흰쥐 관절염 모델에서 족삼리와 대릉 배오 전침의 상호억제 효과 (Mutual Inhibitory Effects of Electroacupuncture Applied to Combination of ST36 and PC7 in the Rat Model of Arthritis)

  • 목적 : Carrageenan(CA)으로 유발시킨 슬관절염 동물 모델에서 족삼리와 대릉을 배오한 전침의 효과를 알아보기 위해 실험을 수행하였다. 두 개 이상의 경혈에 동시에 자침함으로써 상승작용을 일으키는 연구는 많이 보고되었으나, 본 연구에서는 배합한 경혈(족삼리와 대릉)에 의한 전침 진통 작용의 감쇄 효과가 발생할 수 있는지에 대해 알아보고자 하였다. 방법 : CA을 흰쥐의 슬관절강 내에 주입하여 염증성 통증을 유발시켰다. 실험동물을 임의로 4군으로 배정하였다; PC7 군: 양쪽 상지 대릉 전침 군, ST36 군: 양쪽 하지 족삼리 전침 군, ST36+PC7 군: 양쪽 상지 대릉과 양쪽 하지 족삼리 전침 군, CON: 무처치군. 통증 정도를 측정하기 위해서 weight bearing force의 변화를 측정하였고, 척수의 후각에 유도된 COX-2 발현을 측정하기 위해 면역조직화학법과 western blot 분석을 실시하였다. 결과 : ST36+PC7군은 ST36군과 비교하여, 행동실험에서는 weight bearing force가 유의하게 감소하였고, 면역조직화학법과 western blot 분석에서는 COX-2 발현이 상대적으로 유의하게 증가하였다. 결론 : 이상의 실험결과를 통하여 족삼리와 대릉을 배오한 전침 자극은 슬관절염 모델에서 족삼리 전침의 진통 효과를 감소시킴을 알 수 있었다.

아토피 피부염의 한약 치료에 대한 무작위대조군연구 중심의 연구 동향 - CNKI를 중심으로 (Research Trends of Randomized Controlled Trials on Herbal Medicine Treatment for Atopic Dermatitis-Focusing on CNKI)

  • The purpose of this study was to analyze the research trends of randomized controlled trials on herbal medicine treatment for atopic dermatitis in China for the last 5 years. We searched for randomized controlled trials with the intervention of herbal medicine for the treatment of atopic dermatitis in the CNKI (China National Knowledge Infrastructure) from January 2014 to December 2018. For the screening of the paper, we used '特应性皮炎' and '异位性皮炎' which mean atopic dermatitis and search was limited to three areas within Medicine & Public Health: Traditional Chinese Medicine, Traditional Chinese Medicinal Herbs, Combination of Traditional Chinese Medicine with Western Medicine. Among the 136 searched studies, we selected a total of 34 studies and analyzed a year of publication, subject characteristics, study design and intervention, prescribed herbal medicine and herbs, pattern identification, evaluation criteria, and outcomes. Longmu decoction (龍牡湯) and Polia Sclerotium (茯笭) was the most frequently prescribed medicine and herb. The most commonly used pattern identification was Blood deficiency and Wind-dryness (血虛風燥), and among them, the most frequently prescribed herb is Rehmanniae Radix (生地黃). In most studies using the total effectiveness and SCORAD index as an outcome measure, the herbal medicine treatment group showed statistically better results than the control group. As a result of the safety assessment, the herbal medicine treatment group was reported having significantly fewer side effects compared to the control group. Hence, it was confirmed that the intervention including herbal medicine had a significant effect on atopic dermatitis. This study would be able to provide the basis of clinical research on atopic dermatitis and applied to the treatment of atopic dermatitis.

