Purpose: The objective of this study is to evaluate the functional outcome and identify prognosis of retear patients in patients aged 65 years or over undergoing surgical repair for a large to massive full-thickness rotator cuff tear. Materials and Methods: From 1995 September to 2010 March, 147 patients aged 65years or over (40 male, 107 female, with an average age of 69.6) undergoing surgical repair for large to massive full thickness rotator cuff tear (large 67 cases, massive 80 cases). For functional evaluation, preoperative and postoperative 1 year range of motion and muscle power checked. For subjective evaluation, American shoulder and elbow surgeons score and Constant score were checked. For anatomical evaluation, 87 patients were checked shoulder MRI at the time of the postoperative 1 year. Results: ASES score improved from to 50.4 to 88.9, Constant score improved from 47.1 to 75.2. Supraspinatus power improved from 51.1% to 80.8%, external rotator muscle power improved from 64.5% to 83.1%. Forward elevation improved from 117.4 degrees to 153 degrees, external rotation improved from 23.6 degrees to 41.8 degrees. Follow up MRI showed re-tear in 23%, all re-tear patients were from massive tear except one patient. All re-tear patients showed improved clinical outcomes, but supraspinatus and external rotator muscle power were not improved. Conclusions: Patients aged 65 years or over undergoing surgical repair for a large to massive full-thickness rotator cuff tear showed successful outcomes over 90 percent. Re-tear patients also showed successful clinical outcomes. In elderly patients with large to massive full thickness rotator cuff tear, aggressive surgical repair leads good clinical outcomes.

Purpose: To evaluate the clinical usability of reconstructive methods, and how to select flap after wide excision of malignant soft tissue tumor in ankle and foot. Materials and Methods: The 15 cases shown in the 14 patients (In case of a male patient, reconstruction was performed two times due to local recurrence.) with malignant soft tissue tumor in ankle and foot, who underwent reconstruction after wide excision from March 2000 until March 2007. Oncologic, surgical and functional results were evaluated. Results: The method of reconstruction used were anterolateral thigh perforator flap (5cases), Reversed superficial sural artery flap (4 cases), dorsalis pedis flap (3 cases), local flap (3cases). The defect, mean size was $5.5{\times}5.7\;cm$, was reconstructed with rotation flap or free flap, mean size was $5.9{\times}6.0\;cm$, skin graft for remnant. The mean operation time was 310 minutes (120~540 minutes); it took 256 minutes to reconstruct by rotation flap, and 420 minutes by free flap. As oncologic results, 7 patients were no evidence of disease, 6 patients were alive with disease and 1 patient was expired by pulmonary metastasis at the time of the last follow-up. 4 patients had local recurrence and 4 pateints had distant metastases. As functional results, 14 patients were evaluated with average score of 68.8% using the system of the Musculoskeletal Tumor Society. Conclusion: The dorsalis pedis and reverse superficial sural artery rotation flap which is easy procedure, has less complication and takes short operation time, can be primarily considered to reconstruct a small defect. And the anterolateral thigh perforator flap is suitable for coverage of a large defects after wide excision of malignant soft tissue tumor in ankle and foot.
According to improved radiation therapy technology such as IMRT and proton therapy, the accuracy of patient alignment system is more emphasized and IGRT is dominated research field in radiation oncology. We proposed to study the feasibility of cone-beam CT system using simple x-ray imaging systems for image guided proton therapy at National Cancer Center. 180 projection views ($2,304{\times}3,200$, 14 bit with 127 ${\mu}m$ pixel pitch) for the geometrical calibration phantom and humanoid phantoms (skull, abdomen) were acquired with $2^{\circ}$ step angle using x-ray imaging system of proton therapy gantry room ($360^{\circ}$ for 1 rotation). The geometrical calibration was performed for misalignments between the x-ray source and the flat-panel detector, such as distances and slanted angle using available algorithm. With the geometrically calibrated projection view, Feldkamp cone-beam algorithm using Ram-Lak filter was implemented for CBCT reconstruction images for skull and abdomen phantom. The distance from x-ray source to the gantry isocenter, the distance from the flat panel to the isocenter were calculated as 1,517.5 mm, 591.12 mm and the rotated angle of flat panel detector around x-ray beam axis was considered as $0.25^{\circ}$. It was observed that the blurring artifacts, originated from the rotation of the detector, in the reconstructed toomographs were significantly reduced after the geometrical calibration. The demonstrated CBCT images for the skull and abdomen phantoms are very promising. We performed the geometrical calibration of the large gantry rotation system with simple x-ray imaging devices for CBCT reconstruction. The CBCT system for proton therapy will be used as a main patient alignment system for image guided proton therapy.
