Extracorporeal Membrane Oxygenation in a 1,360-g Premature Neonate after Repairing Total Anomalous Pulmonary Venous Return

  • With advancements in complex repairs in neonates with complicated congenital heart diseases, extracorporeal membrane oxygenation (ECMO) has been increasingly used as cardiac support. ECMO has also been increasingly used for low birth weight (LBW) or very low birth weight (VLBW) neonates. However, since prematurity and LBW are risk factors for ECMO, the appropriate indications for neonates with LBW, especially VLBW, are under dispute. We report a case of ECMO performed in a 1,360-g premature infant with VLBW due to cardiopulmonary bypass weaning failure after repairing infracardiac total anomalous pulmonary venous return.

The Variation of Winter Buds among 10 Selected Populations of Kalopanax septemlobus Koidz. in Korea

  • The objective of this study was to understand the conservation of gene resources and provide information for mass selection' of winter bud characters among the selected populations of Kalopanax septemlobus Koidz using analysis of variance(ANOVA) tests. The obtained results are shown below; 1. Ten populations of K. septemlobus were selected for the study of the variation of winter bud characters in Korea. The results of the analysis of variance(ANOVA) tests shows that there were statistically significant differences in all of the winter bud characters among those populations. 2. Correlation analysis shows that width between Height and DBH(Diameter at breast height) characters have negative relationship with all of the characters, as ABL(Apical branch length), ABW(Apical branch width), AWBL(Apical branch winter bud length), AWBW(Apical branch winter bud width), ABT(Apical branch No. of thorns), ABLB(Apical branch No. of lateral bud) and LBL(Lateral branch length), LBW(Lateral branch width), LBT(Lateral branch No. of thorns), LBLB(Lateral branch No. of lateral bud). 3. The result of principal component analysis(PCA) for winter buds showed that the first principal components(PC' s) to the fourth principal component explains about 78% of the total variation. The first principal component(PC) was correlated with AWBW, LWBW, and LBL and the ratio of ABL/ABW and LBL/LBW out of 16 winter bud characters. The second principal component correlated with ABL, ABW, ABLB, LWBL(Lateral branch winter bud length), and LBW and the ratio of AWBL/AWBW. The third principal component correlated with ABL, ABW, LWBL, LBL, and the ratio of LBL/LBW. The fourth principal component correlated with LBL and the ratio of LWBL/LWBW(Lateral branch winter bud width), LBL/LBW. Therefore, these characters were important to analysis of the variation for winter bud characters among selected populations of K. septemlobus in Korea. 4. Cluster analysis using the average linkage method based on 10 selected populations for the 16 winter bud characters of K. septemlobus in Korea showed a clustering into two groups by level of distance 1.1(Fig. 3). As can be seen in Fig. 3, Group I consisted of three areas(Mt. Sori, Mt. Balwang and Mt. Worak) and Group Ⅱ contisted of seven areas(Suwon, Mt. Chuwang, Mt. Kyeryong, Mt. Kaji, Mt. Jiri, Muan, and Mt. Halla). The result of cluster analysis for winter bud characters corresponded well with principal component analysis, as is shown in Fig. 2.

복분자주 주박의 항균, 항산화 및 항혈전 활성 (Anti-microbial, Anti-oxidant, and Anti-thrombosis Activities of the Lees of Bokbunja Wine (Rubus coreanus Miquel))

  • 복분자는 당도가 낮고 신맛이 강해 과실주로 주로 제조되고 있으며, 복분자주 제조 후 부생되는 주박은 대부분 폐기되고 있다. 본 연구에서는 현재까지 생리활성이 보고된 바 없는 복분자주 주박을 대상으로 열수 추출물, ethanol 추출물 및 이들의 다양한 분획물을 조제하여 항균, 항산화 및 항혈전 활성을 평가하였다. 그 결과, 복분자주 주박의 ethylacetate (EA) 분획은 413~459 mg/g의 높은 polyphenol을 함유하고 있으며, 그람양성세균에 대한 강력한 성장억제 활성과, DPPH 음이온, ABTS 양이온에 대한 우수한 radical 소거능, nitrite 소거능 및 환원력을 나타내었다. 또한 혈액내의 thrombin, prothrombin, coagulation factor 에 대한 저해를 통한 혈액응고저해 활성이 우수하였다. 복분자주 주박의 butanol 분획은 우수한 항산화력과 함께 내인성 혈전 생성에 관련된 aPTT 연장활성을 나타내었으며, aspirin에 필적하는 강력한 혈소판 응집저해 활성을 나타내었다. 한편 복분자주 주박의 열수 추출물의 순차적 유기용매 분획 후의 물 잔류물은 aspirin보다 강력한 혈소판 응집저해를 나타내어 새로운 항혈소판제 개발 소재로 이용 가능함을 확인하였다. 본 연구결과는 폐기되고 있는 복분자주 주박을 이용한 고부가가치 식품 및 향장소재 개발이 가능함을 제시하고 있다.

