DEVELOPMENTAL PSYCHOPHARMACOLOGY - DEVELOPMENTAL PHARMACOKINETICS, PHARMACODYNAMICS AND PHARMACOGENETICS - (발달학적 정신약물학 - 발달학적 약동학, 약역학 및 약물유전학 -)

  • The history of pediatric psychopharmacology is very short and the research on safety, efficacy and side effects is preliminary and long-term effect on growth and maturation is not well known yet. Clinical findings have shown that the responses to antidepressants, antipsychotics, CNS stimulants and steroids in children and adolescents might be different from adult populations. Based on these findings, this paper reviewed three issues, Firstly, in developmental pharmacokinetics. the author discussed the developmental factors affecting drug absorption, distribution, protein-binding, metabolism and excretion. Secondly, in developmental pharmacodynamics, developmental characteristics of dopamine, serotonin, norepinephrine receptors and their clinical implications were reviewed. Lastly, in pharamcogenetic part, the clinical utility of pharmacogenetics, pharmacokinetic aspects of pharmacogenetics, the pharmacodynamic aspects of pharmacogenetics, the association studies of dopamine-related alleles in neuropsychiatric disorders such as attention-deficit hyperactivity disorders or Tourette’s disorders, pharmacogenetic studies dopamine-related alleles and the pharmacogenetic studies of serotonin-related alleles. Based on these preliminary research, future pharmacogenetic applications in childhood and adolescent psychiatry were also discussed.

Comparison of Three Methods Assessing the Ergonomic Risks of Manual Lifting Tasks at Ship Engine Manufacturing Facilities (선박용 엔진 제조업 들기작업의 인간공학적 위험 평가를 위한 세 가지 방법 비교)

  • A variety of ergonomic assessment methods of lifting tasks known as a major cause of work-related lower back pain have been used. But there is a limited information in choosing the most appropriate assessment method for a particular job and in finding out strengths and weakness of the methods. The purpose of this study was to assess and compare the ergonomic risks of lifting tasks in a marine diesel engine production industry by three lifting ergonomic assessment tools widely used: the National Institute for Occupational Safety and Health(NIOSH) Revised Lifting Equation(NLE), the Washington Administrative Code 296-62-0517(WAC), and the Snook Tables. Lifting index(weight of load/Recommended Weight Limit) of NLE($LI_{NLE}$) was above 1 at 34 tasks(75.6%) of a total number of 45 lifting tasks. LI of WAC($LI_{WAC}$) was above 1 at 11 tasks(24.4 %). LI of Snook Table($LI_{Snook}$) was above 1 at 29 tasks(64.4%). Thus, LI was high in orders of $LI_{NLE}$ > $LI_{Snook}$ > $LI_{WAC}$. There were significantly high correlations among three Lls(p<0.01). The correlation coefficients between $LI_{NLE}$and the other three Lls($LI_{WAC}$ and $LI_{Snook}$) were r=0.93 and r=0.88, respectively. The linear regression equations were y = 0.444x + 0.11(r=0.93) between $LI_{NLE}$ and $LI_{WAC}$, y = 0.93x + 0.008(r=0.88) between LI(NLE) and $LI_{Snook}$. The LI values by WAC was significantly lower than those by the other tools. The compared features, strength and limitation among these tools were described in this paper.

