과도한 치아 마모와 교합평면 붕괴를 보이는 환자에서 상악 고정성 보철 및 하악 임플란트 보조 국소의치를 통한 구강회복 증례 (A case of oral rehabilitation in a patient with severe tooth wear and occlusal plane collapse, utilizing maxillary fixed prosthesis and mandibular implant-assisted removable partial denture)

  • 다수의 구치를 상실한 환자에서는 전방부 치아들의 마모나 변위, 교합평면의 붕괴와 교합수직고경의 감소가 일어날 수 있다. 특히 편측으로 소수의 치아만 가진 경우 국소의치는 생역학적으로 불리하므로, 소수의 임플란트 서베이드 크라운을 추가한 임플란트 보조 국소의치가 대안이 될 수 있다. 본 증례의 환자는 하악 우측 구치부 상실로 인해 전방부 치아들이 심하게 마모되고, 교합평면이 붕괴되어 있었다. 최소한의 교합 수직고경 거상과 함께 상악 고정성 보철 및 하악 임플란트 보조 국소의치로 수복을 진행하였고, 기능적 및 심미적으로 양호한 임상 결과를 얻을 수 있었다.

Clinical and biochemical outcomes of men undergoing radical prostatectomy or radiation therapy for localized prostate cancer

  • Purpose: We analyzed outcomes of patients with prostate cancer undergoing either radical retropubic prostatectomy (RRP) +/- salvage radiation or definitive radiation therapy (RT) +/- androgen deprivation. Materials and Methods: From 2003-2010 there were 251 patients who underwent RRP and 469 patients who received RT (${\geq}7,560cGy$) for prostate cancer. Kaplan-Meier analysis was performed with the log-rank test to compare biochemical control (bCR), distant metastatic-free survival (DMPFS), and prostate cancer-specific survival (PCSS) between the two groups. Results: The median follow-up was 70 months and 61.3% of the men were African American. For low risk disease the 6-year bCR were 90.3% for RT and 85.6% for RRP (p = 0.23) and the 6-year post-salvage bCR were 90.3% vs. 90.9%, respectively (p = 0.84). For intermediate risk disease the 6-year bCR were 82.6% for RT and 59.7% for RRP (p < 0.001) and 82.6% vs. 74.0%, respectively, after including those salvaged with RT (p = 0.06). For high risk disease, the 6-year bCR were 67.4% for RT and 41.3% for RRP (p < 0.001) and after including those salvaged with RT was 67.4% vs. 43.1%, respectively (p < 0.001). However, there were no significant differences between the two groups in regards to DMPFS or PCSS. Conclusion: Treatment approaches utilizing RRP +/- salvage radiation or RT +/- androgen deprivation yielded equivalent DMPFS and PCSS outcomes. Biochemical control rates, using their respective definitions, appeared equivalent or better in those who received treatment with RT.

A retrospective study on related factors affecting the survival rate of dental implants

  • PURPOSE. The aim of this retrospective study is to analyze the relationship between local factors and survival rate of dental implant which had been installed and restored in Seoul Veterans Hospital dental center for past 10 years. And when the relationship is found out, it could be helpful to predict the prognosis of dental implants. MATERIALS AND METHODS. A retrospective study of patients receiving root-shaped screw-type dental implants placed from January 2000 to December 2009 was conducted. 6385 implants were placed in 3755 patients. The following data were collected from the dental records and radiographs: patient's age, gender, implant type and surface, length, diameter, location of implant placement, bone quality, prosthesis type. The correlations between these data and survival rate were analyzed. Statistical analysis was performed with the use of Kaplan-Meier analysis, Chi-square test and odds ratio. RESULTS. In all, 6385 implants were placed in 3755 patients (3120 male, 635 female; mean age $65{\pm}10.58$ years). 108 implants failed and the cumulative survival rate was 96.33%. There were significant differences in age, implant type and surface, length, location and prosthesis type (P<.05). No significant differences were found in relation to the following factors: gender, diameter and bone quality (P>.05). CONCLUSION. Related factors such as age, implant type, length, location and prosthesis type had a significant effect on the implant survival.

IL-17 and IL-17C Signaling Protects the Intestinal Epithelium against Diisopropyl Fluorophosphate Exposure in an Acute Model of Gulf War Veterans' Illnesses

  • Gulf War Veterans' Illnesses (GWI) encompasses a broad range of unexplained symptomology specific to Veterans of the Persian Gulf War. Gastrointestinal (GI) distress is prominent in veterans with GWI and often presents as irritable bowel syndrome (IBS). Neurotoxins, including organophosphorus pesticides and sarin gas, are believed to have contributed to the development of GWI, at least in a subset of Veterans. However, the effects of such agents have not been extensively studied for their potential impact to GI disorders and immunological stability. Here we utilized an established murine model of GWI to investigate deleterious effects of diisopropyl fluorophosphate (DFP) exposure on the mucosal epithelium in vivo and in vitro. In vivo, acute DFP exposure negatively impacts the mucosal epithelium by reducing tight junction proteins and antimicrobial peptides as well as altering intestinal microbiome composition. Furthermore, DFP treatment reduced the expression of IL-17 in the colonic epithelium. Conversely, both IL-17 and IL-17C treatment could combat the negative effects of DFP and other cholinesterase inhibitors in murine intestinal organoid cells. Our findings demonstrate that acute exposure to DFP can result in rapid deterioration of mechanisms protecting the GI tract from disease. These results are relevant to suspected GWI exposures and could help explain the propensity for GI disorders in GWI Veterans.

