고령운전자 교통사고의 심리적 요인 (Psychological effects on elderly driver's traffic accidents)

  • 고령화가 다른 국가에 비해 상대적으로 빠르게 진행되고 있는 상황에 대비한 고령운전자의 연구가 부족한 실정이다. 본 연구는 고령운전자의 교통사고 원인을 신체적 능력저하보다도 심리적 요인의 변화에 입각하여 고찰해 보려고 한다. - 고령운전자의 교통사고 고령운전자의 교통사고에서 가장 두드러지게 나타나는 행동적 특징은 좌회전시 발생하는 교통사고가 많다는 것이다. 그리고 사고장소에 따른 분석결과에 의하면, 교차로에서 발생하는 사고가 많다는 것이 확인되었다. - 고령운전자의 조심성과 보상행동 고령운전자의 조심성이 운전행동에 미치는 알아보기 위하여 운전자의 운전확신수준을 비교해 보았다 운전확신수준은 4개 요인으로 구분되었고, 고령운전자와 젊은 운전자의 운전확신수준 차이를 검증한 결과를 보면, 고령운전자의 운전확신수준이 젊은 운전자에 비해 현저하게 떨어지는 것을 알 수 있었다 그리고 좌회전을 선택시 소요되는 시간을 분석하여 고령운전자의 조심성을 이해하려고 하였다 고령운전자는 젊은 운전자에 비해 선택소요시간이 현저하게 길어진다는 사실을 발견하였다. - 고령운전자의 운전일탈행동 연령에 따른 운전일탈행동의 변화를 보면, 연령이 증가함에 따라 운전일탈행동의 위반, 오류, 착오의 평균 점수는 감소하는데, 각 요인이 감소하는 정도는 차이가 있었다. 위반점수는 연령의 증가에 따라 급격하게 감소하는 모습을 보이는데 비해 오류와 착오점수는 연령의 증가에 따라 완만하게 감소하는 경향을 보였다.

Ultrasound imaging for age-related differences of lower extremity muscle architecture

  • Objective: To investigate and compare the size of the rectus femoris (RF), tibialis anterior (TA), and medial gastrocnemius (GMM) using ultrasound (US) imaging in young, elderly, and very elderly groups. Design: Cross sectional study. Methods: This study consisted of 25 young (age 20 years), 24 elderly (age 65-74 years), and 25 very elderly (age 75-90 years) people with no physical dysfunctions. The cross sectional area (CSAs) of the RF and muscle thickness of the TA and GMM were measured at rest and during contraction using an US system. Results: The CSA of the RF and thickness of the TA and GMM were significantly smaller in the elderly and very elderly groups than in the young group (p<0.05). There was a significant difference of the CSA of the RF at rest and during contraction between elderly and very elderly group (p<0.05). In the comparison of the TA and GMM thickness between elderly and very elderly groups, there were no significant differences except for the TA thickness during contraction. There was a significant difference in the percentage change in RF CSA among the three groups (p<0.05). Conclusions: Our results revealed loss of muscle mass in the RF, TA, and GMM in elderly and very elderly people (${\geq}65$ years old). In particular, the greatest age-related decline in muscle mass was observed for the RF. Furthermore, the CSA of the RF declined with aging in the very elderly groups (${\geq}75$ years old).

청년층과의 비교를 통한 노년 여성 발의 형태 (Characteristics of Elderly Women's Foot Shape Compared with Young Women)

  • This study was attempted to clarify the characteristics of elderly women's foot shape by examining the various measurements of women over 60 years old as compared with those of women in twenties considered as reference group. The subjects were 321 elderly women and 181 young women. Their right feet were measured directly and indirectly by using scanner and digital camera. The anthropometric measurements were composed of 62 items. They were measured during the months of July and August in 2001. As a result of comparing actual measurements of foot between the two groups, it was found that the elderly women tended to have shorter and lower foot than those among the young women. Among the elderly women, in addition, it was revealed that many deformities related to toes or legs led to the toes concentrated to the central axis of feet resulting in relatively steeper lateral angle of toe, and the heels and legs slanting forwards. As a result of comparing index values of foot between the groups, the elderly women's foot had the relatively wide width and low height in comparison with the length, showing more elliptical flattening than those of the young women. And, the width of the inside was wider than that of the outside, and the toe width was relatively short in comparison with the foot width resulting in severe deformities on the toes or the central axis of foot. Therefore, it was found that there were several deformities including flat foot or deformed toes among elderly women. It is expected that the characteristics of elderly women's foot shown in the results mentioned above will be used as the basic data in designing the footwear for elderly women.

