  • Objectives This study was designed to investigate the correlation between the forward head posture and the spinal alignment. Methods We examined the whole spine x-rays of the 144 student sample. We measured the Craniovertebral angle (CVA), Cervical angle (CA), Thoracic kyphotic angle (TKA), lumbar lordosis angle (LLA) and Ferguson's angle (FA) of the students. We then analyzed the relationship between these angles. Results CVA had correlation with CA, but it was weak. There was significant correlation between CVA and TKA. There were no significant correlation among CVA, LLA and FA. Conclusions According to above results, there is a negative relationship between the CVA and the TKA - in that higher CVAs yielded lower TKAs. But CVA had no significant correlation with LLA or FA.

Long-term Follow-up Results of Short-segment Posterior Screw Fixation for Thoracolumbar Burst Fractures

  • Objective: Despite general agreement on the goals of surgical treatment in thoracolumbar burst fractures, considerable controversy exists regarding the choice of operative techniques. This study is to evaluate the efficacy of short-segment fixation for thoracolumbar burst fractures after long-term follow-up and to analyze the causes of treatment failures. Methods: 48 out of 60 patients who underwent short-segment fixation for thoracolumbar burst fractures between January 1999 and October 2002 were enrolled in this study. Their neurological status, radiological images, and hospital records were retrospectively reviewed. Simple radiographs were evaluated to calculate kyphotic angles and percentages of anterior body compression (%ABC). Results: The average kyphotic angles were $20.0^{\circ}$ preoperatively, $9.6^{\circ}$ postoperatively, and $13.1^{\circ}$ at the latest follow-up. The average %ABC were 47.3% preoperatively, 31.2% postoperatively, and 33.3% at the latest follow-up. The treatment failure, defined as correction loss by $10^{\circ}$ or more or implant failure, was detected in 6 patients (12.5%). 5 out of 6 patients had implant failures. 2 out of 5 patients were related with osteoporosis, and the other 2 were related with poor compliance of spinal bracing. 3 patients with poor initial postoperative alignment had implant failure. 4 patients with screws only on the adjacent vertebrae and not on the injured vertebra itself showed poor initial and overall correction. Conclusion: With proper patient selection, adequate intraoperative reduction with screw fixation involving the injured vertebra, and strict postoperative spinal bracing, the short-segment fixation is an efficient and safe method in the treatment of thoracolumbar burst fracture.

Bone Cement Augmentation of Short Segment Fixation for Unstable Burst Fracture in Severe Osteoporosis

  • Objective : The purpose of this study was to determine the efficacy of short segment fixation following postural reduction for the re-expansion and stabilization of unstable burst fractures in patients with osteoporosis. Methods : Twenty patients underwent short segment fixation following postural reduction using a soft roll at the involved vertebra in cases of severely collapsed vertebrae of more than half their original height. All patients had unstable burst fracture with canal compromise, but their motor power was intact. The surgical procedure included postural reduction for 2 days and bone cement-augmented pedicle screw fixations at one level above, one level below and the fractured level itself. Imaging and clinical findings, including the level of the vertebra involved, vertebral height restoration, injected cement volume, local kyphosis, clinical outcome and complications were analyzed. Results : The mean follow-up period was 15 months. The mean pain score (visual analogue scale) prior to surgery was 8.1, which decreased to 2.8 at 7 days after surgery. The kyphotic angle improved significantly from $21.6{\pm}5.8^{\circ}$ before surgery to $5.2{\pm}3.7^{\circ}$ after surgery. The fraction of the height of the vertebra increased from 35% and 40% to 70% in the anterior and middle portion. There were no signs of hardware pull-out, cement leakage into the spinal canal or aggravation of kyphotic deformities. Conclusion : In the management of unstable burst fracture in patients with severe osteoporosis, short segment pedicle screw fixation with bone cement augmentation following postural reduction can be used to reduce the total levels of pedicle screw fixation and to correct kyphotic deformities.

척추의 만곡과 족저부 압력 분포 및 발각도의 상관성 연구 - 족부 진단기의 임상적 활용 가능성 검토를 위한 예비연구 - (A Study of the Correlation between Spinal Curvatures, Plantar Pressure and Foot Angles)

  • Objectives : The purpose of this study was to identify spinal curvatures, plantar pressure and foot angles in a walking. Methods : 19 outpatients under 19 years old were included. Plantar pressure and foot angle in a walking were measured by using Gaitview AFA-50. Spinal curvatures were measured by using radiograph. Results : The cervical lordotic angle is significantly difference with left and right plantar pressure(p=0.027). The thoracic kyphotic angle is significantly difference with left and right plantar pressure(p=0.026). Cobb's angle is significantly difference with left and right plantar pressure(p=0.027). The other plantar pressure were no difference from spinal curvatures and foot angle in a walking. Conclusion : There were no correlation between plantar pressure, spinal curvatures and foot angle. We consider that needed more additional study.

