic analysis and EMG analysis were conducted and following conclusion were obtained. The Tippelt motions of excellent national gymnasts perform tap motion through the down swing of a large circular movements, and perform kick-out motion rapidly extending shoulder joint angle and hip joint angle with the trunk in a position close to perpendicular position at the vertical downwardness of the grasping the bars. At this time, if handstand starting the movement is too delayed or rapidly down swung, it was shown that from the initial falling, unnecessary muscular power was wasted in trapezius, anterior deltoid, erector spinae, latissimus dorsi, upper rectus abdominis, lower rectus abdominis. The muscular parts in tap motion generating muscle action potential were pectoralis major, rectus femoris, upper rectus abdominis, lower rectus abdominis, and those in kick-out motion were upper rectus abdominis, lower rectus abdominis, trapezius and anterior deltoid.
Objectives : The research was conducted for the use of HT1 in the shoulder disease through correct acupuncture. Methods : (1) The contents were compared through reviewing literature. (2) The literature was studied in conjunction with the shoulder pathology and anatomical structures. Results : (1) The location of HT1 is described as 'in the axilla, over the axillary artery' in many literature, and the depth of HT1 is as shallow as 1 cm. The WHO standard also follows this. (2) There were many references to 'in the axilla, between the big muscles' in later generations, but there were mixed opinions about the exact muscle names. Based on the acupunctural review and the musculoskeletal study, the big muscles are considered to be 'Pectoralis major' and 'Latissimus dorsi'. (3) Among the muscles constituting the rotator cuff, applying acupuncture on HT1 is essential for 'Subscapularis m.'. Therefore, it is effective to stimulate 'Subscapularis m.' to a depth of 1.5 to 4 cm. Conclusions : The accurate acupoint of HT1 enables various uses of flank, armpit, shoulder and arm diseases as well as cardiopathy. Especially, it helps to treat the muscles through safe and effective acupuncture in shoulder rotator cuff disorder.
