Discoid meniscus has been well known to a frequent anomaly of meniscus and much reported in diagnosis and treatment. But, associated X-ray findings have not been known well. We reported its significance of X-ray findings in discoid meniscus. From May 1995 to May 1997, 31 cases of discoid meniscus were compared with 51 cases of nondiseoid meniscus both confirmed by arthroscopy, with simple X-my findings. Both groups were evaluated by X-ray findings in view of lateral joint space widening, peripheral spur of lateral joint space, sclerotic change of lateral tibial plateau, concave lateral tibial plateau, flattening of lateral femoral condyle, hypoplasia of lateral femoral condyle and high riding fibula. The results of analysis were as follows: 1. Widening of lateral joint space, sclerotic change of lateral tibial plateau and cupping of lateral tibial plateau were statistically significant in discoid meniscus. 2. X-ray findings in discoid meniscus were not associated with patient's injury history and symptom duration. 3. Other X-ray findings were not related in discoid meniscus significantly. X-ray findings with patient's history and physical examination arc helpful in the diagnosis of discoid meniscus.

Among the various types of congenital meniscal anomalies, the double-layered lateral meniscus is extremely rare. The double-layered meniscus consists of both the upper additional and the lower normal meniscus. As the upper additional meniscus is mobile, it can be easily displaced, while the lower lateral meniscus is usually normal in shape and volume. A 42-year-old woman suffering from pain and locking of her left knee underwent Magnetic resonance imaging (MRI) examination and an arthroscopic surgery. A rare meniscal abnormality was seen in her left knee, which presented as a double-layered lateral meniscus with displacement. It was remarkable that the upper additional meniscus was displaced over the intercondylar eminence of the tibia and it mimicked a bucket-handle tear. Even though it is rare, it is necessary to consider the possibility of displaced double-layered meniscus in the differential diagnosis of a bucket-handle tear. Here, we report the MRI and arthroscopic findings of a displaced double-layered lateral meniscus, which was similar to the bucket-handle tear.

외측 연골은 형태적으로 내측 연골보다 더 다양하다. 외측 연골의 변이형태는 크기, 형태, 안정성 관점에서 다양하다. 다양한 형태는 모든 연령층에서 발생할 수 있으며, 가장 흔한 형태는 외측 원판형 연골이다. 저자들은 최근 반월상 연골의 전각부가 전방십자인대의 전방부에 부착되어 있는 변형이 동반된 외측 원판형 연골을 경험하여 보고하고자 한다. 45세 여자 환자로 특별한 외상력없이 3~4년간 슬관절 동통을 호소하였으며, 자기공명영상 검사상 외측 원판형 연골로 진단되었다. 관절경 검사상에서는 반월상 연골의 전각부가 전방십자인대의 전방부에 부착되어 있는 변형이 동반된 완전형 외측 원판형 연골로 복합파열이 동반되어 있어 아전절제술을 시행하였으며, 술 후 증상의 호전이 관찰되었다.

Complete radial tears of the lateral meniscus are relatively rare. Once torn, the injury can be debilitating due to disruption of the circumferential fibers of the meniscus. We experienced a case of a lateral meniscus with a complete radial tear at the midbody, where the two torn ends were displaced more than 1 cm and could not be approximated during arthroscopy. Thirteen months after surgery, follow-up MRI and second-look arthroscopic findings showed that the complete radial tear has healed spontaneously. However, twenty nine months after the second-look arthroscopy, the patient complained of severe knee pain during exercise. On follow-up MRI, increased sclerosis and newly developed bone marrow edema were observed in the lateral femoral condyle, compared with previous MR images. Finally, we performed meniscal allograft transplantation due to the defective properties of the completely healed lateral meniscus.

원판형 연골은 주로 슬관절의 외측에 발생하는 것이 보통이며 드물게 내측에도 발생하는 것으로 알려져 있다. 그러나 내측 및 외측에 동시에 발생한 원판형 연골은 보고된 바가 없다. 저자들은 슬관절 동통을 주소로 내원한 15세 남자에서 자기 공명 영상 및 관절경을 이용하여 한쪽 슬관절에서 내측 및 외측에 동시에 발생한 원판형 연골을 진단하고 양측 모두에 대해 관절경하 부분 절제술로 치료하였기에 보고하고자 한다.