파월국군장병의 고엽제 위해에 관한 예비적 역학조사 (A Preliminary Epidemiologic Study on Korean Veterans Exposed to Herbicides in Vietnam War)

  • Among chemical agents in herbicides, dioxin (2, 3, 7, 8-tetrachloro dibenzo-$\rho$-dioxin : TCDD), a chemical contaminant in herbicides sprayed during the Vietnam War has been known to be the major agent causing toxic effects. Approximately 320,000 korean soldiers participated the Vietnam War from 1964 to 1974. Although the potential hazards of the herbicides among Korean veterans exposed were implicated, the problem had not been a public issue until 1991 when Korean veterans were informed U.S. companies, the herbicides manufacturer payed fund, from which a trust fund for New Zealand and Australian Class members were established in 1985. After a series of appeals and demonstration by the Korean Veterans demanding medical care and compensation for their serious health damages, a bill of medical care and compensation for herbicides victims was promulgated in March 1993 and become effective from May 1993, This study was carried out with two major objectives : the first to understand the health problems caused from the herbicides by reviewing literatures published, and the second to examine the nature and extent of health impacts among Korean veterans exposed and to develop valid study methods for the major study by interviewing and reviewing records on a part of veterans (638 persons) registered and completed medical examination in Seoul Veterans Administration Hospital from June to October 1993. The results obtained are as followings: 1. The literature review of 107 papers revealed that 1) Dioxin is teratogenic, carcinogenic and affects almost all organs including nervous, endocrine, and reproductive systems in animal experiments. 2) The diseases showing evidence of causal association were Hodgkin's disease, non-Hodgkin's disease, lung cancer, lymphoma, soft tissue sarcoma, chloroacne and polyneuropathy when judged on the basis of consistency in study results and biological plausibility. 2. Interview and medical record review study on 638 veterans, though limited validity owing to lack of control group, crude estimates of dioxin exposure levels (no biomarkers measurable), and uncertainty of diagnosis, showed that: 1) Most of the study subject's were in their 40's of age and had been dispatched to Vietnam during the period from $1965{\sim}1970$ around one year. 2) Most frequently complained symptoms in medical examination were motor weakness (32%), sensory abnormalities in extremities (23%), skin diseases (22%), and pain in extremities (20%) whereas in Interview they were more frequent in order of skin problem (44%), motor weakness (38%), sensory abnormalities and pain in extremities(17% and 19% each). Kappa indices on the same category of complaints between two sources of information were variable and relatively low. 3) On medical examination, only a part of the 638 subjects had initial impression (442 pts) and final diagnosis (218 pts) suggesting decision making on diagnosis appeared to be difficult even with all available modern medical technologies: in initial impression disorders from peripheral and central neuropathy were predominant whereas in final diagnosis various types of skin disorder were most frequent 4) When dose-response relationship between several conditions (from questionnaire) and arbitrary exposure scores were examined by CMH linear trend test, spontaneous abortion, sexual problems and health problem of offsprings showed statistically significant linear trends. However, pregnancy, accident and suicidal attempts did not show any relationship in this study capacity. 5) Among complaints, psychosis and neurosis (anxiety, phobia) in interview study, and memory disorder and psychosis in medical record study revealed linear trend. 6) Skin disorder was the only condition showing linear trend in initial impression and none in final diagnosis on medical examination. Even though objective to select out dioxin-related disease or group of diseases from this study was not achieved the research experiences provided firm basis for developing various methodological approaches. 3. From this preliminary study we concluded that a larger scale major epidemiologic study on health impacts of herbicides among Korean veterans exposed is not only Indispensible but also well designed study with more valid exposure information and diagnosis may be able to establish causal relationship between certain groups of diseases and exposure to the herbicides among Korean veterans.

Natural Tooth Color Evaluation in the Korean Elderly Population

  • Purpose: This study is to investigate the distribution of natural tooth shades in the Korean elderly population to quantify the correlation and changes of tooth color with age and gender. In addition, the possible effects of habits on tooth color were assessed. Materials and Methods: The tooth color of one of the maxillary central incisors of 200 Korean individuals aged 50 to 89 years, males and female, was measured using the portable intraoral spectrophotometer (VITA Easyshade V). CIELab and CIELCh color coordinates were recorded. We conducted the survey about the tobacco smoking, chronic disease, medication, eating habits, oral health behaviors and satisfaction with tooth color. Experimental data were statistically analyzed by using the t-test (P<0.05), two-way analysis of variance and Pearson's correlation test. Result: The most frequent color in the Korean elderly population was 3M3 & A3.5 shade. L* and h* values decreased, whereas C*, a* and b* values increased progressively with age. There was a significant interaction between age and color coordinates for b* values (r=0.245, P<0.05). Males generally have significantly higher C*, a*, b* values and lower L*, h* value compared to females. Individuals who consumed alcohol had a higher L* value (P<0.05). Conclusion: Within the limitation of this study, the central incisors were getting darker, more reddish, and yellowish with age. Information on the chromatic range of natural teeth by age and gender could help to select colors for esthetic dental restorations.

