  • The purpose of this study was to evaluate the clinical and computed tomographic features of 7 cases of maxillofacial gunshot injuries in the suicidal patients visited the emergency room, Capital Armed Forces General Hospital. The obtained results were as follows : 1. The gunshot wounds were directed from submental area to dorsum of nose(3 cases), frontal area(1 case), orbit(1 case), infraorbital area (1 case), and lateral to nasal wing(l case). Method of Deciding Elastic Modulus of Left and Right Ventricle Reconstructed by Echocardiography Using Finite Element Method and Stress Analysis

  • In order to study the shape and dimensions of heart, a procedure to reconstruct a three dimensional left ventricular geometry from two dimensional echocardiographic images was studied including the coordinate transformation, curve fitting and interpolation utilizing three dimensional position registration arm. Nonlinear material property of the left ventricular myocardium was obtained by finite element method performed on the reconstructed geometry and by optimization techniques which compared the computer predicted 3D deformation with the experimentally determined deformation. Elastic modulus ranged from 3.5g/$cm^2$ at early diastole to l53g/$cm^2$ at around end diastole showing slightly nonlinear relationship between the modulus and the pressure. Afterwards using the obtained nonlinear material propertry the stress distribution related with oxyzen consumption rate was analyzed. The maximum and minimum of ${\sigma}_1$ (max. principal stress) occurred at nodes on the second level intersection points of x-axis with endocardium and with epicardium, respectively. And the tendency of the interventricular septum to be flattened was observed from the compressive ${\sigma}_1$ on the anterior, posterior nodes of left ventricle and from the most significant change of dimension in $D_{RL}$ (septal-lateral dimension of right ventricle).

Protrusion of the infraorbital canal into the maxillary sinus: A cross-sectional study in Cairo, Egypt

  • Purpose: The aim of this study was to investigate the prevalence of infraorbital canal protrusion in an Egyptian subpopulation using cone-beam computed tomography and to describe its radiographic representation. Materials and Methods: This retrospective cross-sectional study was conducted using the records of 77 patients and 123 maxillary sinuses. The full lengths of the sinuses were visible for the detection of infraorbital canal protrusion. The infraorbital canals were classified into 3 types based on their relation to the sinus. If the septum was present, its length and its distance from the sinus floor were measured. Qualitative and quantitative variables were described as percentages and means with standard deviations, respectively. Results: The infraorbital canal most commonly presented as the normal confined type (detected in 78.1% of sinuses), whereas the suspended (or protruded) variant was found in 14.6% of the examined sinuses. The septal length ranged from 0.9 to 5.1 mm, with a mean of 2.8±1.1 mm. The distance to the sinus floor ranged from 5.2 to 29.6 mm depending on the sinus shape and size. Conclusion: The present study indicates that protrusion of the infraorbital canal is not rare, and surgeons that use the maxillary sinuses as corridors for their procedures must be more cautious, especially in the upper lateral confines of the sinus.

피부 제거 없이 시행한 높은 쌍꺼풀의 교정 (Correction of High Fold without Skin Excision)

  • Purpose: Double - eyelid operation is one of the most common cosmetic operations among Korean patients. In such operations, however, the complexity of and individual differences in the patients' anatomical structures may cause various complications, such as asymmetry, retraction of the eyelid, and the occurrence of a high fold. A high fold occurs frequently, and its correction is not very simple. Many methods have been developed to correct it, and among these, the operation involving the excision of the skin between the previous double - eyelid line and the new double - eyelid line is usually selected by plastic surgeons. In many cases, however, patients have insufficient eyelid skins for this operation. In this study, the authors introduce an operation procedure for high - fold correction that does not involve skin excision Methods: From June 2005 to June 2009, 246 cases were treated with this procedure. After the incision of the new double - eyelid line, dissection was done between the previous scar tissue and the levator aponeurosis. Then the orbital septum, orbital fat or the retro - orbicularis oculi fat was slid down and sutured with a tarsal plate. Such sutures were repeated at four to five points, including the lateral and medial limbus, to prevent the reattachment of the previous scar and to create a new double - eyelid line at the end of the orbital septum. Results: Most of the high - fold patients were satisfied with the procedure described above. Their previous scar was hidden under the new double - eyelid line after the operation. In the six cases, the scar was visible in the patients who had a very high and deep inner line. As such, scar revision was undertaken three months after the operation. It is known that scar revision is also required after an operation involving skin excision in the case of a very high inner - eyelid line. Conclusion: This method is an appropriative procedure for high - fold correction for patients who have insufficient upper - eyelid skin.

