LATERAL CEPHALOMETRIC ANALYSIS OF ASYMPTOMATIC VOLUNTEERS AND SYMPTOMATIC PATIENTS WITH TEMPOROMANDIBULAR INTERNAL DERANGEMENT (악관절 내장증 환자와 정상인의 두부방사선규격사진의 분석비교)

  • Study of dentofacial structure relationships relative to TMJ internal derangement is required to increase the predictability of TMJ internal derangement. But few studies have been reported. The purpose of this study is to reveal any correlation of dentofacial characteristics with TMJ internal derangement by lateral cephalometric analysis. Patients were devided into two groups. (1) Symptomatic patients with TMJ internal derangement (2) Asymptomatic volunteers with no TMJ internal derangement. Twenty symptomatic patients with TMJ internal derangement(7male, 13female) were selected from our clinic and had undergone a standarized clinical examination, panorama, transcranical view, TMJ tomography. Twenty asymptomatic volunteers(9male, 11female) were selected from our clinic with no pain, no limitation of motion. All subjects had undergone lateral cephalometric analysis. The results were obtained as follows. 1. No significant difference between ID and normal group is detected in cranial base. 2. Maxilla position of ID group is located more posterioly than normal group. 3. Mandible position of ID group is located more posteriorly than normal group and facial profile is hyperdivergent. 4. Posterior facial height of ID group is less than normal group thus facial profile is hyperdivergent. The patients, as mentioned, have a high prevalance of ID thus it should be careful in TMJ ID diagnosis and treatment.

Condylar position on the lateral individualized corrected tomography in internal derangement of temporomandibular joint (측두하악관절 내장증시 측방 개별화 보정단층방사선사진에서의 하악과두의 위치)

  • Purpose: To examine the possible relationship between condylar position and disk displacement in the temporomandibular joint. Materials and Methods: 79 temporomandibular joints in 40 patients having temporomandibular disorders were classified into three categories: no disk displacement (NDD) , disk displacement with reduction (DDWR), and disk displacement without reduction (DDWOR). Disk positions were assessed from clinical and MRI findings. The relationship between the three categories and condylar positions was evaluated using lateral individualized corrected tomography. Results: Clinical findings regarding the relationship between condyle and disk positions having anterior, centric, and posterior positions were 27%, 27%, and 46%, respectively, in NDD, 43%, 17%, and 40%, respectively, in DDWR, and 44%, 22%, and 34%, respectively, in DDWOR. There were no significant differences in condylar positions between each of the groups (P>0.05). In the relationship between condyle and disk positions with MRI findings, anterior, centric, and posterior positions were 38%, 38%, and 24%, respectively, in NDD, 29%, 21%, and 50%, respectively, in DDWR, and 44%, 9%, and 47% respectively, in DDWOR. There were significant differences in the condylar positions when MRI was utilized (P<0.05) Conclusion: There was a significant correlation between the condyle and disk positions with MRI findings on lateral individualized corrected tomography.

  • A study on the topology optimization of Hard-Disk-Driver(HDD) actuator arm in free vibration is presented. The purpose of this research is to increasse the natural frequency of the first lateral mode of the HDD actuator arm under the constraint of total moment of inertia, so as to facilitate the position control of high speed actuator am. The first lateral mode is an important factor in the position control process. Thus the topology optimization for 2-D model of the HDD actuator arm is considered. A new objective function corresponding to multieigenvalue optimization is suggested to improve the solution of the eigenvalue optimization problem. The material density of the structure is treated as the design variable and the intermediate density is penalized. The effects of different element types and material property functions on the final topology are studied. When the problem is discretized using 8-node element of a uniform density, the smoothly-varying density field is obtained without checker-board patterns incurred. As a result of the study an improved design of the HDD actuator arm is suggested. Dynamic characteristics of the suggested design are compared computationally with those of the old design. With the same amount of the moment of inertia, the natural frequency of the first lateral mode or the suggested design is subsequently increased over the existing one.

Relationship between Lateral Position Change and Sternal Complications after Cardiac Surgery through Median Sternotomy (정중 흉골 절개술을 이용한 심장수술 후 환자의 체위변경과 흉골 합병증 발생과의 관계)

  • Purpose: This study was conducted to examine the relationship between lateral position change and sternal complications after cardiac surgery through median sternotomy. Methods: This study was a retrospective descriptive case-control study, involving 241 patients who underwent cardiac surgery through median sternotomy. Data from October 2011 to September 2014 were collected. Results: Sternal complications (i.e. dehiscence, sternal instability, mediastinitis) developed in 33 patients (13.7%). Primary symptoms of complications were discharge and erythema, and the mean time difference from surgery to appearance of symptoms was 15 days (range, 1-138 days). The factors associated with sternal complications were cancer comorbidity (${\chi}^2=5.22$, p=.039), internal mammary artery procedure (${\chi}^2=4.16$, p=.041), and duration of extra-corporeal membrane oxygenation (p=.033). Position change was not related to incidence of sternal complications (${\chi}^2=0.14$, p=.704). Pressure ulcers appeared in 63 patients (26.1%). Mean time difference from surgery until occurrence of ulcers was 6.7 hours (range, 0-323.0 hours), but position change was started from 132.4 hours (range, 27.1-503.2 hours) after intensive care unit admission. Conclusions: These results provide baseline data to create a standard position change and activity protocol for patients after median sternotomy. Furthermore, the study could help clinical practitioners establish evidence-based nursing practices.

