A RADIOGRAPHIC STUDY OF THE MANDIBULAR ASYMMETRY IN TEMPOROMANDIBULAR DISORDER PATIENTS (악관절장애 환자의 하악골 비대칭에 대한 방사선학적 연구)

  • The purpose of this study was to observe the relationship between mandibular asymmetry and Temporomandibular Disorders by means of the cephalometry using the posteroanterior cephalogram and the submentovertex cephalogram which were taken in 35 Temporomandibular Disorder patients and 35 normal persons ranged from 20S to 30S. The results were as follows: 1. The angulation which was formed by the median line with the ANS-Menton line (MAP) was greater in patients group and there was statistically significant difference. 2. The angulation which was formed to the median line with the Menton-Odontoid process tip line (MES), the difference of the distances from the center of the posterior surface of the both condyles to the most anterior point of the chin (DD), the difference of the distances from the center of the both condyles to the horizontal reference line (DE), the difference of the angulations which were formed by the both condyles axes with the horizontal reference line (DCE), the difference of the lengths of the both condyles (CL) and the difference of the widths of the both condyles (DW) were greater in patients group and there were statistically significant differences. There was reversed correlation between MAP and the difference of the distances from the 3. bilateral points of the lateral margin of the both zygomaticofrontal sutures to the at the lateral inferior margin of the both antegonial protuberances in mandible (DH). There was reversed correlation between MES and DD, DE, DCE. 5. There was correlation between MAP and MES.

3-dimensional reconstruction of mandibular canal at the interforaminal region using micro-computed tomography in Korean

  • PURPOSE. The purpose of this study was to identify the complex course of the mandibular canal using 3D reconstruction of microCT images and to provide the diagram for clinicians to help them understand at the interforaminal region in Korean. MATERIALS AND METHODS. Twenty-six hemimandibles obtained from cadavers were examined using microCT, and the images were reconstructed. At both the midpoint of mental foramen and the tip of anterior loop, the bucco-lingual position, the height from the mandibular inferior border, the horizontal distance between two points, and position relative to tooth site on the mandibular canal were measured. The angle that the mental canal diverges from the mandibular canal was measured in posterior-superior and lateral-superior direction. RESULTS. The buccal distance from the mandibular canal was significantly much shorter than lingual distance at both the mental foramen and the tip of anterior loop. The mandibular canal at the tip of anterior loop was significantly located closer to buccal side and higher than at the mental foramen. And the mental canal most commonly diverged from the mandibular canal below the first premolar by approximately $50^{\circ}$ posterior-superior and $41^{\circ}$ lateral-superior direction, which had with a mean length of 5.19 mm in front of the mental foramen, and exited to the mental foramen below the second premolar. CONCLUSION. These results suggest that it could form a hazardous tetrahedron space at the interforaminal region, thus, the clinician need to pay attention to the width of a premolar tooth from the mental foramen during dental implant placement.

Effect of Gastric Filling on the Images of Hepatic Scintigraphy (위의 충만유무에 의한 간신티그램영상의 변화)

  • To observe the effect of gastric filling on the images of hepatic scintigraphy, we performed hepatic scintigraphy using $^{99m}Tc-tin$ colloid in twenty patients both in the fasting state and after meal, and made the visual comparison of both images. We found that gastric filling could cause some of the following changes in individual patients. 1) In the anterior view, the distance between the liver and the spleen increased and the interlobar notch of the inferior hepatic border was obliterated. The spleen was displaced downwards, its radioactivity in the upper medial part was reduced, and its upper pole was laterally displaced. 2) In the posterior view, the left lobe of the liver was separated from the spleen and the distance between the liver and the spleen increased. 3) In the right lateral view, the notch separating the right and the left lobes of the liver was well demarcated. 4) In the left lateral view, the spleen was separated from the liver and the distance between the liver and the spleen increased. The shape of the spleen was also changed. In case of the follow-up study, above changes should make one consider the possibility of the effect of gastric filling.

