Outcome of Management of Local Recurrence after Immediate Transverse Rectus Abdominis Myocutaneous Flap Breast Reconstruction

  • Background No consensus has been reached regarding the outcome of management of local recurrence after transverse rectus abdominis myocutaneous (TRAM) flap breast reconstruction. This study demonstrated the presentation, management, and outcomes of local recurrence after immediate TRAM breast reconstruction. Methods A comparison was conducted among 1,000 consecutive patients who underwent immediate breast reconstruction with a pedicled TRAM flap (TRAM group) and 3,183 consecutive patients who underwent only modified radical mastectomy without reconstruction (MRM group) from January 2001 to December 2009. The presentation, treatment, and outcome including aesthetics and overall survival rate were analyzed. Results Local recurrences occurred in 18 (1.8%) patients (TRAM-LR group) who underwent TRAM breast reconstruction and 38 (1.2%) patients (MRM-LR group) who underwent MRM only (P=0.1712). Wide excision was indicated in almost all the local recurrence cases. Skin graft was required in 4 patients in the MRM-LR group, whereas only one patient required a skin graft to preserve the mound shape in the TRAM-LR group. The breast mound was maintained in all 17 patients that survived in the TRAM-LR group even after wide excision. The overall survival rate was 94.4% in the TRAM-LR group and 65.8% in the MRM-LR group (P=0.276). Conclusions Local recurrence after immediate TRAM flap breast reconstruction could be detected without delay and managed effectively by multiple modalities without reducing overall survival rates. Breast mound reconstruction with soft autologous tissue allowed for primary closure in most of the cases. In all of the patients who survived, the contour of their reconstructed breast remained.

  • Extended PLR grammars are currently the largest subclass of LR grammars whose grammars are transformed into LL grammars satisfying covering property. This paper suggests a simplified covering transformation of the original covering transformation for extended PLR grammars. The proposed covering transformation reduces the original four rule types to the three rule types.

An Analysis on Traffic Noise at Pusan Shopping Districts by Leq, Lr, TNI Ratings (Leq, Lr, TNI 평가법에 의한 부산시 주요 상가변의 소음도 분석)

  • To evaluate the degree of traffic noise at the main shopping districts, 30 sites, in Pusan and produce the basic data of traffic noise counterplan, this research was conducted. Noise rating methods utilized were Leq(equivalent continuous sound level), Lr (rating sound level) and TNI (traffic noise index). As the results, all sites passed beyond Korean Environmental Noise Standard 70 dB(A) in Leq and 46.7% sites fell under 'widespread complaints'step, 40.0% 'threat of community' action'step, 13.3% 'vigorous community action'step by Lr ratings, and 30.0% sites were over acceptable TNI criteria of 74. Therefore, traffic noise at Pusan shopping districts was very severe.

Predictive Bayesian Network Model Using Electronic Patient Records for Prevention of Hospital-Acquired Pressure Ulcers (전자의무기록을 이용한 욕창발생 예측 베이지안 네트워크 모델 개발)

  • Purpose: The study was designed to determine the discriminating ability of a Bayesian network (BN) for predicting risk for pressure ulcers. Methods: Analysis was done using a retrospective cohort, nursing records representing 21,114 hospital days, 3,348 patients at risk for ulcers, admitted to the intensive care unit of a tertiary teaching hospital between January 2004 and January 2007. A BN model and two logistic regression (LR) versions, model-I and .II, were compared, varying the nature, number and quality of input variables. Classification competence and case coverage of the models were tested and compared using a threefold cross validation method. Results: Average incidence of ulcers was 6.12%. Of the two LR models, model-I demonstrated better indexes of statistical model fits. The BN model had a sensitivity of 81.95%, specificity of 75.63%, positive and negative predictive values of 35.62% and 96.22% respectively. The area under the receiver operating characteristic (AUROC) was 85.01% implying moderate to good overall performance, which was similar to LR model-I. However, regarding case coverage, the BN model was 100% compared to 15.88% of LR. Conclusion: Discriminating ability of the BN model was found to be acceptable and case coverage proved to be excellent for clinical use.

Effect of Feeding Lactobacillus reuteri to Laying Hens on Laying Performance, Availability and Intestinal Microflora (Lactobacillus reuteri의 급여가 산란계의 생산성, 영양소 이용율 및 장내 미생물 변화에 미치는 영향)

  • A feeding trial was conducted to investigate the influence of feeding Lactobacillus reuteri culture (LR) on productive performance, intestinal microflora and availability in laying hens. Four hundred and eighty, Isa-Brown layers, 49 weeks of age, were fed diets supplemented with LR at the level of 0 (control), 0.1, 0.2, and $0.4\%$ of the diets for eight weeks. Egg production and egg weight were measured daily. Feed intake was weighed every two weeks. Egg quality was measured three times at the start, mid-term, and end of the experiment. Intestinal microflora were examined for Lactobacillus spp., E. coli and Salmonella at the end of the experiment. Overall egg production was the highest in $0.2\%$ LR (P<0.05), but that of $0.1\%$ or $0.4\%$ LR treatments did not significantly differ from that of control. Egg weight was significantly higher in LR feeding group than the control (P<0.05). Daily egg mass was significantly higher in $0.2\%$ and $0.4\%$ LR treatments compared to the control and $0.1\%$ LR (P<0.05). The number of jumbo and extra large eggs were increased in LR supplemented groups, especially in $0.1\%$ LR. Feed intake of layers fed LR supplemented diets tended to be lower than the control. However, feed conversion ratio significantly improved in LR supplemented groups (P<0.05). Availability of dry matter and crude protein improved significantly in $0.4\%$ LR treatment (P<0.05). But, those of ether extract and crude ash were not significantly different among treatments. Eggshell breaking strength and eggshell thickness were not significantly influenced by LR supplementation, and Haugh unit and yolk index were also similar to the control. Total number of Lactobacillus spp. in ileum and cecum fed LR supplemented diets were significantly higher than those of the control (P<0.05). There were no significant differences in intestinal E. coli and Salmonella in all treatments. Therefore, it is concluded that dietary supplementation of Lactobacillus reuteri culture can improve the laying performance, feed efficiency and intestinal Lactobacillus.