Wind turbine noise is generally lower than that from other environmental noise sources such as road and railway noise. Nevertheless, some residents living more than 1km away from wind turbines have claimed that they suffer sleep disturbance due to wind turbine noise. Several researchers have maintained that residents near a wind farm may perceive large amplitude modulation of wind turbine noise at night, and this amplitude modulation is the main cause of the noise annoyance. However, to date only few studies exist on the prediction of the amplitude modulation of wind turbine noise. Thus, this study predicts amplitude modulated noise generated from a generic 2.5MW wind turbine. Semi-empirical noise models are employed to predict the modulation depth and the overall sound pressure level of the wind turbine noise. The result shows that the amplitude modulation is observed regardless of atmospheric stability, but the modulation depth in a stable atmosphere is 1~3dB higher than that in an unstable atmosphere near the plane of rotation where the blades move downward. Moreover, using the result of the noise prediction, this study estimates the maximum perceptible distance of the wind turbine noise cause by amplitude modulation. The result indicates that the wind turbine noise can be perceived at a distance of up to 1600m in the range of about 30~60 degree from the on axis in a extremely low background noise environment.

We proposed several systems for 1Giga bit Modem. The first, Binary ASK(Amplitude Shift Keying) system has a high speed shutter transmitter and no IF(Intermediate Frequency) receiver only by symbol synchronization. The advantage of proposed system is that circuitry is very simple without IF process. The disadvantage of proposed system are that line spectrum occurs interference to other channels, and enhancement to 4-level system is impossible due to its large SNR degradation. The second, Binary phase modulation system has a high speed shutter transmitter and IF-VCO(IF-Voltage Controlled Oscillator) control by base-band phase rotation. Polarity of shutter window is changed by the binary data. The window should be narrow same as above ASK. The advantage of proposed system is which error rate performance is superior. The disadvantage of proposed system are that Circuitry is more complex, narrow pull-in range of receiver caused by VCO and spectrum divergence by the non-linear amplifier. The third, 4-QAM(Quadrature Amplitude Modulation)system has a nyquist pulse transmitter and IF-VCO control by symbol clock. The advantage of proposed system are that signal frequency band is a half of 1GHz, reliable pull-in of VCO and possibility of double speed transmission(2Gbps) by keeping 1GHz frequency-band. The disadvantage of proposed system are that circuit complexity of pulse shaping and spectrum divergence by the non-linear amplifier.
The slim-floor solution provides an efficient alternative to the classic slab-over-beam configuration due to architectural and structural benefits. Two deficiencies can be identified in the current state-of-art: (i) the technique is limited to nonseismic applications and (ii) the lack of information on moment-resisting slim-floor beam-to-column joints. In the seismic design of framed structures, continuous beam-to-column joints are required for plastic hinges to form at the ends of the beams. The present paper proposes a slim-floor technical solution capable of expanding the current application of slim-floor joints to seismic-resistant composite construction. The proposed solution relies on a moment-resisting connection with a thick end-plate and large-diameter bolts, which are used to fulfill the required strength and stiffness characteristics of continuous connections, while maintaining a reduced height of the configuration. Considering the proposed novel solution and the variety of parameters that could affect the behavior of the joint, experimental and numerical validations are compulsory. Consequently, the current paper presents the experimental and numerical investigation of two slim-floor beam-to-column joint assemblies. The results are discussed in terms of moment-rotation curves, available rotational capacity and failure modes. The study focuses on developing reliable slim-floor beam joints that are applicable to steel building frame structures located in seismic regions.
