PURPOSE. The present study aimed to investigate the relationships between the crown form of the upper central incisor and their labial inclination, overbite, and overjet. MATERIALS AND METHODS. Maxillary and mandibular casts of 169 healthy dentitions were subjected to 3D dental scanning, and analyzed using CAD software. The crown forms were divided into tapered, square, and ovoid based on the mesiodistal dimensions at 20% of the crown height to that at 40%. The degree of labial inclination of the upper central incisor was defined as the angle between the occlusal plane and the line connecting the incisal edge and tooth cervix. The incisal edges of the right upper and lower central incisor that in contact with lines parallel to the occlusal plane were used to determine the overbite and overjet. One-way ANOVA was performed to compare the labial inclination, overbite, and overjet among the crown forms. RESULTS. The crown forms were classified into three types; crown forms with a 20%/40% dimension ratio of 1.00±0.01 were defined as square, >1.01 as tapered, and <0.99 as ovoid. The labial inclination degree was the greatest in tapered and the least in square. Both overbite and overjet in tapered and ovoid were higher than those in square. CONCLUSION. Upper central incisor crown forms were related to their labial inclination, overbite, and overjet. It was suggested that the labial inclination, overbite, and overjet should be taken into consideration for the prosthetic treatment or restoring the front teeth crowns.
Objective: The aim of this study was to investigate whether labial tooth inclination and alveolar bone loss affect the moment per unit of force ($M_t/F$) in controlled tipping and consequent stresses on the periodontal ligament (PDL). Methods: Three-dimensional models (n = 20) of maxillary central incisors were created with different labial inclinations ($5^{\circ}$, $10^{\circ}$, $15^{\circ}$, and $20^{\circ}$) and different amounts of alveolar bone loss (0, 2, 4, and 6 mm). The $M_t/F$ necessary for controlled tipping ($M_t/F_{cont}$) and the principal stresses on the PDL were calculated for each model separately in a finite element analysis. Results: As labial inclination increased, $M_t/F_{cont}$ and the length of the moment arm decreased. In contrast, increased alveolar bone loss caused increases in $M_t/F_{cont}$ and the length of the moment arm. When $M_t/F$ was near $M_t/F_{cont}$, increases in Mt/F caused compressive stresses to move from a predominantly labial apical region to a palatal apical position, and tensile stresses in the labial area moved from a cervical position to a mid-root position. Although controlled tipping was applied to the incisors, increases in alveolar bone loss and labial tooth inclination caused increases in maximum compressive and tensile stresses at the root apices. Conclusions: Increases in alveolar bone loss and labial tooth inclination caused increases in stresses that might cause root resorption at the root apex, despite the application of controlled tipping to the incisors.
본 연구는 상하악 절치부의 압하를 도모하기 위한 장치의 하나인 분절호선장치를 사용하여 절치부의 순측경사에 따른 저항중심의 위치와 변화양상, 그리고 치축경사 개선과 압하를 동시에 이루기 위해 필요한 최소 후방 견인력의 크기 및 변화양상을 알아보기 위하여 3차원 유한요소법을 이용하여 시행되었으며 다음과 같은 결론을 얻었다. 1. 치축 경사도의 변화에 따른 상악 절치부 저항중심은 1) 정상 치축경사를 가진 경우에서 측절치 브라켓 원심면 후방 6mm에 위치하였다. 2) 순측경사가 $10^{\circ}$ 증가된 경우에서 측절치 브라켓의 원심면 후방 9mm에 위치하였다. 3) 순측경사가 $20^{\circ}$ 증가된 경우에서 측절치 브라켓 원심면 후방 12mm에 위치하였다. 4) 순측경사가 $30^{\circ}$ 증가된 경우에서 측절치 브라켓 원심면 후방 16mm에 위치하였다. 2. 치축 경사도의 변화에 따른 하악 절치부 저항중심은 1) 정상 치축경사를 가진 경우에서 측절치 브라켓 원심면 후방 10mm에 위치하였다. 2) 순측경사가 $10^{\circ}$ 증가된 경우에서 측절치 브라켓 원심면 후방 13mm에 위치하였다. 3) 순측경사가 $20^{\circ}$ 증가된 경우에서 측절치 브라켓 원심면 후방 15mm에 위치하였다. 4) 순측경사가 $30^{\circ}$ 증가된 경우에서 측절치 브라켓 원심면 후방 18mm에 위치하였다. 3. 응력분포 양상은 1) 각 저항중심에서 압하력을 가한 경우에 치주인대에 균일한 압축응력을 나타내었다. 2) 후방 견인력을 동시에 적용한 경우에 순측경사가 증가할수록 응력분포 양상이 복잡해지는 양상을 보였다. 4. 상하악 절치부가 $20^{\circ}$까지 순측경사된 경우에서 Pure intrusion을 위하여 필요한 후방 견인력의 크기도 증가하였다.