The Effect of Neck and Shoulder Self-Stretching Exercise Using Audiovisual Media on Neck Pain, Postural Alignment, and Joint Position Error in Women with Chronic Neck Pain (시청각 매체를 활용한 목, 어깨 자가신장운동이 만성 목통증 여성의 통증, 자세정렬과 관절위치감각에 미치는 영향)

  • Background: The purpose of this study is to investigate the effect of neck and shoulder self-stretching exercise using audiovisual media on neck pain, postural alignment, and joint position error in women with chronic neck pain. Methods: The subjects included 20 women that gave consent to participate in the study voluntarily. They performed the self-stretching exercises using audiovisual media was carried out 20 minutes 5 times a week during 3 weeks. Neck disability index (NDI) and visual analogue scale (VAS) were used to measure the functional disability and pain, A pressure pain threshold was measured using an algometer, and a cervical range of motion (CROM) measurement tool was used to measure the range of motion and error of proprioceptive position sense of the cervical spine. To assess posture alignment, forward head angle (FHA), forward shoulder angle (FSA) were measured using image J software. Results: The neck pain intensity was statistically significantly within group (p<.05). Neck and shoulder functional disability were a statistically significant difference within group (p<.05). Splenius capitis and upper trapezius pressure pain threshold were statistically significant difference in within group (p<.05). The postural alignment was statistically significantly within group (p<.05). The cervical range of motion in neck extension, right and left lateral flexion were statistically significantly within group (p<.05). The joint position error in neck flexion, extension, right and left lateral flexion decreased statistically significantly within group (p<.05). Conclusion: Self-stretching exercise using audiovisual media increased the mobility of the neck, decreased neck pain and joint position error, and improved posture alignment. As a result, there was a positive effect by applying the self-stretching exercise using audiovisual media to people with neck pain. Based on this, it is thought that it can be used as the basis for research related to home training programs for healthy self-management.

A COMPARATIVE STUDY ON THE STANDARDIZED AND INDIVIDUALIZED OBLIQUE LATERAL TRANSCRANIAL RADIOGRAPHS OF THE TEMPOROMANDIBULAR JOINT (규격화 및 개인별 악관절사측방향 투사법에 관한 비교연구)

  • The author obtained 120 oblique-lateral transcranial radiograms by two projection methods from 30 subjects with clinically normal TMJ. The relative position of the condyle within the articular fossa and the quality of the radiographic images were compared in the standardized and individualized radiographic groups. The results were as follows: 1. The condylar position in the individualized radiographic group was more posterior than that in the standardized radiographic group (P<0.001). 2. The horizontal angle between the long axis of condyle and intermeatal line in the individualized group ranged from 0° to 31° (14.38°±7.55°), and the highest prevalence was found between 15 and 19° 3. The individualized group showed prominent radiopaque shadow of the anterior condylar border; the image detail was superior in the standardized group to that of the individualized group (P<0.05).

Simulation of Vehicle Steering Control through Differential Braking (차동 제동을 이용한 조향 제어 시뮬레이션)

  • This paper examines the usefulness of a Brake Steer System (BSS), which uses differential brake forces for steering intervention in the context of Intelligent Transportation Systems (ITS). In order to help the car to turn, a yaw moment can be achieved by altering the left/right and front/rear brake distribution. This resulting yaw moment on the vehicle affects lateral position thereby providing a limited steering function. The steering function achieved through BSS can then be used to control lateral position in an unintended road departure system. A 8-DOF nonlinear vehicle model including STI tire model will be validated using the equations of motion of the vehicle. Then a controller will be developed. This controller, which will be a PID controller tuned by Ziegler-Nichols, will be designed to explore BSS feasibility by modifying the brake distribution through the control of the yaw rate of the vehicle.

  • A 4 years old male mixed breed dog, which weighed 25.5 kg, referred to Veterinary Teaching Hospital at College of Veterinary Medicine in Chonnam National University, because of chronic hindleg lameness. Click sound by flexion and extension of stifle joint, positive reaction by cranial drawer movement, fat pad sign and tibial position in cranial drawer position on the lateral radiographic view, medial patellar luxation (grade II), and lameness score 2.5 at standing and 2 at walking in right hindleg were showed. Under general anesthesia with enflurane, after medial arthrotomy, it was performed to remove remnants of cranial cruciate ligament and torn medial menisci and joint closed. In modified retinacular imbrication technique, one lateral fabellar/tuberosity suture, one medial fabellar/tuberosity suture, and one imbrication suture adjacent to the patella were placed. At 9 days after operation, lameness score 0 was observed and general condition was excellent.

  • This study was done using the 45° oblique lateral old, 18 males and 27 females, with normal occlusion, premolars and molars on upper and lower jaws. Axial inclination to nasal floor, occlusal plane and inter-axial inclination were examined. In addition the position of each tooth was examined in height and depth in upper and lower jaws. The results were obtained as follows; 1. The inclination of long axis of upper 1st premolar was most nearly perpendicular, upper canine was tilted mesially, and 2nd premolar and molars were tilted distally. 2. The inclination of long axis of lowers molar were tilted mesially. 3. There were no severe variation on the inter-axial inclination of canine to mandibular plane, and 2nd molar.
