Lessons Learned from a Comparative Analysis of Surgical Outcomes of and Learning Curves for Laparoscopy-Assisted Distal Gastrectomy

  • Purpose: Before expanding our indications for laparoscopic gastrectomy to advanced gastric cancer and adopting reduced port laparoscopic gastrectomy, we analyzed and audited the outcomes of laparoscopy-assisted distal gastrectomy (LADG) for adenocarcinoma; this was done during the adoptive period at our institution through the comparative analysis of short-term surgical outcomes and learning curves (LCs) of two surgeons with different careers. Materials and Methods: A detailed comparative analysis of the LCs and surgical outcomes was done for the respective first 95 and 111 LADGs performed by two surgeons between July, 2006 and June, 2011. The LCs were fitted by using the non-linear ordinary least squares estimation method. Results: The postoperative morbidity and mortality rates were 14.6% and 0.0%, respectively, and there was no significant difference in the morbidity rates (12.6% vs. 16.2%, P=0.467). More than 25 lymph nodes were retrieved by each surgeon during LADG procedures. The LCs of both surgeons were distinct. In this study, a stable plateau of the LC was not achieved by both surgeons even after performing 90 LADGs. Conclusions: Regardless of the experience with gastrectomy or laparoscopic surgery for other organs, or the age of surgeon, the outcome was quite acceptable; the learning process differ according to the surgeon's experience and individual characteristics.

복강경 보조 유문부보존 위절제술의 초기 경험: 복강경 보조 원위부 위절제술 후 Billroth-I 재건술과의 비교 (The Early Experience with a Laparoscopy-assisted Pylorus-preserving Gastrectomy: A Comparison with a Laparoscopy-assisted Distal Gastrectomy with Billroth-I Reconstruction)

  • 목적: 유문부보존 위절제술(pylorus-preserving gastrectomy, PPG)은 유문륜을 보존하여 잔위의 배출기능을 보존하고 담즙 역류를 방지할 수 있는 기능 보존 수술법으로 조기위암 치료에 적용되고 있다. 저자들은 복강경 보조 유문부보존 위절제술(laparoscopy-assisted pylorus-preserving gastrectomy, LAPPG)의 초기 경험을 LADG 후 Billroth-I 재건술과 비교 분석하였다. 대상 및 방법: 2006년 11월부터 2007년 9월까지 원자력병원 외과에서 복강경 보조 위절제술을 시행 받은 39명의 조기위암 환자 중 LAPPG (n=9)와 LADG 후 Billroth-I 재건술(n=18)을 시행 받은 27명의 환자를 대상으로 하였고, 양 군간의 임상병리학적 변수를 비교하였다. 저자들은 LAPPG 시행 중 유문하동맥, 우위동맥, 미주신경 간지, 유문지 및 복강지를 보존하였으며, 림프절 절제술은 우위동맥 림프절(No. 5)을 제외한 D1+$\beta$술식을 시행하였고, 유문륜 상방 $3{\sim}4\;cm$에서 원위부 위절제를 시행하였다. 결과: LAPPG을 시행 받은 환자 9명의 평균 연령은 $59.9{\pm}9.4$세였으며 남녀 성비는 1.3 : 1.0 (남자 5명, 여자 4명)이었고, LADG 후 Billroth-I 재건술을 시행 받은 환자 18명의 평균 연령은 $64.1{\pm}10.0$세였으며 남녀 성비는 2.6 : 1.0 (남자 13명, 여자 5명)이었다. LAPPG 군과 LADG 후 Biliroth-I 재건술을 시행받은 군에서 절제된 림프절의 개수는 각각 $28.3{\pm}11.9$$28.1{\pm}8.9$개, 수술 시간은 각각 $269.0{\pm}34.4$분, $236.3{\pm}39.6$분, 술 중 출혈량은 각각 $191.1{\pm}85.7\;ml$, $218.3{\pm}156.6\;ml$, 술 후 첫 가스 배출은 각각 $3.6{\pm}0.9$일, $3.5{\pm}0.8$일에 있었고 술 후 첫 유동식은 각각 $5.1{\pm}0.9$일, $5.1{\pm}1.7$일에 섭취하였고 술 후 재원 기간은 각각 $10.1{\pm}4.0$일, $9.2{\pm}3.0$일로 모두 통계학적으로 유의한 차이가 없었다(P>0.05). 술 후 합병증은 LAPPG 군에서 위저류증 1예와 창상 장액종 1예가 발생하였고, LADG 후 Biliroth-I 재건술을 시행 받은 군에서는 좌외측간엽경색 1예가 발생하였다. 결론: 조기위암의 치료에 있어서 LAPPG는 기능 보존 수술 법으로 적용 가능하며, 적절한 적응증을 사용하면 임상종양학적 측면에서도 LADG 후 Billroth-I 재건술과 동등한 결과를 보일 것으로 기대된다.

