폐쇄성수면무호흡 의심환자에서 무호흡-저호흡 지수와 연관이 있는 두개골 계측 변수 : 예비연구 (Cephalometric Variables Significantly Associated with Apnea Hypopnea Index in Suspected Obstructive Sleep Apnea Patients : A Preliminary Study)

  • Objectives The purpose of this study is to find the cephalometric variables which are significantly correlated with the apnea-hypopnea index (AHI) in suspected Korean obstructive sleep apnea (OSA) patients. Methods We examined lateral cephalogram and attended-full night laboratory polysomnography of the 40 participants who complained of OSA symptoms. The correlation analysis was conducted to find the cephalometric variables which are significantly correlated with the AHI. Results The correlation analysis showed that the higher AHI was associated with the longer distance between hyoid and mandibular plane (p = 0.023), the longer distance between C3 and hyoid (p = 0.014), the longer tongue length (p = 0.003), the larger inferior tongue area (p = 0.008), the larger anterior displacement of the hyoid bone (p = 0.024), the longer distance between posterior nasal spine and the tip of the soft palate (p = 0.021), and the larger cross-sectional area of soft palate (p = 0.001) of cephalogram in erect position. The higher AHI was correlated with the longer distance between hyoid and mandibular plane (p = 0.008), the longer tongue length (p = 0.037), the larger inferior tongue area (p = 0.013), the thicker uvula (p = 0.004), the longer distance between retrognathion and hyoid (p = 0.025), and larger cross-sectional area of soft palate (p = 0.001) of cephalogram in supine position. Conclusions The present preliminary results showed the candidate measurements of cephalogram which are significantly correlated with the AHI in suspected OSA.

Use of Reference Ear Plug to improve accuracy of lateral cephalograms generated from cone-beam computed tomography scans

  • Objective: The purpose of this study was to evaluate the effectiveness of the use of Reference Ear Plug (REP) during cone-beam computed tomography (CBCT) scan for the generation of lateral cephalograms from CBCT scan data. Methods: Two CBCT scans were obtained from 33 adults. One CBCT scan was acquired using conventional methods, and the other scan was acquired with the use of REP. Virtual lateral cephalograms created from each CBCT image were traced and compared with tracings of the real cephalograms obtained from the same subject. Results: CBCT scan with REP resulted in a smaller discrepancy between real and virtual cephalograms. In comparing the real and virtual cephalograms, no measurements significantly differed from real cephalogram values in case of CBCT scan with REP, whereas many measurements significantly differed in the case of CBCT scan without REP. Conclusion: Measurements from CBCT-generated cephalograms are more similar to those from real cephalograms when REP are used during CBCT scan. Thus, the use of REP is suggested during CBCT scan to generate accurate virtual cephalograms from CBCT scan data.

Immediate effects of mandibular posterior displacement on the pharyngeal airway space: A preliminary study

  • Objective: This study aimed to evaluate the immediate effects of mandibular posterior displacement on the pharyngeal airway space (PAS) by using cephalometric evaluations and to investigate how the surrounding structures are schematically involved. Methods: In this retrospective study, 38 subjects with functional Class III malocclusion and two lateral cephalograms were selected. The first lateral cephalogram was taken with the mandible in the habitual occlusal position, and the second in anterior edge-to-edge bite. Paired t-test was used to analyze changes in the PAS, hyoid bone, tongue, and soft palate, followed by mandibular posterior displacement. Pearson's correlation analysis was used to determine the relationship between the amount of mandibular posterior displacement and other variables. Results: A statistically significant decrease was observed in the PAS following mandibular posterior displacement. Along with mandibular posterior displacement, the tongue decreased in length (p < 0.001) and increased in height (p < 0.05), while the soft palate increased in length, decreased in thickness, and was posteriorly displaced (p < 0.001). The hyoid bone was also posteriorly displaced (p < 0.05). There was no correlation between the amount of mandibular posterior displacement and the measured variables. Conclusions: The PAS showed a statistically significant decrease following mandibular posterior displacement, which was a consequence of retraction of the surrounding structures. However, there were individual variances between the amount of mandibular posterior displacement and the measured variables.

