Anti-inflammatory Effects of Cistanche deserticola Water Extracts in LPS-stimulated RAW 264.7 Macrophages (육총용(肉蓯蓉) 열수추출물이 RAW 264.7 대식세포에 미치는 항염증 효과)

  • Objectives : The purpose of this study is to confirm the anti-inflammatory effects of Cistanche deserticola Water Extracts(CDWE). Methods : In this study, cell viability was determined by MTT assay. NO production was determined with Griess reagent, and IL-6 production was determined by ELISA. And western blot analysis was performed to confirm the protein expression of NOS2, COX-2, I𝜅B-𝛼, p-I𝜅B-𝛼, and NF-𝜅B. Results : CDWE was not cytotoxic at 15.625-1,000㎍/㎖ in RAW 264.7 cells. CDWE inhibited NO production in a concentration-dependent manner, with inhibitory effect on expression levels of IL-6. Expression level of NOS2, NF-𝜅B p65, and p-I𝜅B-𝛼 decreased at 250-1,000㎍/㎖. mRNA expression of COX-2 suppressed at 250, 1,000㎍/㎖. Conclusions : These results suggest that CDWE has anti-inflammatory effects and can be used as an anti-inflammatory therapeutic material.

The Effect of Gamisungmagalguntang(加味升麻葛根湯) on Chronic Urticaria Identified as "Wind Heat" Pattern (풍열형(風熱型) 만성 두드러기에 대한 가미승마갈근탕(加味升麻葛根湯)의 치료효과)

  • Objective : This case series study was designed to assess the effect of Gamisungmagalguntang for chronic urticaria identified as "Wind-Heat" pattern by retrospective methods. Methods : We treated 19 patients for chronic urticaria with Gamisungmagalguntang(加味升麻葛根湯), who visited to care chronic urticaria at the department of Ophthalmology, Otolaryngology and Dermatology of Korean Medicine at Kyunghee University Korean Medical Center from 1st January 2013 to 31st August 2013. We analyzed information and body conditions of 19 patients. And by using Urticaria Activicy Score(UAS), frequency and number of wheals, itch severity and total score on a 4-point(0-3) scale were assessed in 19 patients. The change of each criterion and total scores between baseline and follow up were analyzed using paired t-test and Wilcoxon test(p<0.05). Results : 1. UAS improvement was statistically significant(p<0.001). Effects of symptom improvement were excellent(31.58%) that means symptom remission and good(42.10%) that means symptom improved over 50%. 2. Heat sensitivity as suspected provocation factor could be a marker for the Wind-Heat pattern urticaria. 3. Among the Wind-Heat pattern urticaria patients, normal digestion(52.63%) and no thirst(78.95%) were higher than abnormal conditions. Whereas about sweat item, abnormal conditions(57.89%) were higher than normal. That means abnormal sweat conditions could be a marker for a defense qi(衛氣) dysfunction of the Wind-Heat pattern urticaria. Conclusions : Our findings suggest that Gamisungmagalguntang could be effective on the Wind-Heat pattern urticaria.

A Network Pharmacology-based Study to Explore the Potential Mechanism of Artemisia capillaris Thunb. for Psoriasis Vulgaris (네트워크 약리학을 활용한 심상성 건선에 대한 인진호(茵蔯蒿)의 잠재적 작용 기전 탐색 연구)

  • Objectives : The purpose of this study is to investigate the potential mechanism of Artemisia capillaris Thunb. for psoriasis vulgaris. Methods : We conducted the network pharmacological analysis. It contains the process that search the compounds of the herb, the target proteins of the compounds, related genes of psoariasis vulgaris and the pathway/terms of the common protein lists between psoriasis vulgaris and Artemisia capillaris Thunb.. Results : 13 compounds and 30 protein targets of Artemisia Capillaris Herba were searched. And 997 psoriasis-related genes were searched. The common proteins were 11, and the core genes were 3; AKT1, CASP3, MAPK8. The related pathway/terms of 11 proteins were analyzed. ω-hydroxylase P450 pathway(60%), nitric oxide(NO) biosynthetic process(20%) were resulted. Also, 19 proteins of Artemisia Capillaris Herba were analyzed, and sterol homeostasis(78.95%), sterol biosynthetic process(15.79%), Type 2 diabetes mellitus(5.26%) were resulted. Conclusion : The Artemisia Capillaris Herba can potentially act through the ω-hydroxylase P450 pathway and nitric oxide(NO) biosynthetic process for psoriasis. Also, the metabolism of sterol biosynthesis and homeostasis can be involved in a roundabout way for psoriasis.