The Early Experience of Laparoscopy-assisted Gastrectomy for Gastric Cancer at a Low-volume Center

  • Purpose: Laparoscopy-assisted gastrectomy (LAG) has become a technically feasible and safe procedure for early gastric cancer treatment. LAG is being increasingly performed in many centers; however, there have been few reports regarding LAG at low-volume centers. The aim of this study was to report our early experience with LAG in patients with gastric cancer at a low-volume center. Materials and Methods: The clinicopathologic data and surgical outcomes of 39 patients who underwent LAG for gastric cancer between April 2007 and March 2010 were retrospectively reviewed. Results: The mean age was 68.3 years. Thirty-one patients had medical co-morbidities. The mean patient ASA score was 2.0. Among the 39 patients, 4 patients underwent total gastrectomy and 35 patients underwent distal gastrectomy. The mean blood loss was 145.4 ml and the mean operative time was 259.4 minutes. The mean time-to-first flatus, first oral intake, and the postoperative hospital stay was 2.8, 3.1, and 9.3 days, respectively. The 30-day mortality rate was 0%. Postoperative complications developed in 9 patients, as follows: anastomotic leakage, 1; wound infection, 1; gastric stasis, 2; postoperative ileus, 1; pneumonia, 1; cerebral infarction, 1; chronic renal failure, 1; and postoperative psychosis, 1. Conclusions: LAG is technically feasible and can be performed safely at a low-volume center, but an experienced surgical team and careful patient selection are necessary. Furthermore, for early mastery of the learning curve for LAG, surgeons need education and training in addition to an accumulation of cases.

Pylorus-Preserving Gastrectomy for Gastric Cancer

  • Pylorus-preserving gastrectomy (PPG) is a function-preserving surgery for the treatment of early gastric cancer (EGC), aiming to decrease the complication rate and improve postoperative quality of life. According to the Japanese gastric cancer treatment guidelines, PPG can be performed for cT1N0M0 gastric cancer located in the middle-third of the stomach, at least 4.0 cm away from the pylorus. Although the length of the antral cuff gradually increased, from 1.5 cm during the initial use of the procedure to 3.0 cm currently, its optimal length still remains unclear. Standard procedures for the preservation of pyloric function, infra-pyloric vessels, and hepatic branch of the vagus nerve, make PPG technically more difficult and raise concerns about incomplete lymph node dissection. The short- and long-term oncological and survival outcomes of PPG were comparable to those for distal gastrectomy, but with several advantages such as a lower incidence of dumping syndrome, bile reflux, and gallstone formation, and improved nutritional status. Gastric stasis, a typical complication of PPG, can be effectively treated by balloon dilatation and stent insertion. Robot-assisted pylorus-preserving gastrectomy is feasible for EGC in the middle-third of the stomach in terms of the short-term clinical outcome. However, any benefits over laparoscopy-assisted PPG (LAPPG) from the patient's perspective have not yet been proven. An ongoing Korean multicenter randomized controlled trial (KLASS-04), which compares LAPPG and laparoscopy-assisted distal gastrectomy for EGC in the middle-third of the stomach, may provide more clear evidence about the advantages and oncologic safety of PPG.

위암에서 복강경보조 원위부 위아전절제술 및 비장합병절제술 좌위동맥의 보존 증례 보고 (The Preservation of Left Gastric Artery in Laparoscopy-Assisted Subtotal Gastrectomy with Splenectomy of Stomach Cancer)

  • 원위부 위암으로 위아전절제술을 시행할 때는 잔위의 경색을 방지하기 위해 비장을 보존하여 단위동맥으로 하여금 잔위의 혈액공급을 할 수 있게 한다. 위아전절제술을 하던 중 비장손상을 받아 지혈을 위해 계획에 없던 비장절제술을 하게 되는 경우에는 blue stomach을 방지하기 위해 종종 위전절제술로 전환해야 하는 경우도 있다. 본원에서는 2개월간의 소화불량을 주소로 내원한 67세 여자환자가, 내시경상 원위부 위암의 소견과 함께 술 전 복부 CT에서 비장의 전이를 확실하게 배제할 수 없는 1.5 cm 크기의 혈관종 의증 소견이 보여 확진을 위한 검사 및 치료의 목적으로 원위부 위아전절제술 및 비장절제술을 계획하였으며 복강경보조로 좌위동맥과 정맥을 보존하면서 림프절 곽청을 포함한 근치적 위아전절제 및 비장 절제술을 성공적으로 시행하였다. 수술 후 잔위 경색을 비롯한 합병증 없이 만족스러운 경과를 보였으며 최종적으로 비장의 종양은 림프관종이었다. 병기는 T1 (sm1) N0 (0/26) M0으로 1aI기로, 현재 외래 추적 관찰 중이다.

