The Importance of Age as a Factor of Carpal Tunnel Syndrome management (수근관 증후군 치료결정의 한 요인으로 연령의 중요성)

  • Background : Carpal tunnel syndrome(CTS) is the most common entrapment neuropathy that refers to a group of signs and symptoms resulting from compression of the median nerve at the wrist. The course of CTS in older patients is different from the younger patients. This difference may be the result of different underlying mechanisms. The different nerve conduction studies of CTS may signify different approaches in management. This study was done to assess the differences in nerve conduction study of CTS in younger and older patients. Methods : This study involved 224 patients who visited Gachon Medical School, Gil Medical Center and was diagnosed by nerve conduction study from October 1997 to October 1999. We compared the results of nerve conduction study to age, especially in between those under 60 years and those 60 years or over CTS patients. Nerve conduction study consists of motor studies of both median nerves(terminal latency, compound action potential) and sensory studies(nerve conduction velocity, nerve action potential). And we also evaluated the variables between younger and older patients group. Those variables include sex, symptom period, laterality, abnormal physical findings and radiculopathy. Results : We found that a significant increase of terminal latency(p<0.1), but a decrease in compound motor action potential(p<0.05) in older patient's group. There was no significant differences in sensory nerve conduction velocity and action potential between those under 60 years and those 60 years or even patients. And also there was no significant difference in sex, symptom period, laterality, abnormal physical findings, radiculopathy between older and younger patients. Conclusions : This study showed a significant increase in the terminal latency and a decrease in compound action potential in older patients. The different nerve conduction studies of CTS by age effect may need different approaches in management.

A Clinical Study of Pediatric Inguinal Hernia Before and After Using Ultrasonography for Diagnosis (소아 서혜부탈장 진단에 초음파검사 실시 후 임상 양상의 변화)

  • The purpose of this retrospective study was to evaluate the effects of diagnostic sonography in pediatric patients with inguinal hernias. The patients were classified into two groups. Group A included the patients who had been operated upon for inguinal hernia in 1980's, when diagnostic sonography was not available. Group B included the patients, operated upon for inguinal hernia from 2001 to 2002, when inguinal sonography was employed to detect potential bilateral hernias. The age distribution, sex ratio, laterality, bilaterality, and concomitant symptoms were compared between group A and group B. There were 296 cases in group A and 377 cases in group B. The prevalent age group was from 1 to 5 years. There was no difference in age group distribution between both groups. The male to female ratio was 5.3:1 in group A and 3.5:1 in group B. The ratio of unilateral to bilateral hernia was 5:1 in group A and 3:1 in group B. In cases with a unilateral hernia, the ratio of right to left was 1.5:1 in group A and 1.8:1 in group B. In cases with bilateral hernia, the simultaneous bilateral hernia was 33 cases (67.4 %) in group A and 75 cases (80.6 %) in group B. The sequential bilateral hernia was 16 cases (32.7 %) in group A and 18 cases (19.4 %) in group B. Although the ratio of bilateral hernia was increased in group B, the portion of the sequential bilateral hernia was significantly decreased in group B. In conclusion, there were no differences in the age distribution and the laterality between group A and B. The ratio of female patients and the incidence of bilateral hernia were increased in group B even though the portion of the sequential bilateral hernia was decreased. This result shows that the preoperative inguinal sonography in unilateral hernia with potential bilateral hernia is useful in early detection of the sequential contralateral hernia.

The Sex-Related Differences of EEG Coherences between Patients with Bipolar Disorder and Controls (양극성장애 환자와 대조군에서 뇌파 코히런스의 성별 차이)

  • Objectives Sex hormones exposure during the prenatal period has an effect on cerebral lateralization. Male brains are thought to be more lateralized than female brains. Bipolar disorder was known to show abnormalities in cerebral laterality whose characteristics could be estimated by electroencephalography (EEG) coherences. We studied sex-related differences of EEG coherences between healthy controls and patients with bipolar disorder to examine the sex effects in the genesis of bipolar disorder. Methods Participants were 25 patients with bipolar disorder (11 male, 14 female) and 46 healthy controls (23 male, 23 female). EEG was recorded in the eyes closed resting state. To examine dominant EEG coherence associated with sex differences in both groups within five frequency bands (delta, theta, alpha, beta, and gamma) across several brain regions, statistical analyses were performed using analysis of covariance. Results Though statistically meaningful results were not found, some remarkable findings were noted. Healthy control females showed more increased interhemispheric coherences than control males in gamma frequency band. There were no differences in the intrahemispheric coherences between the healthy control males and females. In patients with bipolar disorder, female dominant pattern in interhemispheric coherences was attenuated compared with healthy control. Conclusions Sex differences of EEG coherences, which could be a marker for cerebral laterality, were attenuated in patients with bipolar disorder compared with healthy controls. These results imply that abnormal sex hormone exposure during early development might play some role in the pathogenesis of bipolar disorder.