A Study on the Change of Occlusal Contacts and Lateral Cephalometric Variables after Stabilization Splint Therapy in Temporomandibular Disorders Patients

  • Purpose: The aim of this study is to assess the relationship between possible occlusal change after stabilization splint therapy and the research diagnostic criteria for temporomandibular disorders (RDC/TMD) Axis I diagnoses and lateral cephalometric variables. Methods: Clinical and radiographic records of 47 TMD patients wearing stablization splint were reviewed. The number of occluding teeth was recorded and lateral cephalogram was taken at pre-treatment and 6-month post-treatment. They were divided into two groups. The control group consists of patients with the unchanged number of occluding teeth throughout 6-month splint therapy (19 females and 4 males), and occlusal-loss group with the number of occluding teeth decreased (19 females and 5 males). The difference of RDC/TMD diagnoses and cephalometric variables were compared between two groups. Results: In the control group, RDC group I, muscle disorders, was 39.1% (9/23), group II, disc displacements, was 17.4% (4/23), group III OA, osteoarthritis/osteoarthrosis, was 73.9% (17/23), and group III pain, arthralgia, was 82.6% (19/23). In the occlusal-loss group, group I was 41.7% (10/24), group II was 41.7% (10/24), group III OA was 70.8% (17/24), and group III pain was 83.3% (20/24). The frequency of RDC groups was not different between two groups, analyzed by binomial logistic regression. Pre-treatment cephalometric variables were not different between two groups. However, articular angle, AB to mandibular plane and ODI decreased and gonial angle increased significantly in the occlusal-loss group, implying clockwise rotation of the mandible, between pre-treatment and 6-month post-treatment, while none of cephalometric variables showed any statistical difference in the control group. Conclusions: Change in the number of occluding teeth was not related to the RDC/TMD diagnoses. Cephalometric values changed only in the occlusal-loss group as a result of mandibular clockwise rotation. None of cephalometric variables before the stabilization splint therapy was statistically different between the control and occlusal loss group.

  • Purpose: This study was intended to perform a cephalometric comparison between the patients with and without obstructive sleep apnea (OSA). The factors influencing the OSA in the lateral cephalogram were also investigated. Methods: Fifty patients who had visited the Sleep Disorder Clinic at the Ajou University Hospital and evaluated with the polysomnograph (PSG) and cephalogram, were included in the study. The patients had the apnea-hypopnea episode over 5 times per hour (apnea-hypopnea index $[AHI]{\geq}5$) were diagnosed as OSA after the overnight PSG. To evaluate the hard and soft tissue profiles, the cephalometric radiograms were taken at the maximal intercuspation. The correlation between the patient's age, height, weight, body mass index (BMI) and AHI was inspected in the OSA and control group. The difference between the OSA and control group was evaluated (Mann-Whitney U Test). The cephalometric influencing factors to OSA were analyzed (Pearson's correlation coefficient) statistically using SPSS statistics. Results: The OSA Group had a significantly higher BMI than the control group. The mean lower facial height (ANS-Me) was longer in the OSA group; however, statistically significant difference was not detected in the anteroposterior craniofacial measurements. The distance between mandibular plane and hyoid bone of the OSA group was significantly longer than that of the control group. The hyoid position (MP-Hyoid) had a positive correlation between AHI (P<0.001). However, the measurements of oropharyngeal airway were not different between the two groups. The hypothesis, that the antero-posteriorly narrow oropharyngeal airway may aggravate the airway resistance and give rise to a higher AHI, was rejected in the study. Conclusion: We suggest that the lateral cephalogram may be utilized as a useful method to evaluate OSA. The patients with a lower hyoid position can be expected to have higher risks of OSA. However, a comprehensive intraoral inspection, including the soft palate and tonsilar hypertrophy, is emphasized, as the lateral cepahlogram cannot visualize the oropharyngeal status completely.