Analysis of Exit Site of Guide Pin Using Tansglenoid Suture Technique in Bankart and SLAP Lesion (Bankart 병변과 SLAP 병변에서 경견관절와 술식시 Guide Pin 출구의 분석)

  • Purpose: To Analyze the exit site of pin inserted at the anterior glenoid rim in the reconstruction of the Bankart lesion and SLAP lesion using transglenoid suture technique. Materials and Methods: In the twenty adult right cadeveric scapula, insertions of pin were performed using guide at the position of 1, 2, 3 O'clock of glenoid rim. We measured the exit site of dorsal surface of the scapula by medial distance from sagittal plane of lateral border of scapular spine and the vertical distance from posterior border of the scapular spine. Results: When the pin was inserted caudally within 10 degree, at the position of 1, 2, 3 O'clock, the medial distance from lateral border of the scapula is averaged 29.4, 19.2, 34.0 mm respectively and the vertical distance from posterior border of the scapular spine is averaged 15.0, 18.6, 17.2 mm respectively. When the pin was inserted caudally within 20-30 degree, the medial distance is averaged 14.6, 14.2, 15.8 mm respectively and the vertical distance is averaged 31.6, 31.9, 32.1 mm respectively. Conclusion: When the pin was inserted caudally within ten degrees using the guide, the pin exit appeared at the more medial side of the base of scapular spine and the more inferior of scapular spine. This can make the firm suture tied over scapular spine during repair SLAP and the Bankart lesion, and also prevent the injury of suprascapular nerve.

Acupuncture Treatment about Medial Meniscus Posterior Horn Rupture : A Case Report

  • Objectives : In this case, the knee joint inconvenience with deteriorating pain has been relieved by acupuncture treatment for a patient was 51-year-old male with a left medial meniscal posterior horn rupture in 2012. Methods : Twenty-four times of acupuncture treatments were performed for Twelve weeks from July 2016 to improve the disease. SP8(Jigi; 地機), BL63(Geummun; 金門), KI3(Taegye; 太谿), LI4(Hapgok; 合谷) were chosen for treatment by principles of Traditional Korean Medicine. Results : Numeric Rating Scales, which means subjective pain, decreased from 6 points to 3 points out of 10 points in total. Pressure Pain Threshold, which means sensitivity to pressure applied to the affected area, increased from 21N to 47N on the lateral-inferior side and from 19N to 50N on the lateral side. K-WOMAC, which indicates discomfort of knee-related activity, was 56 points out of 96 points in total before treatment and 4 points after treatment, 13 points after two years, and 15 points after four years. However, MRI tests conducted before and after treatment did not identify any significant changes. As a result, we confirmed that a total of 24 acupuncture treatments had resulted in the relief objective and subjective pain and functional recovery, especially in the case of the knee function, maintained until after four years later. There was no significant substrate recovery in meniscal rupture.

A COMPUTERIZED TOMOGRAPHIC STUDY ON THE STRUCTURE OF THE MANDIBULAR RAMUS (전산화단층사진을 이용한 하악지구조분석)

  • This study was conducted for the purpose of suggestion of the new technique of sagittal split ramus osteotomy pararell to the true sagittal plane. This pararellism is the important concept of the sagittal split ramus osteotomy to reduce the condylar sagging including mandibular hypomobility, temporomandibular disorder, occlusal relapse and other complications. We used 26 adult dry manibles(52 rami), and obtained the computed tomographs through the sagittal, horizontal and coronal sections. The results were obtained as follows. 1. On sagittal section, mean area of S1 was $8.63{\pm}2.10cm^2$, S2 was $8.93{\pm}1.94cm^2$, S3 was $9.49{\pm}2.15cm^2$, S6 was $10.72{\pm}2.22cm^2$. The wider area of sagittal section, the more lateral section, But, no significant differency between the areas of the sagittal sections(P>0.05). 2. On horizontal section, The distance between the inferior alveolar canal and the lateral cortical plate of the mandibular ramus were $6.73{\pm}1.24mm$ minum, $7.70{\pm}1.44mm$ maximum. 3. On coronal section, Outer mandibular angle were $4.84{\pm}2.37^{\circ}$ right side, $4.93{\pm}2.12^{\circ}$ left side. 4. The design of the ideal true sagittal split ramus osteotomy is that posterior border of osteotomy must be limited vertically, at the right posterior point of lingula mandibularis and anterior of osteotomy must be extended to mandibular body, anteroinferiorly.

