Study on the Correlation between Extracellular Matrix Protein-1 and the Growth, Metastasis and Angiogenesis of Laryngeal Carcinoma

  • Objective: To investigate the correlation between extracellular matrix protein-1 (ECM1) and the growth, metastasis and angiogenesis of laryngeal carcinoma. Materials and Methods: Forty-five samples with laryngeal benign and malignant tumors confirmed by pathology in Laiwu City People's Hospital from March 2006 to March 2011 were collected, in which there were 29 cases with laryngeal carcinoma and 16 with benign tumors. The expression of ECM1 and factor VIII-related antigens in patients with laryngeal carcinoma and those with benign tumors was respectively detected using immunohistochemical method, and the correlation between ECM1 staining grade and microvessel density (MVD) was analyzed. Results: In laryngeal carcinoma tissue, ECM1 was mainly expressed in cytoplasm, less in cytomembrane or intercellular substance. With abundant expression in the tissue of laryngeal benign tumors (benign mesenchymoma and hemangioma), ECM1 was primarily expressed in the connective tissue, which was different from the expression in laryngeal carcinoma tissue. The proportion of positive ECM1 staining (++) in patients with laryngeal carcinoma was dramatically higher than those with benign tumors (p<0.05), and that of strongly-positive ECM1 staining (+++) slightly higher. The results of Spearman nonparametric correlation analysis revealed that ECM1 staining grade in laryngeal carcinoma tissue had a significantly-positive correlation with MVD (r=0.866, p=0.000). Conclusions: ECM1 expression in laryngeal carcinoma is closely associated with tumor cell growth, metastasis and angiogenesis, which can be considered as an effective predictor in the occurrence and postoperative recurrence of laryngeal carcinoma.

A Case of Metachronous Multiple Primary Tumor Involving the Thyroid and the Larynx (갑상선과 후두에 발생한 다발성 원발암 1례)

  • Multiple primary tumors in the head and neck are not uncommon, however those in the thyroid and the larynx are known to be very rare. In most cases of multiple primary tumors involving the thyroid and the larynx, lesions are observed usually simultaneously and thyroid tumors are found incidentally during the laryngeal tumor surgery. In rare cases, thyroid tumors are found metachronously after radiation therapy of laryngeal cancer. The authors recently experienced a case of multiple primary tumor involving the thyroid and the larynx, in which thyroid papillary carcinoma was the index tumor and the laryngeal squamous carcinoma was the meatachronous second tumor. Both tumors showed aggressive local extension and regional nodal meatastasis with tumor collision in the same node. The patient died of recurrent or of residual squamous carcinoma shortly after main surgical treatment index thyroid cancer.

Detection of Human Papillomavirus in Laryngeal Squamous Cell Carcinomas (후두편평세포암종에서 인유두종 바이러스의 검출)

  • Human papillomavirus(HPV) is epitheliotrophic virus invading the anogenital tract and the upper aerodigestive tract HRV produces a diversity of benign and maljgnant tumors. In this study, the author determined the frequency of association of human papillomavirus(HPV) and laryngeal carcinomas and investigated the significance of HRV infection of different subtypes in the tumorigenesis of laryngeal carcinoma. Laryngeal squamous cell cancinomas from 34 patients who did not have preexisting papillomas by clinical history were retrieved from formalin-fixed, paraffin-embedded blocks and analyzed for HPV. Nineteen cases were tumors of the true vocal folds, 11 were supraglottic and 4 were transglottic. HPV detection was dane using polymerase chain reaction amplification with HPV L$_1$consensus primer. HPV type was determined by the same method using HPV-6, 11 and 16,-18 type-specific E6 primers. The results were as follows : 1) HPV DNA was detected in 7 cases among the 34 patients(20.6%). According to the type of HPV DNA HPV-11 was detected in 3 cases, HPV-16 was detected in 2 cases and HPV-6 and HPV-18 were detected in 1 case, respectively. 2) These 7 HPV-positive patients were advanced cancinoma cases. From these results, we concluded that HPV was thought to be the etiological factor of laryngeal squamous cell carcinomas.

A Case of Angioleiomyoma of Larynx (후두에 발생한 혈관평활근종 1예)

  • Angioleiomyomas are a vascular subtype of leiomyomas or benign smooth muscle tumors. The majority of these tumors occur in the extremities. Angioleiomyoma of the larynx has been reported but is exceedingly rare. Laryngeal angiomyomas can present with hoarseness, dyspnea, or globus sensation, and often misdiagnosed as asthma. We report a case of a 74-year old man with laryngeal angioleiomyoma misdiagnosed as asthma.

Three Cases of Neuroendocrine Carcinoma of the Larynx (후두에 발생한 신경 내분비암종 3예)

  • Although primary neuroendocrine carcinomas(NEC) are uncommon tumors of the larynx, they represent the most common nonepidermoid malignancy of this organ. Whereas typical carcinoid tumors and small-cell carcinomas occur very rarely in the larynx, the majority of laryngeal NEC belong to a broad intermediate group between these two extremes of NEC. Through many case studies, a major protion of clinicopathologic characteristics of laryngeal NEC have been revealed; however, the gistogenesis of laryngeal NEC is still unknown. The only well-documented normal neuroendocrine structures of the larynx are paraganglions, but the incidence of laryngeal paragangliomas are much lower than that of NEC. Here, we report on three cases of NEC affecting the larynx and the results of searching for neuroendocriene cells in non-neoplastic larynx tissue.