THE COMPARATIVE EVALUATION OF FILM-SCREEN COMBINATIONS (증감지와 필름에 따른 방사선상 변화에 대한 연구)

  • This study was to compare the quality of image by different screen and film combinations. Using the sensitometer measured the speed and average gradient of blue sensitive films and orthochromatic films. The films was combined with rare earth screen LR, LM, LF and conventional screen OM, OH, XOR, OKa and exposed the step wedge to impulse 2, 3, 4, 6, 10, 15, 24,3 8, 60 and measured the density. The following results were obtained: 1. The density of film and film-screen combinations showed significant difference, then in film-screen combinations was significantly different by the screens than films. 2. The speed of blue sensitive films was little different, the TMG of orthochromatic films produced high speed, and the AX film was high average gradient. 3. The relative speed of film-screen combinations showed significant difference, and was high in the OKa of the conventional screens and in the LR of the rare earth screens, especially that of LR screen in the combination with blue sensitive films was high. 4. The average gradient of film-screen combinations showed no significant difference, and was high in the OKa screen and LR/OG combination, and that of OKa/AX combination was highest. 5. The latitude of film-screen combinations showed significant difference by screens, and was high in the LM screen in combination with blue sensitive films and in the OM screen in combination with orthochromatic films. 6. The subject contrast of film-screen combinations showed significant difference by screen, and was high in the LR screen in combination with blue sensitive AX film and orthochromatic TMG film.

A Large Scale Separation & Purification of Cyanobacterial Toxins (남조류 독성물질의 대량분리 및 정제)

  • It is very difficult to separate and purify the microcystins, cyanobacterial toxins since it exist in a trace level in natural lakes. In this paper, we developed a new analytical method to separate and purify the microcystin RR and LR from the freeze-dried cyanobacterial cells in natural lakes. We used 7.5 g silica gel as a stationary phase and ethyl acetate: isopropanol: water (30: 45: 25) as a mobile phase and microcystins were eluted using an open column. The eluting solvent was collected in a small bottle at the intervals of 3 mL and the fractions were chromatographed with HPLC to confirm the microcystin RR and LR.

Study on characteristic for Larson's ratio of water treatment plants (국내 정수장의 Larson's ratio 특성에 관한 연구)

  • In many countries in order to manage corrosion of water treatment process, it is currently using Langelier index (LI). However, management of the Larson's ratio (LR) to compare corrosion management and LI which can be generated by the water treatment process is required. In this study, in order to ensure data LR, factors associated with the actual corrosion resistance of water treatment plant was measured. Using the measured data, the model equation can be estimated alkalinity, and using the statutory water quality data, LR and alkalinity is estimated. At comparison of the measured value and estimated value of alkalinity, it appeared in $R^2$ = 0.629, using the statutory water quality data and estimated alkalinity model, LR and alkalinity (Whole water treatment plants : 472) is estimated. Concentration of estimated alkalinity is 0.5 mg/L to 107.5 mg/L (average : 23.2 mg/L), and LR is 0.1 to 10 (average : 1.3). At tendency to corrosion of investigated LR, "No metal tendency" (>0.5) is 39 water treatment plants, 8.26 %, and "corrosion metal tendency" is 433 water treatment plants, 91.74%.

Security Proof for a Leakage-Resilient Authenticated Key Establishment Protocol

  • At Asiacrypt 2003, Shin et al., have proposed a new class for Authenticated Key Establishment (AKE) protocol named Leakage-Resilient AKE ${(LR-AKE)}^{[1]}$. The authenticity of LR-AKE is based on a user's password and his/her stored secrets in both client side and server side. In their LR-AKE protocol, no TRM(Tamper Resistant Modules) is required and leakage of the stored secrets from $.$my side does not reveal my critical information on the password. This property is useful when the following situation is considered :(1) Stored secrets may leak out ;(2) A user communicates with a lot of servers ;(3) A user remembers only one password. The other AKE protocols, such as SSL/TLS and SSH (based or PKI), Password-Authenticated Key Exchange (PAKE) and Threshold-PAKE (T-PAKE), do not satisfy that property under the above-mentioned situation since their stored secrets (or, verification data on password) in either the client or the servers contain enough information to succeed in retrieving the relatively short password with off-line exhaustive search. As of now, the LR-AKE protocol is the currently horn solution. In this paper, we prove its security of the LR-AKE protocol in the standard model. Our security analysis shows that the LR-AKE Protocol is provably secure under the assumptions that DDH (Decisional Diffie-Hellman) problem is hard and MACs are selectively unforgeable against partially chosen message attacks (which is a weaker notion than being existentially unforgeable against chosen message attacks).

Applications of the ANFIS and LR in the prediction of strain in tie section of concrete deep beams

  • Recent developments in Artificial Intelligence (AI) and computational intelligence have made it viable in the construction industry and structural analysis. This study usesthe Adaptive Network-based Fuzzy Inference System (ANFIS) as a modelling tool to predict the strain in tie section for High Strength Self Compacting Concrete (HSSCC) deep beams. 3773 experimental data were collected. The input data andits corresponding strains in tie section as output data were recorded at all loading stages. Results from ANFIS are compared with the classical linear regression (LR). The comparison shows that the ANFIS's results are highly accurate, precise and satisfactory.