Prevalence and characteristics of pneumatized articular tubercle: First large series in Iranian people

  • Purpose: This study was performed to determine the prevalence and characteristics of pneumatized articular tubercle or eminence among a defined group of Iranian people. Materials and Methods: Digital panoramic radiographs of 1694 patients in the Department of Oral and Maxillofacial Radiology, Hamadan Dental School, Iran were evaluated retrospectively to detect the above lesion. Finally, 1563 radiographs were selected according to inclusion criteria. Then, a review was done of 10 large case series found using a MEDLINE search of the literature. Chi-squared test was used to analyze the differences in variables such as age, gender, laterality, and locularity in our case series. Results: The average age of our samples was $32.6{\pm}7.63$ years. Pneumatized articular tubercle was found in 98 cases, representing a prevalence of 6.2% with a mean age of $22.8{\pm}7.9$ and a range of 8 to 60 years. Sixty-four (65.3%) pneumatized articular tubercles were unilateral, with 30 lesions on the right and 34 on the left side. Bilateral lesions were found in 34 (34.7%) patients. 52 (53.06%) of the pneumatized articular tubercles were of the unilocular type and 46 (46.94%) were multilocular. The results showed no statistically significant differences regarding age (p=0.454), gender (p=0.634), laterality (p=0.252), or locularity (p=0.807) among the samples. Conclusion: Among ten large case series from other countries, the prevalence of pneumatized articular tubercle (6.2%) in Iranian patients was higher than that of all eight of the case series that used the same detection method as the present study of panoramic radiography.

Language Lateralization Using Magnetoencephalography (MEG): A Preliminary Study (뇌자도를 이용한 언어 편재화: 예비 연구)

  • Backgrounds: MEG can measure the task-specific neurophysiologic activity with good spatial and time resolution. Language lateralization using noninvasive method has been a subject of interest in resective brain surgery. We purposed to develop a paradigm for language lateralization using MEG and validate its feasibility. Methods: Magnetic fields were obtained in 12 neurosurgical candidates and one volunteer for language tasks, with a 306 channel whole head MEG. Language tasks were word listening, reading and picture naming. We tested two word listening paradigms: semantic decision of meaning of abstract nouns, and recognition of repeated words. The subjects were instructed to silently name or read, and respond with pushing button or not. We decided language dominance according to the number of acceptable equivalent current dipoles (ECD) modeled by sequential single dipole, and the mean magnetic field strength by root mean square value, in each hemisphere. We collected clinical data including Wada test. Results: Magnetic fields evoked by word listening were generally distributed in bilateral temporoparietal areas with variable hemispheric dominance. Language tasks using visual stimuli frequently evoked magnetic field in posterior midline area, which made laterality decision difficult. Response during task resulted in more artifacts and different results depending on responding hand. Laterality decision with mean magnetic field strength was more concordant with Wada than the method with ECD number of each hemisphere. Conclusions: Word listening task without hand response is the most feasible paradigm for language lateralization using MEG. Mean magnetic field strength in each hemisphere is a proper index for hemispheric dominance.

Cone-beam computed tomographic evaluation of the root canal anatomy of the lower premolars and molars in a Brazilian sub-population

  • Purpose: This study evaluated anatomical variations in the root canals of the lower premolars and molars in a Brazilian sub-population using cone-beam computed tomography (CBCT). Materials and Methods: In total, 121 CBCT images of patients were selected from a database. All images contained lower first and second premolars and molars on both sides of the arch, fully developed roots, and no treatment, resorption, or calcifications. In each image, the root canals of the lower premolars and molars were evaluated according to the Vertucci classification in On-Demand 3D software in the multiplanar reconstruction with dynamic navigation. Twenty-five percent of the images were re-assessed to analyze intraobserver confidence with the kappa test. Data were statistically evaluated with linear regression to evaluate the correlations of anatomic variations with age and sex, and the Wilcoxon test to analyze the laterality of variations, with a significance level of 5%. Results: The intraobserver agreement (0.94) was excellent. In general, the root canals of lower premolars and molars showed a higher prevalence of type I than other Vertucci classification types, followed by type V in premolars and type II in molars. When the molar roots were evaluated separately, type II was more frequent in mesial roots and type I in distal roots. Although age showed no correlations with the results, sex and laterality showed correlations with tooth 45 and the lower second premolars, respectively. Conclusion: The lower premolars and molars of a Brazilian sub-population showed a wide range of root canal anatomic variations.