Sp1-Induced SETDB1 Overexpression Transcriptionally Inhibits HPGD in a β-Catenin-Dependent Manner and Promotes the Proliferation and Metastasis of Gastric Cancer

  • Purpose: Gastric cancer (GC) has high morbidity and mortality, the cure rate of surgical treatment and drug chemotherapy is not ideal. Therefore, development of new treatment strategies is necessary. We aimed to identify the mechanism underlying Sp1 regulation of GC progression. Methods and Methods: The levels of Sp1, β-catenin, SET domain bifurcated 1 (SETDB1), and 15-hydroxyprostaglandin dehydrogenase (HPGD) were detected by quantitative reverse transcription polymerase chain reaction and western blot analysis. The targets of SETDB1 were predicted by AnimalTFDB, and dual-luciferase reporter assay was used for confirming the combination of Sp1, β-catenin, and SETDB1. HGC27 or AGS cells (1×106 cells/mouse) were injected into mice via the caudal vein for GC model establishment. The level of Ki67 was detected using immunohistochemistry, and hematoxylin and eosin staining was performed for evaluating tumor metastasis in mice with GC. Results: HPGD was inhibited, while the protein levels of Sp1, β-catenin, and SETDB1 were up-regulated in GC tissues and cell lines. HPGD overexpression or SETDB1 silencing inhibited the proliferation, invasion, and migration of GC cells, and Sp1 regulated the proliferation, invasion, and migration of GC cells in a β-catenin-dependent manner. Furthermore, HPGD served as a target of SETDB1, and it was negatively regulated by SETDB1; additionally, Sp1 and β-catenin bound to the SETDB1 promoter and negatively regulated HPGD expression. We proved that Sp1 regulated GC progression via the SETDB1/HPGD axis. Conclusions: Our findings revealed that Sp1 transcriptionally inhibited HPGD via SETDB1 in a β-catenin-dependent manner and promoted the proliferation and metastasis of GC cells.

Ginsenoside Re prevents 3-methyladenine-induced catagen phase acceleration by regulating Wnt/β-catenin signaling in human dermal papilla cells

  • Background: The human hair follicle undergoes cyclic phases-anagen, catagen, and telogen-throughout its lifetime. This cyclic transition has been studied as a target for treating hair loss. Recently, correlation between the inhibition of autophagy and acceleration of the catagen phase in human hair follicles was investigated. However, the role of autophagy in human dermal papilla cells (hDPCs), which is involved in the development and growth of hair follicles, is not known. We hypothesized that acceleration of hair catagen phase upon inhibition of autophagy is due to the downregulation of Wnt/β-catenin signaling in hDPCs, and that components of Panax ginseng extract can increase the autophagic flux in hDPCs. Methods: We generated an autophagy-inhibited condition using 3-methyladenine (3-MA), a specific autophagy inhibitor, and investigated the regulation of Wnt/β-catenin signaling using the luciferase reporter assay, qRT-PCR, and western blot analysis. In addition, cells were cotreated with ginsenoside Re and 3-MA and their roles in inhibiting autophagosome formation were investigated. Results: We found that the unstimulated anagen phase dermal papilla region expressed the autophagy marker, LC3. Transcription of Wnt-related genes and nuclear translocation of β-catenin were reduced after treatment of hDPCs with 3-MA. In addition, treatment with the combination of ginsenoside Re and 3-MA changed the Wnt activity and hair cycle by restoring autophagy. Conclusions: Our results suggest that autophagy inhibition in hDPCs accelerates the catagen phase by downregulating Wnt/β-catenin signaling. Furthermore, ginsenoside Re, which increased autophagy in hDPCs, could be useful for reducing hair loss caused by abnormal inhibition of autophagy.

Adipogenesis관련 유전자발현감소와 Cell Cycle Arrest를 통한 EGCG와 Glucosamine-6-Phosphate의 Anti-Obesity 효과 (Anti-obesity effect of EGCG and glucosamine-6-phosphate through decreased expression of genes related to adipogenesis and cell cycle arrest in 3T3-L1 adipocytes)