A retrospective study of incidental findings occurring in a consecutive case series of lateral cephalograms of 12- to 20-year-old patients referred for routine orthodontic treatment

  • Purpose: Lateral cephalograms of orthodontic patients may contain incidental findings that could potentially lead to harm. Materials and Methods: The lateral cephalograms of 1765 consecutive 12- to 20-year-old patients, being considered for routine orthodontic treatment, were retrospectively reviewed. These patients were considered normal, because no abnormalities were found either in their medical history or on their clinical examination. Results: The overall prevalence of incidental findings was 18.8%, of which 10.3% were ponticulus posticus and 4.2% were bridging of the sella turcica. Although occipital spurs and ponticulus posticus were more prevalent in males, the size of the sella turcicas did not differ between sexes. Of the 1156 patients completing treatment about 2 years later, only one lateral cephalogram displayed progression of the ponticulus posticus in that time. Conclusion: The prevalence of incidental findings on lateral cephalograms of otherwise normal orthodontic 12- to 20-year-old patients was almost a fifth, of which ponticulus posticus, vertebral fusion, and enlarged parietal foramina were clinically significant.

3-D CT를 이용한 악교정술 전후의 하악과 설골의 위치에 관한 연구 (THE POSITIONAL RELATIONSHIP BETWEEN THE MANDIBLE AND THE HYOID BONE IN MANDIBULAR PROTRUSION AFTER ORTHOGNATHIC SURGERY EVALUATED WITH 3-D CT)

  • Purpose : This study was intended to evaluate the positional relationship between the hyoid bone and the mandible in patients with mandibular protrusion after mandibular set-back surgery by means of 3D-CT. Materials and methods : Preoperative(3 weeks before) and postoperative (6 weeks after) 3D-CT & cephalogram were taken on 32 patients(12 male, 20 female, mean age of 23.2) treated by bilateral sagittal split osteotomy with rigid fixation. The angular measurement on 3D-CT basilar view were deviation of Me & H, long axis angle of left & right cornu majus. The lineal measurement on 3D-CT basilar view were composed of intercondylar line and coordinates(x,y) of Me & H. The angular & lineal measurement of lateral cephalogram were composed of mandibular plane angle, SNA, SNB, ANB, FH-NA & FH-NB, and coordinates(x,y) of B, Pog, Me & H, PAS, Lpw, MPH and IAS. On the frontal cephalogram, deviation of Me were evaluated. Results : The mean mandibular set-back was 8.0mm horizontally and mandibular plane angle was slightly increased. The hyoid bone was displaced postero-inferiorly, the distance between MP(mandibular plane) and H(hyoid bone) was increased and the posterior airway space values (PAS, Lpw, IAS) were decreased. The coordinates Me(x,y), H(x,y) and deviation angle Me'& H' were revealed the strong positive correlation. Conclusion : The results revealed that the horizontal, vertical and transverse relationship of the mandibular and the hyoid bone movements were significantly correlated in patients performed mandibular set-back surgery.

소아 수면호흡장애의 예측 인자 평가 (Assessment of Predicting Factors for Pediatric Sleep Disordered Breathing)