Risk Factors for Preterm Birth and Low Birth Weight Among Pregnant Indian Women: A Hospital-based Prospective Study

  • Objectives: The present study was undertaken to study the maternal risk factors for preterm birth (PTB) and low birth weight (LBW) with a special emphasis on assessing the proportions of maternal genitourinary and periodontal infections among Indian women and their association with adverse pregnancy outcomes. Methods: A hospital-based prospective study comprising 790 pregnant women visiting the obstetrics clinic for a routine antenatal check-up was undertaken. Once recruited, all study participants underwent clinical and microbiological investigations for genitourinary infections followed by a dental check-up for the presence of periodontitis. The study participants were followed up until their delivery to record the pregnancy outcomes. Infectious and non-infectious risk factors for PTB and LBW were assessed using univariate and multivariate Cox regression analysis. Independent risk factors for PTB and LBW were reported in terms of adjusted relative risk (ARR) with the 95% confidence interval (CI). Results: Rates of PTB and LBW in the study population were 7.6% and 11.4%, respectively. Previous preterm delivery (ARR, 5.37; 95% CI, 1.5 to 19.1), periodontitis (ARR, 2.39; 95% CI, 1.1 to 4.9), Oligohydramnios (ARR, 5.23; 95% CI, 2.4 to 11.5), presence of Nugent's intermediate vaginal flora (ARR, 2.75; 95% CI, 1.4 to 5.1), gestational diabetes mellitus (ARR, 2.91; 95% CI, 1.0 to 8.3), and maternal height <1.50 m (ARR, 2.21; 95% CI, 1.1 to 4.1) were risk factors for PTB, while periodontitis (ARR, 3.38; 95% CI, 1.6 to 6.9), gestational hypertension (ARR, 3.70; 95% CI, 1.3 to 10.8), maternal height <1.50 m (ARR, 2.66; 95% CI, 1.3 to 5.1) and genital infection during later stages of pregnancy (ARR, 2.79; 95% CI, 1.2 to 6.1) were independent risk factors for LBW. Conclusions: Our study findings underscore the need to consider screening for potential genitourinary and periodontal infections during routine antenatal care in developing countries.

신생아 집중치료실에서 퇴원한 정상 체중아와 저출생 체중아의 재입원에 관한 비교 (Comparison of Rehospitalization during the First Year of Life in Normal and Low Birth Weight Infants Discharged from NICU)