Compairison of Effect of Manual Myofascial Release and Self Myofascial Release Technique Using a Foam Roller on Pain Thresholds and Body Schema in Subjects with Chronic Tension-type Headache (도수근막이완기법과 폼롤러를 이용한 자가근막이완기법이 만성 긴장성 두통 환자의 통증문턱값 및 신체 도식에 미치는 효과 비교)

  • Purpose: Headache is a very common disease experienced at least once in daily life, and tension-type headaches have a high and increasing prevalence. Chronic headaches can cause functional damage and huge socioeconomic impacts. This study aimed to compare the effects of myofascial release technique with manual therapy and self-myofascial release technique using a foam roller on the pain threshold and body schema in patients with chronic tension-type headaches. Methods: The study was conducted on 20 patients living in Busan with chronic tension headaches. Myofascial release technique with manual therapy was performed on the suboccipital, sternocleidomastoid, scalene, and upper trapezius muscles. The self -myofascial release technique using a foam roller was applied to the cervical and thoracic muscles. A laterality test was performed using a recognized neck application developed to evaluate body schema ability. A pressure-pain threshold test was performed using an electronic pressure algometer to compare the results before and after the myofascial release technique. Results: After applying myofascial release with manual therapy and a foam roller, the pressure-pain threshold values showed significant changes in both groups (p<.05). As a result of the laterality test, myofascial release with manual therapy and a foam roller were applied to the painful area. The values showed significant changes in both groups (p<.05), but only the group using the foam roller showed a significant difference (p<.05) in painless areas. Conclusion: The myofascial release technique with manual therapy can be the primary treatment technique for pain control in painful areas. The self-muscle release technique using a foam roller can be an effective method when there is no pain or when maintenance is needed after pain control.

  • Primary aldosteronism is a group of disorders in which the autonomous secretion of aldosterone is associated with hypertension and hypokalemia. It is crucial to determine the laterality of aldosterone hypersecretion because treatment options differ accordingly. Adrenal venous sampling (AVS) is considered the most reliable method for assessing the laterality of primary aldosteronism. This procedure is often technically challenging because of the small size and varied locations of the adrenal veins. A better understanding of anatomical variations and careful review of imaging studies would improve sampling success. This report presents three cases of anatomical variations encountered during AVS.

Quantitative Electroencephalogram Markers for Predicting Cerebral Amyloid Pathology in Non-Demented Older Individuals With Depression: A Preliminary Study (비치매 노인 우울증 환자에서 대뇌 아밀로이드 병리 예측을 위한 정량화 뇌파 지표: 예비연구)

  • Objectives: When elderly patients show depressive symptoms, discrimination between depressive disorder and prodromal phase of Alzheimer's disease is important. We tested whether a quantitative electroencephalogram (qEEG) marker was associated with cerebral amyloid-β (Aβ) deposition in older adults with depression. Methods: Non-demented older individuals (≥ 55years) diagnosed with depression were included in the analyses (n = 63; 76.2% female; mean age ± standard deviation 73.7 ± 6.87 years). The participants were divided into Aβ+ (n = 32) and Aβ- (n = 31) groups based on amyloid PET assessment. EEG was recorded during the 7min eye-closed (EC) phase and 3min eye-open (EO) phase, and all EEG data were analyzed using Fourier transform spectral analysis. We tested interaction effects among Aβ positivity, condition (EC vs. EO), laterality (left, midline, or right), and polarity (frontal, central, or posterior) for EEG alpha band power. Then, the EC-to-EO alpha reactivity index (ARI) was examined as a neurophysiological marker for predicting Aβ+ in depressed older adults. Results: The mean power spectral density of the alpha band in EO phase showed a significant difference between the Aβ+ and Aβ- groups (F = 6.258, p = 0.015). A significant 3-way interaction was observed among Aβ positivity, condition, and laterality on alpha-band power after adjusting for age, sex, educational years, global cognitive function, medication use, and white matter hyperintensities on MRI (F = 3.720, p = 0.030). However, post-hoc analyses showed no significant difference in ARI according to Aβ status in any regions of interest. Conclusion: Among older adults with depression, increased power in EO phase alpha band was associated with Aβ positivity. However, EC-to-EO ARI was not confirmed as a predictor for Aβ+ in depressed older individuals. Future studies with larger samples are needed to confirm our results.

A study on the apparel sizing system of adult women (성인여성 기성복의 치수 간격설정에 관한 연구)

  • The purposes of this paper were to suggest the procedures for a sizing sys- tem which can provide good fitting of apparel and minimize the loss due to excessive inventory, and to determine an adequate apparel sizing standard. An anthropometric database used for this study was the 1992 National Anthro- pometric Survey of Koreans. The database was limited to 1,336 women who were 18-51 years old. They study was conducted by classifying ages into two groups (18-33, 34-51) using Wilk's lamda. Three principal components : laterality( fullness), linearity(length) and characteristic of torso were selected to describe body types, and these three body type classification of each group were selected by cluster analysis. It was found that all intervals between standard sizes were not equal. They were narrow around the center with high frequencies of the customers but wide in both tail with low frequencies. It was also found that the optimal sizes suing the loss function can be applied well in practice.