  • 이 연구의 목적은 수면호흡장애를 가진 어린이들을 선별하기 위해 휴대용 간이수면검사를 이용한 무호흡 저호흡 지수(apneahypopnea index, AHI)를 기준으로, 치과 진료실에서 사용 가능한 측모두부계측 방사선 사진, 임상 검사, 소아수면설문지(pediatric sleep questionnaire, PSQ)에서 유용한 수치들을 찾아내고 평가하는 것이었다. 교정치료를 위해 전북대학교 소아치과에 내원한 5 - 10세의 어린이들을 대상으로 하였다. 총 61명이 이 연구에 포함되었으며, AHI 값이 1보다 크게 나타난 어린이들을 SDB군으로, 1 이하로 나타난 어린이들은 정상군으로 분류하였다. 61명의 어린이 중 49(80%)명이 SDB군에 포함되었다. SDB군과 정상군과의 통계 분석 결과, 휴대용 간이수면검사의 ODI(oxygen dehydration index)와 코골이 시간, 설문지를 통한 PSQ scale, 측모두부계측 방사선 사진에서 아데노이드 비대율과 하악평면에서 설골까지의 거리 그리고 gonial angel이 통계적으로 유의한 차이를 보였다. 이 연구를 통해 잠재적인 SDB 환자를 선별해내기 위해 측모두부계측 방사선 사진을 통한 아데노이드 비대, 설골의 위치, gonial angle과 소아수면설문지를 평가하는 것이 유용할 것으로 보인다.

전두동의 크기와 하악골 성장예측에 관한 연구 (A STUDY ON THE MANDIBULAR GROWTH PREDICTION AND SIZE OF THE FRONTAL SINUS)

  • 성장기 환자의 치료 계획 수립시 치아 이동만으로 치료를 할 것인지, 악 정형 치료를 동반할 것인지, 아니면 성장후 수술을 할 것인지를 결정하는 일은 쉬운 일이 아니며 특히, 악골의 성장을 예측하는 과정을 치료계획 수립에 매우 중요한 부분이다. 전두동은 조기에 성장이 완료되고 하악골은 20세까지 성장을 지속한다는 점에 착안하여, 측모 두부방사선 사진상에 나타나는 전두동의 크기와 하악골 크기간에 상관성을 알아보기 위해 228명을 골격선 제I급, 제II급, 제III급 부정교합의 3군으로 분류하고 하악골의 장경과 악골의 전후방 관계를 나타내는 3가지 지수(ANB, APDI, Wits)를 측정하여, 서로간의 상관성을 검토한 결과 다음의 결론을 얻었다. 1. 전두동의 크기와 ANB(-0.3633), APDI(0.296), Wits(-0.2380), 하악골 장경(0.2704)은 높은 상관성을 (p<0.001) 보였다. 2. 골격성 제III급 부정교합군에서, 측모두부 방사선 사진상에 나타나는 전두동의 크기가 골격성 제 I 급 부정교합군이나 II 급 부정교합군보다 크게 나타났다.

악관절장애 환자의 하악골 비대칭에 대한 방사선학적 연구 (A RADIOGRAPHIC STUDY OF THE MANDIBULAR ASYMMETRY IN TEMPOROMANDIBULAR DISORDER PATIENTS)

  • The purpose of this study was to observe the relationship between mandibular asymmetry and Temporomandibular Disorders by means of the cephalometry using the posteroanterior cephalogram and the submentovertex cephalogram which were taken in 35 Temporomandibular Disorder patients and 35 normal persons ranged from 20S to 30S. The results were as follows: 1. The angulation which was formed by the median line with the ANS-Menton line (MAP) was greater in patients group and there was statistically significant difference. 2. The angulation which was formed to the median line with the Menton-Odontoid process tip line (MES), the difference of the distances from the center of the posterior surface of the both condyles to the most anterior point of the chin (DD), the difference of the distances from the center of the both condyles to the horizontal reference line (DE), the difference of the angulations which were formed by the both condyles axes with the horizontal reference line (DCE), the difference of the lengths of the both condyles (CL) and the difference of the widths of the both condyles (DW) were greater in patients group and there were statistically significant differences. There was reversed correlation between MAP and the difference of the distances from the 3. bilateral points of the lateral margin of the both zygomaticofrontal sutures to the at the lateral inferior margin of the both antegonial protuberances in mandible (DH). There was reversed correlation between MES and DD, DE, DCE. 5. There was correlation between MAP and MES.