우심방에 발생한 원발성 악성 림프종의 수술적 치험 - 1예 보고 - (Primary Malignant Cardiac Lymphoma in Right Atrium - A case report-)

  • 심장에 발생하는 원발성 악성 림프종은 매우 드물다. 68세 남자 환자가 점점 심해지는 호흡곤란으로 본원 흉부외과에 입원하였다. 심장 초음파, 흉부컴퓨터단층촬영 후, 우심방과 우심실에 걸쳐 있는 거대 종양을 발견하였다. 뇌경색 등의 위험과 혈역학적 불안정으로 응급수술을 결정하였다. 우측 심장에 있는 거대 종양을 심방 중격의 일부와 전외측 우심방 벽을 함께 포함하여 제거하고, 우형 심내막 패치로 재건하였다. 최종 조직검사상 원발성 악성 림프종으로 진단되었으며, 환자와 보호자가 경제적 이유 등으로 항암치료를 거부하고 귀가 퇴원을 하였다.

회전-신전법의 Mulliken 변형을 이용한 편측 구순열 수술 (Repair of Unilateral Cleft Lip using Mulliken's Modification of Rotation Advancement)

  • Unilateral cleft lip is not a simple and independent problem in all aspects. nasal deformity results from the cleft lip, maxillary hypoplasia, and abnormal muscular pull on the nasal structures, including abnormal muscular tension on the alar base and abnormal position of the orbicularis oris muscle. Its gross and histopathologic characteristics include widening of the alar base, a midline deviation of the columella and septum to the noncleft side, dorsal displacement of the dome, lateral rotation of medial crura, buckling of the alar cartilage, and underdevelopment of the pyriform aperture. Since Dr. Millard first presented his method for repair of the unilateral cleft lip and nasal deformity in 1955, no other technique has gained as much popularity as the rotation-advancement principle. Principles established more than 50 years ago and techniques are evolving continuously. Unlike earlier procedures, this repair gives the surgeon the opportunity to manipulate the individual cleft elements through various modifications while maintaining Millard's original surgical and anatomical goals. Although this strategy is applied worldwide, successful execution is variable and highly operator dependent. Millard and many other surgeons have made technical variations to adjust the procedure to each specific patient, to address some of its faults, and to gain new advantages. We will review the Mulliken's modifications that Dr. Millard made to his original rotation-advancement principle and inform cases applied modifying the rotation-advancement principle.

  • 대하 아가미의 형태와 미세구조를 광학현미경과 전자현미경으로 조사하였다. 대하는 수지형 아가미를 가진 다. 아가미는 출새관과 입새관을 구분하는 세로 격벽을 가진다. 각각의 새판은 전자밀도가 다른 다층구조의 큐터클로 싸여 있다. 새판 기저세포들은 편평형이며, 전자밀도가 높은 세포질을 가진다. 상피세포층은 단층으로 큰 핵을 가진 편평상피로 구성된다. 새판의 기둥구조는 세로방향의 미세소관들과 측면 membrane interdigitation을 가진다. AB-PAS음성 분비세포들은 다세포선의 형태이다. 활성화된 선에서 각 세포의 경계부는 뚜렷하지 않으며, 세포질에는 활면소포체, 미토콘드리아, 전자밀도가 낮으며 막을 가진 분비소포들과 granular rosettes를 가진다. 비 활성화된 선에서는 세포 경계가 뚜렷하며, 세포질에서는 전자밀도가 높은 다수의 작은 과립들이 산재한다. 그리고 alcian blue에 양성인 단세포 분비세포들은 세포질에 발달된 다수의 골지체와 조면소포체들을 함유한다.