Two Cases of Radial Forearm Free Flap Reconstruction after Wide Vertical Hemipahryngolaryngectomy (진행된 이상와암의 광범위 수직인후두부분절제술 후 요전완 유리피판을 이용한 재건술 2예)

  • Wide vertical hemilaryngopharyngectomy with immediate glottic and pharyngeal reconstruction using a radial forearm free flap is reported in 1991 by Chantrain et al. This procedure was designed for the preservation of healthy hemilarynx and resection of pharynx with safe oncological margin in especially piriform sinus cancer or supraglottic cancer invading the hypopharynx. In the original paper, they used palmaris longus tendon for reconstruction of neoglottis. In other groups, they used rib cartilage instead of palmaris longus tendon. In this paper, we report two cases of piriform sinus cancer patients who treated with wide vertical hemilaryngectomy with radial forearm free flap reconstruction. In one case, the operation was performed as Chantrain et al described. But in another case, the ipsilateral forearm was impossible due to the positive Allen's test. So the contralateral forearm flap and rib cartilage graft was done. This reconstructive technique make large resection possible. As the dissection of thyroid cartilage and lateral displacement makes direct visualization and manipulation of piriform sinus lesions, sufficient resection margin in lateral and inferior pharyngeal wall cab be obtained.

  • Spontaneous intramuscular hematoma of the abdominal wall is a rare condition characterized by acute abdominal pain. It is often misdiagnosed as a surgical condition. It used to be associated with risk factors such as coughing, pregnancy, and anticoagulant therapy. Most cases of abdominal wall hematomas were rectus sheath hematomas caused by the rupture of either the superior or inferior epigastric artery, but spontaneous internal oblique hematoma was extremely rare. In this report, we present a case of spontaneous internal oblique hematoma in a 69-year-old man with non-dialysis chronic kidney disease who was taking cilostazol. The patient complained of abrupt abdominal pain with a painful palpable lateral abdominal mass while sleeping. The abdominal computed tomography showed an 8 cm-sized mass in the patient's left internal oblique muscle. The administration of cilostazol was immediately stopped, and the intramuscular hematoma of the lateral oblique muscle disappeared with conservative management.

Cranioplasty and temporal hollowing correction with a three-dimensional printed bioresorbable mesh and double vertical suture anchor muscle sling: a case report

  • Postoperative temporal hollowing is a common complication of craniotomy. Damage and repositioning of the temporalis muscle can lead to a depression in the temporal side of the skull with inferior bulging, worsening aesthetic outcomes. We report a case of cranioplasty with three-dimensional (3D) printed mesh involving an additional correction using a temporalis muscle sling to help address this problem. A 3D-printed bioabsorbable mesh was prepared based on preoperative facial computed tomography, and was fixed to the hollowed area for tissue augmentation. The temporalis muscle was elevated and fanned out to its original position, and a sling was attached to a screw that was fixed to the mesh. For reinforcement, an additional sling was attached to another screw fixed to the mesh 2-3 cm vertically above the first screw. Aesthetic results were confirmed immediately after surgery and later during outpatient follow-up. Both depression and lateral bulging were resolved, and there was no delayed drooping of the temporalis muscle on 6-month follow-up. There were no complications, and the patient was satisfied with the appearance. This is a simple yet effective technique with a low risk of complications, and should be considered for postoperative temporal hollowing patients, especially those with severe lateral bulging.

The Effect of Meniscectomy on Clinical Result After ACL Reconstruction (전방십자인대 재건술에서 반월상 연골 절제술의 영향)

  • Purpose: To evaluate the effects of total or subtotal meniscectomy on anterior cruciate ligament reconstruction. Materials and Methods: We reviewed the 455 cases of arthroscopic ACLR (anterior cruciate ligament reconstruction) from February, 2003 to February, 2007 and followed-up more than 1 year. The 93 cases were enrolled. The 45 cases who underwent only ACLR were included and the 48 cases who underwent ACLR with total or subtotal meniscectomy were included in this study except grade 3 or 4 chondral lesion, partial meniscetomy or meniscal repair. We divided the patient into 4 groups which were isolated ACLR group (group I, 45cases), ACLR with lateral meniscectomy group (group II, 10cases), ACLR with medial meniscectomy group (group III, 28cases) and ACLR with both medial and lateral meniscectomy group (group IV, 10cases). The clinical evaluation was done by range of motion (ROM), IKDC subjective score, Lysholm score, anterior drawer test, Lachman test, Pivot shift test and KT-1000 arthrometer. Results: At final follow up, group IV was inferior than group I in IKDC subjective score and Lysholm score, and inferior than group II in IKDC subjective score. In KT-1000 arhtometric test, group I had better results than group III and group IV. Also in anterior drawer test and Lachman test, group 1 had better result than group III and group IV. In pivot shift test, there was no significant difference among four groups. Conclusion: Medial or both medial and lateral meniscectomy had greater laxity in anterior drawer test, Lachman test and KT-1000 arthrometric test and both medial and lateral meniscectomy had a lower subjective score than both meniscus intact group.