Background: To test the hypothesis that in profile smiling view, for ideal aesthetics, a tangent to the labial face of the maxillary central incisor crowns should be approximately parallel to the true vertical line and thereby perpendicular to the true horizontal line. Methods: An idealized female image was created with computer software and manipulated using the same software to construct an "ideal" female profile image with proportions, and linear and angular soft tissue measurements, based on currently accepted criteria for idealized Caucasian profiles. The maxillary incisor labial face tangent was altered in 5° increments from 70 to 120°, creating a range of images, shown in random order to 70 observers (56 lay people and 14 clinicians), who ranked the images from the most to the least attractive. The main outcome was the preference ranks of image attractiveness given by the observers. Results: The most attractive inclination of a tangent to the labial face of the maxillary incisor crowns in profile view in relation to the true horizontal line was 85°, i.e. 5° retroclined from a perpendicular 90° inclination. The most attractive range appears to be between 80 and 90°. Excessive proclination appeared to be less desirable than retroclination. Beyond 105° most observers recommend treatment. Conclusion: In natural head position, the ideal inclination of the maxillary incisor crown labial face tangent in profile view will be approximately parallel to the true vertical line and thereby approximately perpendicular to the true horizontal line.
Purpose: The aim of this study was to investigate and identify the main causes of periodontal tissue change associated with labial gingival recession by examining the anterior region of patients who underwent orthodontic treatment. Methods: In total, 45 patients who had undergone orthodontic treatment from January 2010 to December 2015 were included. Before and after the orthodontic treatment, sectioned images from 3-dimensional digital model scanning and cone-beam computed tomography images in the same region were superimposed to measure periodontal parameters. The initial labial gingival thickness (IGT) and the initial labial alveolar bone thickness (IBT) were measured at 4 mm below the cementoenamel junction (CEJ), and the change of the labial gingival margin was defined as the change of the distance from the CEJ to the gingival margin. Additionally, the jaw, tooth position, tooth inclination, tooth rotation, and history of orthognathic surgery were investigated to determine the various factors that could have affected anterior periodontal tissue changes. Results: The mean IGT and IBT were 0.77±0.29 mm and 0.77±0.32 mm, respectively. The mean gingival recession was 0.14±0.57 mm. Tooth inclination had a significant association with gingival recession, and as tooth inclination increased labially, gingival recession increased by approximately 0.2 mm per 1°. Conclusions: In conclusion, the IGT, IBT, tooth position, tooth rotation, and history of orthognathic surgery did not affect labial gingival recession. However, tooth inclination showed a significant association with labial gingival recession of the anterior teeth after orthodontic treatment.
This study was undertaken to establish the roentgenocephalometric standards of the Korean children in Hellman dental age III C. The subjects consisted of 33 males and 33 females with the normal occlusion and acceptable profile. The lateral cephalometric films were taken with the teeth in centric occlusion, the soft tissue outline of the nose, lips, and chin was made visible by the low-speed films, 70Kvp, 100Mas. Their linear and angular measurements were performed by Jarabak's methods. The following results were obtained; 1) The author made the tables of standard deviation from the measured values. 2) Each linear measurement of the skull was greater in males than in females. 3) The maxillary basal bones were more protrusive in Korean children than in Caucasian. 4) The degree of the facial convexity was larger in Korean children than in Caucasian. 5) The labial inclination of the upper & lower incisors was greater in Korean children than in Caucasian. The labial inclination of the upper incisor was greater in females, but the labial inclination of the lower incisor was greater in males.