  • The purpose of this study to conform the effect of the aseptic dressing method to prevent infusion phlebitis. One quaxi-experimental, nonequvalent control group post-test design was used to evaluate prevention of phlebitis between a control group and an experimental group. The data for the control group were collected from 100 hospitalized patients from July 1 to November 30, 1999. The data for the experimental group were collected from 100 hospitalized patients from December 1, 1999 to March 5, 2000. The control group used paper tape on the IV site and the experimental group used a sterile gauze dressing which was changed every 24 hours. Two sets of instruments were used for this study. First, instrument developed Weinstein(1993) and modified by the researcher was used for judging phlebitis. The second, instrument developed Park(1996) was used for assessment records concerning the phlebitis which developed. Catheter sites were inspected on a daily basis by unit nurses and development of phlebitis was grade and documented. Data were analyzed using $x^2-test$ and stepwise regression The results are summarized as follows : 1. The incidence of phlebitis according to the duration of catheter insertion decreased in the experimental group($x^2=3.56$, p<.05). 2. The incidence of phlebitis according to the duration of catheter insertion decreased in the experimental group($x^2=28.79$, P<.0001). 3. No significant difference was found between the experimental and control groups in the severity of phlebitis. 4. A statistically significant difference between the two groups was found in the incidence of phlebitis according to the location of the insertion site. 5. No statistically significant difference between two groups was found in the incidence of phlebitis by age 6. A statistically significant difference between two groups was found in the incidence of phlebitis by sex($x^2=3.88$, p<.05) 7. Further study revealed that the duration of catheter and sex were predictors of occurrence of the phlebitis, explaining 38.2%, 14.2% of the total variance respectively. In conclusion, the aseptic dressing method is recommended to be prevent infusion phlebitis.

  • Cirsium setidens Nakai, a wild perennial, is widely consumed as a food and traditional medicine in Korea. In addition, diverse functionalities of C. setidens Nakai, including anti-inflammatory and antioxidant effects, have been reported. However, whether or not C. setidens Nakai and its major compound, pectolinarin have high nutritional value and functional properties remains unknown. This paper investigated the proximate compositions, mineral contents, hepatoprotective activities, hepatic fat accumulation inhibitory activities, and anti-inflammatory and anti-oxidant activities of C. setidens Nakai and its component parts, including of pectolinarin. The result showed that C. setidens Nakai and its major compounds have potential as a functional food material with natural antioxidant and anti-inflammatory activities.

Effects of Sour Tastes on Blood Components in Anemic mice - Animal Experiment for Nursing Intervention of Female Anemia - (신맛(酸味)이 용혈성 빈혈흰쥐의 혈액성분에 미치는 효과 - 여성빈혈의 간호중재개발을 위한 동물실험 -)

  • Many women in the world have suffered from anemia produced by menstruation, pregnancy and delivery. In the theory of oriental medicine, a sour taste is believed to have a tonic effect on the blood. Thus this paper is to investigate the effect of sour tastes on the improvement in anemia-induced female mice, using citric acid and Fructus schizandrae(五味子). The method used in this experiment was the change of RBC, WBC, Hemoglobin, Hematocrit, Fe, and TIBC in the blood of female mice who were fed citric acid and Fructus schizandrae(五味子). The results obtained were as follows ; 1. Compared with the control group (anemia- induced group without treatment), the mean number of RBC in the blood of mice was significantly increased only in evaluating the change of sample group fed Fructus schizandrae (五味子) 500mg/kg. 2. Compared with the control group (anemia- induced group without treatment), the mean number of WBC in the blood of mice was not significantly larger in both sample groups fed citric acid and Fructus schizandrae(五味子). 3. Compared with the control group (anemia- induced group without treatment), the mean number of Hemoglobin in the blood of mice was significantly larger only in evaluating the change of the sample group fed Fructus schizandrae(五味子). 4. Compared with the control group (anemia- induced group without treatment), the mean percentage of Hematocrit in blood of mice was significantly increased only in evaluating the change of sample group fed Fructus schizandrae(五味子) 500mg/kg and sample group fed Fructus schizandrae(五味子) 250mg/kg for 3days. 5. Compared with control group (anemia- induced group without treatment), the mean volume of Fe in serum of mice was significantly increased only in evaluating the change of sample group fed Fructus schizandrae(五味子) 500mg/kg. 6. Compared with control group (anemia- induced group without treatment), the mean TIBC in serum of mice was significantly increased only in evaluating the change of sample group fed Fructus schizandrae(五味子) 500mg/kg and sample group fed Fructus schizandrae(五味子) 250mg/kg for 7days. According to these results, a sour taste is presumed to have a general tonic effect on anemia, but more study must be taken on the effects of citric acid in improving female anemia.