  • 널리 음용되고 있는 녹차의 EGCG과 우리나라 국민의 상당수가 복용하고 있는 건강기능성 식품 성분인 글루코사민은 이전의 연구들을 통해서 지방세포의 분화를 억제하는데 효과가 있다고 보고되어왔다. 이 두 물질의 병합처리로 기대되어지는 지방세포에서의 adipogenesis 및 지방축적감소에 대한 상승효과는 검증된 바 없으며, 효과에 대한 cell cycle 차원에서의 접근은 없었다. 본 연구 결과에서 EGCG와 Glucosamine 6-phosphate는 adipogenesis 전사인자인 $PPAR{\gamma}$, $C/EBP{\alpha}$, SREBP1에 대한 직접적인 발현 억제 뿐아니라, $PPAR{\gamma}$, $C/EBP{\alpha}$, SREBP1와 매개된 FAS, ACSL1, LPL과 같은 adipogenic target 유전자의 발현 감소를 통하여 지방세포의 분화와 지방세포 내 지방축적을 감소시키는 효과를 나타냈다. 그리고 HSL과 perilipin의 발현조절을 통해 부분적인 lipolytic effet도 나타냈다. 또한 지방세포의 분화가 개시되는데 있어 중요한 DNA의 remodeling 과정인 mitotic clonal expansion (MCE) 과정 중 G0/G1 phase 단계에서 cell cycle 정지 유도와 그로인한 S phase 및 G2/M phase로 세포주기이행의 방해를 통해 지방세포가 분화되는 것은 억제하였다. 이러한 효과들은 EGCG 농도가 높아질수록, 그리고 EGCG를 단독으로 처리한경우보다 Glucosamine 6-phosphate와 병합하였을 때 효과적이었다. 따라서 EGCG 단독처리 및 glucosamine 6-phosphate와의 병합처리는 지방세포에서 adipogenesis와 adipogenic관련 유전자들의 발현 억제 및 MCE 단계의 cell cycle arrest를 통해 지방세포의 분화를 억제하고 지방축적을 감소시켜 항비만 효과를 나타냈으며, 이러한 효과는 두 성분의 병합처리에서 조금 더 효과적이었다고 할 수 있다. 비록 두 성분의 병합처리가 기대했던 만큼은 아니었으나 항비만 효과에 대한 상승효과가 있다고 볼 수 있다.

골육종세포에 미치는 레이저 조사의 광독성 효과 (PHOTOTOXIC EFFECTS OF LOW LEVEL LASER IRRADIATION ON HUMAN OSTEOSARCOMA CELLS)

  • 광역동치료(PDT)는 갖가지 고형 종양의 치료를 위한 임상적시도로 시작되어 화학요법 및 방사선 요법에 내성이 있는 종양의 대체 치료법의 하나로 제시되고 있다. PDT는 전신적 또는 국소적으로 투여하여 유해한 조직에 선택적으로 농축되도록 한 광증감제와 이를 활성화하는 적절한 파장과 에너지를 가진 레이저광의 조합에 기초한다. 이 연구에서는, 인체 골육종 세포(HOS)에 미치는 레이저 EIT 21의 광독성 효과에 대해 알아보았으며, 그런 광독성 효과가 세포고사를 유발하는가를 규명하고자 하였다. 본 연구는 레이저 EIT 21이 HOS 세포에 대해 광독성을 효과를 가진다는 것을 증명했다. 세포 죽음이 세포괴사에 의해 유발되는지 아니면 세포고사에 의해 유발되는지를 알아보기 위해 세포 고사를 평가 하는 여러 실험 기법을 이용하였다. TUNEL 분석은 극소수만이 응축된 핵의 양성반응을 보여주었다. Hemacolor와 AO/EB 염색 또한 대부분의 세포가 괴사로 죽는 것을 보여주었다. 레이저 EIT로 조사된 HOS 세포에서 응축되거나 분절된 핵을 발견하는 것은 어려웠다. DNA 전기영동에서, 세포고사에서 보여지는 DNA 분절의 전형적인 특징인 사다리형 절편 형태(ladder fragmentation pattern)가 나타나지 않았다. Western blotting에 의한 분석에서 p53의 발현은 일정하게 나타났고 레이저로 조사된 세포는 caspase-3과 PARP의 분열을 나타내지 않는 것으로 보아 레이저 유도 세포 죽음(laser-induced cell death)은 p53과는 관련이 없는 것 같다.