Comparison of Learning Curves and Clinical Outcomes between Laparoscopy-assisted Distal Gastrectomy and Open Distal Gastrectomy

  • Purpose: Most stomach surgeons have been educated sufficiently in conventional open distal gastrectomy (ODG) but insufficiently in laparoscopy-assisted distal gastrectomy (LADG). We compared learning curves and clinical outcomes between ODG and LADG by a single surgeon who had sufficient education of ODG and insufficient education of LADG. Materials and Methods: ODG (90 patients, January through September, 2004) and LADG groups (90 patients, June 2006 to June 2007) were compared. The learning curve was assessed with the mean number of retrieved lymph nodes, operation time, and postoperative morbidity/mortality. Results: Mean operation time was 168.3 minutes for ODG and 183.6 minutes for LADG. The mean number of retrieved lymph nodes was 37.9. Up to about the 20th to 25th cases, the slope decrease in the learning curve for LADG was more apparent than for ODG, although they both reached plateaus after the 50th cases. The mean number of retrieved lymph nodes reached the overall mean after the 30th and 40th cases for ODG and LADG, respectively. For ODG, complications were evenly distributed throughout the subgroups, whereas for LADG, complications occurred in 10 (33.3%) of the first 30 cases. Conclusions: Compared with conventional ODG, LADG is feasible, in particular for a surgeon who has had much experience with conventional ODG, although LADG required more operative time, slightly more time to get adequately retrieved lymph nodes and more complications. However, there were more minor problems in the first 30 LADG than ODG cases. The unfavorable results for LADG can be overcome easily through an adequate training program for LADG.

A Novel Technique of Hand-Sewn Purse-String Suturing by Double Ligation Method (DLM) for Intracorporeal Circular Esophagojejunostomy

  • Purpose: The optimal method for intracorporeal esophagojejunostomy remains unclear because a purse-string suture for fixing the anvil into the esophagus is difficult to perform with a laparoscopic approach. Therefore, this study aimed to evaluate our novel technique to fix the anvil into the esophagus. Materials and Methods: This retrospective study included 202 patients who were treated at our institution with an intracorporeal circular esophagojejunostomy in a laparoscopy-assisted total gastrectomy with a Roux-en-Y reconstruction (166 cases) or a laparoscopy-assisted proximal gastrectomy with jejunal interposition (36 cases). After incising 3/4 of the esophageal wall, a hand-sewn purse-string suture was placed on the esophagus. Next, the anvil head of a circular stapler was introduced into the esophagus. Finally, the circular esophagojejunostomy was performed laparoscopically. The clinical characteristics and surgical outcomes were evaluated and compared with those of other methods. Results: The average operation time was 200.3 minutes. The average hand-sewn purse-string suturing time was 6.4 minutes. The overall incidence of postoperative complications (Clavien-Dindo classification grade ${\geq}II$) was 26%. The number of patients with an anastomotic leakage and stenosis at the esophagojejunostomy site were 4 (2.0%) and 12 (6.0%), respectively. All patients with stenosis were successfully treated by endoscopic balloon dilatation. There was no mortality. Regarding the materials and devices for anvil fixation, only 1 absorbable thread was needed. Conclusions: Our procedure for hand-sewn purse-string suturing with the double ligation method is simple and safe.

개복 위절제술에 경험이 풍부한 술자에 의한 복강경 보조하 원위부 위절제술의 Learning Curve (Learning Curve of a Laparoscopy Assisted Distal Gastrectomy for a Surgeon Expert in Performing a Conventional Open Gastrectomy)

