Scapulothoracic dissociation is a rare injury caused by blunt force or massive traction to the anterolateral shoulder girdle with disruption of scapulothoracic articulation. This injury is characterized by massive soft-tissue swelling of the shoulder with intact skin, lateralization of damaged scapular. It frequently is accompanied by various musculoskeletal injuries and neurovascular injuries. 33-year-old man sustained a scapulothoracic dissociation without fracture and Vascular injuries during arresting. We report diagnostic method, clinical finding and non-operative management with literatures.

MIT has been applied for nonfluent aphasic patients on the basis of lateralization of brain hemisphere. However, its applications for different languages have some inquiry for aphasic patients because of prosodic and rhythmic differences. The purpose of this study is to develop the Korean Melodic Intonation Therapy program using personal computer and its clinical effects for nonfluent aphasic patients. The algorithm was composed to voice analog signal, PCM, AMDF, Short-time autocorrelation function and center clipping. The main menu contains pitch, waveform, sound intensity and speech files on window. Aphasic patients' intonation patterns overlay on selected kMIT patterns. Three aphasic patients with or without kMIT training participated in this study. Four affirmative sentences and two interrogative sentences were uttered on CSL by stimulus of ST. VOT, VD, Hold and TD were measured on Spectrogram. In addition, articulation disorders and intonation patterns were evaluated objectively on spectrogram. The results indicated that nonfluent aphasic patients with kMIT training group showed some clinical effects of speech intelligibility based on VOT, TD values, articulation evaluation and prosodic pattern changes.
The purpose of this research was to investigate dichotic listening, that is, whether the left and right ears have different functionalities in actual listening. Unlike previous studies, which used short words or a list of two or three numbers, these researchers investigated more comprehensive listening ability using the Toeic listening test. Sixty-three university students participated in this study. The subjects took three tests in total: one using only the right ear, another using the left ear, and the third using both. T-analysis of SPSS on the data revealed that there is a significant difference between the test results with only the right or left ear and the ones with both ears. So we cannot find right-ear priority in this study. We can see that in comprehensive listening, right hemisphere plays an important role as well as left hemisphere. When grouped according to language ability, test result of low group is the same as the total test result. However, in high group there is no significant difference between the test results with left ear and the results with both ears in high group. In mid group there is no significant difference between the test results with right ear and the results with both ears. The different functionalities of left and right ears and suggestions for future research are presented.
Objectives Sex hormones exposure during the prenatal period has an effect on cerebral lateralization. Male brains are thought to be more lateralized than female brains. Bipolar disorder was known to show abnormalities in cerebral laterality whose characteristics could be estimated by electroencephalography (EEG) coherences. We studied sex-related differences of EEG coherences between healthy controls and patients with bipolar disorder to examine the sex effects in the genesis of bipolar disorder. Methods Participants were 25 patients with bipolar disorder (11 male, 14 female) and 46 healthy controls (23 male, 23 female). EEG was recorded in the eyes closed resting state. To examine dominant EEG coherence associated with sex differences in both groups within five frequency bands (delta, theta, alpha, beta, and gamma) across several brain regions, statistical analyses were performed using analysis of covariance. Results Though statistically meaningful results were not found, some remarkable findings were noted. Healthy control females showed more increased interhemispheric coherences than control males in gamma frequency band. There were no differences in the intrahemispheric coherences between the healthy control males and females. In patients with bipolar disorder, female dominant pattern in interhemispheric coherences was attenuated compared with healthy control. Conclusions Sex differences of EEG coherences, which could be a marker for cerebral laterality, were attenuated in patients with bipolar disorder compared with healthy controls. These results imply that abnormal sex hormone exposure during early development might play some role in the pathogenesis of bipolar disorder.
This study examined the characteristics of linguistic prosody in terms of cerebral lateralization in three groups of 9 healthy speakers and 14 speakers with a history of stroke (7 with left hemisphere damage (LHD), 7 with right hemisphere damage (RHD)). Specifically, prosodic characteristics related to speech rate, duration, pitch, and intensity were examined in three types of interrogative sentences (wh-questions, yes-no questions, alternative questions) with auditory perceptual evaluation. As a result, the statistically significant key variables showed flaws in production of the linguistic prosody in the speakers with LHD. The statistically significant variables were more insufficiently produced for wh-questions than for yes-no and alternative questions. This trend was particularly noticeable in variables related to pitch and speech rate. This result suggests that when Korean speakers process linguistic prosody, such as that of lexico-semantic and syntactic information in interrogative sentences, the left hemisphere seems to be superior to the right hemisphere.
The critical bandwidth represents response interactions with respect to a signal tone and their neighboring bands. This study was to analyze the critical bandwidths of a clinically important 500 Hz tone in Korean young male and female subjects (male = 10, female = 10) at a conversational level (60 dB HL). Data were measured with notched band noise and two alternative forced choice methods. Results showed that the critical bandwidth was slightly greater (95 Hz) than the previous Western measures. There were no statistically significant differences in gender, nor were there any significant differences in lateralization of the ear (p > 0.05). These results may have implications in optimizing effective tinnitus masking or the related clinical applications.

