음성장애가 있는 직업적 음성사용자와 비직업적 음성사용자의 음성장애 중증도와 유형에 따른 자기보고식 음성평가 차이 (Comparison of Self-Reporting Voice Evaluations between Professional and Non-Professional Voice Users with Voice Disorders by Severity and Type)

  • The purpose of this study was to compare professional (Pro) and non-professional (Non-pro) voice users with voice disorders in self-reporting voice evaluation using Korean-Voice Handicap Index (K-VHI) and Korean-Voice Related Quality of Life (K-VRQOL). In addition, those were compared by voice quality and voice disorder type. 94 Pro and 106 Non-pro were asked to fill out the K-VHI and K-VRQOL, perceptually evaluated on GRBAS scales, and divided into three types of voice disorders (functional, organic and neurologic) by an experienced speech-language pathologist and an otolaryngologist. The results showed that the functional (F) and physical (P) scores of K-VHI in Pro group were significantly higher than those in Non-pro group. As the voice quality evaluated by G scale got worse, the scores of all aspects except emotional (E) of K-VHI and social-emotional (SE) of K-VRQOL were higher. All scores of K-VHI and K-VRQOL in neurologic voice disorders were significantly higher than those in functional and organic voice disorders. In conclusion, professional voice users are more sensitive to their functional and physical handicap resulted by their voice problems and that goes double for the patients with severe and neurologic voice disorders.

  • Motor delay, when present, is usually the first concern brought by the parents of children with developmental delay. Cerebral palsy that is the most common motor delay, is a nonspecific, descriptive term pertaining to disordered motor function that is evident in early infancy and is characterized by changes in muscle tone, muscle weakness, involuntary movements, ataxia, or a combination of these abnormalities. A wide range of causative disorders and risk factors have been identified for cerebral palsy, and broadly classified into 5 groups; perinatal brain injury, brain injury related to prematurity, developmental abnormalities, prenatal risk factors, and postnatal brain injury. Delay in attaining developmental milestones is the most distinctive presenting complaint in children with cerebral palsy. A detailed history and thorough physical and neurologic examinations are crucial in the diagnostic process. The clinician should be cautious about diagnostic pronouncement unless the findings are unequivocal. Several serial examinations and history review are necessary. All children with cerebral palsy should undergo a neuroimaging study, preferably MRI, because an abnormality is documented on head MRI(89%) and CT(77%). The high incidence rates for mental retardation, epilepsy, ophthalmologic defects, speech and language disorders and hearing impairment make it imperative that all children with cerebral palsy be screened for mental retardation, ophthalmologic and hearing impairments, and speech and language disorders; nutrition, growth, and swallowing also should be closely monitored.

음성위생교육 만족도에 대한 예비 연구 (A Survey on Participants' Satisfaction of Vocal Hygiene Education: A Preliminary Study)

  • Vocal hygiene education is an indirect training approach to improve vocal function by educating all facets of optimal vocal health. Satisfaction levels of participants might be an important component of this indirect therapy for voice disorders. The authors aimed to investigate the satisfaction levels of vocal hygiene education in 51 patients with voice problems. We classified voice disorders of the participants according to three etiological categories (subgroups): organic, neurogenic, and functional. The survey consisted of three parts: 1) a condition of vocal hygiene education, 2) a degree of satisfaction of the present education, and 3) a request for future education. Participants responded to each item of the survey using a five-point Likert scale of 1 to 5 (1 being not at all and 5 being extremely). They also wrote down personal comments of improvement. Participants scored the vocal hygiene education offered by the speech-language pathologists between '3' and '4'. Specifically, the participants were highly satisfied with the specific and comprehensible explanation/instruction given by their speech-language pathologists. However, they were less satisfied with the tuition fee for the therapy sessions. Vocal hygiene education is offered individually to people in a clinical setting. Our results support the notion that vocal hygiene education can be an integral aspect of the treatment of voice problems in most cases.