The Analysis of Research Trend about Management of Low Back Pain (요통관리에 관한 연구동향 분석)

  • The result of studying papers about management of lower back pain published in domestic and foreign nursing and medical magazines in these 10 years is as follows ; 1. General characteristic of lower back pain ; 1) In sex distribution, there were more men than women in 5 papers and more women than men in 4 papers among 9 papers surveryed. 2) In age distribution, thirties to forties of patients had more lower back pain as surveyed by general hospitals and fifties to sixties of patients had more lower back pain as surveyed by oriental medical hospitals. 3) In cause factor, there were 50 to 65% of sprain, 32 to 44% of herniated intervertebral disc and 13 to 29% of degenerative changes. 4) In symtom distribution, there were 26 to 57% of lower back pain, 42 to 65% of lower back pain with radiating pain and 34 to 99% of paravertevral muscle spasm. 5) In period of pain management distribution, 18 to 40% of patients experienced pain for less than 6 months and 59 to 82% of them experienced pain for more than 6 months in 3 papers among 4 papers. 6) In surveying the treatment, 66 to 88% of patients had conservative treatment and there were treatments of general hospital, oriental medicine, self remedy and traditional practice in conservative treatment. 7) In job distribution, 12 to 50% of them were housekeepers, 23 to 31% office workers, 4.6 to 36% blue color workers and 11 to 15% students. 2. As psychological character lower back pain paients had anxiety, depression, anger-hostility, phobic anxiety, neurasthenia, hypochondriasis, and interpersonal sensitivity. 3. To distinguish the cause of lower back pain, plain lumbar roentgenogram, straight leg rasing test, eletromyelogic findings, somatosensory evoked potentials CT and MRI were performed. 4. To relieve lower back pain. epidural adhesiolysis, epidulal injection of local anesthetic in mixture with steroid, lumbar spinal root block, low level laser therapy, acupuncture like transcutaneous nerve stimulation(AL TENS), topical capsaicin and lumbar orthotics were used in medical field, and relaxation technique was used in nursing field. 5. Mckenzie's extension exercise and William's flexsion exercise for lower back pain were used in medical field and Yoga exercise was applied in nursing field. 6. The more school education and self efficacy were high, the better they had active coping lower back pain positively and the less self efficacy was the more they had serious pain. As a result of studying the paper there have been very little research for lower back pain in nursing fields of Korea and foreign countries. Because 60 to 80% of population expeience lower back pain at least more than once, it is necessary to develop the study and clinical practice for management of lower back pain.

  • Purpose: This paper describes a electron field presence of magnetic field, intensity and shape surface dose variation to clinical application possibility. Materials and Methods: The using 6 MeV electron and $10{\times}10\;cm^2$ field size, 9 hole to shielding block make the by measure the film, when the magnetic field position inside and outside of the X-Omat film and parallel plate ionization chamber using the surface dose measured. Results: Present of 4 cm to the side at angle about 3 degree from beam center, use of ring type magnetic is 0.9% increase the surface dose, lens block located in the magnetic field the surface dose 1.58% increase, half magnetic field's position on the side of them at the field center of the 3.6% increase of the surface dose. Conclusion: Surface dose variation is with magnetic field about the mean electron beam of progress direction change, orbit region patient's is inconvenient without surface dose increase percentage case goodness will be used as a useful way.

  • This paper reports the purification of myomodulin A (MMA) and myomodulin E (MME) from the sea hare. The central nervous systems of 500 sea hare were extracted in an acidified solvent, after which four HPLC column systems were used to obtain pure peptides, The phasic contraction bioassay using a Mytilus edulis anterior byssus retractor muscle (ABRW) was applied to monitor all collected fractions. The pure peptides were submitted to Edman degradation based automated microsequencing. Mass spectrometry and chemical synthesis confirmed the sequence. The primary structures of MMA and MME were Pro-Met-Ser-Met-Leu-Arg-Leu-$NH_2$, (847.41 Da) and Cly-Leu-Gln-Met-Leu-Arg-Leu-$NH_2$, (830.50 Da), respectively. Synthetic peptides showed a modulating activity of phasic contraction in the ABRM of Mrtilus edulis.