한국적 의학 기준에 근거한 고혈압환자의 Angiotensin II Receptor Blockers와 Calcium Channel Blockers의 약물 평가 (Evaluation of Therapeutic Differences of Angiotensin II Receptor Blockers and Calcium Channel Blockers Among Hypertensive Patients Classified by Oriental Traditional Way)

  • Background: Oriental lifestyle for treating diseases has been developed and well-accepted for a long time among Koreans. Sasang Constitution theory, originated from Korean traditional medicine, suggests that medication treatment should be differentiated by each patient's body classification (So-yang [SY], So-eum [SE], Tae-yang [TY], and Tae-eum [TE]), in contrary to western medicine's theory that medication should be applied equally by disease indication without such classification. However, the pharmacotherapeutic outcomes of these theories have not been compared to date. In this study, we aimed to compare the two theories by evaluating blood pressure (BP), which is lowered as a therapeutic outcome, among hypertensive patients taking angiotensin II receptor blockers (ARBs) or calcium channel blockers (CCBs), two most commonly used antihypertensive classes in Korea. Methods: From April 2006 to June 2012, we retrospectively collected data on hypertensive patients with Sasang Constitution classification at Kyunghee University Hospital at Gangdong, one of the East-West collaborative medical centers in Korea. We collected information on age, gender, underlying diseases, antihypertensive drugs (ARB, CCB, ARB+CCB), and BP by reviewing the electronic medical records. We excluded patients with missing blood pressure at baseline or follow-up, or those who had a change in their antihypertensive drug class during follow-up. Results: We selected a total of 573 patients (SY: 165, SE: 158, TY: 0, TE: 250). Baseline BPs were on average 139.0/82.0 mmHg for SY, 137.8/78.5 mmHg for SE, and 138.7/79.2 mmHg for TE. In all three groups, CCBs were the most prescribed, followed by combination therapy with ARB+CCB, then ARBs. BP reduction after 1 month of initial medication was significantly different among the drug classes, but not in Sasang constitutional classification (ARB [SY: -12.4/-4.7, SE: -12.3/-2.5, TE: -8.6/-1.8], CCB [SY: -12.3/-5.4, SE: -13.0/-2.3, TE: -10.8/-6.0], ARB+CCB [SY: -15.6/-6.7, SE: -18.4/-8.1, TE: -20.2/-6.7], drug [$P{\leq}0.05$/P>0.05], constitutional type [P>0.05/P>0.05]). Conclusion: We observed significant differences in reduction of blood pressure by classes of drugs (ARB+CCB>CCB>ARB) but not by Sasang constitutional classification. Therefore, current approach of antihypertensive pharmacotherapy assisted by Western medicine is appropriate for treatment of hypertension. However, further larger scale or prospective studies are required in order to confirm these results.

Paclitaxel, Lenvatinib 및 방사선 병용 요법의 역형성 갑상선암에서의 항암 작용 (Anti-cancer Activity of Paclitaxel, Lenvatinib and Radiation Combination Therapy on Anaplastic Thyroid Cancer in Vitro and in Vivo)

  • Background/Objectives: Although anaplastic thyroid carcinoma (ATC) is rare, it is one of the deadliest forms of thyroid cancer. The fatality rate for ATC is high, and the survival rate at one year after diagnosis is <20%. The present study aimed to investigate the anti-tumor activities of paclitaxel, radiation, and tyrosine kinase inhibitor (TKI) combined therapy in anaplastic thyroid cancer cells both in vitro and in vivo and explore its effects on apoptotic cell death pathways. Materials & Methods: ATC cell line was exposed to TKI, lenvatinib in the presence or absence of paclitaxel with radiation, and cell viability was determined by MTT assay. Effects of the combined treatment on cell cycle and intracellular signaling pathways were assessed by flow cytometry and western blot analysis. The ATC cell line xenograft model was used to examine the anti-tumor activity in vivo. Results: Our data revealed that the combined administration of paclitaxel, TKI, and radiation decreased cell viability in ATC cells, and also significantly increased apoptotic cell death in these cells, as demonstrated by the cleavage of caspase-3 and DNA fragmentation. This combination therapy reduced anti-apoptotic factor levels in ATC cells, while significantly decreasing tumor volume and increasing survival in ATC xenografts. Conclusion: These results indicate that administering the combination of paclitaxel, TKI, and radiation therapy may exert significant anticancer effects in preclinical models, potentially suggesting a new clinical approach for treating patients with ATC.