The Effect of Coordinative Locomotor Training Using Elastic Bands on the Flexibility and Body Alignment of Elementary School Right-Handed Baseball Players

  • Purpose: This study sought to identify the effects of coordinative locomotor training (CLT) using elastic bands on the flexibility and body alignment of right-handed baseball players in elementary school. Methods: The subjects were 20 instructed right-handed baseball players in elementary school. They were classified into the experimental group with 10 players (n=10) to be given coordinative locomotor training using elastic bands and the control group with 10 players (n=10). Flexibility was measured by the sit and reach exercise. Body alignment was measured using formetric 4D method to measure the trunk imbalance angle (TI), trunk torsion angle (TT), pelvic tilt angle (PTi), pelvic torsion angle (PTo), pelvic rotation angle (PR), kyphotic angle (Ky), and lordotic angle (Lo). Results: There was a significant increase in the flexibility within the experimental group (p<0.05). after the intervention. There was also a significant increase in the difference between the experimental group and the control group (p<0.05). There was a significant decrease in PR during body alignment in the experimental group after intervention (p<0.05). An analysis of the differences between groups showed a significant decrease in the PR of the experimental group compared to the control group (p<0.05). Conclusion: These findings show that coordinative locomotor training using elastic bands would be effective for enhancing the flexibility and pelvic rotation of elementary school right-handed baseball players.

초등학교 야구선수들의 우세손에 따른 족저압과 신체 정렬간의 상관관계 분석 (Correlation Analysis between Plantar Pressure and Body Alignment According to the Dominant Hand of Elementary School Baseball Players)

  • PURPOSE: This study analyzed the correlation between the plantar pressure and body alignment according to the dominant hand of elementary school baseball players. METHODS: The subjects consisted of 32 elementary school baseball players. The 32 players were classified in the right-hand dominant group (Rt. Group, n = 22) and left-hand dominant group (Lt. group, n = 10). The plantar pressure was measured using a pedoscan to determine the maximum pressure and pressure distribution. The body alignment was measured using a fometric 4D to measure the trunk imbalance angle (TI), pelvic tilt angle (PTi), pelvic torsion angle (PTo), pelvis rotation angle (PR), kyphotic angle (Ky), lordotic angle (Lo), and trunk torsion angle (TT). RESULTS: Participants showed correlations in the Rt. group, according to the left maximum pressure, Lo (r = .592, p < .05), and TT (r = .514, p < .05); according to the right maximum pressure, PR (r = .539, p < .05), and Lo (r = .503, p < .05). In the left pressure distribution, the PR (r = -.521, p < .05) showed a negative correlation. In the Lt. group, the PT (r = -.591, p < .05) showed a negative correlation in the left pressure distribution. CONCLUSION: These results can be used as basic data for a body analysis study of elementary school baseball players in the future.

Postoperative Results of Kyphoplasty for Osteoporotic Vertebral Compression Fractures

  • Objective: We analyze pain relief, deformity correction and complication rate after percutaneous kyphoplasty for osteoporotic vertebral compression fractures. Methods: The authors retrospectively reviewed medical records and radiological findings of 32 patients who underwent percutaneous kyphoplasty for osteoporotic vertebral compression fractures. Results: The patients had significant pain improvement with the procedure. The visual analogue scale score reduced from 8.6 to 3.4 significantly after the procedure. The midline vertebral body height significantly increased postoperatively, but mean kyphotic angle did not. There was no serious complication except one case of epidural cement leakage without neurological impairment. Conclusion: Balloon kyphoplasty safely can reduce severe back pain and returned geriatric patients to higher activity levels. The midline vertebral height is restored significantly. However kyphotic deformity correction is not significant as contrary to what we expected from the present study before it was carried out.

고등학교 야구선수들의 신체정렬과 족저압 분포 간의 상관관계 분석 (Correlation between body alignment and foot pressure in high School Baseball Players)