Background: Restoration of external (ER) and internal rotation (IR) after Grammont-style reverse shoulder arthroplasty (RSA) is often unreliable. The purpose of this systematic review was to evaluate the influence of RSA medio-lateral offset and subscapularis repair on axial rotation after RSA. Methods: We conducted a systematic review of studies evaluating axial rotation (ER, IR, or both) after RSA with a defined implant design. Medio-lateral implant classification was adopted from Werthel et al. Meta-analysis was conducted using a random-effects model. Results: Thirty-two studies reporting 2,233 RSAs were included (mean patient age, 72.5 years; follow-up, 43 months; 64% female). The subscapularis was repaired in 91% (n=2,032) of shoulders and did not differ based on global implant lateralization (91% for both, P=0.602). On meta-analysis, globally lateralized implants achieved greater postoperative ER (40° [36°-44°] vs. 27° [22°-32°], P<0.001) and postoperative improvement in ER (20° [15°-26°] vs. 10° [5°-15°], P<0.001). Lateralized implants with subscapularis repair or medialized implants without subscapularis repair had significantly greater postoperative ER and postoperative improvement in ER compared to globally medialized implants with subscapularis repair (P<0.001 for both). Mean postoperative IR was reported in 56% (n=18) of studies and achieved the minimum necessary IR in 51% of lateralized (n=325, 5 cohorts) versus 36% (n=177, 5 cohorts) of medialized implants. Conclusions: Lateralized RSA produces superior axial rotation compared to medialized RSA. Lateralized RSA with subscapularis repair and medialized RSA without subscapularis repair provide greater axial rotation compared to medialized RSA with subscapularis repair. Level of evidence: 2A.
Purpose: To investigate the localization and functional lateralization of the supplementary motor area (SMA) in motor activation tests in comparison to that of the primary motor area. Materials and Methods: Seven healthy volunteers obtained echoplanar imaging blood oxygen level dependent technique. This study was carried on 1.5T Siemens Magnetom Vision system with the standard head coil. Parameters of EPI were followed as; TR/TE : 1.0/66.0msec, flip angle: $90^{\circ}$, field of view: $22cm{\times}22cm,{\;}matrix:{\;}128{\times}128$, slice number/slice thickness/gap: 1O/4mm/0.8mm with fat suppression technique. Motor task as finger opposition in each hand consisted of 3 sets of alternative rest and activation periods. Postprocessing were done on Stimulate 5.0 by using cross-correlation statistics. To compare the functional lateralization of the SMA in the right and left hand tests, each examination was evaluated for the percent change of signal intensity and the number of activated voxels both in the SMA and in the pri¬mary motor area. Hemispheric asymmetry was defined as difference of summation of the activted voxels between each hemisphere. Results: Percent change of signal intensity in the SMA (2.49 -3.06%) is lower than that of primary motor area(4.4 -7.23%). Percent change of signal intensity including activated voxels were observed almost equally in the right and left SMA. As for summation of activated voxels, primary motor area had significant difference between each hemisphere but not did the SMA. Conclusion: Preferred contralateral dominant hemisphere and hemispheric asymmetry were detected in the primary motor area but not in the SMA.

Purpose : We performed EEG and PET on children with epilepsy concomitantly in order to evaluate the effects of epileptiform and non-epileptiform discharge of EEG on glucose metabolism. Methods : Seventy three children with epilepsy who had PET and EEG simultaneously were included in our study. The subjects were classified in two ways: (1) based on the frequency of epileptiform discharge and (2) the severity of non-epileptiform discharge. We evaluated the clinical aspects of their seizures, the severity of focal slow waves during the interictal period with the frequency of spikes or sharp waves in order to compare with the PET results. Results : The subjects were divided by the frequency of epileptiform discharge, with 13 in the no/rare group, 7 in the occasional group, and 53 children in the frequent group. The concordant rates with PET in each group were 0%, 42.9%, and 67.9%, respectively, showing high correlations with the frequency of epileptiform discharge (P<0.05, r=0.491). The subjects as divided by the severity of non-epileptiform discharge were 15 in the no group, 25 in the infrequent group, 17 in the intermediate group, and 16 in the continuous group. The concurrence rates with PET for each group were 13.3%, 52.0%, 64.7%, and 68.8%, respectively, also showing a high correlation with the severity of non-epileptiform discharge (P<0.05, r= 0.365). Conclusion : Epileptiform discharge and non-epileptiform discharge in EEG showed a certain association with hypometabolism in PET. We recommend EEG to reduce false lateralization and to localize lesions in cases of high frequency and severity.
Purpose: We developed a computer-aided classifier using artificial neural network (ANN) to discriminate the cerebral metabolic pattern of medial and lateral temporal lobe epilepsy (TLE). Materials and Methods: We studied brain F-18-FDG PET images of 113 epilepsy patients sugically and pathologically proven as medial TLE (left 41, right 42) or lateral TLE (left 14, right 16). PET images were spatially transformed onto a standard template and normalized to the mean counts of cortical regions. Asymmetry indices for predefined 17 mirrored regions to hemispheric midline and those for medial and lateral temporal lobes were used as input features for ANN. ANN classifier was composed of 3 independent multi-layered perceptrons (1 for left/right lateralization and 2 for medial/lateral discrimination) and trained to interpret metabolic patterns and produce one of 4 diagnoses (L/R medial TLE or L/R lateral TLE). Randomly selected 8 images from each group were used to train the ANN classifier and remaining 51 images were used as test sets. The accuracy of the diagnosis with ANN was estimated by averaging the agreement rates of independent 50 trials and compared to that of nuclear medicine experts. Results: The accuracy in lateralization was 89% by the human experts and 90% by the ANN classifier Overall accuracy in localization of epileptogenic zones by the ANN classifier was 69%, which was comparable to that by the human experts (72%). Conclusion: We conclude that ANN classifier performed as well as human experts and could be potentially useful supporting tool for the differential diagnosis of TLE.