고양이에서 비강 림프종의 영상 진단 증례 (Diagnostic Imaging of Nasal Lymphoma in a Cat)

  • A spayed female, 5-year-old, weighing 2.7 kg, domestic short hair cat was referred to Veterinary Medical Teaching Hospital, Seoul National University. The clinical signs of this patient were weight loss, sneezing, respiratory distress, nasal discharge, epistaxis, ocular discharge, left exophthalmos, and left facial edema and deformity. The laboratory tests represented mild leukocytosis. On the skull radiographs, soft tissue density filled nasal cavity with loss of turbinate detail was found. Destructive and lytic changes of the left nasal bone with soft tissue swelling were identified. On the thorax radiographs, there were a tracheobronchial lymph node swelling and a soft tissue round mass in the left caudal lung field. On computed tomographic scan images, asymmetrical destruction of turbinate and nasal septum and increased soft tissue opacity in the nasal cavity were identified. Destruction of the lateral maxillary bone, invasion to the left retrobulbar region, and craniodorsal deviation of the left eye were seen. Also, there was lysis of hard palate and cribriform plate. Invasion to the brain was found. The patient was diagnosed as nasal lymphoma by cytology and histopathology.

Surgical implications of anatomical variation in anterolateral thigh flaps for the reconstruction of oral and maxillofacial soft tissue defects: focus on perforators and pedicles

  • Objectives: To gain information on anatomical variation in anterolateral thigh (ALT) flaps in a series of clinical cases, with special focus on perforators and pedicles, for potential use in reconstruction of oral and maxillofacial soft tissue defects. Materials and Methods: Eight patients who underwent microvascular reconstructive surgery with ALT free flaps after ablative surgery for oral cancer were included. The number of perforators included in cutaneous flaps, location of perforators (septocutaneous or musculocutaneous), and the course of vascular pedicles were intraoperatively investigated. Results: Four cases with a single perforator and four cases with multiple perforators were included in the ALT flap designed along the line from anterior superior iliac spine to patella. Three cases had perforators running the septum between the vastus lateralis and rectus femoris muscle (septocutaneous type), and five cases had perforators running in the vastus lateralis muscle (musculocutaneous type). Regarding the course of vascular pedicles, five cases were derived from the descending branch of the lateral circumflex femoral artery (type I), and three cases were from the transverse branch (type II). Conclusion: Anatomical variation affecting the distribution of perforators and the course of pedicles might prevent use of an ALT free flap in various reconstruction cases. However, these issues can be overcome with an understanding of anatomical variation and meticulous surgical dissection. ALT free flaps are considered reliable options for reconstruction of soft tissue defects of the oral and maxillofacial area.

피뿔고동 ( Rapana venosa Valenciennes )의 Osphardium 에 관한 형태학적 연구 (Morphological Study on the Osphradium of Rapana venosa (Gastropoda : Muricidae))

  • The authors observed histochemical and ultrastructural characters on the osphradium of Rapana venosa Valenciennes using light microscope, scanning and transmission electron microscpes. The results were as follows:1)The basic structure of osphradium was bipectinated shape, which consisted of a septum situating in the center of osphradium and numerous osphradial leaflets. On the other hand, Epidermis of ospradial leaflets formed the structure of pseudostratified ciliated columnar epithelium which was composed of an epithelial cell layer, a basal cel layer and a neuropile. 2) Ciliated dpithelial cells:A large number of these cells were observed on the lateral and ventral regions but a small number of them were observed on the dorsal region. These cells had cylindrical microvilli, slender mitochondria and serve fibers.3) Supporting cells: These cells had cylindrical microvilli, spongy layer, electron dense granules, mitochondria and nerve fibers4) Four types secretory epothelial cells: Four distinct types of secretory epithelial cells were recognized and were arbitrily designated as Type I, Type II, Type III and Type IV.cell type I: These cells contained electron denwe granules(diameter, 0.94-1.56${\mu}{\textrm}{m}$), well developed Golgi apparatus and rough endoplasmic reticula, cell type II: These cills contained two types of granules of the different electron density. One was high electron density granules which were 0.4-1.0${\mu}{\textrm}{m}$ in diameter, The other was low electron density granules which were 0.75-1.2${\mu}{\textrm}{m}$ in diameter.cell type III:These cells had fibrous secretory materials and exhibited strongly positive reaction with Toluidine blue.cell type IV:A large number of this type of cells were observed on the ventral region of ospgradial leaflets and positively reacted with periodic acid Schiff reagent. 5)Dark cells contained several electron dense cillaty rootlets and unmerous granules but cellular organelles were not observed.6) Four types basal cells: Four distinci types of basal cells were recognized and arbitrarily designated as Type I, Type II, Type III and Type IV.Cell type I(light cell): These cells exhibited low electuon density and contained short smooth endoplasmic reticula, several vacuoles and granules.