Analysis of Potential Active Ingredients and Treatment Mechanism of Ponciri Fructus Immaturus for Dermatitis Accompanied by Pruritus Using Network Pharmacology (네트워크 약리학을 이용한 소양증을 동반한 피부 염증에 대한 지실(枳實)의 잠재적 치료기전 탐색)

  • Objectives : To identify the active ingredient of Poncirus Trifoliata Immaturus and to explore the mechanism expected to potentially act on dermatitis accompanied by pruritus. Methods : We conducted the network pharmacological analysis. We selected effective ingredients among the active compounds of Poinciri Fructus Immaturus. We found the target protein of the selected active ingredient, disease(dermatitis accompanied by pruritus) and fexofenadine. Then we established the network between the proteins which Poinciri Fructus Immaturus and fexofenadine intersected with disease respectively, and the coregene was also extracted. After that, the active pathways in the human body involving the groups and coregenes were searched. Results : Total of 7 active ingredients were selected, and 202 target proteins were collected. There were 756 proteins related to inflammatory skin disease accompanied by pruritus, and 75 proteins were related to fexofenadine. 42 proteins crossed by Poinciri Fructus Immaturus with a disease, and 31 proteins crossed by fexofenadine with a disease. 12 proteins were found as a coregene from the proteins that cross Poinciri Fructus Immaturus and disease. Coregenes are involved in 'Nitric-oxide synthase regulator activity', 'Epidermal growth factor receptor signaling pathway'. 2 groups that extracted are invloved in 'Fc receptor signaling pathway', 'Central carbon metabolism in cancer', 'Phosphatidylinositol 3-kinase complex, class IB', and 'omega-hydroxylase P450 pathway'. Conclusion : It is expected that Poinciri Fructus Immaturus will be able to show direct or indirect anti-pruritus and anti-inflammatory effects on skin inflammation accompanied by pruritus in the future. And it is also expected to have a synergy effect with fexofenadine on skin disease.

A Case of Adult Otitis Media with Effusion Accompanied by Atopic Dermatitis (아토피피부염을 동반한 성인 삼출성 중이염 치험례)

  • Objectives : This study is to report a case of otitis media with effusion accompanied by atopic dermatitis improved by Korean medicine monotherapy. Methods : A 31-year-old woman patient visited our clinic for aural fullness, hearing loss, and tinnitus of the left ear on August 14th, 2021. She also had abdominal pain, diarrhea, and general weakness because of side effects of antibiotics. Besides, she had nasal congestion, rhinorrhea, and dry skin. Through otoscopy, her tympanum was fully filled with yellow effusion. She only took Korean medicine therapy including acupuncture and herbal medicine weekly. The treatment continued for about 4 months(total 14 visits) until December 4th, 2021. Otoscopy, numerical rating scale(NRS), scoring atopic dermatitis(SCORAD) index were used to assess the symptoms. Results : On August 30, 2021, her intestinal symptoms improved. The left tympanum was recovered to normal condition as the treatment continued. The effusion disappeared on October 16th, 2021. On November 6th, 2021, peritympanic blood congestion also almost alleviated. Subjective symptoms including aural fullness, hearing loss, and tinnitus disappeared on October 16, 2021. Nasal symptoms and dry skin also improved. The SCORAD index dropped from 15 to 4. Conclusions : These results suggests that otitis media with effusion can be sufficiently managed by Korean medicine therapy. Further, active application of Korean medicine therapy to common otolaryngological diseases is necessary.

A Study about Applicable Acupoints for Atopic Dermatitis : Focused on Frequent Herbs (아토피 피부염에 운용 가능한 경혈(經穴) 탐구 - 상용(常用) 본초(本草)를 중심으로)

  • Objectives : The purpose of this study is to investigate acupoints for atopic dermatitis which have similar impact with frequent herbs. Methods : We collected research materials in "The Journal of Korean Medicine Ophthalmology & Otolaryngology & Dermatology" published from Feburary, 2000 to September, 2017. The search terms are 'atopic Dermatitis', 'antiallergic', 'anti-inflammatory', 'antianaphylaxis', 'dermatitis' and we elected experimental researches about herbal medicine. Among the herbal medicine, we extracted frequent herbs using more than 5 times, and categorized through "Herbal Medicine". By Consulting "Principles of Meridians & Acupoints; A Guidebook for College Students", we matched frequent herbs and acupoints. Results : 1. There are 38 frequent herbs for study except Glycyrrhiza uralensis FISCH. because it was used for balance in herbal medicine. Arrange meridians in order of frequency, the most is 'Lung meridian'. Among the organs, the most related one with skin is 'Lung' and 'Lung meridian' can treat skin diseases. 2. We researched acupoints called 'specific acupoints' under elbow, knee because of clinical usage. Exceptively, SP6 and GV20 are not included in 'specific acupoints' but they can be effective for atopic dermatitis treatment. 3. Researching on MEDLINE about frequent herbs and acupoints, there are many studies that acupuncture is effective for reducing pruritus. Therefore, the acupoints which were mentioned in this study can be choosed for atopic dermatitis. Conclusions : We found several applicable acupoints for Atopic Dermatitis. We hope it will be useful in experiment such as randomized controllized trial on Atopic Dermatitis and acupuncture and futhermore in clinic.