BIOLOGICAL STUDIES IN CHILD AND ADOLESCENT DEPRESSION (소아 ${\cdot}$ 청소년 우울장애의 생물학적 연구)

  • Research on biological aspects on adult depression has been subjected to more than 25 years of systematic research, while biologic investigations regarding childhood and adolescent depression are only now being initiated. Although no unifying, explanatory theory of the biologic etiology of childhood depression emerges from the results of studies reviewed above, the findings do support that biological factors may be involved in the genesis of childhood depression. The research reviewed in this paper suggests that age and pubertal factors have major effects in most biological markers of depression. Some of these markers, like sleep EEG and neuroendocrine markers should be broken down by decades during adult life span. Thus, although adult data are very valuable points of departure for biological research on child and adolescent depression, it is very hard to transfer the adult data to prepubertal children and adolescents, ignoring the biological changes that take place in growth and development, pubety and aging. A great deal of work in basic developmental neuroscience remains to be done. It will be crucial for further advances in this field to determine the normal patterns of neurotransmitter interaction in this age group and to study children at high risk for depression. It will be also crucial to use primate models of depressive illness in order to be able to answer the many queations that cannot be investigated in humans for ethical issues. Conclusively, much closer collaboration between developmental and neurobiological and behavioral studies in primates and in humans will be essential for further development.

Effect of Different Surface Treatment on the Shear Bond Strength between Yttria-Tetragonal Zirconia Polycrystal and Non-10-Methacryloyloxydecyl Dihydrogen Phosphate-Containing Resin Cement

  • Purpose: To evaluate the effect of different surface treatment methods (yttria-tetragonal zirconia polycrystal [Y-TZP] primers, air-abrasion, and tribochemical surface treatment) on the shear bond strength between (Y-TZP) ceramics and etch-and-rinse non-10-methacryloyloxydecyl dihydrogen phosphate (MDP)-containing resin cements. Materials and Methods: Y-TZP ceramic surfaces were ground flat with 600-grit silicone carbide abrasives paper and then divided into seven groups of ten. They were treated as the following: untreated (control), Monobond Plus (IvoclarVivadent), Z-PRIME Plus (Bisco Inc.), ESPE Sil with CoJet (3M ESPE), air-abrasion, Monobond Plus with air-abrasion, and Z-PRIME Plus with air-abrasion. The surface of Y-TZP specimens was analyzed under a scanning electron microscope (SEM). Non-MDP-containing cements were placed on the surface-treated Y-TZP specimens. After thermocycling, shear bond strength test was performed. Bond strength values were statistically analyzed using one-way analysis of variance and Student-Newman-Keuls multiple comparison test (P<0.05). Result: The Z-PRIME Plus treatment in combination with air-abrasion produced the highest bond strength ($14.94{\pm}1.70MPa$) followed by Monobond Plus combined with air-abrasion ($10.70{\pm}1.71MPa$), air-abrasion ($10.47{\pm}1.60MPa$), ESPE Sil after CoJet treatment ($10.38{\pm}0.87MPa$), Z-PRIME Plus application ($10.00{\pm}1.70MPa$), and then Monobond Plus application ($9.25{\pm}0.86MPa$). The control ($6.70{\pm}1.49MPa$) indicated the lowest results (P<0.05). The SEM results showed different surface morphologies according to surface treatment methods compared with the Y-TZP control. Conclusion: The shear bond strength between the Y-TZP ceramic and the non-MDP-containing resin cement was the greatest when the surface was treated with air-abrasion and MDP-containing Z-PRIME Plus primer.