  • 본 연구는 고등학교 야구선수를 대상으로 신체정렬과 족저압 분포 분석하여 올바른 자세지도와 스포츠 손상 및 부상 예방 프로그램에 참고 자료를 제공하는데 그 목적이 있다. 연구 대상자 32명을 대상으로 실시하였다. 신체정렬은 척추에서 몸통의 좌우기울기, 등뼈의 뒤굽음각, 허리뼈의 앞굽음각, 골반에서는 골반의 기울기, 골반의 뒤틀림을 측정하였고 족저압은 각 발의 좌우 체중분포, 각 발의 최대압력을 측정하였다. 측정항목에 대한 평균과 표준편차를 산출하여 도표화하였고, 신체정렬과 족저압 분포의 상관관계를 알아보기 위해 pearson's cerrelation analysis를 사용하여 분석하였다. 모든 통계학적 유의 수준은 0.05로 설정하였다. 신체정렬과 족저압 분포 간의 상관관계를 분석한 결과 척추에서 허리뼈의 앞굽음각과 좌우 최대압력에서 양의 상관관계가 나타났다. 몸통의 좌우기울기, 등뼈의 뒤굽음각, 골반의 기울기, 골반의 뒤틀림과 각 발의 좌우 체중분포, 각 발의 최대압력에서는 상관관계가 나타나지 않았다. 본 연구를 통해 허리뼈의 앞굽음각이 증가될수록 발의 한쪽에 힘이 더 실리게 되어 최대압력이 증가하는 양의 상관관계를 나타내는 것을 알 수 있었다. 향후 운동선수들의 신체정렬과 족저압 분포에 대한 운동프로그램의 참고 자료가 될 것으로 사료된다.

골다공증성 흉요추부 압박 골절에서의 보존적 치료 (Conservative treatment of Osteoporotic Compression Fractures in Thoracolumbar spine)

  • Backgrounds: To evaluate the effectiveness of conservative treatment in osteoporotic thoraco-lumbar compression fractures and to identify the factors influencing the progression of compression. Materials and Methods: From January 2003 to October 2004, Patients who were admitted to our hospital for osteoporotic thoraco-lumbar compression fractures were reviewed retrospectively with follow-up more than 12 months (ave. 14.6 months). With simple x-ray lateral view, we evaluate wedge compression ratio (WCR) and kyphotic angle (KA) at initial and final follow-up. We separate the patients into two groups baesed on the amount of progression of vertebral compression and evaluate a relation with BMD, vertebral fracture level, initial WCR, initial KA. All datas were statistically analyzed. Results: An average of T-score was -3.5 and the changes of KA between initial and final follow-up were average $3.5^{\circ}$. Compression of anterior column were progressed to 8.5%. The changes between initial and final WCR in Group I (N=24) was 17.8%, and Group II (N=18) was 3.3%. T-score in group I was -3.4 and group II was -3.8 (p=0.228). vertebral fracture level were 10 T12, 12 L1, 2 L2 in group I; 6 T12, 6 L1, 6 L2 in group II (p=0.156). Initail WCR was 0.74 in group I, 0.63 in group II, and there was statistical difference between two groups(p=0002). Initial kyphotic angle was $13.9^{\circ}$in group I, $16.2^{\circ}$in group II repectively (p=0.392). Conclusion: The conservative treatment with short-term bed rest and early embulation is effective and valuable method to patients who have osteoporotic thoraco-lumbar compression fractures. There was no statistical difference between two groups according to BMD, vertebral fracture level, KA. But in comparison with initail WCR between two groups, there was statistical difference. That means, in the case of small initial compression of anterior column, the progression of compression was bigger than else. In these patients, more strict use of appropriate brace and careful follow-up should be needed.

Analysis of Factors Contributing to Repeat Surgery in Multi-Segments Cervical Ossification of Posterior Longitudinal Ligament

  • Objective : Cervical ossification of the posterior longitudinal ligament (OPLL) can be treated via anterior or posterior approach, or both. The optimal approach depends on the characteristics of OPLL and cervical curvature. Although most patients can be successfully treated by a single surgery with the proper approach, renewed or newly developed neurological deterioration often requires repeat surgery. Methods : Twenty-seven patients with renewed or newly developed neurological deterioration requiring salvage surgery for multi-segment cervical OPLL were enrolled. Ten patients (group AP) underwent anterior approach, and 17 patients (group PA) underwent posterior approach at the initial surgery. Clinical and radiological data from initial and repeat surgeries were obtained and analyzed retrospectively. Results : The intervals between the initial and repeat surgeries were $102.80{\pm}60.08months$ (group AP) and $61.00{\pm}8.16months$ (group PA) (p<0.05). In group AP, the main OPLL lesions were removed during the initial surgery. There was a tendency that the site of main OPLL lesions causing renewed or newly developed neurological deterioration were different from that of the initial surgery (8/10, p<0.05). Repeat surgery was performed for progressed OPLL lesions at another segment as the main pathology. In group PA, the main OPLL lesions at the initial surgery continued as the main pathology for repeat surgery. Progression of kyphosis in the cervical curvature (Cobb's angle on C2-7 and segmental angle on the main OPLL lesion) was noted between the initial and repeat surgeries. Group PA showed more kyphotic cervical curvature compared to group AP at the time of repeat surgery (p<0.05). Conclusion : The reasons for repeat surgery depend on the type of initial surgery. The main factors leading to repeat surgery are progression of remnant OPLL at a different segment in group AP and kyphotic change of the cervical curvature in group PA.