음성장애 환자에서 시행되는 청지각적 평가에 대한 논의 (Discussions on Auditory-Perceptual Evaluation Performed in Patients With Voice Disorders)

  • The auditory-perceptual evaluation of speech-language pathologists (SLP) in patients with voice disorders is often regarded as a touchstone in the multi-dimensional voice evaluation procedures and provides important information not available in other assessment modalities. Therefore, it is necessary for the SLPs to conduct a comprehensive and in-depth evaluation of not only voice but also the overall speech production mechanism, and they often encounter various difficulties in the evaluation process. In addition, SLPs should strive to avoid bias during the evaluation process and to maintain a wide and constant spectrum of severity for each parameter of voice quality. Lastly, it is very important for the SLPs to perform a team approach by documenting and delivering important information pertaining to auditory-perceptual characteristics in an appropriate and efficient way through close communication with the laryngologists.

  • The purpose of this study was to evaluate the effects of multisensory(AVK: Auditory, Visual and Kinethetic) treatment on reading pronunciation with phonological prcessing - tensification, palatalization, and lateralization for the middle school students with delayed language development caused by mental retarded. Participants were three children with reading pronunciation difficulties in phonological processing. The following conclusions were arrived. First, three children are improved on tensifiication, palatalization, and lateralization by multisensory treatment program. Second, multisensory treatment was effective in facilitating generalization. Three children presented prominent generalization effcects in lateralization. Third, they were found to maintain partially their performance rates of the later phase of the reading with phonological processing intervention three weeks after the termination of the intervention.

The Long-Term Outcome and Rehabilitative Approach of Intraventricular Hemorrhage at Preterm Birth

  • Technological advances in neonatology led to the improvement of the survival rate in preterm babies with very low birth weights. However, intraventricular hemorrhage (IVH) has been one of the major complications of prematurity. IVH is relevant to neurodevelopmental disorders, such as cerebral palsy, language and cognitive impairments, and neurosensory and psychiatric problems, especially when combined with brain parenchymal injuries. Additionally, severe IVH requiring shunt insertion is associated with a higher risk of adverse neurodevelopmental outcomes. Multidisciplinary and longitudinal rehabilitation should be provided for these children based on the patients' life cycles. During the infantile period, it is essential to detect high-risk infants based on neuromotor examinations and provide early intervention as soon as possible. As babies grow up, close monitoring of language and cognitive development is needed. Moreover, providing continuous rehabilitation with task-specific and intensive repetitive training could improve functional outcomes in children with mild-to-moderate disabilities. After school age, maintaining the level of physical activity and managing complications are also needed.

  • Cerebrovascular accident(CVA) is a leading cause of death, and severe sequelae, like motor disturbance, mental disorder, dysphagia, recognition disorder, speech disorder(aphasia) often occur. Most of medical cure about CVA sequelae lay emphasis on motor disturbance, so speech disorder(aphasia) has been neglected. But speech disorder therapy is essential for social rehabiltation. Recently, inside and outside South Korea, various clinical approaches and potential medical cures for speech disorder (aphasia) have been researched. In Korean Medicine, papers pertaining to speech disorders have been but a few. In this study two cases of aphasic stroke patients who were treated for speech and language disorders through Korean medical therapy are reported.

Recent advances in genetic studies of stuttering

  • Speech and language are uniquely human-specific traits, which contributed to humans becoming the predominant species on earth. Disruptions in the human speech and language function may result in diverse disorders. These include stuttering, aphasia, articulation disorder, spasmodic dysphonia, verbal dyspraxia, dyslexia and specific language impairment. Among these disorders, stuttering is the most common speech disorder characterized by disruptions in the normal flow of speech. Twin, adoption, and family studies have suggested that genetic factors are involved in susceptibility to stuttering. For several decades, multiple genetic studies including linkage analysis were performed to connect causative gene to stuttering, and several genetic studies have revealed the association of specific gene mutation with stuttering. One notable genetic discovery came from the genetic studies in the consanguineous Pakistani families. These studies suggested that mutations in the lysosomal enzyme-targeting pathway genes (GNPTAB, GNPTG and NAPGA) are associated with non-syndromic persistent stuttering. Although these studies have revealed some clues in understanding the genetic causes of stuttering, only a small fraction of patients are affected by these genes. In this study, we summarize recent advances and future challenges in an effort to understand genetic causes underlying stuttering.