A case of improved Wegener's granulomatosis with Oriental-Western Medicine Treatment (한 양방 동시 치료를 통해 호전된 베게너 육아종증 1예)

  • Objectives : The aim of this study is to report the improved case of Wegener's granulomatosis with Oriental-Western Medicine Treatment. Methods : The patient was treated by herbal medicine(Manhyeongja-san, Banhabaekchulchenma-tang, Jaeumgeonbi-tang), acupuncture and moxa constantly. And she was also treated by glucocorticoids treatment and immunosuppressive therapy provided by department of Rheumatology. The otorhinolaryngologic and the ophthalmologic Western medication treatment were also supplied. The information was collected retrospectively. Results & Conclusions : Chronic otitis media of both ear and hearing loss were improved by Oriental-Western medicine treatment. Ophthalmagia was controlled as less. Dizziness and facial palsy were also disappeared. Oriental-Western medicine treatment may be effective on Wegener's granulomatosis patient when pain controling and fast improvement of symptoms are needed.

Increasing Effect of Myricetin of Biotae Semen & Biotae Orientalis Folium on the Melanogenesis (백자인과 측백엽의 성분인 myricetin이 멜라닌 생성에 미치는 영향)

  • The unique biochemical pathways in melanocytes responsible for melanogenesis provide us with a rational mechanism-based means for developing both pharmacological regulators of pigmentation and cytotoxic chemotherapeutic drugs for melanoma, Myricetin is a polyphenolic antioxidant and a component from Biotae Semen, Biotae orientalis Folium. Therefore, the present study was conducted to evaluate the effects of myricetin on the melanogenesis in human melanoma (HM₃KO) cells. The cells were treated for 5 days with myricetin at several concentrations (10 - 100 μg/ml). Treatment with myricetin dose-dependently suppressed cell growth in HM₃KO cells, But melanin formation was markedly increased by myricetin as a dose dependent manner. Myricetin did not affect to tyrosinase activity, which is a key enzyme for melanogenesis, but significantly increased the level of tyrosinase protein expression, These results suggest that myricetin stimulate melanin synthesis of human melanoma cells through the modification of tyrosinase protein expression.

Relationship between Nasal Endoscopy Index for Pattern Identification and Cold-heat Pattern Identification in Allergic Rhinitis Patients (알레르기 비염 환자의 비내시경 평가척도와 寒熱 변증과의 상관성)

  • Objective : The aim of this study was to suggest guidelines using Nasal endoscopy index for objective pattern identification in allergic rhinitis patients. we performed a clinical study to investigate the relationship between Nasal endoscopy index and Cold-heat pattern identification for allergic rhinitis patients.Methods : We assessed 32 patients with allergic rhinitis using Nasal endoscopy index and the patients filled in Cold-Heat pattern questionnaires. Then, we analyzed the relationship between Nasal endoscopy index and Cold-heat pattern identification.Results : Pale and watery rhinorrhea scores were positively correlated with Cold questionnaire score (P<0.05).Conclusion : The results suggest that pattern identification using nasal endoscopy for allergic rhinitis can be useful for assessing the diagnosis of Cold-heat pattern identification and deciding guidelines of treatment.Acknowledgments : This work was supported by a Grant of the Traditional Korean Medicine R&D Project, Ministry of Health and Welfare, Republic of Korea (HI12C1889 and HI13C0530).

A Clinical Analysis on Outpatients with Ear Diseases of Korean Medicine Ophthalmology, Otolaryngology & Dermatology Department at Daejeon Korean Medicine Hospital - From March, 2013 to February, 2019- (대전한방병원 한방안이비인후피부과로 내원한 귀 질환 환자 특성 분석 -2012년 3월부터 2019년 2월까지-)

  • Objectives : This study is designed to analyze the outpatients who received medical treatments for ear diseases in ophthalmology & otolaryngology & dermatology clinic of Daejeon Korean Medical Hospital from March, 2013 to February, 2019. Methods : We classified the otology outpatients who received medical treatments in ophthalmology & otolaryngology & dermatology clinic of Daejeon Korean Medical Hospital from March, 2012 to February, 2019 according to gender, year, age, season, anatomical parts, and main diagnosis. The statistical analysis was performed using SPSS 25.0. Results : 1. The number of outpatients with ear diseases were 880; 505 female and 375 male. The number of female patients were approximately 1.7 times higher. 2. The average age of the outpatients with ear diseases were 50.0. The number of middle-aged (40-50s) patients were 439, which is almost half the number(49.9%) of the whole patients. 3. By analyzing the number of patients classified by year, we found out that the number of the outpatients with ear diseases has increased gradually for the past 7 years. 4. In the analysis of the number of patients classified by the anatomical parts of the ear, the inner ear diseases group were the largest, amounting to 79.3% of the whole patients. 5. In the analysis of the number of patients classified by main diagnosis, Tinnitus turns out to be the largest group with 338 patients, followed by sudden sensorineural hearing loss, dizziness, otalgia, Meniere's syndrome, otitis, auditory tube dysfunction, BPPV. 6. No statistical-significant difference were shown in the analysis of the number of patients classified by season. Conclusions : It turns out that patients with ear diseases had increased by the years, and middle aged patients (40-50s) were the largest among the age groups. Most of the patients came for the inner-ear diseases, and tinnitus, sudden sensorineural hearing loss took up the largest proportions.