고령자와 청장년층의 발바닥 압력분포에 따른 보행패턴 비교 분석 (Comparative Analysis on Gait Patterns of the Elderly and the Young Regarding to Foot Pressure)

  • The purpose of this study was to find the difference in gait patterns when elderly and young people walk by analyzing COP, Gait Line, Foot pressure pattern, and ensuring the original biomechanics technology of developing high performance footwear for the elderly. The subjects who took part in the test consist of 20 elderly people and 20 young people. The physical features of the elderly people that were recruited for the study are as below: 20 healthy male subjects(elderly people) with an average age of 75.43 yrs(S.D 6.46 yrs), weight of 68.10 kg(S.D 0.94 kg) and a height of 168.65 cm(S.D 1.47 cm). Foot pressure pattern data was collected using a EMED-AT system(Novel Gmbh, Germany) operating at the 50 Hz during walking. The results are as follow : COP route of the elderly leans to lateral compared to the young, and Gait Line from heel to toe is not clear and laterally curved. At the same time, a contact are aonthe midfoot is high compared to the young, and maximum force of the forefoot is low. As a result of analysis, in order to develop high performance footwear for the elderly, it is necessary to develop lasts and soles reflecting the elderly's gait patterns.

Lower Extremities' Joint Stability during the Elderly Woman's Walking

  • Objective: The aim of this study was to investigate the periodicity of the lower extremity joint flexion/extension angle to compare the local stability between young and elderly women during walking on a treadmill. Method: Eighteen young women (mean $age=21.2{\pm}1.6y$; mean $mass=57.1{\pm}6.1kg$; mean $height=1.61{\pm}0.04m$) and 18 elderly women (mean $age=66.4{\pm}1.2y$; mean $mass=55.4{\pm}8.3kg$; mean $height=1.56{\pm}0.04m$) participated in this study. Approximate entropy (ApEn) was used to determine the periodicity in the lower limb joint angles. Results: The ApEn values of the two groups were statistically greater in the surrogate data test than in the original time series data (p<.05). The periodicity of the hip and ankle flexion/extension angles decreased in the elderly women group compared with the young women group (p<.05). The periodicity of the lower extremity joint flexion/extension angle showed that the ankle joint increased dominatingly in both groups (p<.05); the hip joint decreased compared with the knee joint in the young women group; and the knee joint decreased compared with the hip joint in the elderly women group (p<.05). Conclusion: These results suggest that the lower extremity joint flexion/extension angles of the young and elderly women during walking contained random noises as well as biological signals. In addition, the differences in the periodicity in the lower extremity joint between the young and elderly women may provide some insight in predicting potential falls and be used as a characteristic indicator for determining local stability in elderly women during walking.

  • As in our $20^{th}$ century, the dramatic advance of the science and medical technologies has led the advance of living standards. Even though the elderly people are increasing, the underwear for elderly women is not so various. As getting older, women's body shape is changing. But the most of underwear in the market are designed for the twenties whose body shape and proportion is generally stable. Besides, as the color and material of underwear are designed for young women, the elderly women have a poor choice of the underwear. The results of this study are listed in the following: (a) analyzing the current status of the underwear market for elderly women (b) searching what elderly take inconvenience in underwear, and (c) wearing test for search what elderly take inconvenience in brassier.

노인과 청장년의 족근관절 골절에 대한 임상적 비교 (Clinical Comparison of Ankle Fractures Between the Young and the Elderly)

  • Purpose: We compared the clinical features of the ankle fractures treated by operation between the elderly and the young, and reviewed the principles of treatment of the ankle fractures in the elderly. Materials and Methods: We reviewed 49 cases of the ankle fractures, which were treated by open reduction from August 1991 to July 2002. Patients aged more than 60 were designated as the elderly, and patients aged between 15 and 33 were designated as the young. The average follow-up period was 13.2 months. Using the Lauge-Hansen classification, We defined stage I or II fractures as low stage and stage III or IV fractures as high stage fractures. Results: There were 15 cases of high stage fractures (78.9%) in the elderly and 11 cases (36.7%) in the young. High stage fracture rate was significantly higher in the elderly (P=0.004). Hospital day, period between primary injury and operation, and union time were significantly longer in the elderly (P<0.001). However, there was no statistical difference in immobilization time between the two groups. The results of treatment were satisfactory clinically and radiologically by the Meyer's criteria in both groups. Conclusion: In the elderly, high stage fractures were more common and longer hospitalization and union time were needed than the young. However, the result of surgical treatment was satisfactory.