한국어-영어 이중언어 화자의 사용 언어, 문맥, 언어 능숙도에 따른 기본 주파수 변화 (Changes in fundamental frequency depending on language, context, and language proficiency for bilinguals)

  • 본 연구에서는 영어-한국어 이중언어 화자의 평균 기본주파수를 분석하여 언어 및 과제에 따른 차이가 있는지 살펴보고, 거주년수 및 언어 능숙도에 따라 언어 간 평균 기본주파수 상이한지 실펴보고자 하였다. 본 연구의 대상자는 영어권 나라에 7년 이상 거주한 한국어-영어 균형적 이중언어 집단 총 28명(남녀 각각 14명, 균형적 이중언어집단), 언어 노출기간 및 언어 능숙도에 차이를 두기 위해 추가로 모집한 영어권 나라 거주년수 3년 이하인 한국어 우세 이중언어 집단 총 20명(남녀 각각 10명, 한국어 우세 이중언어 집단)이다. 이들은 한국어와 영어로 읽기 및 자발화 과제를 수행하였고 Praat을 통해 녹음한 음성파일의 평균 기본주파수를 분석하였다. 이후 각 성별 내 언어 ${\times}$ 과제 이원 배치 분산 분석(two-way repeated ANOVA) 및 집단 ${\times}$ 언어의 이원 혼합 분산 분석(two-way mixed ANOVA)을 실시하였다. 그 결과, 언어 및 과제에 따라 균형적 이중언어 집단의 남녀 대상군 별로 상이한 결과가 나타났다. 여성의 경우 한국어보다 영어의 평균 기본주파수가 유의하게 높았으며(p=.003), 자발화과제보다 읽기 과제에서 평균 기본주파수가 유의하게 높았다(p=.002). 그러나 남성의 경우, 어떠한 조건에서도 유의한 결과가 나타나지 않았다. 두 번째로, 언어 능숙도에 따른 집단 간 비교에서 여성 대상자들에서만 언어 간 기본주파수 변화가 통계적으로 유의하게 나타났다(p=.000). 즉, 한국어 읽기 과제에서 평균 기본주파수가 영어 읽기 과제보다 유의하게 높았다. 그러나 언어 능숙도에 따른 평균 기본주파수의 변화는 통계적으로 유의하지 않았다(p=.830). 남성 대상자의 경우는 어떤 조건에서도 유의한 결과는 없었다. 결과적으로, 이중언어 화자의 언어 간 평균 기본주파수의 변화는 언어의 능숙도와 관계없이 성별에 따라 상이하게 나타났으며 본 연구를 바탕으로 언어 간 평균 주파수의 차이가 해부생리학적 요인이나 언어학적 요인보다도 사화문화적인 요인에 의한 것임을 생각해 볼 수 있도록 한다.

말소리장애 아동의 말명료도와 음향학적 측정치 간 상관관계 (The Correlation between Speech Intelligibility and Acoustic Measurements in Children with Speech Sound Disorders)

  • Purpose : This study investigated the correlation between speech intelligibility and acoustic measurements of speech sounds produced by the children with speech sound disorders and children without any diagnosed speech sound disorder. Methods : A total of 60 children with and without speech sound disorders were the subjects of this study. Speech samples were obtained by having the subjects? speak meaningful words. Acoustic measurements were analyzed on a spectrogram using the Multi-speech 3700 program. Speech intelligibility was determined according to a listener's perceptual judgment. Results : Children with speech sound disorders had significantly lower speech intelligibility than those without speech sound disorders. The intensity of the vowel /u/, the duration of the vowel /${\omega}$/, and the second formant of the vowel /${\omega}$/ were significantly different between both groups. There was no difference in voice onset time between the groups. There was a correlation between acoustic measurements and speech intelligibility. Conclusion : The results of this study showed that the speech intelligibility of children with speech sound disorders was affected by intensity, word duration, and formant frequency. It is necessary to complement clinical setting results using acoustic measurements in addition to evaluation of speech intelligibility.