Clinical Characteristics of Female Laryngeal Cancer (여성후두암의 임상적 특성)

  • The laryngeal cancer is a cancer of secondary sex organ, such as malignant tumors of the mammary gland, endometrium, and prostate. The clinical characteristics of the female laryngeal cancers are considered somewhat to be different from that of male. As cancer of the larynx is principally a disease of men, many investigations have showed the characteristics of the male laryngeal cancers. For understanding the clinical characteristics of the female laryngeal cancers, we analyzed 21 cases of laryngeal cancer in women, diagnosed and treated in our institute during the last 10 years. The results were, 1) In female subjects, supraglottis was most common subsite of laryngeal cancer(85.7%). 2) On histopathologic grade, the moderately differentiated squamous cell carcinoma was the most common (80.9%). 3) The positive neck nodes were 19 %, considered to be lower than that of total laryngeal cancer. 4) The treatment results, the 2 year disease free rate and 5 year survival rate were 88.9%, 83.3%, respectively. These results suggest, therefore, female laryngeal cancers are more likely to be supraglottic cancer than glottic cancer. In spite of high incidence of supraglottic cancer, the nodal metastases are rare, the prognosis appeared to be good.

Component Analysis of Laryngeal Cancer Incidence Dynamics in Kazakhstan from 1999 to 2014

  • Background: In this study, we examined epidemiological aspects of dynamic changes in incidences of laryngeal cancer in male and female populations in Kazakhstan. Materials and Methods: Primary data were for registered patients with malignant laryngeal tumors in the whole country during the period of 1999-2014. Evaluation of changes in laryngeal cancer incidence in the population of Kazakhstan was performed using component analysis. Results: It was determined that the number of patients with laryngeal cancer in the whole country is decreasing although with conflicting impacts of different factors. Despite population growth (all - ${\Delta}_P=+66.1%$, men - ${\Delta}_P=+70.9%$ and women - ${\Delta}_P=+46.4%$), and aging (all - ${\Delta}_A=+45.1%$, men - ${\Delta}_A=+54.3$ and women - ${\Delta}_A=+22.2$), the reduction in risk of developing the disease (all - ${\Delta}_R=-165.6%$, men - ${\Delta}_R=-170.9%$ and women - ${\Delta}_R=-141.0%$) was overwhelming. Conclusions: This investigation was the first epidemiological study of dynamics of laryngeal cancer by component analysis in population of Kazakhstan. Implementation of the results of the study is recommended in management of anti-cancer activities for laryngeal cancer.

Relationships Between Clinical Behavior of Laryngeal Squamous Cell Carcinomas and Expression of VEGF, MMP-9 and E-Cadherin

  • The biological mechanisms of cancer and associations with behavior of tumours need to be studied to understand progression and determine appropriate treatments. Here we investigated expression of VEGF, MMP-9 and E-cadherin in laryngeal SCCs and their relations with clinical behavior. This prospective study was based on 38 surgical specimens from patients with primary laryngeal SCC and data recorded in their cards. Expression of the three factors in tumor tissue was examined using immunohistochemistry and correlations with clinical parameters of primary tumors, regional lymph node metastases, stage of disease, histopathologic differentiation, and vascular/cartilage invasion were investigated. Regarding the cases with positive MMP-9 expression, the difference between well and moderately/poorly differentiated tumors was statistically significant. However, differences between early stage (stage I and II) and late-stage (stage III and IV) tumours, and between positive and negative for pLN metastasis were not. No significant relationship between positive VEGF and tumor differentiation or stage was apparent, but E-cadherin levels significantly differed between well and moderately/poorly differentiated tumours and with the presence of pLN metastasis. E-cadherin staining did not vary between MMP-9 positive and negative cases. In conclusion, MMP-9 may be a negative predictor of differentiation in laryngeal SCC, while E-cadherin is a predictor of differentiation and nodal metastases. Even if the difference between VEGF expression and tumor stage was not statistically significant, it seems that there exists some relationship, which might be clarified with a greater number of cases.

Syndrome of Inappropriate ADH Secretion Associated with Laryngeal Carcinoma, Cured by Total Laryngectomy (후두암에 동반된 SIADH)

  • The syndrome of inappropriate ADH secretion(SIADH) is a clinical disorder in which there is continual release of antidiuretic hormone(ADH) unrelated to plasma osmolality. It may occur usually in association with malignant tumors below the clavicle and other pulmonary disorders which are capable of synthesizing, storing and releasing ADH into the circulation but rarely with the head and neck tumors yet. We experienced a patient with unexplained hyponatremia associated with laryngeal carcinoma. The clinical and laboratory diagnostic studies suggested that patient's sodium deficiency was secondary to the SIADH. Subsequent resection of the neoplasm, total laryngectomy led to resolution of hyponatremia, suggesting that a tumor associated humoral factor, such as vasopressin or vasopressin like substance, was responsible for the electrolyte disturbance.

A Case of Dysplastic Epiglottis in Elderly Patient (노령 환자에서 발견된 후두개 이형성증 1예)

  • Dysplastic epiglottis is extremely rare congenital malformation, which usually occurs in association with other laryngeal anomalies. Hypoplasia is the most common type in epiglottic malfomations. Other abnormalities include rudimentary, aplasia and bifid etc. Mostly, they are found in infancy and early childhood, and diagnosis at adulthood is extremely rare. A 69-year-old man with chronic cough and globus sense visited our clinic. Laryngoscopic findings revealed a unique form of epiglottis. He had no history of laryngeal trauma, tumors, head and neck surgery, and radiation. There was no another anomaly in the laryngo-pharynx. Because of a mass-like lesion at the apex of epiglottis, we performed the laryngeal microsurgery. The pathology revealed as granulation tissue. We report a rare and unique case of dysplastic epiglottis in elderly patient with a brief literature review.