Electroglottographic Measurements of Glottal Function in Voice according to Gender and Age

  • Electroglottography (EGG) is a common method for providing non-invasive measurements of glottal activity. EGG has been used in vocal pathology as a clinical or research tool to measure vocal fold contact. This paper presents the results of pitch, jitter, and closed quotient (CQ) measurements in electroglottographic signals of young (mean = 22.7 years) and elderly (mean = 74.3 years) male and female subjects. The sustained corner vowels /i/, /a/, and /u/ were measured at around 70 dB SPL since the most notable among EGG variables is the phonation intensity, which showed positive correlation with closed phase. The aim of this paper was to measure EGG data according to age and gender. In CQ, there was a significant difference between young and elderly female subjects while there was no significant difference between young and elderly male subjects. The mean value for young males was higher than that for elderly males while the mean value for young females was lower than that for elderly females. Thus, it can be said that in mean values, increased CQ was related to decreased age for females, while CQ decreased for males as the speaker's age decreased. Although the laryngeal degeneration due to increased age seems to occur to a lesser extent in females, the significant increase of CQ in elderly female voices could not be explained in terms of age-related physiological changes. In standard deviation of pitch and jitter, the mean values for young and elderly males were higher than that for young and elderly females. That is, male subjects showed higher in mean values of voice variables than female subjects. This result could be considered as a sign of vocal instability in males. It was suggested that these results may provide powerful insights into the control and regulation of normal phonation and into the detection and characterization of pathology.

고혈압을 가진 저소득층 여성노인의 자가간호 행위와 우울증상 -초기노인과 후기노인의 비교- (Self-Care Behaviors and Depressive Symptoms of Low-Income Elderly Women with Hypertension)

  • Purpose: This study aimed to compare self-care behaviors and depressive symptoms between the young old (65-74 yr) and the old-old (75-84 yr) in low-income women with hypertension. Methods: This study used a descriptive research design. The subjects of this study were 136 elderly women over 65 yr living in D city. Data was collected from September to December 2007 through personal interviews using a questionnaire. The collected data was analyzed using the SPSS WIN 12.0 Program. Results: Self-care behaviors, functional status, and number of medications showed a significant difference between young elderly and middle elderly. There was a negative correlation between self-care behaviors and depressive symptoms. 52.2% of variance in self-care behaviors of young elderly and 76.8% of variance in self-care behaviors of middle elderly were explained by depressive symptoms, number of medications, and functional status. Conclusion: It is necessary to manage depressive symptoms to improve the self-care behaviors of low-income elderly with hypertension. Depressive symptoms need to be considered in planning hypertension programs for low-income elderly women.

서울시내 일개 복지관에 내원하는 노인의 대사증후군 발생양상과 관련요인에 대한 연구 (The Metabolic Syndrome and Associated Lifestyle Factors among Older Adults)

  • Purpose: The purpose of the present study was to investigate the prevalence and lifestyle factors related to the metabolic syndrome in Korean elderly. Methods: A total number of 122 elderly over 60 yr were recruited from a community elderly center. Elderly were classified into young old (60-74 yr) and old (75-84 yr). Metabolic syndrome was defined by third report of the national cholesterol education program (NCEP) expert panel on Adult Treatment Panel III (ATP III). Abdominal obesity was determined by Korean society for the study of Obesity criteria. The nutritional status was measured by mini nutritional assessment (MNA). Results: The prevalence of the metabolic syndrome was significantly higher in the old than young old elderly (p<0.05, respectively). Abdominal obesity and hypertension were the most common risk factors. The related factors to metabolic syndrome were high Body mass Index (BMI) and currently not participating in exercise. Abdominal obesity, low HDL cholesterol and hypertension were associated with specific nutrient intakes. Conclusion: A strategy to decrease the metabolic syndrome in the elderly is urgently needed particularly so for the old elderly. A nutritional and exercise program for young old elders may be helpful reducing the prevalence of metabolic syndrome in later stage.