여성 난임 한의표준임상진료지침 개작을 위한 한의사의 인식과 치료에 관한 실태조사 (A Survey on Korean Medicine Doctors' Recognition and Treatment for Developing Korean Medicine Clinical Practice Guideline of Female Infertility)

  • Objectives: This study was conducted to develop a Korean Medicine(KM) Clinical Practice Guidelines(CPG) of female infertility. We conducted this questionnaire survey to reflect the experiences of Korean Medicine doctors(KMD) and clinical field in Korea. Method: We sent a questionnaire survey to KMD belonging to the Association of Korean Medicine by e-mail. We received 665 responds, and analyzed the answers. Results: 51.2% of respondents knew the previously developed CPG of female infertility, and 18.3% actually used. 83.3% agreed about the necessity of CPG of female infertility, and 80.3% had practical use plan. 90.2% of respondents treated less than 5 infertility woman for a month. 22.7% of respondents treated 50% of patients with collaborative treatment of KM and Western medical treatments. The main age group of patients was '35~40 years'(54.7%), and the most common cause was unexplained infertility(61.7%). The most common pattern identification of female infertility patients was Kidney deficiency(55.4%). KMD used 'a combination of decoction of herbal medicine, acupuncture and moxibustion treatment' the most(43%), and 'a decoction of herbal medicine treatment alone' was next(35%). 84.2% conducted lifestyle modification education about diet, stress, exercise etc. Conclusion: We figured out Korean Medicine doctors' recognition about CPG of female infertility, preference of treatments, and also characteristics of patients visiting Korean Medical clinics to make a practical CPG reflecting clinical situation.

만응환(萬應丸) 에탄올 추출물의 메티실린 내성 포도상구균에 대한 항균활성 및 내성억제 효과 (Antibacterial Activity and Inhibition of Resistance in Methicillin-resistant Staphylococcus aureus by Maneung-hwan Ethanol Extract)

  • Objectives In this study, we investigated the antimicrobial activity of a 70% ethanol extract of Maneung-hwan (MEH), which is prescribed by practitioners of oriental medicine for use against methicillin-resistant Staphylococcus aureus (MRSA). Methods The antibacterial activity of MEH against MRSA strains was evaluated using the disc diffusion method, broth microdilution method (minimal inhibitory concentration, MIC), checkerboard dilution test, and time-kill test. The mechanism of action of MEH was investigated by bacteriolysis using detergents or ATPase inhibitors Additionally, mRNA and protein expression were investigated by quantitative reverse transcription-polymerase chain reaction and western blot assay, respectively. Results The MIC of MEH was 25~1,600 ㎍/mL against all the tested bacterial strains. We showed that MEH extract exerts strong antibacterial activity. In the checkerboard dilution test, the fractional inhibitory concentration index of MEH in combination with antibiotics indicated synergism or partial synergism against S. aureus. The time-kill study indicated that the growth of the tested bacteria was considerably inhibited after a 24-h treatment with MEH and selected antibiotics. To measure the cell membrane permeability, MEH (3.9 ㎍/mL) was combined with Triton X-100 (TX) at various concentrations N,N-dicyclohexylcarbodimide (DCCD) was also tested as an ATPase inhibitor. TX and DCCD cooperation against S. aureus exhibited synergistic action. Accordingly, the antimicrobial activity of MEH in the context of cell membrane rupture and ATPase inhibition was assessed. Additionally, the expression of genes and proteins associated with resistance was reduced after exposing MRSA to MEH. Conclusions These results suggest that MEH possesses antibacterial activity and acts as a potential natural antibiotic against MRSA.