  • 목적: 조기위암 치료에 있어서 복강경 위암 수술이 새로운 패러다임으로 정착하고 있다. 기존에 시행하고 있던 개복에 의한 위절제술에 익숙한 경험 많은 외과의사들은 복강경 위암 수술은 시간이 많이 소요되며 기술적으로 습득하는데 많은 제약이 있음을 느끼며 복강경 수술에 소극적 자세를 취하는 경향이 있다. 이에 저자 등은 개복 위절제술에 경험이 풍부한 외과의사에 의한 복강경 위절제술의 learning curve를 결정하고, 이에 영향을 미치는 요인들을 분석하였다. 대상 및 방법: 2005년 4월부터 2006년 3월까지 한 명의 술자에 의하여 수술전 조기 위암(cT1N0)으로 진단 받고 복강경 보조하 원위부 위절제술(LADG) 및 $D1+{\beta}$ 림프절 곽청을 시행 받은 62명을 대상으로 하였으며 복강경 위절제술 시행을 위하여 전문팀을 구성하였다. 대상 환자를 6명씩 한 그룹으로 총 10그룹(마지막 그룹은 8명으로 구성함)으로 나누어 각 그룹의 평균 수술시간을 비교하여 learning curve 극복 전후의 나이, 성별, 수술 후 합병증, 절개창의 길이, 수혈유무, 적출된 림프절 개수, 수술 전 후 혈색소 변화 등을 분석하였다. 결과: 평균 수술 시간을 분석한 결과 여섯 번째 그룹 (31st case)부터 수술 시간의 Plateau를 보였다. 이에 저자들은 learning curve 극복시점을 30th case (7개월)로 간주하였으며 극복 전후 집단 간의 평균 수술시간을 분석하였을 때 각각 $239.0{\pm}69.7$분과 $170.0{\pm}32.6$분으로 유의하게 나타났다(P<0.05), 양 군 간 평균나이, 성별, BMI, 수술 전후의 혈색소 수치변화, 적출된 림프절 개수 등은 유의한 차이를 보이지 않았다. 또한 절개창의 길이, 수혈유무, 수술 후 합병증 유무도 양 군 간에 통계적으로 유의하지 않았다. 결론: 저자들의 LADG learning curve 극복은 30예(7개월)로 다른 보고보다 일찍 도달할 수 있었다. 이는 개복 수술의 풍부한 경험, 전문 수술팀 구성, 그리고 단기간 집중적인 시술에 의한 것으로 생각된다.

Clinical Outcome of Modified Laparoscopy-Assisted Proximal Gastrectomy Compared to Conventional Proximal Gastrectomy or Total Gastrectomy for Upper-Third Early Gastric Cancer with Special References to Postoperative Reflux Esophagitis

  • Purpose: This study evaluated the functional and oncological outcomes of proximal gastrectomy (PG) in comparison with total gastrectomy (TG) for upper-third early gastric cancer (EGC). Materials and Methods: The medical records of upper-third EGC patients who had undergone PG (n=192) or TG (n=157) were reviewed. The PG group was further subdivided into patients who had undergone conventional open PG (cPG; n=157) or modified laparoscopy-assisted PG (mLAPG; n=35). Patients who had undergone mLAPG had a longer portion of their intra-abdominal esophagus preserved than patients who had undergone cPG. Surgical morbidity, recurrence, long-term nutritional status, and the incidence of reflux esophagitis were compared between the groups. Results: The rate of postoperative complications was significantly lower for PG than TG (16.7% vs. 31.2%), but the five-year overall survival rate was comparable between the two groups (99.3% vs. 96.3%). Postoperative levels of hemoglobin and albumin were significantly higher for patients who had undergone PG. However, the incidence of reflux esophagitis was higher for PG than for TG (37.4% vs. 3.7%; P<0.001). mLAPG was related to a lower incidence of reflux esophagitis after PG (P<0.001). Conclusions: Compared to TG, PG showed an advantage in terms of postoperative morbidity and nutrition, and there was a comparable prognosis between the two procedures. Preserving the intra-abdominal esophagus may lower the incidence of reflux esophagitis associated with PG.

Long-Term Survival Following Port-Site Metastasectomy in a Patient with Laparoscopic Gastrectomy for Gastric Cancer: A Case Report

  • A 78-year-old man underwent laparoscopy-assisted total gastrectomy for gastric cancer (pT3N0M0). Multiple port sites were used, including a 10 mm port for a videoscope at the umbilical point and three other working ports. During the six-month follow-up evaluation, a 2 cm enhancing mass confined to the muscle layer was found 12 mm from the right lower quadrant port site, suggesting a metastatic or desmoid tumor. Follow-up computed tomography imaging two months later showed that the mass had increased in size to 3.5 cm. We confirmed that there was no intra-abdominal metastasis by diagnostic laparoscopy and then performed a wide resection of the recurrent mass. The histologic findings revealed poorly differentiated adenocarcinoma, suggesting a metastatic mass from the stomach cancer. The postoperative course was uneventful, and the patient completed adjuvant chemotherapy with TS-1 (tegafur, gimeracil, and oteracil potassium). There was no evidence of tumor recurrence during the 50-month follow-up period.