  • 목 적 : 출생 후 첫 1년은 유병율과 사망율이 높은 시기이고, 특히 저출생 체중아가 차지하는 비율이 높아 신생아 중환자실에 입원했던 미숙아나 저출생 체중아에 대한 추적관찰이나 예후에 대한 연구가 많이 되어왔었다. 그러나 신생아 집중치료실에 입원한 환아의 약 반 수정도가 정상 체중아임에도 불구하고 정상 체중아에 대한 추적관찰은 국내뿐 아니라 국외에서도 거의 이루어지지 않았다. 이에 저자들은 신생아 집중 치료실에서 치료를 받고 퇴원한 정상 체중아와 저출생 체중아의 생후 1년 동안의 재입원율과 재입원 원인을 알아보고 재입원에 영향을 미친 주산기 인자들에 대하여 조사하였다. 방 법 : 2000년 1월 1일부터 12월 31일까지 1년간 전주 예수병원 신생아 집중치료실에서 치료 후 생존하여 퇴원한 2,500 gm 이하의 저출생 체중아 131명과 2,501 gm 이상인 정상 체중아 302명을 대상으로 입원병력지 및 외래 추적관찰 기록을 검토하였고, 또한 전화를 통하여 정보를 얻었다. 본원 입원 중 다른 병원으로 전원한 경우는 포함하지 않았고 또한 입원 중 사망한 환자도 포함하지 않았다. 저출생 체중아의 경우 다시 1,500 gm 미만군, 1,500-1,999 gm군, 2,000-2,500 gm 군으로 세분하였고, 정상 체중아군과 같이 재입원 빈도와 재입원 원인 질환, 재입원 당시의 연령을 비교하였다. 또한 저출생 체중아와 정상 체중아를 재입원군과 비입원군으로 나누어 아기의 성별, 재태주령, 인공호흡기 치료 유무 및 기간, 뇌실 내 출혈, 패혈증, 신생아 집중치료실에서의 입원기간, 부당 경량아, 선천성 기형, 산모의 연령과 교육 수준 등 여러 주산기 인자들과의 관계를 비교 분석하였다. 결 과 : 1) 정상 체중아의 21.2%(64/302)가 총 76회의 재입원(1.19회/명)을 하였고, 저출생 체중아는 23.0%(30/131)가 총 37회(1.23회/명)의 재입원을 하였으며, 두 군간의 재입원율이나 재입원 횟수에 있어서 유의한 차이를 보이지 않았다. 2) 재입원 원인으로 호흡기계 질환이 저출생 체중아군과 정상 체중아에서 각각 36.0%, 32.1%로 가장 많은 빈도를 차지하였으며, 그 외 원인으로 저출생 체중아에서는 위장관계 질환(26.0%), 요로감염(6.0%), 외과적 문제(6.0%), 심장질환(6.0%) 등이 있었고, 정상 체중아에서는 위장관계 질환(26.2%), 요로감염(11.9%), 외과적 문제(10.7%), 심장질환(7.1%), 선천성/발달장애(1.2%) 등의 순이었다. 3) 재입원시의 연령은 저출생 체중아와 정상 체중아 모두 1-3개월 사이에서 가장 많은 빈도(35.1%, 31.6%)를 차지하였으나 통계적 유의성은 없었다. 4) 산모의 연령과 교육 수준은 저출생 체중아와 정상 체중아 모두에서 재입원과 유의한 상관관계가 없었다(P>0.05). 5) NICU에서 퇴원한 생존 환아들에서 재입원에 유의한 영향을 미치는 주산기 인자들에는 인공호흡기 치료 유무, 6일 이상의 인공호흡기 치료, 선천성 기형의 존재 등이 있었고, 재태 연령, 출생체중, 성별, 출생시 부당 경량아, 뇌실 내 출혈, 패혈증, 신생아 집중 치료실 입원 기간 등은 의미있는 관계가 없었다. 정상 체중아에서도 동일한 결과를 보였으나 저출생 체중아에서는 재입원과 의의있게 연관된 주산기 인자는 없었다. 결 론 : 신생아 집중치료실에서 생존하여 퇴원한 환아들에서 정상 체중아나 저출생 체중아 모두 생후 1년동안 비슷한 재입원율을 보였으며, 정상 체중아에서는 재입원에 영향을 미치는 인자로 인공호흡기 치료 유무, 6일 이상의 인공호흡기 치료, 선천성 기형의 존재 등이 있었으나 저출생 체중아에서는 의의있게 연관된 주산기 인자가 없었다. 따라서 신생아 집중치료실에 입원하는 정상 체중아들도 퇴원 후 추적관찰이 중요할 것으로 사료된다.

Anti-Thrombosis Activity of Sinapic Acid Isolated from the Lees of Bokbunja Wine

  • From the lees of bokbunja wine (LBW) made from Rubus coreanus Miquel, we have identified six compounds (1: trans-4-hydroxycinnamic acid; 2: trans-4-hydroxy-3-methoxycinnamic acid; 3: 3,4-dihydroxycinnamic acid; 4: 4-hydroxy-3-methoxybenzoic acid; 5: 3,5-dimethoxy-4-hydroxybenzoic acid; and 6: 3,5-dimethoxy-4-hydroxycinnamic acid (sinapic acid)) through silica gel chromatography and UHPLC-MS. The compounds 1-6 showed strong anticoagulation and platelet aggregation inhibitory activities without hemolytic effect against human red blood cells. To date, this is the first report of the in vitro anti-thrombosis activity of sinapic acid. Our results suggest that different cinnamic and benzoic acid derivatives are closely linked to the anti-thrombosis activity of LBW, and sinapic acid could be developed as a promising anti-thrombosis agent.