한국인에서 고엽제 관련 노출과 건강영향 및 보상정책 (Agent Orange-related Chemical Exposure: Health Effects and Compensation Policy in Korea)

  • Several US veterans stationed in Korea have told the press that around 250 fifty-five gallon drums of herbicides, including Agent Orange, were buried at Camp Carroll in 1978. Joint Korean-US Agent Orange investigation and environmental and health studies were started in July 2011. Korean soldiers and military personnel who served in Vietnam during 1964-1973 or near the demilitarized zone in Korea during 1967-1970 were exposed to dioxincontaminated Agent Orange. The joint Korean-US Agent Orange investigation team found that herbicides, pesticides, solvents and other chemicals -not Agent Orange- were buried at Camp Carroll. However, there remains the possibility that Agent Orange was stored and buried at Camp Carroll or other military camps in Korea. Adverse health effects have not been clearly explained despite a number of health studies among veterans in Korea with potential Agent Orange exposure. Although the Korean government has been compensating veterans and military personnel with 18 presumptive-service-connected-diseases and their offspring with three diseases, there are many veterans, military personnel and civilians who require the government°Øs support. The environmental study on contaminated sites and health studies among veterans and civilians were initiated three or four decades after possible Agent Orange contamination and exposure. Several toxic chemicals, including dioxin-contaminated Agent Orange, could remain in the environment and could have hazardous effects on the health of exposed people for more than several decades. Further environmental investigations and health studies are needed to ensure public safety and health, and government support should be guaranteed for people potentially exposed to these toxic chemicals.

Agent Orange Exposure and Prevalence of Self-reported Diseases in Korean Vietnam Veterans

  • Objectives: The aim of this study was to evaluate the association between Agent Orange exposure and self-reported diseases in Korean Vietnam veterans. Methods: A postal survey of 114 562 Vietnam veterans was conducted. The perceived exposure to Agent Orange was assessed by a 6-item questionnaire. Two proximity-based Agent Orange exposure indices were constructed using division/brigade-level and battalion/ company-level unit information. Adjusted odds ratios (ORs) for age and other confounders were calculated using a logistic regression model. Results: The prevalence of all self-reported diseases showed monotonically increasing trends as the levels of perceived self-reported exposure increased. The ORs for colon cancer (OR, 1.13), leukemia (OR, 1.56), hypertension (OR, 1.03), peripheral vasculopathy (OR, 1.07), enterocolitis (OR, 1.07), peripheral neuropathy (OR, 1.07), multiple nerve palsy (OR, 1.14), multiple sclerosis (OR, 1.24), skin diseases (OR, 1.05), psychotic diseases (OR, 1.07) and lipidemia (OR, 1.05) were significantly elevated for the high exposure group in the division/brigade-level proximity-based exposure analysis, compared to the low exposure group. The ORs for cerebral infarction (OR, 1.08), chronic bronchitis (OR, 1.05), multiple nerve palsy (OR, 1.07), multiple sclerosis (OR, 1.16), skin diseases (OR, 1.05), and lipidemia (OR, 1.05) were significantly elevated for the high exposure group in the battalion/company-level analysis. Conclusions: Korean Vietnam veterans with high exposure to Agent Orange experienced a higher prevalence of several self-reported chronic diseases compared to those with low exposure by proximity-based exposure assessment. The strong positive associations between perceived self-reported exposure and all self-reported diseases should be evaluated with discretion because the likelihood of reporting diseases was directly related to the perceived intensity of Agent Orange exposure.

후두실과 진성대에 발생한 점액종 1예 (Myxoma in the Laryngeal Ventricle and the True Vocal Cord:A Case Report)

  • Myxoma is an uncertain mesenchymal cell origin, characterized by irregular round, stellate or spindle cells surrounded by the matrix containing abundant mucoid material and scant vascularity. Their occurrence in descending order of frequency is in the heart, subcutaneous tissue, bone and genitourinary tract. In the head and neck region, the most predilection sites are mandible and maxilla(more than 80%). Laryngeal myxoma is extremely rare:only 5 cases have been reported in the English literature. We report a rare case of laryngeal myxoma. A 60-year-old man with hoarseness visited the out-patient department. The mass was located between the vocal fold and the vocal ligament, filling with the left laryngeal ventricle. We planned the laryngo-microsurgery and successfully excised using $CO_2$ laser. The histopathologic finding revealed the myxoma. After 18 months of surgery, there is no evidence of recurrence and mucosal scarring in the vocal fold. This report is the first case of laryngeal myxoma involving the laryngeal ventricle and the true vocal cord together.