Objective: To assess the labial and lingual alveolar bone thickness in adults with maxillary central incisors of different inclination by cone-beam computed tomography (CBCT). Methods: Ninety maxillary central incisors from 45 patients were divided into three groups based on the maxillary central incisors to palatal plane angle; lingual-inclined, normal, and labial-inclined. Reformatted CBCT images were used to measure the labial and lingual alveolar bone thickness (ABT) at intervals corresponding to every 1/10 of the root length. The sum of labial ABT and lingual ABT at the level of the root apex was used to calculate the total ABT (TABT). The number of teeth exhibiting alveolar fenestration and dehiscence in each group was also tallied. One-way analysis of variance and Tukey's honestly significant difference test were applied for statistical analysis. Results: The labial ABT and TABT values at the root apex in the lingual-inclined group were significantly lower than in the other groups (p < 0.05). Lingual and labial ABT values were very low at the cervical level in the lingual-inclined and normal groups. There was a higher prevalence of alveolar fenestration in the lingual-inclined group. Conclusions: Lingual-inclined maxillary central incisors have less bone support at the level of the root apex and a greater frequency of alveolar bone defects than normal maxillary central incisors. The bone plate at the marginal level is also very thin.
Objective: This study was performed to investigate the changes in alveolar bone after maxillary incisor intrusion and to determine the related factors in deep-bite patients. Methods: Fifty maxillary central incisors of 25 patients were evaluated retrospectively. The maxillary incisors in Group I (12 patients; mean age, $16.51{\pm}1.32years$) were intruded with a base-arch, while those in Group II (13 patients; mean age, $17.47{\pm}2.71years$) were intruded with miniscrews. Changes in the alveolar envelope were assessed using pre-intrusion and post-intrusion cone-beam computed tomography images. Labial, palatal, and total bone thicknesses were evaluated at the crestal (3 mm), midroot (6 mm), and apical (9 mm) levels. Buccal and palatal alveolar crestal height, buccal bone height, and the prevalence of dehiscence were evaluated. Two-way repeated measure ANOVA was used to determine the significance of the changes. Pearson's correlation coefficient analysis was performed to assess the relationship between dental and alveolar bone measurement changes. Results: Upper incisor inclination and intrusion changes were significantly greater in Group II than in Group I. With treatment, the alveolar bone thickness at the labial bone thickness (LBT, 3 and 6 mm) decreased significantly in Group II (p < 0.001) as compared to Group I. The LBT change at 3 mm was strongly and positively correlated with the amount of upper incisor intrusion (r = 0.539; p = 0.005). Conclusions: Change in the labial inclination and the amount of intrusion should be considered during upper incisor intrusion, as these factors increase the risk of alveolar bone loss.
Objective: The objective of this two-arm parallel trial was to compare the type of tooth movement during en masse retraction of the maxillary anterior teeth using labial versus lingual biocreative therapy. Methods: Twenty-eight subjects were randomized in a 1 : 1 ratio to either the labial or lingual group. En masse anterior retraction was performed using labial biocreative therapy in group A and lingual biocreative therapy in group B. Cone beam computed tomography scans were taken before and after retraction and the primary outcome was the type of tooth movement during anterior retraction. Data were analyzed using paired t-tests for comparisons within each group and independent-sample t-test for comparison of the mean treatment changes between the two groups. Results: Significant differences were found between the two groups in relation to the type of tooth movement (labiolingual inclination of the central incisor; mean difference, $5.85{\pm}1.85^{\circ}$). The canine showed significant distal tipping in the lingual group (mean difference, $6.98{\pm}1.25^{\circ}$). The canine was significantly more intruded in the lingual group (mean difference, $1.67{\pm}0.49mm$). Good anchorage control and significant soft tissue changes occurred in both groups. No serious adverse effects were detected. Conclusions: With a 10-mm retraction hook, the labial biocreative technique with the reverse curve overlay provided anterior retraction with good torque control, while in the lingual group, anterior retraction occurred with controlled tipping movement with significant distal tipping and intrusion of the canine (trial registration: The trial was registered at ClinicalTrials.gov [NCT03239275]).
This study was undertaken to find out the characteristic craniofacial morphology of Class II Division 2 malocclusions in children by means of roentgenocephalometry. The subjects consisted of twelve boys and thirteen girls with Class Ii, Division 2, thirty seven boys and fifty three girls with Class II, Division 1, and forty six boys and eighty one girls with normal occlusion, ranged from 10 years old to 18 years old. The following results were obtained; 1. The anteroposterior relationship of the maxilla to the cranium in the Class II, Division 2 malocclusion similar to the normal occlusion, but the mandible was the posterior position in th the cranial anatomy. 2. There were no significant differences in the anteroposterior relationship of the maxilla and the mandible between Class II, Division 1 and Class II, Division 2 malocclusions. 3. In Class II, Division 1 the axes of maxillary incisors showed labial inclination, but lingual inclination in Class II, Division 2 malocclusions. 4. Overbite was prominent one in Class II, Division 2, on the other hand overjet was distinguished in Class II, Division 1.