  • This study is to compare the weldability of Zircaloy-4 end cap of nuclear fuel pin using by GTA and Nd :YAG LB. The welding parameters which affect bead width and penetration depth have been investigated. The effect of joint geometry of end cap for GTAW and LBW has been studied and optimum conditions of Zircaloy-4 endcap welding have been found. Microstructures and microhardness of GTA and LB welded zones have been also compared.

조사연료봉 봉단마개의 레이저용접기술 (Technology of the End Cap Laser Welding for Irradiation Fuel Rods)

  • Various welding methods such as Gas Tungsten Arc Welding(GTAW), magnetic force electrical resistance welding and Laser Beam Welding(LBW) are now available for end cap closure of nuclear fuel rods. Even though the resistance and GTA welding processes are widely used in manufacturing commercial fuel rods, they can not be recommended for the remote seal welding of fuel rods in the hot cell Facility due to the complexity of the electrode alignment, the difficulty in replacing parts in a remote manner and the large heat input for the thin sheath. Therefore, the Nd:YAG laser system using optical fiber transmission was selected for the end cap welding of irradiation fuel rods in the hot cell. The remote laser welding apparatus in the hot cell Facility was developed using a pulsed Nd:YAG laser of 500 watt average power with an optical fiber transmission. The weldment quality such as microstructure and mechanical strength was satisfactory. The optimum conditions of laser welding for encapsulating irradiation fuel rods in the hot cell were obtained.

  • This study is to investigate the optimum conditions of Nd:YAG laser beam welding for Zircaloy-4 end cap closure by optical fiber transmission. Laser welding parameters which affect the penetration depth and bead width were experimentally examined using the various beam radius by the beam quality analyzer, joint geometries of end cap and the laser parameters which mean pulse width, repetition rate and pulse energy. Also, an optimum welding speed and the effect of assistant gas with varying the flow rate of He were investigated. We found that the laser average power for the end cap welding will be 230W and rotation speed must not exceed 8 RPM, the best position of focus using optical fiber with 600.mu.m will be 2 to 3mm below the surface of the material.

Determination of Genetic Diversity Using 15 Simple Sequence Repeats Markers in Long Term Selected Japanese Quail Lines

  • Japanese quail is still used as a model for poultry research because of their usefulness as laying, meat, and laboratory animals. Microsatellite markers are the most widely used molecular markers, due to their relative ease of scoring and high levels of polymorphism. The objective of the research was to determine genetic diversity and population genetic structures of selected Japanese quail lines (high body weight 1 [HBW1], HBW2, low body weight [LBW], and layer [L]) throughout 15th generations and an unselected control (C). A total of 69 individuals from five quail lines were genotyped by fifteen microsatellite markers. When analyzed profiles of the markers the observed ($H_o$) and expected ($H_e$) heterozygosity ranged from 0.04 (GUJ0027) to 0.64 (GUJ0087) and 0.21 (GUJ0027) to 0.84 (GUJ0037), respectively. Also, $H_o$ and $H_e$ were separated from 0.30 (L and LBW) to 0.33 (C and HBW2) and from 0.52 (HBW2) to 0.58 (L and LBW), respectively. The mean polymorphic information content (PIC) ranged from 0.46 (HBW2) to 0.52 (L). Approximately half of the markers were informative ($PIC{\geq}0.50$). Genetic distances were calculated from 0.09 (HBW1 and HBW2) to 0.33 (C and L). Phylogenetic dendrogram showed that the quail lines were clearly defined by the microsatellite markers used here. Bayesian model-based clustering supported the results from the phylogenetic tree. These results reflect that the set of studied markers can be used effectively to capture the magnitude of genetic variability in selected Japanese quail lines. Also, to identify markers and alleles which are specific to the divergence lines, further generations of selection are required.