The treatment of tumors along curved surfaces with stationary electron beams using cone collimation may lead to non-uniform dose distributions due to a varying air gap between the cone surface and patient. For large tumors, more than one port may have to be used in irradiation of the chest wall, often leading to regions of high or low dose at the junction of the adjacent ports. Electron-beam arc therapy may elimination many of these fixed port problems. When treating breast tumors with electrons, the energy of the internal mammary port is usually higher than that of the chest wall port. Bolus is used to increase the skin dose or limit the range of the electrons. We invertiaged the effect of various arc beam parameters in the isodose distributions, and combined into a single arc port for adjacent fixed ports of different electron beam eneries. The higher fixed port energy would be used as the arc beam energy while the beam penetration in the lower energy region would be controlled by a proper thickness of bolus. We obtained the results of following: 1. It is more uniform dose distribution of electron to use rotation than stationary irradiation. 2. Increasing isocenter depth on arc irradiation, increased depth of maximum dose, reduction in surface dose and an increasing penetration of the linear portion of the curve. 3. The deeper penetration of the depth dose curve and higher X-ray background for the smaller field sized. 4. If the isocenter depth increase, the field effect is small. 5. The decreasing arc beam penetration with decreasing isocenter depth and the isocenter depth effect appears at a greater depth as the energy increases. 6. The addition of bolus produces a shift in the penetration that is the same for all depths leaving the shape of the curves unchanged. 7. Lead strips 5 mm thick were placed at both ends of the arc to produce a rapid dose drop-off.
Most animals, including human beings, live in a cyclic pattern of lift that is influenced by the ambient changes of environment. The regular changes occurred by rotation of the Earth itself its revolving around the Sun, and the local environment, are reflected by the distinct behavior in the living organisms. These regular changes of environment have been imprinted into the genes within the living organisms through the evolutionary process over a long period of time. The genes are expressed by rhythms during the process of fetal development followed by growth. The environmental modifications ultimately are settled in genes, serving as a biological clock that is located putatively in the hypothalamus. Thus the biological clock governs a large number of rhythms and affects the time of birth and death lift expectancy, behavior, physiology, cell division, biochemical reaction, etc. The rhythms are readjusted to the changes of environmental cues. The biological clock has the great advantage of predicting and preparing the regular changes of environment.
In breast cancer, the treatment volume presents a relatively complex three dimensional structure. Effective radiation therapy requires the delivery of adequate dose to a large target volume using complex beam arrangements. The technique proposed here is our department's method using asymmetric jaw with appropriate couch, collimator and gantry rotation. This technique has the following advantages: 1) all treatments are given in a single clinical set up 2) it does not require half beam blocks 3) it produces exact geomatric match 4) it is very convenient and easy to use 5) it has daily reproducibility.

Turbo-pumps have weak points, such as the pumping operation is unstable on the positive slope of the head curve and/or the cavitation occurs at the low suction head. To improve simultaneously both weak points, the first author invented the unique pumping unit composed of the tandem impellers and the peculiar motor with the double rotational armatures. The front and the rear impellers are driven by the inner and the outer armatures of the motor, respectively. Both impeller speeds are automatically and smartly adjusted in response to the pumping discharge, while the rotational torques between both impellers/armatures are counter-balanced. Such speeds contribute to suppress successfully not only the unstable operation at the low discharge but also the cavitation at the high discharge, as verified with the axial flow type pumping unit in the previous paper. Continuously, this paper investigates experimentally the effects of the tandem impeller profiles on the pump performances and the rotational speeds against the discharge, using the impellers whose loads are low and/or high at the normal discharge. The worthy remarks are that (a) the unstable operation is suppressed as expected and the shut off power is scarcely large in the smart control, (b) the blade profile contributes to determine the discharge giving the maximum/minimum rotational speed where the reverse flow may incipiently appears at the front impeller inlet, (c) the tandem impeller profiles scarcely affect the rotational speeds, while the loads of the front and the rear impellers are same, but (d) the impeller with the low load must run faster and the impeller with the high load must run slower at the same discharge to take the same rotational torque, and (e) the reverse flow at the inlet and the swirling velocity component at the outlet of the front impeller with the high load require making the rotational speed of the rear impeller with low load fairly faster at the lower discharge.