원판형 반월상 연골 중 Wrisberg ligament 형은 매우 드문 질환이다. 저자들은 후방 분리가 동반된 완전형 원판형 반월상 연골이 양측성으로 발생한 예를 경험하였기에 이를 보고하고자 한다.

슬관절에서 외측 원판형 연골판은 흔히 발견되고 있으나, 내측 원판형 연골판의 빈도 및 보고가 드물다. 본 증례에서는 수평 파열과 더불어 상순에 국한된 변연부 파열이 관찰되었으며 이는 정상의 반월상 연골뿐만 아니라 원판형 외측 반월상 연골에서도 보기 드문 형태이다. 다른 정상 연골판이나 원판형 외측 반월상 연골 파열의 형태와 비교하여 본 증례의 파열 기전을 논하였다.

목적 : 단순 방사선 사진에서 슬관절 외측 원판형 연골판을 진단하는데 도움이 되는 새로운 방사선 소견인 대퇴외과 절삭 징후(cut-off sign)를 관찰하여 이를 보고하고, 이의 진단적 의의를 특이도, 민감도, 양성 예측도, 음성 예측도와 함께 보고하고자 한다. 대상 및 방법 : 관절경 시술로 확진 된 50예의 완전 원판형 연골군과 50예의 정상 대조군을 대상으로 하였으며, Tunnel view에서 보이는 대퇴 외과와 내과의 내측부의 최장 길이를 측정하여 비율을 비교 분석하였다. 결과 : 원판형 연골군에서 대퇴 내과부 길이와 외과부 길이의 비율의 평균은 0.716 이였으며, 정상 대조군에서는 0.902 이었다. 이를 T-test로 분석했을 때, t-value는 -11.13 (p<0.0001)로 두 군간에 매 우 유의 한 차이를 나타내었으며, 또한 경계값(cut point)을 0.8로 하여 chi-square test로 분석했을 때 통계적으로 두 군간에 유의한 차이가 있었으며, 민감도 $76\%$, 특이도 $100\%$였으며, 양성 예측도 $100\%$, 음성 예측도 $80.6\%$로 나타났다. 절삭 징후는 육안적으로도 뚜렷한 차이를 보여 원판형 외측 반월상 연골을 단순 방사선 사진에서 진단할 수 있는 매우 좋은 간접적인 소견이라고 생각한다. 결론 : 대퇴 외과 절삭 징후는 단순 방사선 검사의 Tunnel view에서 원판형 연골을 진단할 수 있는 좋은 방사선 소견이며, $100\%$의 특이도와 $100\%$의 양성 예측도를 나타내는 우수한 진단소견으로 판단된다.

소아에서의 슬관절 탄발음은 주로 원판형 연골에 의해서 발생되며 슬관절의 신전제한은 원판형연골의 파열을 강력히 시사하는 소견으로 알려져 있으며, 특히 3세미만 소아에서의 보고는 극히 적은 실정이다. 또한 3세 미만의 증상이 있는 원판형 연골의 치료방법에 있어서는 정설이 없고 기술적으로도 어려운 점이 있을 것으로 예상된다. 저자들은 신전제한을 주소로 내원한 25개월 여아에서 자기 공명영상검사상 외측 원판형 연골 파열을 확인하고 관절경하 부분 절제술 시행 후 2년 9개월간 추시 한 바 합병증이나 증상의 재발없이 임상적 및 기능적으로 만족할만한 결과를 보여 이에 대한 증례를 문헌 고찰과 함께 이를 보고하는 바이다.

A ganglion cyst is a soft tissue mass that is surrounded by a dense connective-tissue capsule. The capsule is filled with a viscous fluid that is rich in hyaluronic acid and other mucopolysaccharides. But, Ganglion cysts in the knee joint are rare. There are very few case reports of ganglion cysts related to the surface of the anterior cruciate ligament, Posterior cruciate ligament and medial meniscus. We are reporting a case of a ganglion cyst in the anterior aspect of the anterior cruciate ligament accompanying with discoid lateral meniscus in the right knee of a 46-year-old woman without any history of trauma. The cyst and discoid lateral meniscus were treated successfully with arthroscopic excision and partial meniscectomy.
